Objective:This paper aims to investigate the awareness and demand for medical liability insurance of doctors, and to provide policy recommendations for the further development of medical liability insurance in China. Method:A stratified random sampling survey is conducted on doctors from a Top3 general hospital in Guangzhou, and the relevant departments and individual doctors were interviewed. Results:The higher the risk in a department, the higher their awareness of medical liability insurance becomes;the longer the length of service for a doctor, the more understanding of medical liability insurance the doctor has. Medical liability insurance is generally supported by doc-tors with a strong willingness of demand, but they opinions on bearing and sharing the insurance premium are quite different. Discussion:Doctors still lack sufficient knowledge of medical liability insurance due to the lack of effective publicity. The structural mode of medical liability insurance is still relatively vague, and the government leadership and market operation ability have a serious shortage. Suggestion:This paper suggest to improve the relevant laws and regulations of medical liability insurance, to strengthen government supervision and publicity, to build a reasonable development model of medical liability insurance in line with the national circumstances, and to strengthen an effec-tive combination of government leadership and market operation.

BACKGROUND:Intraarticular hyaluronic acid (HA)has been used as one common method in the treatment of osteoarthritis. But its application in treating Kashin-Beck disease (KBD) is generalizing. The effect of HA on chondrocyte metabolism in KBD patients remains unclear.OBJECTIVE:To investigate the effects of HA on the synthesis of collagen Ⅱ and aggrecan in KBD chondrocytes cultured in vitro to understand its clinical treatment for KBD.DESIGN,TIME AND SETTING:Comparative observation. The experiment was performed in the Laboratory Center of Biomedicine and Endemic Disease Laboratory of College of Medicine,Xi'an Jiaotong University from September 2006 to January 2009.MATERIALS:Samples of KBD were from cartilage and corpus liberum excised from 6 KBD patients undergoing knee joint operation, and normal samples of fresh knee cartilages were provided by 6 people undergoing amputation or death by accident METHODS:Chondrocytes of KBD and normal control were separated and cultured in vitro. Varying dosages of HA (100,500 gm/L) were administered to normal and KBD chondrocytes cultured in vitro for 6 days. Negative control was not treated with HA.MAIN OUTCOME MEASURES:Effects of HA on collagen Ⅱ and aggrecan mRNA expression in KBD patients and normal people by RT-PCR.RESULTS:Compared with normal control group, the mRNA level of collagen Ⅱ and aggrecan were lower in KBD (P<0.05).After administration of HA (100,500 mg/L) the mRNA level of collagen Ⅱ and aggrecan were significantly increased, and the effects of 500 mg/L was more significant.CONCLUSION:HA can increase the collagen Ⅱ and aggrecan mRNA level of KBD chondrocytes cultured in vitro. In particular,500 mg/L HA significantly promoted chondrocyte metabolism compared with 100 mg/L HA.

Objective To study the molecular pathway of osteoblastic differentiation induced by substance P (SP), a neurotransmitter. Methods Mesenchymal stem cells were isolated and cultured, and treated with SP or its receptor (NK1) antagonist to induce osteoblastic cell differentiation, respectively. Alkaline phosphatase activity was determined; Osterix gene expression was detected by RT-PCR after 1-2 weeks for three times. The data of each culture condition were analyzed using SPSS12.0 statistical software to determine whether the differences between conditions were significant. Results After 4-5 days' culture, bone marrow stromal cells became spindle-shaped, triangular or polygonic. They covered the plate surface, formed extensive cell sheets in each group after 11-12 days of culture, and then induced differentiation to osteoblast. SP up-regulated the important transcription factor Osterix gene expression significantly (P<0.05). Conclusion The up-regulation of Osterix gene expression by SP may stimulate osteoblastic cell differentiation. SP's regulation depends on its receptor NK1.

This is an in vitro study to explore the effects of cryopreservation and resuscitation on the biological characteristics of osteoblasts at different times. Osteoblasts taken from the crania of newly born SD rats were cultured. Comparative studies were made on the cells' proliferation, the activity of alkaline phosphatase (ALP), and the number of live cells among fresh cultured osteoblasts and cells after the inception of cryopreservation and resuscitation at time-periods of one, three, six months respectively. The results showed that there were no significant differences among four groups in cell proliferation and in activity of ALP (P > 0.05). Yet, after cryopreservation and resuscitation, there were significant differences between the six-month group and the other three groups (P < 0.05). The results also showed, after cryopreservation and resuscitation, there were no significant differences between the control group and the one-month and three-month groups, respectively (P > 0.05). These findings indicated that the live cells might decrease in number after the osteoblasts were cryopreserved for too long a period, but after cryopreservation and resuscitation, the cells still retained the original biological characteristics of osteoblasts.

Journal of Biomedical Engineering 2006;23(2):366-369
This study was conducted to find perfect temperature sintered bone as carrier of bone morphogenetic protein (BMP). The different temperature active sintered bones, which were made up of calcine bone and bone morphogenetic protein, were implanted into the defects of rabbit radius. Compared with the sintered bone of 600 degrees C, the sintered bone of 900 degrees C and 1200 degrees C could induce more pieces of bone formation and be replaced by new bone. There were more pieces of new bone formation in sintered bone of 900 degrees C and 1200 degrees C than those in sintered bone of 600 degrees C (P<0.05). There was no difference between the sintered bone of 900 degrees C and 1 200 degrees C (P>0.05). In comparison with the sintered bone of 600 degrees C and 1200 degrees C, the sintered bone of 900 degrees C may be the choicest carrier of bone morphogenetic protein.
