1.Analysis of Inpatient Mortality Cases in a Tertiary General Hospital in Beijing Based on Diagnosis-related Groups

		                        			 To analyze inpatient mortality cases in a tertiary general hospital in Beijing based on diagnosis-related groups (DRG), with the aim of providing references for healthcare quality management. We retrospectively collected DRG data of hospitalized patients admitted to a tertiary general hospital in Beijing from January 1, 2015, to December 31, 2023. Mortality cases were analyzed according to mortality risk stratification, with a focus on the temporal trends, departmental distribution, and DRG composition of low/medium-low mortality risk cases. Among 927 304 DRG-classified hospitalizations, 2346 cases resulted in death (stratified into 130 low-risk, 209 medium-low-risk, 411 medium-high-risk, and 1596 high-risk cases), yielding an overall mortality rate of 0.25%. The mortality rates were 0.02% (130/680 939) in the low-risk group and 0.16% (209/130 449) in the medium-low-risk group. From 2015 to 2023, the mortality rate showed a significant downward trend ( Critical care units accounted for the highest proportion of mortality cases, with surgical patients having complications/comorbidities representing the major DRG-related factors for low/medium-low-risk deaths. Hospitals should prioritize these findings by identifying areas for improvement, implementing multidisciplinary case reviews, and strengthening patient safety measures.
		                        		
		                        	
2.Progress in ERAS-based anesthetic management for patients undergoing deep inferior epigastric perforator flap breast reconstruction

		                        			At present,the development of enhanced recovery after surgery(ERAS)pathways in deep inferior epigastric perforator(DIEP)flap breast reconstruction is still in the initial stage worldwide,lacking established guidelines.In the multidisciplinary ERAS pathways,the department of anesthesiology is responsible for some core elements such as optimizing anestheticprotocols,perioperative fluid management and homeostasis regulation,prevention of hypothermia,improvement of perioperative analgesia,and postoperative nausea and vomiting prophylaxis.We summarized the anesthetic management in the ERAS pathways for patients undergoing DIEP flap breast reconstruction in Fudan University Shanghai Cancer Center,along with the recent progress,aiming to establish and improve the perioperative strategy based on ERAS pathways in DIEP flap breast reconstruction.
		                        		
		                        		
		                        		
		                        	
3.Construction and practice of the integrated medical course of Molecular Medicine

The curriculum group actively explores and practices the mode of medical integration through reconstruction of the curriculum system and optimization of the course content. 4.Progress of important clinical research of breast cancer in China in 2023

		                        			Breast cancer is the most prevalent malignant tumor among women globally,posing a serious threat to women's health.With the establishment of staging and typing principles for breast cancer diagnosis and treatment,and the development and application of novel antitumor drugs,the survival and quality of life of breast cancer patients have been continuously improving.In China,the large base of breast cancer patients possesses unique incidence characteristics,necessitating ongoing exploration of more appropriate treatment strategies;the volume and level of clinical research are also continuously advancing.In 2023,significant clinical research results were reported for different subtypes of breast cancer.In surgical treatment,clinical trials on targeted axillary lymph node dissection and the establishment of a predictive model BRCA-CRisk for contralateral breast cancer risk provide more evidence for de-escalation in surgical treatment.In the area of human epidermal growth factor receptor 2(HER2)-positive breast cancer,pyrotinib has shown significant efficacy in advanced breast cancer treatment.In triple-negative breast cancer,precision subtype treatment and immunotherapy continue to improve patient survival.For hormone receptor-positive breast cancer,significant research results were obtained in exempting low-risk patients from chemotherapy and exploring alternative options after resistance to endocrine therapy.In the aspect of BRCA mutations,BGB-290-201 further confirmed the therapeutic efficacy and safety of poly(ADP-ribose)polymerase(PARP)inhibitors for the Chinese population.For advanced HER2-negative patients carrying germline BRCA(gBRCA)1/2 mutations,pamiparib will be an ideal treatment choice.This article reviews the important clinical research in the field of breast cancer in China in 2023,summarizes key results,and aims to provide reference ideas for future clinical research.
		                        		
		                        		
		                        		
		                        	
5.Efficacy comparison of botulinum toxin A injection and extraocular muscle surgery in the treatment of large angle acute acquired concomitant esotropia

		                        			 AIM: To explore the variance in efficacy between botulinum toxin A(BTA)injection and extraocular muscle surgery in managing large-angle(≥+60 PD)acute acquired concomitant esotropia(AACE).METHODS: A retrospective analysis was conducted on clinical data of 60 patients with AACE treated at our hospital from June 2020 to December 2022. Patients were divided into three groups based on different treatments: 2.5 IU BTA injection group(14 cases), 5.0 IU BTA injection group(29 cases), and surgical group(17 cases). Follow-up was conducted for 6 mo after treatment to observe the degree of strabismus after the correction of refractive error, visual function, treatment effectiveness, and occurrence of complications after BTA injection.RESULTS: At 6 mo post-treatment, the degree of strabismus in the surgical group and the 5.0 IU BTA injection group was lower than that in the 2.5 IU BTA injection group(P<0.017). However, there was no significant difference in the degree of strabismus between the surgical group and the 5.0 IU BTA injection group(P>0.017). The effective rate of the 5.0 IU BTA injection group was higher than that of the 2.5 IU BTA injection group(86% vs 43%, P<0.017). There was no difference in visual function among the three groups(P>0.05). The incidence of complications after treatment was not significantly different between the 2.5 IU BTA injection group and the 5.0 IU BTA injection group(43% vs 52%, P>0.05).CONCLUSION: For AACE patients with esotropia degree ≥+60 PD, bilateral medial rectus injection of 5.0 IU BTA can yield outcomes comparable to traditional extraocular muscle surgery, with the advantages of minimal trauma and simple and convenient operation. 
		                        		
		                        		
		                        		
		                        	
6.Epidural Labor Analgesia: Comfort and Safety Run in Parallel, with Teaching and Quality Control Complementing Each Other

		                        			Epidural labor analgesia aims to provide effective medical services to alleviate labor pain in parturients, while adhering to the principles of voluntary participation and clinical safety. In 2018, Peking Union Medical College Hospital(PUMCH)became one of the first pilot units for labor analgesia in China, and has achieved satisfactory results in high-quality development of labor analgesia. This article mainly introduces the achievements and experience of labor analgesia at PUMCH, including: (1) prioritizing maternal and infant safety, arranging personnel rationally, and developing standardized treatment processes through multidisciplinary collaboration to ensure safe and comfortable childbirth; (2) leveraging the hospital's comprehensive capabilities in emergency treatment, and improving collaborative rescue plans for critically ill parturients and newborns; (3) implementing advanced teaching methods to effectively train and conduct simulated drills for labor analgesia and rescue of critically ill parturients; (4) conducting patient education and informative lectures to help parturients acquire a scientific understanding of labor analgesia. 7.Research progress on tuberous sclerosis complex-associated neuropsychiatric disorders

		                        			Tuberous sclerosis complex (TSC) is a rare autosomal dominant disorder.Primarily involving the skin and central nervous system, it also impacts the heart, lungs, kidneys, and other organs.The vast majority of TSC patients may experience neuropsychiatric symptoms during their lifetime, including behavioral, mental, intellectual, academic, neuropsychological, and psychosocial disorders, which are collectively referred to as TSC-associated neuropsychiatric disorders(TAND).The TAND Checklist is a screening tool designed to identify potential neuropsychiatric disorders by facilitating dialogues between TSC patients, their families, and clinicians.This article focuses on the concept and research history of TAND and the application of the TAND Checklist, contributing to the comprehensive and systematic clinical evaluation and understanding of the prognosis of children with TAND.
		                        		
		                        		
		                        		
		                        	
8.Effect of postoperative local injection of platelet-rich plasma in ankle fractures associated with lower tibiofibular injuries on the patients'early inflammatory response and functional recovery

		                        			Objective To observe the effect of postoperative local injection of platelet-rich plasma(PRP)on patients'early inflammatory response and functional recovery in ankle fractures with lower tibiofibular injuries.Methods Fifty ankle fracture patients with lower tibiofibular injuries admitted to our hospital from January 2017 to December 2022 with postoperative local injection of PRP were selected as the PRP group,and 50 ankle fracture patients with lower tibiofibular injuries undergoing surgical treatment with ligament repair protocols in the same time period with their matched gender and age were selected as the conventional group.We compared the postoperative conditions,postoperative 3 d swelling,postoperative pain visual analogue(VAS)scores and levels of platelet-derived growth factor(PDGF),transforming growth factor β1(TGF-β1),and inflammatory mediators in the two groups,and counted the 24-week postoperative good rate,ankle plantarflexion-dorsiflexion mobility,the quality of the restoration,and the complications in the two groups.Results The swelling grading in the PRP group was lower than that in the conventional group(28 cases of grade Ⅰ,20 cases of grade Ⅱ,and 2 cases of gradeⅢ)in the 3 d postoperative period(15 cases of grade Ⅰ,23 cases of grade Ⅱ,and 12 cases of gradeⅢ),and the VAS scores in the 3 d and 7 d postoperative period[(3.24±0.61)and(2.40±0.42)points]were lower than those in the conventional group[(3.78±0.74)and(2.96±0.55)],and the lower limb weight-bearing activity time(4.01±0.65)weeks,fracture healing time(3.05±0.42)months,and postoperative return to normal work time(3.17±0.47)months were shorter than those in the conventional group[weight-bearing activity time of lower limbs(4.29±0.74)weeks,fracture healing time(3.23±0.48)months,and postoperative return to normal work time(3.40±0.59)months].While the operation time(106.53±19.77)min,number of intraoperative fluoroscopies(14.47±3.23)times,and intraoperative blood loss(84.07±25.60)ml were not statistically different from those of the conventional group[operation time(108.20±18.41)min,number of intraoperative fluoroscopies(14.53±3.06)times,and intraoperative blood loss(86.23±22.17)ml]were not statistically different when compared(P>0.05).There was no statistical difference in the comparison of preoperative inflammatory mediators,PDGF,and TGF-β1 between the two groups(P>0.05).In both groups,PDGF was higher and interleukin-6(IL-6),TGF-β1,C-reactive protein(CRP),and tumour necrosis factor-α(TNF-α)were lower at 12 weeks postoperatively compared with the preoperative period(P<0.05).In the PRP group,the postoperative PDGF was higher than that of the conventional group,and the inflammatory mediators and TGF-β1 were lower than those of the conventional group at 12 weeks postoperatively(P<0.05).In the PRP group,the postoperative 24 week good rate and ankle plantarflexion-dorsiflexion mobility were higher than that of the conventional group,and the inferior tibiofibular coalition anterior spacing(ITFACS)and inferior tibiofibular coalition posterior spacing(ITFPCS)were lower than that of the conventional group,and there was no statistically significant difference in the rate of complications when comparing the rate with that of the conventional group(P>0.05).Conclusion Local PRP injection combined with ligament repair protocol for treatment of ankle fractures with lower tibial fibula injuries accelerates patients'early functional recovery and improves the quality of anatomical repositioning,which may be related to the inhibition of inflammatory mediator release and the promotion of bone repair.
		                        		
		                        		
		                        		
		                        	
9.Research Hotspots on Systemic Lupus Erythematosus in Pregnancy Based on Bibliometrics

		                        			Objective To explore the research hotspots on systemic lupus erythematosus(SLE)in pregnancy based on the bibliometric analysis of the related articles published from 2018 to 2023 and provide di-rections for the future research in this field.Methods PubMed,Web of Science,and Embase were searched for the articles on SLE in pregnancy that were published from January 1,2018 to December 31,2023.VOSviewer was used to visualize the high-frequency keywords in the selected articles.Results A total of 266 articles were selected,from which 25 high-frequency keywords were extracted.The bibliometric analysis showed that the available studies about SLE in pregnancy mainly focused on maternal complications,maternal and fetal outcomes,and medica-tions.The studies were limited regarding the predictors,autoimmunity,immunotherapy,and long-term outcomes of offspring.Conclusion Maternal complications,maternal and fetal outcomes,and medications are currently hotspots in the research on SLE in pregnancy,while predictors,autoimmunity,immunotherapy,and long-term outcomes of offspring may become future research directions.
		                        		
		                        		
		                        		
		                        	
10.The targeting relationship and function of miR-199b-3p and CRIM1 in gastric cancer cells

		                        			Background and Aims:Studies have shown that miR-199b-3p is downregulated in gastric cancer tissues,while cysteine-rich transmembrane BMP regulator 1(CRIM1)is upregulated in these tissues.However,the role and mechanism of miR-199b-3p in the biological behavior of gastric cancer cells are still unclear,as is its potential association with CRIM1.Therefore,this study was conducted to investigate whether there is an interaction between miR-199b-3p and CRIM1 and how they affect the function of gastric cancer cells. Methods:qRT-PCR and immunohistochemistry were used to detect the expression levels of miR-199b-3p and CRIM1 in gastric cancer tissues and adjacent non-cancerous tissues.Gastric cancer MGC803 cells were used to assess changes in cell proliferation,invasion/migration abilities,and apoptosis rates after overexpression of miR-199b-3p(using miR-199b-3p mimics)or knockdown of CRIM1(using si-CRIM1).Bioinformatics analysis was used to predict the targeting relationship between miR-199b-3p and CRIM1,which was further validated by dual-luciferase reporter assay and confirmed through Western blot analysis. Results:The results of qRT-PCR indicated that,compared to adjacent non-cancerous tissues,miR-199b-3p expression was significantly lower in gastric cancer tissues,while CRIM1 expression was higher(both P<0.05).Immunohistochemistry results demonstrated positive expression of CRIM1 in cancerous tissues,while it was negative in non-cancerous tissues.Overexpression of miR-199b-3p or CRIM1 knockdown resulted in decreased proliferation and invasion/migration abilities of MGC803 cells,along with increased apoptosis rates(all P<0.05).Bioinformatics prediction and dual-luciferase reporter assays confirmed that CRIM1 is a target of miR-199b-3p.Western blot analysis showed that CRIM1 expression was significantly reduced after transfection with miR-199b-3p mimics(P<0.05). Conclusion:CRIM1 is a target gene of miR-199b-3p,which can inhibit the proliferation,invasion,and migration of gastric cancer cells while promoting apoptosis by targeting and regulating CRIM1 activity.The miR-199b-3p/CRIM1 pathway may serve as a potential therapeutic target for gastric cancer.
