1.Effect of Tongfeng Decoction (痛风汤) on NLRP3 Inflammatory Vesicles and Neutrophil Extracellular Traps in Synovial Tissues of Ankle Joints in Acute Gouty Arthritis Model Rats

		                        			ObjectiveTo explore the possible mechanism of Tongfeng Decoction (痛风汤) for preventing and treating acute gouty arthritis. MethodsSixty Wistar male rats were divided into normal group, model group, colchicine group and low-, medium- and high-dose Tongfeng Decoction groups according to the random number table, 10 rats in each group. Tongfeng Decoction of 11.34, 22.68 and 45.36 g/kg were given by gavage to low-, medium- and high-dose Tongfeng Decoction groups, colchicine 3.15×10-4 g/(kg·d) to the colchicine group, and normal saline 10 ml/(kg·d) to the normal group and model group respectively for 7 consecutive days. After 1 hour of gavage on day 5, rats in all groups except the normal group were modelled as acute gouty arthritis in the ankle joint of the right hind limb with the modified Coderre's method; rats in the normal group were injected with 0.2 ml of normal saline at the same location. The swelling degree of the ankle joint was measured before modelling and after 6 h, 12 h, 24 h and 48 h of modelling, respectively. HE staining was used to observe the histopathological and morphological changes in the synovial tissue of the ankle joint; immunoblotting and RT-qPCR were used to detect NOD-like receptor protein 3 (NLRP3), apoptosis-associated speck-like protein (ASC), cysteine aspartate protease 1 (Caspase-1), and interleukin 1β (IL-1β) protein and mRNA expression levels in ankle synovial tissues, respectively; immunohistochemistry was used to detect the positive expression of Caspase-1 and IL-1β in ankle synovial tissues; ELISA was used to detect the tumour necrosis factor alpha (TNF-α), IL-1β, and interleukin in serum; immunofluorescence staining was used to observe the formation of neutrophil extracellular traps (NETs) in the synovial tissues of the ankle joints. ResultsCompared with the normal group, rats in the model group showed elevated joint swelling at all time points, significantly increased inflammatory cell infiltration and the number of synovial cell layers in the synovial tissues of the ankle joints, elevated NLRP3, Caspase-1, ASC and IL-1β protein and mRNA expression, elevated positive expression of Caspase-1 and IL-1β, increased formation of NETs, and elevation of TNF-α, IL-6, and IL-1β in serum (P<0.05). Compared with the model group, there was an improvement in synovial cell proliferation and inflammatory cell infiltration of the ankle joint in the colchicine group, high- and medium-dose Tongfeng Decoction groups. In the colchicine group and the high-, medium- and low-dose Tongfeng Decoction groups, the degree of joint swelling, positive expression of Caspase-1, IL-1β and formation of NETs in the synovial tissue of the ankle joint, and TNF-α, IL-6 and IL-1β in serum reduced 24 h and 48 h after modelling; in colchicine group and high-dose Tongfeng Decoction group, NLRP3, ASC, Caspase-1, IL-1β protein and mRNA expression in the synovial tissues of ankle joints all reduced (P<0.05). The colchicine group and high-dose Tongfeng Decoction group were superior to the low-dose Tongfeng Decoction group in reducing ASC, Caspase-1, IL-1β protein and mRNA expression in the ankle synovial tissues, the positive expression of Caspase-1, as well as TNF-α, IL-6, and IL-1β level in serum (P<0.05). ConclusionTongfeng Decoction showed effectiveness for the prevention and treatment of acute gouty arthritis, and its mechanism may be related to the inhibition of the assembly and activation of NLRP3 inflammatory vesicles in ankle synovial tissues, the inhibition of the release of inflammatory factors, and the formation of NETs. 
		                        		
		                        		
		                        		
		                        	
2.Effect of Guizhi Shaoyao Zhimu Decoction on collagen-induced arthritis rat based on OPG/RANKL/RANK signaling pathway

		                        			Objective To study the effects of Guizhi Shaoyao Zhimu Decoction on factors related to bone destruction and bone protection in rats with collagen-induced arthritis(CIA)based on osteopro-tegerin(OPG)/receptor activator of NF-κB ligand(RANKL)/receptor activator of NF-κB(RANK)signaling pathway.Methods According to the body weight,60 female Wistar rats were randomly di-vided into the following six groups:the normal group,the model group,the Triperygium wilfordii mul-tiglucoside group(0.01 g/kg),the Guizhi Shaoyao Zhimu Decoction low-dose group(8.6 g/kg),the Guizhi Shaoyao Zhimu Decoction medium-dose group(17.2 g/kg),and the Guizhi Shaoyao Zhimu Decoction high-dose group(34.4 g/kg)(n=10 rats per group).The rats in all groups except for the normal group were given 100 μg bovine type Ⅱ collagen on the 1st and 8th days to establish the CIA model,and was injected into the left foot sole and tail root of the rats.After the successful modeling,the rats were treated by gavage for 4 weeks.The general state,body weight,and arthritis index(AI)score of rats were recorded,and the contents of RANKL and OPG in rat serum were determined by enzyme-linked immunosorbent assay.The mRNA and protein expressions of RANKL,RANK,and OPG in the ankle joint were determined through real-time PCR and Western blotting,respectively.Results Com-pared with the normal group,the general state of the model group was poor,the toe swelling was obvious,the AI score was increased,the serum RANKL content was increased,the serum OPG content was de-creased,and the mRNA and protein expressions of RANKL and RANK in the ankle joint were increased(P<0.05).Compared with the model group,the degree of toe swelling and the AI score of rats in the Guizhi Shaoyao Zhimu Decoction low-,medium-,and high-dose groups were decreased,the serum RANKL content was decreased,the serum OPG content was increased,the mRNA and protein expres-sions of RANKL and RANK in the ankle joint were decreased,the mRNA and protein expressions of OPG were increased,and the RANKL/OPG ratio of the ankle joint was decreased(P<0.05).Conclusion Guizhi Shaoyao Zhimu Decoction can improve the destruction of joint bone in CIA rats,and its mecha-nism of action may be related to reducing RANKL level,reducing RANKL/OPG ratio,and regulating bone balance.
		                        		
		                        		
		                        		
		                        	
3.Application effect of modified installing method of high flexion rotating platform prosthesis trial model in double compartment knee arthroplasty

		                        			Objective To explore the application effect of modified installing method of high flexion rotating platform prosthesis trial model in double compartment knee arthroplasty.Methods A total of 20 patients(26 knees)with severe knee osteoarthritis admitted to Hebi People's Hospital from January 2021 to April 2022 and required double compartment knee arthroplasty were selected as the research subjects.Patients were divided into the control group(10 knees)and the observation group(16 knees)based on the installing method of high flexion rotating platform prosthesis trial model.The patients in the control group used the conventional installing method of high flexion rotating platform prosthesis test model(placing the tibial side test model,the femoral side test model and finally the pad test model),while patients in the observation group used the modified installing method of high flexion rotating platform prosthesis test model(matching the femoral test model,pad and tibial test model when the patient flexed 60° to 90°,and placing them together into the knee joint osteotomy groove).The intraoperative bleeding volume and surgical time of patients in the two groups were recorded.Before surgery,2 weeks after surgery and 18 months after surgery,the knee joint function of patients was evaluated by the American hospital for special surgery(HSS)scoring system,and the range of motion(ROM)of knee joint of patients was measured by protractor.Results The intraopera-tive bleeding volume of patients in the observation group was significantly less than that in the control group,and the surgical time was significantly shorter than that in the control group(P<0.05).There was no statistically significant difference in preoperative HSS score and ROM of knee joint of patients between the two groups(P>0.05).At 2 weeks and 18 months after surgery,the HSS score and ROM of knee joint of patients in the observation group were significantly higher than those in the control group(P<0.05).Conclusion Compared with the conventional installing method of high flexion rotating platform prosthesis trial method,the modified installing method of high flexion rotating platform prosthesis trial model in double compart-ment knee arthroplasty for patients with severe knee osteoarthritis can effectively improve surgical efficiency,reduce intraopera-tive bleeding,improve knee function and increase ROM of knee joint.

5.Construction of primary prostate cancer cell bank using conditional reprogramming cell technology

		                        			Objective:To cultivate human-derived prostate cancer (PCa) cells via conditional reprogramming cell (CRC) technology, and establish individualized cell bank for PCa research in vitro.Methods:We obtained three fresh PCa tissue samples from different patients between January 2019 and April 2019. Then each sample was divided into two parts. One was used for cancer nature confirmation by intraoperative biopsy. Another part was sent to the laboratory and digested into single primary cancer cells with 0.25% EDTA enzyme for CRC technology. The details were described as followed: 1. The primary PCa cells were co-cultured with 3T3-J2 cells irradiated by 30 Gy (feeder cells) in conditioned medium, and observed for the growth of cell clones, 2. The feeder cells were removed by 0.25% EDTA trypsin for 1 minute before primary PCa cells digested for passage. All primary PCa cells were validated by multiple experiments such as immunofluorescence, immunohistochemistry, immunoblotting and fluorescence in situ hybridization (FISH).Results:Total three cases of human-derived PCa cell lines were successfully established during 15days through CRC technology. All those primary PCa cells could be steadily and continuously passaged, which also expressed AR, CK5, CK18, P504s and PSA. FISH demonstrated that each cell line harbored≥1.6% TMPRSS2/ERG fusion and conformed to the features of PCa.Conclusion:CRC technology can be used for stable and continuous PCa cell culture in vitro.
		                        		
		                        		
		                        		
		                        	
6.Analysis of clinical diagnosis and treatment of 12 cases of intraparotid facial nerve schwannoma

		                        			Objective:To investigate the clinical manifestation and management strategy of intraparotid facial nerve schwannoma.Methods:The clinical data of 12 patients with parotid schwannoma treated in Henan Cancer Hospital from January 2009 to January 2019 were analyzed retrospectively, including 5 males and 7 females, aged from 23 to 72 years. All the 12 patients complained of a mass in the parotid region, of whom 4 patients had local discomfort and pain, and 6 patients had varying degrees of pain during palpation. Ultrasound examination showed solid mass in parotid region in 8 cases and mixed mass in 4 cases. The treatment methods, the relationship between tumor and facial nerve, the modes of treatments and the nerve functions before and after operation were analyzed and summarized with House-Brackmann grade.Results:All 12 patients received surgical treatment: 8 patients underwent tumor resection plus partial or total superficial parotidectomy, 1 patient underwent tumor resection and total parotidectomy, and 3 patients underwent tumor resection, with 1 patient in whom tumor could not be removed completely due to tumor extention to the skull base. In 9 cases, the tumor occurred in the trunk or main branch of the facial nerve, and there was no facial nerve injury occurred after surgery; in 1 case, the tumor occurred in the terminal branch of the facial nerve, and part of the severe small branches of adhesion were cut off during the operation; in 1 case, the total trunk of the facial nerve was not found and the broken end of the nerve could not be found during the operation, so the nerve transplantation was not performed. In 1 case, nerve grafting was performed after nerve transection because it was impossible to separate the trunk of the facial nerve from the tumor during the operation. Preoperatively, House-Brackmann grade (H-B)Ⅰfacial nerve function was shown in 10 patients, H-BⅡ in 1 patient and H-B Ⅴin 1 patient. Postoperatively, the facial nerve function recovered to H-B Ⅰfor 7 patients, H-B Ⅱfor 2 patients, H-B Ⅲ for 1 patient, grade Ⅳ for 1 patient with nerve transplantation, and H-B Ⅴfor 1 patient with a revised surgery due to tumor recurrence.Conclusions:The intraparotid facial nerve schwannoma is rare, and it is easy to be misdiagnosed before operation. Surgery is a main treatment for intraparotid facial nerve schwannoma. 7.Effects of nuclear factor 5 of activated T cells on proliferation and apoptosis of human gastric cancer MGC803 cells

		                        			Objective: To investigate the effects of nuclear factor 5 of activated T cells (NFAT5) on proliferation and apoptosis of human gastric cancer MGC803 cells and to explore the possible mechanisms. Methods: Three siRNAs targeting NFAT5 gene (siRNA2567, siRNA2714 and siRNA4562) and one negative control siRNA were designed and chemically synthesized before transfected into human gastric cancer cell line MGC803 by liposome. Real-time PCR was used to detect the changes of N F AT 5 mRNA expression in MGC803 cells to further pick out the siRNA that most effectively inhibit the expression of N F AT 5 . Further, Real-time PCR and Western blotting assay were carried out to test mRNAand protein levels of NFAT5 and S100A4 in cells 48 h after N F AT 5 -siRNAtransfection. Then, CCK-8 assay and FCM assay were used to detect the influence of silencing N F AT 5 on cell proliferation and apoptosis, respectively. Results: siRNA2567 was the most effective siRNA that significantly inhibited the expression of N F AT 5 mRNA ( P <0.01), and thus was validated as NFAT5-siRNA. Real-time PCR and Western blotting assay confirmed that both mRNA and protein levels of NFAT5 and S100A4 were down-regulated in cells 48 h after N F AT 5 -siRNAtransfection. Compared with NC-siRNAgroup, the proliferation ability of MGC803 cells in the N F AT 5 siRNAgroup was significantly down-regulated at 72 h and 96 h ( P <0.01).And FCM assay showed that compared with NC-siRNA group, cell apoptosis rate of N F AT 5 -siRNA group was significantly increased from (2.7±0.2)% to (7.9±0.2)%, ( P <0.01) 48 h after N F AT 5 -siRNA transfection. Conclusion: N F AT 5 -siRNA transfection can silence N F AT 5 gene expression in gastric cancer MGC803 cells effectively. 8.Effects of interleukin-17 on human laryngeal carcinoma Hep-2 cells

		                        			Objective To investigate the effects of interleukin-17 (IL-17) on the cell proliferation, apoptosis and migration of human laryngeal carcinoma Hep-2 cells.Methods IL-17 was transiently transfected into Hep-2 cells, and at the same time empty vector group (pEGFP-N1) and normal control group were set up.The efficiency of transfection was evaluated by fluorescence microscope, and the mRNA and protein expressions of IL-17 were detected by reverse transcription-polymerase chain reaction (RT-PCR) and Western blotting.The proliferation of cells was detected by methyl thiazolyl tetrazolium (MTT) method, and the apoptosis was detected by flow cytometry.The migration ability was detected by wound-healing assay and Transwell assay.ResultsHep-2 cells transfected with empty vector pEGFP-N1 and IL-17 showed green fluorescence under the fluorescence microscope.Hep-2 cells expressed IL-17 at both mRNA and protein levels after transfection with IL-17.Compared with the normal control group, the proliferation of IL-17 transfected Hep-2 cells was significantly inhibited after 48 h transfection (0.34±0.03 vs.0.46±0.04, P=0.006).The apoptotic rate of IL-17 transfected cells was higher than that of normal control group (26.80%±0.80% vs.2.90%±0.31%, P=0.000).According to the wound-healing assay, compared with the normal control group, the scratch width of IL-17 transfected cells was significantly greater (1.59±0.01 vs.1.36±0.01, P=0.000).Transwell migration experiment showed that the migration of IL-17 transfected cells was significantly lower than that of the normal control group (26.33±2.08 vs.49.33±1.53, P=0.000).Conclusion IL-17 can inhibit the proliferation of human laryngeal carcinoma Hep-2 cells, reduce their migration ability and enhance their apoptosis ability.Therefore, IL-17 may inhibit the occurrence and development of laryngeal carcinoma through a variety of mechanisms.
		                        		
		                        		
		                        		
		                        	
9.Feasibility study of coronary CT angiography in single cardiac cycle in patients with high heart rate using 256-row detector CT

		                        			Objective To investigate the feasibility of coronary CT angiography in single cardiac cycle and to analyze the image quality and radiation dose in patients with high heart rate(HR) using 256-row detector CT. Methods Ninety-two consecutive patients between October and November 2015 who were suspected coronary artery disease underwent coronary CT angiography(CCTA) were enrolled, which was performed with a 256-row detector CT(Revolution CT, GE Healthcare) using prospective ECG-triggered volume CCTA within a single cardiac cycle with snapshot freeze(SSF) technique. The patients were grouped by HR during CT scans: group A(80—89 bpm, n=56), group B(90—99 bpm, n=20), and group C(≥100 bpm, n=16). Image quality was compared before and after using SSF technique reconstructions in seventy-four patients. The image quality of coronary artery was evaluated blindly by 2 experienced radiologists using a four-point scale based on the 18-segment model according to the Society of Cardiovascular Computed Tomography guidelines. The differences in age, body mass index, heart rate and CT dose index volume,effective dose(ED) among the three groups were compared by using ANOVA analysis or Kruskal-Wallis test, the image quality and interpretability using χ2 test. Comparisons of image quality between standard and SSF were performed with paired Wilcoxon rank sum test. Kappa coefficient was used to test inter-observer agreement. Results A total of 1 065 coronary artery segments, 98.97%(1 054/1 065) met the requirements for diagnosis. No significant difference was found(χ2=1.274, P=0.563) for the diagnostic image quality of coronary artery segments among the 3 groups with 98.64%(651/660), 99.57%(232/233), 99.42%(171/172), respectively. Significant difference(χ2=68.811, P<0.05) was found for diagnostic image quality before and after using SSF with increase from 90.29%(772/855) to 99.44%(881/886). Image quality was improved with the use of SSF reconstructions and the diagnostic segments were also increased. The median of ED for group A, B and C was 2.03, 1.93, 2.37 mSv, respectively. There was no significant difference in ED among group A, B and group C(H=2.412,P>0.05). Conclusions Single cardiac cycle scan is feasibility for coronary CT angiography in patients with high heart rate using 256-row detector CT. This scan mode can maintain the diagnostic image quality with low radiation dose. 10.Analysis and prevention of postoperative delayed hemorrhage associated with radical D2 gastrectomy

		                        			Objective:To investigate the cause, treatment, and prognosis of delayed hemorrhage in patients who underwent radical gastrectomy. Methods:The clinical data of 294 patients who underwent radical gastrectomy in the Second Hospital Affiliated from Nanchang University from January 2015 to October 2015 were retrospectively analyzed. Results:A total of 15 patients suffered from delayed hemorrhage and accounted for 5.1%of the gastric cancer cases in our hospital for the same period of radical gastrectomy. Of the 15 patients, 9 underwent laparoscopic radical gastrectomy and 6 received open radical gastrectomy resection. Large vascular hemorrhage was found in 7 cases. Anastomosis and anastomotic ulcer induced hemorrhage were observed in 3 cases. Duodenal stump rupture induced hemorrhage was detected in 2 cases. Hemorrhage was also observed in some parts in 2 cases. Likewise, hemorrhage occurred in 1 case, but the affected parts were unknown. Of the 11 patients who underwent a second operation, 2 were subjected to digital subtraction angiography (DSA) and transcathete arterial embolization (TAE) to stop hemorrhage. Endoscopic hemostasis was performed to stop hemorrhage in 1 case. Conservative treatment was administered to stop hemorrhage in 1 case. The secondary surgery rate was 73.3%(11/15) with mortality and curative rates of 40%(6/15) and 60%(9/15), respectively. Conclusion:For delayed hemorrhage after D2 of gastric cancer, a second radical surgery and death rates were high. Therefore, patients suffering from hemorrhage should be subjected to comprehensive clinical treatment and positive measures. Major vascular bleeding, anastomotic leakage, anastomotic ulcer, and duodenal stump rupture are relevant risk factors. Anastomotic fistula and celiac artery bleeding complications caused hemorrhage is the leading cause of death. Extensive bleeding and unstable vital signs should be checked. A second operation and abdominal drainage should also be timely conducted to as effective methods. Realistic and conservative treatment can be administered to patients with stable vital signs and low amount of blood loss. Endoscopic hemostasis can be applied to alleviate simple anastomotic ulcer bleeding. DSA can be initially performed to detect unknown bleeding sites.
