1.Analysis of the mechanism of phellodendron amurense polysaccharide in the treatment of liver injury based on network pharmacology and in vivo experiments

Objective To analyze the effect and molecular mechanism of phellodendron amurense in the treatment of liver injury based on network pharmacology,and to verify the relevant prediction targets and the protective effect of phellodendron amurense extract-Phellodendron amurense polysaccharide on immune liver injury through mice.Methods TCMSP and Swiss target prediction databases were used to retrieve and screen phellodendron amurenses active components and action targets,and then obtain disease-related targets on GeneCards and OMIM websites,and take compounds and disease intersection targets for protein interaction.Analysis,GO biological function and KEGG signaling pathway enrichment analysis,followed by molecular docking of compounds and key target proteins,and finally established a mouse liver injury model induced by Daodou protein A(Con A)to explore the mechanism of phellodendron amurense extract in the treatment of liver injury.Results 37 active ingredients were screened.The key targets for their treatment were tumor necrosis factor α(TNF-α),serine/threonine protein kinase 1(AKT1),signal transduction and transcription activation factor 3(STAT3),epidermal growth factor receptor(EGFR)anditin.Enzyme 3(CASP3)and other enrichment analysis showed that phellodendron amurense might play a protective role in protecting the liver through molecular mechanisms such as positive regulation of MAPK cas-cade reaction,oxidative stress response and regulatory PI3K-Akt signaling pathway,lipid and atherosclerosis.Ani-mal experiments had found that the gastric treatment of phellodendron amurense polysaccharide could improve the activity of superoxide dismutase(SOD)and catalase(CAT)in liver tissue,reduce the levels of serum alkaline phosphatase(ALP),aspartate aminotransferase(AST)and malonaldehyde(MDA)in liver tissue,and regulate serum inflammatory factor while the expression of intercitin(IL)-6,IL-1 β,tumor necrosis factor α(TNF-α),ac-tivated the expression of transforming growth factor β1(TGF-β1),and reduced TNF-α mRNA expression in liver tissue.Conclusion Phellodendron amurense can intervene in lipid and atherosclerosis pathways by acting on tar-gets such as TNF-α,AKT1,STAT3,EGFR and CASP3 to reduce oxidative stress and inflammatory reactions and achieve liver protection.
2.Mechanism of ginsenoside Rg1 inhibiting the proliferation and metastasis of tongue squamous cell carcinoma

Objective To investigate the inhibitory effect of Ginsenosides Rg1(GS-Rg1)on the proliferation and metastasis of tongue squamous cell carcinoma(TSCC),and its related mechanisms of action.Methods TSCC cells were treated with GS-Rg1 at concentrations of 1.25,2.5,5.0 and 10.0 μmol·L-1 for 48 hours.The proliferation ability of cells at different concentrations was measured by cell counting kit-8(CCK-8)experiment,and the IC5ovalue of GS-Rg1 at CAL-27 for 48 hours was calculated.TSCC cells CAL-27 were divided into control group and GS-Rg1 group.The control group and GS-Rg1 group were treated with 0.9%NaCl and IC50concentration of GS-Rg1 for 48 hours,respectively.The cell cycle distribution of each group was detected by flow cytometry,and the cell metastasis ability of each group was detected by Transwell experiment.Construct TOP/FOP Flash plasmid,transfect control group and GS-Rg1 group,and detect the effect of GS-Rg1 effect on wnt/β-catenin signaling pathway activity in TSCC cell CAL-27 using luciferase assay.Using wnt/β-catenin pathway inhibitor XAV939 treated GS-Rg1 group cells(XAV939+GS-Rg1 group),and wnt/β-catenin pathway activator HLY78 was used to treat GS-Rg1 group cells(HLY78+GS-Rg1 group)and detect changes of wnt/β-catenin signaling pathway activity,the cell proliferation ability,cell cycle distribution,and metastasis ability in XAV939+GS-Rg1 group,HLY78+GS-Rg1 group and GS-Rg1 group.The expression of wnt/β-catenin signaling pathway related proteins β-catenin,and its downstream cell cycle related proteins cellular myelocytomatosis oncogene(cMYC),Cyclin dependent kinase 4(CDK4),andcyclinD1,as well as metastasis related proteins E-cadherin,N-cadherin and matrix metalloproteinase 2(MMP-2)were detected by Western blotting in each group of cells.Results GS-Rg1 significantly inhibited the proliferation ability of TSCC cells CAL-27(P<0.05),and the IC50value of GS-Rg1 on CAL-27 was(5.46±1.58)μmol·L-1.The ratio of GO/G1 phase cells in the control group and GS-Rg1 group were(60.65±2.16)%and(71.20±2.38)%,respectively;the number of cell transmembrane penetration were 87.33±7.51 and 50.67±3.21,respectively;the luciferase activity were 1.00±0.02 and 0.35±0.06,respectively.Compared with the control group,the GS-Rg1 group showed cell cycle arrest in GO/G1 phase,decreased cell metastasis ability,and the activity of wnt/β-catenin signaling pathway decreased(P<0.05,P<0.01).Compared with the GS-Rg1 group,the activity of the wnt/β-catenin signaling pathway was decreased,cell proliferation ability and metastasis ability was decreased(P<0.05),while the number of GO/G1 phase cells was increased(P<0.05),the expression of β-catenin,cMYC,CDK4,cyclinD1,E-cadherin and MMP-2 proteins were decreased(P<0.05),while the expression of N-cadherin protein increased in XAV939+GS-Rg1 group cells.However,the result were opposite in the HLY78+GS-Rg1 group of cells.Conclusion GS-Rg1 downregulates wnt/β-catenin signaling pathway inhibits the proliferation and metastasis ability of TSCC cells.
3.Effect of AMH Levels on Embryo Quality and Clinical Outcomes of Fresh Embryo Transfer in Young Infertile Female Patients Undergoing IVF/ICSI Treatment

Objective To investigate the impact of different levels of anti-Müllerian hormone(AMH)on embryo quality and clini-cal outcomes of fresh embryo transplantation in young infertile female patients undergoing treatment with in vitro fertilization/intracytoplas-mic sperm injection(IVF/ICSI).Methods The clinical data of 528 young infertile female patients who underwent IVF/ICSI treatment at the Center of Reproductive Medicine,Jinhua People's Hospital from January 2020 to December 2021 were retrospectively analyzed,and they were divided into three groups based on serum AMH levels:low AMH group(n=93),medium AMH group(n=273)and high AMH group(n=162).Baseline information,laboratory indicators,and clinical outcomes of the three groups were compared.Multiple linear regression analysis was used to determine the correlation between AMH and various laboratory indicators,Logistic regression analysis was adopted to correct for the impact of different baseline information on clinical outcomes in three groups.Results The results of multi-ple linear regression analysis showed that the level of AMH was an independent factor affecting the number of retrieved oocytes and normal fertilization rate,and positively correlated to the number of retrieved oocytes(β=0.630,95%CI:0.405-0.854,P<0.05),and nega-tively correlated with normal fertilization rate(β=-1.965,95%CI:-2.897-1.033,P<0.05).The results of Logistic regression a-nalysis showed that there was no significant difference in clinical outcomes among different AMH groups(P>0.05).Conclusion AMH is an independent influencing factor for predicting the number of retrieved oocytes and normal fertilization rate in young infertile female pa-tients,and AMH is a good indicator of ovarian reserve capacity,but it has no significant ability to predict clinical outcomes in fresh em-bryo transplantation.Even those with low AMH can achieve satisfactory pregnancy outcomes as long as they obtain embryos,and high AMH does not necessarily mean they can achieve better pregnancy outcomes.
4.Exploring a definition of healthy longevity in Chinese population based on Delphi method

Objective:To explore a definition of healthy longevity in the Chinese population based on the Delphi method.Methods:Through a comprehensive literature review and expert consultation, the dimensions in the definition of healthy longevity were identified, and a preliminary list of questions was created. Experts in clinical medicine, public health, basic research, and the elderly care service industry, who had been working in the field of geriatric health for at least 5 years, were invited to participate in the Delphi survey from August to December 2022. The survey questionnaires were administered via email in two rounds, and experts were asked to select the optimal options from the provided questions. The active coefficients were expressed by the response rate, and a consensus was reached when the largest number of experts agreed for single-choice questions and more than 70% agreed for multiple-choice questions.Results:In the two rounds, the active coefficients were 96.00% (24/25) and 79.17% (19/24), respectively, and a consensus was finally reached on nine items, including age, physical health, common metabolic indicators, mental health, cognitive function, functional ability, social activity, self-rated health, and subjective well-being. Following discussions among the research team and experts, a final definition of healthy longevity was determined. Healthy longevity could refer to a state of good physical, psychological, cognitive function and social adaptation, as well as subjective well-being, in individuals aged 90 and above. Specifically, individuals with healthy longevity should be free from diseases associated with high disability rates and mortality, such as stroke, cancer, and Parkinson′s disease. They should also maintain reasonable levels of common non-communicable disease indicators, such as blood pressure and blood glucose, and exhibit favorable mental health and cognitive function using validated measurement tools. In addition, individuals with healthy longevity should engage in social interactions with friends and relatives, care for family members, and go out to do things. Meanwhile, with the ability to complete the visual and hearing functions of daily life and communication, and the ability to complete basic activities such as walking, eating, bathing, toileting, dressing, continence of urination, and bowel movement independently, they could rate themselves to be in good health and experience a relatively high level of life satisfaction.Conclusion:A definition of healthy longevity in the Chinese population is established through the two-round Delphi consultation.
5.Exploring implementation strategies for healthy longevity among the elderly population in China based on the delphi method

The Delphi method was employed from August to December 2022, inviting 25 experts from various disciplines such as clinical medicine, public health, basic research, and the elderly care services industry. Experts were sent consultation questionnaires via email to assess the importance, feasibility, judgment basis and familiarity of each implementation strategy. Active coefficient, authority coefficient, and harmony coefficient were analyzed to ultimately determine the important and feasible implementation strategies for healthy longevity that were suitable for the Chinese elderly population.Results:The expert active coefficients of the two rounds were 96.00% (24/25) and 79.17% (19/24). The authority coefficients were (0.76±0.19) and (0.77±0.17). The average scores of importance were (4.32±0.84) and (4.36±0.82), and the corresponding scores of feasibility were (3.72±1.04) and (3.80±0.92). The harmony coefficients for the importance score were 0.269 ( χ 2=594.084, P<0.001) and 0.159 ( χ 2=193.624, P<0.001). The harmony coefficients for feasibility scores were 0.205 ( χ 2=452.008, P<0.001) and 0.167 ( χ 2=202.878, P<0.001). The final eight implementation strategies were identified after two rounds of consultation. Conclusion:Through two rounds of Delphi consultations, eight important and feasible implementation strategies for promoting healthy longevity that are suitable for the Chinese context have been proposed.
6.Promotion mechanism of astragaloside on axon repair and regeneration in experimental autoimmune encephalomyelitis mice

Objective To investigate the effects of astragaloside Ⅳ(AS-Ⅳ)on axon growth inhibitory factor A(Nogo-A)and its downstream pathway protein RHO-associated coiled spiral kinase 2(ROCK2)in experimental autoimmune encephalomyelitis(EAE)mice,and to explore the mechanism by which it promotes axon repair and regeneration.Methods EAE model was induced in C57BL/6 female mice by subcutaneous injection of myelin oligodendrocyte glycoprotein 35-55(MOG35-55).Mice were randomly divided into EAE group and AS-Ⅳ group(n=8 per group).EAE group received intraperitoneal injection of PBS on the 3rd day post-immunization,while AS-Ⅳ group was administered AS-Ⅳ at a dosage of 30mg/(kg.d)once daily,0.2 ml per injection,for 25 consecutive days.On the 28th day post-immunization,the expression levels of growth-associated protein 43(GAP-43),neuronal core antigen(NeuN),microtubule associated protein 2(MAP-2),glial fibroacidic protein(GFAP),and Iba1 in the spinal cord were detected using immunofluorescence assay.Real-time fluorescence quantitative PCR(qRT-PCR)was conducted to detect mRNA expression levels of GAP-43,Nogo-A,and Nogo receptor(NgR)genes.Western blotting was utilized to determine the expression levels of GAP-43,Nogo-A,ROCK2,phosphorylated myosin phosphatase(p-MYPT1),B-lymphoblastoma-2(Bcl-2),and Bcl-2 associated X protein(Bax).Results Compared with EAE group,AS-Ⅳ treatment significantly reduced the positive cell expression rates of Iba1 microglia and GFAP astrocyte in spinal cord(P<0.01 and P<0.001,respectively),while it also increased the positive expression rates of NeuN and MAP-2(P<0.001 and P<0.05,respectively).The treatment also upregulated the expression level of anti-apoptotic factor Bcl-2(P<0.001)and downregulated the expression level of pro-apoptotic factor Bax(P<0.05),leading to an increase in Bcl-2/Bax ratio(P<0.05).Furthermore,AS-Ⅳ enhanced the expression of GAP-43 protein(P<0.05)and decreased the mRNA expression levels of neuroregeneration inhibitor Nogo receptor(NgR)and ROCK2 gene(P<0.001,P<0.05,respectively);as well as decreased the expression levels of Nogo-A,ROCK2 and p-MYPT1 proteins(P<0.05,P<0.001).Conclusion AS-Ⅳ may inhibit the activation of microglia and astrocytes and neuronal apoptosis in EAE mice by inhibiting Nogo-A and downstream pathway ROCK 2,thereby promoting the expression of GAP-43,NeuN and MAP-2,alleviating neuronal damage,and facilitating axon repair and regeneration.
7.Cost-effectiveness analysis of different cleaning frequency on reducing pathogens on hospital bed-side curtains

Objective To explore the pathogen contamination and cost-effectiveness of hospital bed-side curtains under different cleaning frequencies.Methods Fourteen departments in a hospital with relatively higher incidence but smaller fluctuation of incidence of infection from January to June 2023 were selected.Nine wards in each depart-ment were selected,and two curtains were selected from each ward.According to the cleaning frequency,wards were divided into low,medium and high frequency groups.The qualified rate of environmental hygienic detection on pathogens on the bed-side curtains and bacterial contamination on the surface of curtains after 6 months were com-parted,cost-effectiveness analysis was conducted.Results The qualified rate of environmental hygienic detection of bed-side curtains was the highest in high frequency group,at 88.10%.Bacterial counts on the upper,middle,and lower segments of the bed-side curtains in the high frequency group were all lowest(P<0.05).Within 6 months,the incidence of healthcare-associated infection(HAI)in patients in the high and the medium frequency groups(0.16%and 0.25%,respectively)were both lower than 0.85%in the low frequency group(x2=5.653,3.915,respectively,both P<0.05).Cost-effectiveness analysis showed that for every additional unit of qualified rate for the high and medium frequency groups,the costs increase by 124.51 and 44.10 Yuan,respectively.Conclusion The increase of cleaning frequency can effectively reduce bacterial contamination of bed-side curtains,decrease the risk of HAI,and improve the qualified rate of environmental hygiene of bed-side curtains in the wards.Cleaning once every 2 months can achieve better cost-effectiveness.
8.Research progress on role of MFG-E8 in regulating function of microglia

Microglia are resident immune cells in the central nervous system(CNS),it play an important role in both physio-logical and pathological conditions of the CNS,and can be activated by a variety of physicochemical factors to produce pro-inflammato-ry phenotype(M1 type)and anti-inflammatory phenotype(M2 type).Milk fat globule epidermal growth factor 8(MFG-E8)is closely related to the biological function of microglia,which can regulate microglia neuroprotective effects such as promoting phagocytosis,anti-inflammatory,and inhibiting oxidative stress in CNS related diseases.It also regulates microglia pro-tumor effect,which in turn affects the outcome of the disease.This article reviews the role of targeting MFG-E8 in regulating the function of microglia,and provides a basis for finding therapeutic targets for CNS related diseases in the future.
