1.Application evaluation of detection system based on microfluidic technology in initial screening of blood donors

		                        			【Objective】 To evaluate the screening efficacy and practical value of the portable microfluidic biochemical analyzer in the detection of blood donors before blood donation. 【Methods】 Blood donor samples, clinical blood samples and constant quality control products were collected. Referring to the documents of ISO15189 and National Health Industry Standard, the precision and accuracy of hemoglobin (Hb) and alanine aminotransferase (ALT) were verified and compared with other detection systems. 【Results】 The MS200 biochemistry instrument has an intra-batch precision of 1.40% to 1.46%, inter-batch precision of 1.91% to 1.94%, and correctness bias of -0.9% to -1.3% for Hb test, and an intra-batch precision of 3.77% to 4.86%, inter-batch precision of 4.92% to 6.02%, and correctness bias of -3.0% to -4.8% for ALT test, which were within the range of quality requirements of industry standard. Comparison of Hb test results between MS200 biochemistry and Hb201 analyser on 1 189 peripheral blood samples from donors showed no statistically significant difference (P>0.05). 65 samples showed positive correlation between MS200 biochemistry and XS-900i automated hematology analyzer on Hb test results (R2=0.986, P=0.000). Correlation analysis of all the results of ALT detection by MS200 biochemical analyzer and AU480 biochemical analyzer in 1 065 samples showed a positive correlation (R2=0.965, P=0.000). The elevated ALT samples did not affect the Hb test results, and the samples with abnormal Hb value did not affect the ALT test results, with no interference between the two items in the detection. 【Conclusion】 The MS200 biochemical analyzer based on microfluidic technology has reliable methodological performance and can meet the need of pre-donation testing.
		                        		
		                        		
		                        		
		                        	
2.Healthy Lifestyle and the Risk of Metabolic Dysfunction-Associated Fatty Liver Disease: A Large Prospective Cohort Study

		                        			 Background:
		                        			The incidence density of metabolic dysfunction-associated fatty liver disease (MAFLD) and the effect of a healthy lifestyle on the risk of MAFLD remain unknown. We evaluated the prevalence and incidence density of MAFLD and investigated the association between healthy lifestyle and the risk of MAFLD. 
		                        		
		                        			Methods:
		                        			A cross-sectional analysis was conducted on 37,422 participants to explore the prevalence of MAFLD. A cohort analysis of 18,964 individuals was conducted to identify the incidence of MAFLD, as well as the association between healthy lifestyle and MAFLD. Cox proportional hazards regression was used to calculate the hazard ratio (HR) and 95% confidence interval (CI) with adjustments for confounding factors. 
		                        		
		                        			Results:
		                        			The prevalence of MAFLD, non-alcoholic fatty liver disease, and their comorbidities were 30.38%, 28.09%, and 26.13%, respectively. After approximately 70 thousand person-years of follow-up, the incidence densities of the three conditions were 61.03, 55.49, and 51.64 per 1,000 person-years, respectively. Adherence to an overall healthy lifestyle was associated with a 19% decreased risk of MAFLD (HR, 0.81; 95% CI, 0.72 to 0.92), and the effects were modified by baseline age, sex, and body mass index (BMI). Subgroup analyses revealed that younger participants, men, and those with a lower BMI experienced more significant beneficial effects from healthy lifestyle. 
		                        		
		                        			Conclusion
		                        			Our results highlight the beneficial effect of adherence to a healthy lifestyle on the prevention of MAFLD. Health management for improving dietary intake, physical activity, and smoking and drinking habits are critical to improving MAFLD. 
		                        		
		                        		
		                        		
		                        	
3.Tubridge flow diverter for the treatment of recurrent internal carotid blood blister-like aneurysms after stent-assisted embolization

		                        			Objective:To investigate the safety and efficacy of Tubridge flow diverter for the treatment of recurrent internal carotid blood blister-like aneurysms after stent-assisted embolization.Methods:From June 2018 to April 2021, patients with recurrent internal carotid blood blister-like aneurysms treated with Tubridge flow diverter in the Department of Neurosurgery, Changhai Hospital, Naval Medical University were enrolled retrospectively. The perioperative safety, immediate postoperative and follow-up results were analyzed.Results:A total of 6 patients with recurrent internal carotid blood blister-like aneurysm after stent-assisted embolization were enrolled. The time interval from the first stent-assisted embolization to Tubridge placement was 14 to 90 d. Tubridge implantation alone was used in 4 patients, and Tubridge was implanted in the other 2 patients after the coils were packed. There were no complications during the perioperative period, and no rebleeding was observed after clinical follow-up for 5 to 36 months. Five patients were followed up by angiography for 1-3 months, and the aneurysms disappeared completely.Conclusion:Tubridge flow diverter for the treatment of recurrent internal carotid blood blister-like aneurysms is safe and effective.
		                        		
		                        		
		                        		
		                        	
4.Study on Puerariae Lobamle Radix with UPLC fingerprint

		                        			Objective:To establish the UPLC fingerprint analysis method for Puerariae Lobamle Radix. Methods:The column was Agilent ZORBAX Eclipse Plus C18 column (2.1 mm×100 mm, 1.8 μm). The mobile phase consisted of acetonitrile (A)-0.1% formic acid (b), gradient elution, flow rate was 0.3 ml/min. The detection wavelength was 250 nm. The column temperature was set at 30 ℃. The sample volume was 2 μl, and the similarity evaluation system of traditional Chinese medicine fingerprint was adopted. The cluster analysis, principal component analysis and partial least square method in SPSS software were used to judge the differences of Pueraria lobata from different habitats.Results:With puerarin as reference peak, 22 common peaks were calibrated, and 6 peaks were identified. The similarity of 25 batches of samples was above 0.990 except S22 was 0.935, which indicated that the samples were with good consistency. Through cluster analysis, principal component analysis and partial least square analysis, 25 batches of Puerariae Lobamle Radix can be clustered into 4-5 categories, and different components such as 3'-hydroxy puerarin were found. The extraction process of Puerariae Lobamle Radix was optimized based on analytic hierarchy process and multi-index orthogonal test. The results showed that adding 50% ethanol 40 ml and refluxing for 40 min was the best extraction process. Conclusion:UPLC fingerprint is suitable for Puerariae Lobamle Radix, and the results are reliable. 5.Cognitive impairment in patients with intracranial aneurysm

		                        			In addition to causing high disability and high fatality rates, ruptured intracranial aneurysms can also cause cognitive impairment. Although preventive surgical treatment can avoid intracranial aneurysm rupture and bleeding, patients may still have a certain degree of cognitive impairment, even in patients with good clinical recovery after surgery. There is no systematic review on the effect of different surgical methods on cognitive function, and the best surgical method is still inconclusive. 6.Effect of high dose irradiation on thermoluminescence detector

		                        			Objective To investigate effect of high dose irradiation on the performances of thermolumines-cence detectors (LiF:Mg, Cu, P). Methods The high-dose irradiated thermoluminescence detector was annealed by a thermoluminescence annealing furnace until the annealing was completed, and then the annealed thermoluminescent detector was irradiated 0.5Gy by 137Cs irradiator to verify the accuracy of the thermoluminescentdetector. Results The thermoluminescence detector after high-dose irradiation could not be completely annealed under the temperature condition of 240 ℃, and it could be completely annealed at a high temperature as 400℃. After 0.5 Gy irradiation by 137Cs irradiator, the measurement results of the annealed thermoluminescence detector were significantly smaller, and the dose response and dispersion of the detector were also changed significantly. Conclusions After a more than 5 Gy irradiation, the crystal structure of the thermoluminescence detector has changed, and a high temperature peak above 240 ℃ has appeared, which leads to the failure to completely anneal at normal temperature. 7.Study on the relationship between thyroid imaging reporting and data system and the Bethesda system for reporting thyroid cytopathology

		                        			Objective To compare effect between thyroid imaging reporting and data system (TI-RADS) and the Bethesda system for reporting thyroid cytopathology (TBSRTC) in the qualitative diagnosis of thyroid nodule. Methods The clinical data of 665 patients with thyroid nodule (1 598 nodules) from April 2013 to October 2016 were retrospectively analyzed. TBSRTC and TI-RADS were used for qualitative diagnosis of thyroid nodule before operation. Pathological diagnosis was performed after the operation. The diagnostic effect of TI-RADS and TBSRTC were assessed, and the factors leading to the diagnostic errors were analyzed. Results Of 1 598 thyroid nodules, the pathological diagnosis showed that benign nodules were in 202, and malignant thyroid nodules were in 1 396. The diagnostic sensitivity and 85.64%(173/202) vs. 74.75%(151/202) and specificity of malignant nodules by TBSRTC were significantly higher than that by TI-RADS: 91.76% (1 281/1 396) vs. 87.11% (1 216/1 396), and there was statistical difference (P<0.01). The diagnosis positive rate of malignant nodules with diameter<1 cm by TBSRTC was significantly higher than that by TI-RADS: 77.63% (59/76) vs. 47.37% (36/76), and there was statistical difference (P<0.01); there was no statistical difference in diagnosis positive rate of malignant nodules with diameter ≥ 1 cm between 2 methods (P>0.05). Univariate analysis result showed that the diameter of thyroid nodules in patients with TI-RADS false negative was significantly smaller than that in patients with benign thyroid nodules: (1.01 ± 0.48) cm vs. (1.51 ± 0.45) cm, the incidence of malignant thyroid nodules combined with other thyroid diseases was significantly higher than that in patients with benign thyroid nodules: 41.18% (21/51) vs. 11.32% (158/1 396), and there were statistical differences (P<0.05); the rate of thyroid dysfunction in patients with TBSRTC false positive was significantly higher than that in patients with malignant thyroid nodules: 18.26% (21/115) vs. 6.93% (14/202), and there was statistical difference (P<0.05). Conclusions The qualitative diagnosis of thyroid nodule requires the cross-reference of TI-RADS and TBSRTC, and the combination of other clinical indicators of patients can improve the detection rate of malignant thyroid nodules.
		                        		
		                        		
		                        		
		                        	
8.Treatment of thoracolumbar tuberculosis by transforaminal endoscopic debridement combined with allograft and percutaneous internal fixation

		                        			Objective To discuss the feasibility,the advantages and disadvantages,the clinical efficacy and the indications of minimally invasive transforaminal endoscopic debridement combined with allograft and posterial percutaneous internal fixation for thoracolumbar spinal tuberculosis.Methods All of 22 patients with thoracolumbar tuberculosis treated in our department from January 2012 to December 2013 were retrospectively reviewed.There are 11 male and female cases separately,with an average age of 54.1 ±10.2 years and with an average disease duration of 5.3 ± 1.9 months.Endoscopic lesion removal and allograft bone grafting combined with posterial percutaneous immobilization were performed on all these cases.The data of these patients were complete,and all patients had been followed up for more than 36 months.The clinical and radiographic results were recorded and analyzed.Results In this group,22 patients were followed-up for 41.9±2.5 months(36-48 months).The spinal kyphosis was not improved 3 months after surgery (t=0.3546,P=0.7029),but the amount of blood loss (30.5±7.9 ml) was less in the operation,the amount of postoperative analgesics(0.3±0.1 g) was low and the bed time(1.5±0.3 days) was short.No recurrence and no internal fixation failure was found after long term follow-up.Good clinical outcomes were achieved with the fusion rate reached above grade 2 in all patients(95.5%) except one.The neuralgia was relieved,and the spinal cord injury was recovered to ASIA E.The VAS score and SF-36 score which were recorded 1 month and 3 months after operative were all improved significantly compared with those before operation,and patients' life quality in the early period after operation was excellent.The incidence of complications was low(9.0%),and the patients were satisfied with the treatment process.Conclusion It may be a potential way to treat spinal tuberculosis with minimally invasive transforaminal endoscopic debridement combined with allograft and percutaneous internal fixation,which could be a powerful supplement to other therapeutic measures,and is worthy of further research and development.
		                        		
		                        		
		                        		
		                        	
9.Radiological protection assessment of a hospital Clinac 23EX medical electron accelerator construction project

		                        			Objective To evaluate the radiation protection of occupational hazards in a hospital Clinac 23EX medical electronic accelerator construction project so as to ensure the health and safety of the relevant people involved.Methods According to the relevant laws,regulations and standards of China,combined with the relevant materials provided by the construction unit,the radiation protection tests and comprehensive assessment of this project were carried out.Results The performance test results of the medical electron accelerator met the requinements of GB/T 19046-2013 The ambient dose equivalent rate in the workplace was between the background dose rate (0.10 μ,Sv/h) and 11.5 μSv/h,which suggested the computer room shielding met the requirements of radiation protection.The total body effective dose,the for 7 radiation workers were 0.85,1.19,1.64 mSv,respectively,which were lower than the dose management control values of the construction unit and the national standards.Radiation protection supplies and the management system of the construction unit met the national requirements.Conclusions The construction project can effectively control the radioactive occupational hazard factors in normal operation,and the radiation protection facilities have reached the completion requirements.
		                        		
		                        		
		                        		
		                        	
10.Analysis of surveillance results on iodine deficiency disorders in Zaoyang City of Hubei Province in 2015

		                        			Objective To find out the prevalence of iodine deficiency disorders (IDD) and population iodine nutritional status,and to evaluate the intervention effect of iodized salt during the Twelfth Five-Year Plan period,and to provide a scientific basis for adjusting the intervention programs.Methods One town was selected respectively from the east,the west,the south,the north and the central of the entire city,4 villages were selected from each town,15 residents were selected to determine iodide content by quantitative detection;1 primary school was selected respectively from those 5 chosen towns,40 subjects aged from 8 to 10 years old (half male and half female) in each school were selected to collect urine samples for determination of urinary iodine,and to detect the volume of thyroid by palpation;20 pregnant women were chosen respectively from those 5 towns to detect their urinary iodine content and to determine iodine content of their salt samples,10 pregnant women were selected to investigate their healthy knowledge.Results A total of 300 samples of edible salt were tested,the median of salt iodine was 21.2 mg/kg,coverage rate of iodized salt was 97.7% (293/300),the consumption rate of eligible iodized salt at household level was 90.3% (271/300);the median of urinary iodine level was 232.4 μg/L in 200 8-10 years old chindren;232.9 μg/L in 100 pregnant women;the prevalence rate of goiter in children aged 8-10 was 3.0% (6/200);the healthy knowledge rate of pregnant women was 82.0% (41/50).Conclusions During the Twelfth Five-Year Plan period,Zaoyang has achieved the stage of elimination of IDD,but the situation of iodine nutritionrich children and the iodine nutrition-lack pregnant women are still exist.Therefore,we still need to strengthen the salt market management and insist to spread the knowledge of IDD,to eliminate the non-iodized salt from the market,and to ensure all people in appropriate iodine nutrition.
