1.Exploring Detoxication Mechanism of Dioscoreae Bulbiferae Rhizoma Processed with Phaseoli Radiati Semen Juice Based on Target Organ Ferroptosis

		                        			ObjectiveTo investigate the attenuating effect of Dioscoreae Bulbiferae Rhizoma(DBR) processed with Phaseoli Radiati Semen(PRS) juice, and explore the attenuating mechanism based on ferroptosis of the main toxic target organ. MethodSixty male ICR mice were randomly divided into blank group, DBR group, water roasted DBR group(hereinafter referred to as water group), PRS juice-roasted DBR group 1(DBR-PRS 10∶1, stuffy moistening for 40 min, stir-fried at 130 ℃ for 18 min, hereinafter referred to as group 1), PRS juice-roasted DBR group 2(DBR-PRS 10∶1, stuffy moistening for 80 min, stir-fried at 100 ℃ for 14 min, hereinafter referred to as group 2), PRS juice-roasted DBR group 3(DBR-PRS=20∶3, stuffy moistening for 40 min, stir-fried at 160 ℃ for 14 min, hereinafter referred to as group 3). The raw and processed groups of DBR were gavaged with their corresponding 95% ethanol extract at a dose of 3 g·kg-1·d-1, while the blank group was gavaged with an equal volume of 0.5% sodium carboxymethyl cellulose, once a day for 14 consecutive days. Hematoxylin-eosin(HE) staining was used to observe the histopathological changes of mouse liver. Alanine aminotransferase(ALT) and aspartate aminotransferase(AST) levels in serum, as well as malondialdehyde(MDA), ferrous ions(Fe2+), reduced glutathione(GSH) and superoxide dismutase(SOD) levels in liver tissue were detected by the biochemical detection. Western blot was used to detect the expression of iron key proteins such as ferritin heavy chain 1(FTH1) and glutathione peroxidase 4(GPX4). ResultHE staining results showed that the liver tissue structure of the blank group was clear, the morphology of hepatocytes was normal, the cytoplasms of hepatocytes in the DBR group and water group were loose and vacuolar, with obvious pathological damages, and the pathologic damages of mice in the group 1-3 were significantly improved. Compared with the blank group, the levels of ALT, AST, MDA and Fe2+ in mice from the DBR group were significantly increased(P<0.01), while GSH and SOD levels were significantly reduced(P<0.01), and the protein expression levels of FTH1 and GPX4 were significantly decreased(P<0.01). Compared with the DBR group, the ALT, AST,MDA and Fe2+ levels of mice in the group 1-3 were significantly reduced(P<0.05, P<0.01), the GSH and SOD levels and the protein expression levels of FTH1 and GPX4 were significantly increased(P<0.01). Compared with the water group, the AST and MDA levels of mice in the group 1-3 were significantly reduced(P<0.05, P<0.01), the SOD level significantly increased(P<0.05, P<0.01), the FTH1 protein expression significantly increased(P<0.01), and the serum ALT level of mice in the group 2-3 significantly reduce(P<0.01), Fe2+ level significantly reduced(P<0.01), GSH level significantly increased(P<0.05, P<0.01), and GPX4 protein expression significantly increased(P<0.05, P<0.01). Among the group 1-3, the group 3 had the best detoxification effect. ConclutionProcessing with PRS juice can reduce the liver injury induced by DBR, and the mechanism may be related to the inhibition of ferroptosis in the liver. 
		                        		
		                        		
		                        		
		                        	
2.Spatial Heterogeneity and Risk Factors of Dental Caries in 12-Year-Old Children in Shanxi Province,China

		                        			Objective This study aimed to explore the spatial heterogeneity and risk factors for dental caries in 12-year-old children in Shanxi province,China. Methods The data encompassed 3,721 participants from the two most recent oral health surveys conducted across 16 districts in Shanxi Province in 2015 and 2018.Eighteen specific variables were analyzed to examine the interplay between socioeconomic factors,medical resources and environmental conditions.The Geo-detector model was employed to assess the impacts and interactions of these ecological factors. Results Socioeconomic factors(Q=0.30,P<0.05)exhibited a more substantial impact compared to environmental(Q=0.19,P<0.05)and medical resource factors(Q=0.25,P<0.05).Notably,the urban population percentage(UPP)demonstrated the most significant explanatory power for the spatial heterogeneity in caries prevalence,as denoted by its highest q-value(q=0.51,P<0.05).Additionally,the spatial distribution's heterogeneity of caries was significantly affected by SO2 concentration(q=0.39,P<0.05)and water fluoride levels(q=0.27,P<0.05)among environmental factors. Conclusion The prevalence of caries exhibited spatial heterogeneity,escalating from North to South in Shanxi Province,China,influenced by socioeconomic factors,medical resources,and environmental conditions to varying extents.
		                        		
		                        		
		                        		
		                        	
3.Surgical treatment strategy for acute type A aortic dissection involving severe stenosis or occlusion of the carotid arteries

		                        			Objective:To explore surgical strategies for acute type A aortic dissection involving severe stenosis or occlusion of the carotid arteries.Methods:From January 2019 to March 2023, a total of 29 patients with acute type A aortic dissection involving severe stenosis or occlusion of the carotid arteries were included in the study. All patients underwent emergency surgery, with simultaneous intraoperative neck incision and replacement of the unilateral or bilateral carotid arteries. Among them, there were 19 males with a mean age of(49.57±2.14)years old. Preoperative brain CT indicated abnormalities in 15 cases, transient neurological dysfunction occurred in 5 cases, and syncope in 1 case.Results:Procedures included ascending aorta replacement in 10 cases, Bentall procedure in 18 cases, and Wheat procedure in 1 case. Arch operations involved partial arch replacement in 3 cases and Sun’s procedure in 26 cases. Simple left carotid artery replacement was performed in 6 cases, simple right carotid artery replacement in 19 cases, and bilateral carotid artery replacement in 4 cases. Cerebral protection measures during circulatory arrest included unilateral cerebral perfusion in 24 cases and bilateral cerebral perfusion in 5 cases. The mean operation time was(7. 6±0. 3) h, with a mean cardiopulmonary bypass time of(196. 3±8. 7) min, aortic cross-clamp time of(113.2±6.4) min, ischemic time 12(5-16.5) min, and lowest temperature of(26.3±0.4)°C. One patient experienced in-hospital mortality. Postoperatively, new neurological dysfunction occurred in 2 cases, including 1 case with coma and permanent neurological deficit.Conclusion:In patients with acute type A aortic dissection involving severe stenosis or occlusion of the carotid arteries, simultaneous carotid artery replacement via neck incision during aortic surgery is a safe and reliable surgical approach.
		                        		
		                        		
		                        		
		                        	
4.Sleeping quality in civil aviation pilots and associated influence factors

		                        			Background Civil aviation pilots are actual operators of civil aircraft. Their job operations are directly associated with passenger safety and flight safety. Unsafe aviation operations are related to fatigue caused by poor sleep quality. Recently, with the promotion of China's air transportation business, irregular working hours of civil aviation pilots rise gradually. However, there is still a lack of relevant research on the influence of working conditions on sleep quality in this group. Objective To explore potential impact factors of sleep quality among civil aviation pilots, for the purposes of improving sleep quality and health level of this group and ensuring aviation flight safety by formulating health management suggestions in a targeted manner. Methods All pilots of an aviation company were approached when they visited the Shanghai Hospital of Civil Aviation Administration of China for their health examinations. After informed consent, an online questionnaire survey was conducted. Self-made questionnaires were used to collect information on general conditions, lifestyle, and subjective work stress levels. The Pittsburgh Sleep Quality Index (PSQI) was used to assess sleep quality. A total of 1204 valid questionnaires were recovered. Statistical analysis was conducted using SPSS 26.0 software. Results There were 410 pilots (16.8%) who reported sleep disorders, including insufficient sleep time, difficulty falling asleep, and poor sleep quality, and 894 pilots (74.3%) who reported moderate to severe work stress. Associations were identified between sleep quality of pilots and work stress or lifestyle indicators (P<0.05), while work stress showed the strongest association (r=0.28). Further multiple-factor analysis results showed that severe work stress (OR=4.25, 95%CI: 2.89, 6.30) and alcohol use (OR=1.72, 95%CI: 1.31, 2.27) associated with an increased risk of sleep disorders. Regular breakfast (OR=0.62, 95%CI: 0.42, 0.91) and physical exercise (OR=0.68, 95%CI: 0.45, 1.03) associated with a lower risk of sleep disorders. Conclusion A certain degree of sleep disturbance is reported in the civil aviation pilot group, which is closely related to work stress and lifestyle indicators. 5.Anesthesiologists′ proficiency and training needs in flexible bronchoscope-guided awake fiberoptic intubation in China: a nationwide prevalence survey

		                        			Objective:To investigate the Chinese anesthesiologist′s proficiency, training experience and needs of flexible bronchoscope-guided awake flexible bronchoscopy intubation (AFBI) using a questionnaire method.Methods:The cluster sampling was used, and self-designed questionnaires that addressed 54 questions in 5 categories were distributed through WeChat and online platforms. The survey took one month, and the answers were automatically recorded by the WeChat server.Results:A total of 1 250 anesthesiologists participated in the survey in 30 provinces of China, 9 of them were not anesthesiologists, and 1 241 (99.28%) questionnaires were validated. In the valid questionnaires, 52.70% (654) of the anesthesiologists were from tertiary hospitals, and 74.78% (928) of the anesthesiologists were attending physicians or above, only 7.57% (94) of the anesthesiologists had sufficient confidence in AFBI. Twenty-five point two two percent (313) of the anesthesiologists preferred fiberoptic intubation as the first tool when dealing with the anticipated difficult airway. Forty-eight point one one percent (597) of the anesthesiologists had implemented AFBI. Among them, 80.74% (482) had experienced unsuccessful AFBI practices. Eight hundred and ninety-four anesthesiologists had received AFBI training, and the most common AFBI training strategy was theoretical lectures. In addition, the degree of satisfaction regarding the theoretical lectures quality, technical training, clinical practice relativity and non-technical skills training was 21.47% (192), 14.32% (128), 12.3% (110) and 17.90% (160), respectively. The degree of satisfaction with all the 4 training elements mentioned above was 7.27% (65).Conclusions:The awareness and practice of Chinese anesthesiologists in terms of clinical application of AFBI to treat difficult airways need to be strengthened at present, and the lack of high-quality AFBI training may be the key.
		                        		
		                        		
		                        		
		                        	
6.Midterm clinical results of Sun's procedure with median resternotomy

		                        			Objective:To summarize the clinical data of Sun's procedure(total aortic arch replacement with frozen elephant trunk technique) in the treatment of complex aortic arch disease after previous cardiac surgery.Methods:From January 1, 2018 to December 31, 2020, a total of 55 patients underwent resternotomy Sun's procedure in the Aortic Surgery Center of Beijing Anzhen Hospital, including 41 males and 14 females, with a mean age of(45.4±12.7) years old, were retrospectively analyzed. The indications of primary cardiac surgery included type A aortic dissection, aortic root or ascending aortic aneurysm, heart valve surgery, and coronary heart disease. Indications for reoperation included residual aortic dissection larger than 55 mm in diameter, aortic aneurysm dilation, new type A aortic dissection, anastomotic leakage with symptoms, and pseudoaneurysm. All the operations were performed under general anesthesia and median resternotomy, total aortic arch replacement with the stented elephant trunk implantation and were performed by anterograde unilateral or bilateral cerebral perfusion.Results:There was no intraoperative death, and the postoperative mortality was 9.1%(5/55). The causes of death were 2 cases of low cardiac output, 1 case of respiratory failure, 1 case of cerebral complications, and 1 case of gastrointestinal bleeding. Except death, there were 2 cases of postoperative cerebral complications(2/50, 4%), 5 cases of spinal cord injury(transient paraplegia)(5/50, 10%), the median duration of ventilator use was 17 hours(14-42 h). Other postoperative complications included respiratory insufficiency requiring ventilatory support longer than 48 hours(8/50, 16%), renal insufficiency requiring temporary dialysis(2/50, 4%). The follow-up time was(25.9±11.2) months(10-47 months), during which 1 case died due to cerebral complication, 4 cases underwent total thoracoabdominal aorta replacement, and 1 case underwent anastomotic leakage repair.Conclusion:It is safe and effec to perform Sun's procedure(total aortic arch replacement with frozen elephant trunk technique) in the treatment of complex arch disease after previous cardiac surgery.

		                        			The end of the COVID-19 infection peak in 2022 prompts a backlog of cardiovascular surgical patients to gradually return to the hospital, resulting in a surge in cardiovascular surgeries. However, against the backdrop of the COVID-19 pandemic, the clinical practice of cardiovascular surgery faces many problems. Therefore, organized by Beijing Anzhen Hospital, experts in cardiovascular surgery and related fields have formulated hospital expert experience on perioperative treatment principles of cardiovascular surgery for patients infected with COVID-19. This article summarizes the clinical decision-making of patients requiring cardiovascular surgery after COVID-19 infection, and advises on the corresponding recommendations according to the existing evidence-based medical evidence as well as the actual clinical practice experience of relevant experts. The main content of the article includes special requirements for cardiovascular surgical treatment indications in patients with COVID-19 infection, selection of surgical timing, special requirements of preoperative, intraoperative and postoperative management, etc., which aims to provide COVID-19-infected patients with guidance on rational decision-making when receiving cardiovascular surgery.
		                        		
		                        		
		                        		
		                        	
8.Research on Wnt2/β⁃catenin pathway changes in liver regeneration and repair in C57BL/6 mice

		                        			Objective:
		                        			To explore the changing characteristics of Wnt2/β⁃catenin pathway in liver regeneration and repair in C57BL/6 mice.
		                        		
		                        			Methods  :
		                        			Male C57BL/6 mice were randomly divided into a sham⁃operated group (Sham group) , 1 d post⁃operative group , 2 d post⁃operative group , 4 d post⁃operative group , 6 d post⁃operative group , and 8 d post⁃operative group. Mice in the operated group underwent partial hepatectomy ( PHx) to remove the left and middle lobes of the liver, respectively. Plasma and liver tissues were collected on postoperative days 1 , 2 ,4 ,6 , and 8 , and plasma ALT and AST activity was measured by ALT(alanine aminotransferase) and AST( aspartate aminotransferase) biochemical analysis kits; Ki67 ⁃positive cells were identified by immunohistochemistry in each group;the number of HNF4⁃α and Ki67 double positive cells , the number of LYVE1 and Ki67 double positive cells , and the number of β ⁃catenin transferred into the nucleus were determined by immunofluorescence method. The expression of Wnt2 protein in each group was detected by Western blot , and the time characteristics of its expression during liver regeneration were analyzed. 
		                        		
		                        			Results:
		                        			Liver weight and liver/body weight ratio peaked on day 6 after PHx and approached the level of the Sham group. After PHx , there was severe liver damage on day 1 , but it had normalized by day 4. On day 2 after PHx , mainly hepatocytes proliferated;and on days 4 and 6 , mainly liver sinusoidal endothelial cells proliferated ,while the Wnt2/β⁃catenin pathway was activated.
		                        		
		                        			Conclusion
		                        			The liver has a powerful regenerative repair ability ,which is closely related to the rapid proliferation of hepatocytes and liver sinusoidal endothelial cells , and the activation of the Wnt2/β⁃catenin pathway is activated in the regenerative repair of the mouse liver.
		                        		
		                        		
		                        		
		                        	
9.Optimal Ratio of Rehmanniae Radix and Lilii Bulbus in Baihe Dihuangtang Based on Antidepressant Activity and Its Mechanism

		                        			ObjectiveBased on the antidepressant activity, the optimal ratio of Rehmanniae Radix and Lilii Bulbus in Baihe Dihuangtang was optimized and its possible mechanism was preliminarily explored. MethodA total of 100 male mice were randomly divided into normal group, model group, Rehmanniae Radix single decoction group, Lilii Bulbus single decoction group, and 5 different ratios of Baihe Dihuangtang group (Rehmanniae Radix-Lilii Bulbus 5∶6, 5∶5 , 5∶4, 5∶3, 5∶2), fluoxetine hydrochloride group. Except for the normal group, the remaining 9 groups were treated with chronic unpredictable mild stress method (CUMS) for 21 consecutive days to establish the depression mouse model.The Rehmanniae Radix single decoction group, Lilii Bulbus single decoction group and 5 different ratios of Baihe Dihuangtang group were administered with corresponding drugs at a dose of 2 g·kg-1 respectively, and fluoxetine hydrochloride group was administered fluoxetine hydrochloride at a dose of 0.01 g·kg-1, the normal group and the model group were respectively given an equal volume of normal saline by gavage for 21 consecutive days. The antidepressant effect of Baihe Dihuangtang with different ratios was evaluated by behavioral experiments. Western blot was used to detect the expression levels of brain-derived neurotrophic factor (BDNF) and tyrosine kinase receptor B (TrkB) in hippocampus. The level of total superoxide dismutase (SOD) in hippocampus was detected by hydroxylamine method. The level of reduced glutathione (GSH) in hippocampus was detected by spectrophotometric method. The levels of glutathione peroxidase (GPX) and glutathione S-transferase (GST) in hippocampus was detected by colorimetric method. The level of malondialdehyde (MDA) in hippocampus was detected by thiobarbituric acid method, and preliminary exploration of its antidepressant mechanism. ResultCUMS caused a significant decrease in the preference rate of sugar water and a significant prolongation of forced swimming and tail suspension immobility time (P<0.01), while the different ratios of Baihe Dihuangtang and the single decoction of Rehmanniae Radix and Lilii Bulbus were significantly reversed to varying degrees. Abnormalities of the above indicators were observed (P<0.05, P<0.01), especially when the ratio of 5∶6 had the most obvious effect on the above indicators, Moreover, CUMS caused the hippocampal BDNF and the expression of TrkB, SOD and glutathione antioxidant-related indexes were significantly decreased (P<0.01), and the levels of lipid peroxidation product MDA was significantly increased (P<0.01). However, the superior ratio of Baihe Dihuangtang (Rehmanniae Radix-Lilii Bulbus 5∶6, 5∶5, 5∶4) and the single decoction of Rehmanniae Radix and Lilii Bulbus significantly reversed the abnormality of the above indicators to varying degrees (P<0.05, P<0.01). ConclusionIn summary, 5 different ratios of Baihe Dihuangtang and the single decoction of Rehmanniae Radix and Lilii Bulbus can produce antidepressant effect, especially when the ratio of 5∶6 had the most obvious effect on the above indicators, and the antidepressant effect gradually weakens with the proportion of Lilii Bulbus in the recipe decreases, and its mechanism may involve enhancing the nutritional level and antioxidant defense ability of hippocampal central nervous system. 
		                        		
		                        		
		                        		
		                        	
10.Surveillance of Japanese encephalitis related mosquitoes in Mengla County, Yunnan Province

		                        			ObjectiveTo analyze the epidemic characteristics and trend of mosquito vectors related to Japanese encephalitis (JE) in Mengla County, and to provide scientific evidence for JE prevention and control. MethodsThe JE related mosquito vector monitoring data in Mengla County from 2016 to 2020 were collected and subjected to further statistical analysis. ResultsA total of 1 689 mosquitoes were captured at the JE mosquito vector monitoring sites in Mengla County, 36.3% of which were captured in 2020 and 13.3% in 2017. The density of Culex tritaeniorhynchus was the highest (3.04 per lamp per day), and that of Anopheles sinensis was the lowest (1.03 per lamp per day). The distribution of mosquito species showed significant difference in the same year. The mosquito density in pig house was 12.93 per lamp∙day, and that in human house was 4.67 per lamp∙day. The mosquito density of different mosquito species in pig house was higher than that in human house. There was no significant difference in the site distribution of Anopheles sinensis, but there were significant differences in the site distribution of Culex tritaeniorhynchus, Culex pipiens quinquefasciatus and other mosquito species. The mosquito density peaked in May (12.78 per lamp per day) and July (10.28 per lamp per day). The temporal distribution showed that the vector density decreased gradually from May to October, however, the species population structure also varied significantly, and the trends of each mosquito species also varied greatly. In Mengla, Culex tritaeniorhynchus peaked in May. ConclusionThe JE epidemic situation in Mengla County is still severe. It is recommended to strengthen prevention and control in the peak season of mosquito activities, such as actively carrying out patriotic health campaigns, and effective public education.
