1.Study on epidemiological prevalence and serological marker characteristics of hepatitis E infection

Objective:This study aims to explore the prevalence of hepatitis E virus (HEV) infection in patients and the screening value of serological indicators for HEV infection patients.Methods:Retrospective analysis was conducted on 97 440 cases of anti-HEV IgM and IgG simultaneously tested in two Beijing hospitals from January 1, 2018 to August 31, 2023. Among them, there were 61 005 males and 36 435 females, with an average age of 51.65±13.05 years old. According to the positivity of anti HEV specific antibodies, they were divided into anti-HEV IgM positive group (3 588 cases), anti-HEV IgG positive group (18 083 cases), and anti-HEV antibody negative group (78 892 cases). Results of HEV RNA, liver function, AFP, PIVKA-Ⅱ and PT were collected, and their basic clinical information were recorded. The prevalence of HEV infection in patients, as well as the relationship between the positivity of anti-HEV specific antibodies and the patient′s age group, HEV RNA, and clinical characteristics were analyzed.Results:Among 97 440 patients who tested anti-HEV IgM and IgG simultaneously, the positivity rate of anti-HEV IgM was 3.68% (3 588/97 440), and was 18.56% for anti-HEV IgG (18 083/97 440). The overall positivity rates of anti-HEV IgM in two Beijing hospitals from 2018 to 2023 were 2.51%, 2.53%, 3.02%, 4.59%, 5.72%, and 4.26% ( χ2=1 401.73, P<0.001), while the positivity rates of anti-HEV IgG were 12.56%, 12.32%, 12.85%, 22.65%, 27.42%, and 26.66% ( χ2=1 058.29, P<0.001). These rates showed a gradual increase until 2023 when a decline was observed. The positivity rates of anti-HEV IgM (2.28%, 3.60%, 4.47%) ( χ2=89.62, P<0.001) and IgG (4.71%, 17.86%, 25.94%) ( χ2=2 017.32, P<0.001) increased with age in patients who aged 1-30, >30-60, and over 60 years old. The age and ALB values of patients in the anti-HEV IgM positive group were lower than the IgG-positive group, while the proportion of males, TBIL, ALT, AFP and PT values were higher than the IgG-positive group, and the differences were statistically significance ( P<0.05). Furthermore, patients in both the anti-HEV IgM and IgG positive groups had higher age, male proportion, TBIL, ALT, AFP, PIVKA-Ⅱ, and PT values than the anti-HEV negative group. Additionally, both groups had lower ALB values than the anti-HEV negative group, all of which were statistically significant ( P<0.05). 2 162 HEV infected patients were grouped based on HEV RNA positivity. The proportion of anti-HEV IgM single positive, IgG single positive, IgM+IgG double positive, and antibody negative patients in the HEV RNA positive group were 5.42% (18/332), 3.62% (12/332), 90.36% (300/332), and 0.60% (2/332), respectively. Among them, the proportion of anti-HEV IgM+IgG double positive patients in the HEV RNA positive group was higher than that in the HEV RNA negative group ( χ2=302.87, P<0.001), while the proportion of anti-HEV IgG single positive ( χ2=174.36, P<0.001) and anti-HEV antibody negative patients ( χ2=59.28, P<0.001) were lower than that in the HEV RNA negative group, both of which were statistically significant ( P<0.001). In addition, the positive rates of HEV RNA in anti-HEV IgM positive, IgG positive, and antibody negative patients were 29.23% (318/1 088), 17.59% (312/1 774), and 0.65% (2/306), respectively. Conclusion:The HEV infection rate among patients declined in 2023. HEV infection is age-related, with older individuals being more susceptible. Abnormal liver function and jaundice were commonly observed during HEV infection. It is crucial to note that the absence of anti-HEV specific antibodies cannot rule out HEV infection; therefore, additional testing for HEV RNA and/or HEV Ag is necessary for accurate diagnosis.
2.Observations of lower limb lymphatic reflux and para-uterine lymphatic reflux by injecting indocya-nine green

Objective To observe the differences between the lymphatic reflux in the lower extremities and near the uterus by interphalangeal and cervical injection of indocyanine green(ICG).Methods A total of 50 patients with early-stage endometrial cancer or cervical cancer admitted to Zhoukou Central Hospital from June 2019 to November 2022 were selected as the research subjects.According to the ICG injection site during the surgery,patients were divided into the interphalangeal injection group(n=20)and the cervical injection group(n=30).The patients in the two groups were injected with ICG at the toes or cervix uteri,respectively.The lower limb lymphatic reflux pathway in the pelvic cavity and the para-uterine lymphatic reflux pathway were observed under fluorescence laparoscopy,and the differences between the two groups were analyzed.Results Among the patients with the interphalangeal injection of ICG(20 patients,40 sides),the lower limb lymphatic reflux was developed on 33 sides of 18 patients but not developed on both sides of 2 patients.Among the 18 patients,26 sides showed the lower limb lymphatic reflux through deep inguinal lymph nodes,circumflex iliac lymph nodes,external iliac lymph nodes,and common iliac lymph nodes;5 sides showed the lower limb lymphatic reflux to deep inguinal lymph nodes,circumflex iliac lymph nodes,obturator lymph nodes,internal iliac lymph nodes,and common iliac lymph nodes;and 2 sides showed the lower limb lymphatic reflux to deep inguinal lymph nodes,obturator lymph nodes,internal iliac lymph nodes,and common iliac lymph nodes.Among the patients with the cervical injection of ICG(30 patients,60 sides),pelvic lymph nodes were developed on 55 sides of 29 patients but not developed bilaterally in 1 patient.In the 29 patients,2 sides showed para-uterine lymphatic reflux to obturator lymph nodes,circumflex iliac lymph nodes,external iliac lymph nodes,and common iliac lymph nodes,in which circumflex iliac lymph nodes were non-sentinel lymph nodes;40 sides showed para-uterine lymphatic reflux to medial iliac lymph nodes(or obturator lymph nodes)and common iliac lymph nodes along the superior paracervical lymphatic reflux pathway;and 13 sides showed para-uterine lymphatic reflux to the internal iliac or presacral lymph nodes along the inferior paracervical lymphatic reflux pathway.The shared pathway of lower limb lymphatic reflux and para-uterine lymphatic reflux was upward reflux from obturator,external iliac and common iliac lymph nodes.The circumflex iliac lymph node developing rates in the interphalangeal and cervical injection groups were 93.94%(31/33)and 3.63%(2/55),respectively.The interphalangeal injection group had a significantly higher circumflex iliac lymph node developing rate than the cervical injection group(P<0.05).Conclusion The application of ICG under fluorescence laparoscopy intuitively observed the lower limb lymphatic reflux and the para-uetine lymphatic reflux pathway.The difference between the two is that the lower limb lymphatic reflux flows through the circumflex iliac lymph nodes at the distal end of the external iliac lymph nodes,while cervical cancer and endometrial cancer rarely transfer there.
3.Clinical characteristics of patients with moderate or severe valvular heart disease

Objective:To describe the characteristics, etiology and patterns of outpatients and inpatients patients with moderate or severe valvular heart disease (VHD).Methods:This is a cross-sectional study. Outpatients and inpatients with moderate or severe VHD who underwent transthoracic echocardiography for first examination from 1 st January 2001 to 1 st January 2020 in Southwest Hospital, Army Medical University were enrolled. Data were collected from medical records and big data platform of Southwest Hospital. Characteristics of age and gender, etiology and types of VHD were descriptively analysed. Results:A total of 68 354 patients with moderate or severe VHD were enrolled. The age was 63 (50, 72) years. And 35 706 (52.24%) patients were female. (1) Age characteristics: There was similar age trend between male and female patients with moderate or severe VHD. The number of patients increased firstly and then decreased and reached its peak in the age group of 65-69 years old. The peak age of mitral stenosis patients was 45-49 years, which was earlier than that of whole patients with moderate or severe VHD. The median age of patients with bicuspid aortic valve was 42 years. (2) Gender characteristics: The proportion of tricuspid regurgitation, pulmonary regurgitation, mitral regurgitation, mitral stenosis and valve surgery in female patients with moderate or severe VHD were higher than those in male patients. The proportion of aortic regurgitation, aortic stenosis and bicuspid aortic valve in male patients with moderate or severe VHD were significantly higher than those in female patients (all P<0.05). (3) Etiology: The proportion of rheumatic VHD was 13.07% (8 934/68 354), which was higher than that of degenerative VHD (0.67% (458/68 354)). (4) Types of VHD: Tricuspid regurgitation made contribution to the largest proportion with 60.72% (41 503/68 354), followed by mitral regurgitation, aortic regurgitation, mitral stenosis, pulmonary regurgitation and aortic stenosis. Conclusions:There are certain regional characteristics in the prevalence of moderate or severe VHD in southwest China, suggesting different attention should be paid on the whole process of refined management of moderate or severe VHD.
4.Application of family integrated care based on family ward in premature infants with bronchopulmonary dysplasia

Objective To explore the application effect of family integrated care (FIC) based on family ward (FW) on premature infants with bronchopulmonary dysplasia.Methods A total of 171 premature in-fants with bronchopulmonary dysplasia and their parents in the neonatology department of a hospital from March 2022 to March 2023 were selected as the research subjects.According to the wishes of parents,they were divided into three groups:NICU-FIC group,FW-FIC group and no accompanying group.In the NICU-FIC group,the parents entered the centrally managed neonatal intensive care unit to take care of premature in-fants at the bedside.The parents in the FW-FIC group shared a single ward with the premature infants,and participated in the care throughout the day.The parents in the unaccompanied group did not participate in the care of premature infants during hospitalization.The conditions of the three groups of premature infants at discharge and on 30 d after discharge were compared among 3 groups.Results A total of 167 premature in-fants completed the trial.At discharge,the breastfeeding rate,total oxygen days,and total hospitalization days of the NICU-FIC group and FW-FIC group were significantly different from those of the unaccompanied group (P<0.05).However,there was no statistically significant difference between the NICU-FIC group and FW-FIC group(P>0.05).After 30 d of discharge,the breastfeeding rate,weight gain,proportion of home oxygen therapy,and readmission rate of the NICU-FIC group and FW-FIC group were significantly different from those of the unaccompanied group (P<0.05).The breastfeeding rate,weight gain and readmission rate in the FW-FIC group were significantly different from those in the NICU-FIC group (P<0.05).Conclusion The FIC method based on the family ward is consistent with the FIC method based on the open neonatal intensive care unit in promoting the clinical prognosis of premature infants with bronchopulmonary dysplasia,moreover the FIC method based on the family ward has better strengthening effect and out-of-hospital continuity.
5.Efficacy and safety of endoscopic retrograde cholangiopancreatography during pregnancy and puerperium: A comparative analysis

ObjectiveTo investigate the efficacy and safety of endoscopic retrograde cholangiopancreatography (ERCP) during pregnancy and puerperium through a comparative analysis. MethodsA retrospective analysis was performed for the clinical data of 22 patients in pregnancy and 39 patients in puerperium who received ERCP in Digestive Endoscopy Center of The First Affiliated Hospital of Nanchang University from January 2007 to August 2022. The two groups of patients were compared in terms of baseline data, diagnosis during ERCP, interventions, laboratory results before and after ERCP, post-ERCP complications, and delivery and fetal outcomes. The independent-samples t test was used for comparison of continuous data between two groups, and the chi-square test or the Fisher’s exact test was used for comparison of categorical data between two groups. ResultsThere were no significant differences between the patients in pregnancy and those in puerperium in all baseline data (all P>0.05) except the rate of CT examination (22.73% vs 58.97%, χ2=7.44, P=0.006). During the ERCP procedure, compared with the patients in puerperium, the patients in pregnancy had a significantly lower proportion of the patients who underwent biliary stone extraction or endoscopic papillary balloon dilation (χ2=4.007 and 4.315, both P<0.05) and a significantly higher proportion of the patients who did not receive X-ray fluoroscopy (χ2=12.103, P=0.001). After ERCP, both groups had significant improvements in white blood cell count, total bilirubin, direct bilirubin, alanine aminotransferase, and aspartate aminotransferase (all P<0.05). The overall incidence rate of post-ERCP complications was 7.04% (5/71) for all patients, with post-ERCP biliary infection as the most common complication (3/71,4.23%), and there was no significant difference in the incidence rate of post-ERCP complications between the two groups (P>0.05). As for delivery and fetal outcomes, compared with the patients in puerperium, the patients in pregnancy had a significantly higher proportion of patients with early termination of pregnancy (50.00% vs 0,χ2=20.528, P<0.001) and a significantly lower proportion of patients with cesarean section (36.36% vs 76.92%, χ2=4.756, P=0.029). There were no significant differences in the incidence rates of adverse events such as abnormal fetal development and stillbirth between the two groups (P>0.05). ConclusionWith strict control of ERCP indications and timing, ERCP is effective and relatively safe in both pregnancy and puerperium and has a comparable incidence rate of post-ERCP complications to that in the general population, with relatively good delivery and fetal outcomes.
6.Effects of growth hormone combined with triptoreline acetate on growth and sex hormone in girls with central precocious puberty

Objective:To investigate the effects of growth hormone combined with triptoreline acetate on growth and sex hormone in girls with central precocious puberty.Methods:Sixty-two girls with central precocious puberty who received treatment in Xin'an International Hospital from January 2017 to December 2019 were included in this study. They were randomly assigned to receive treatment with either triptorelin acetate (control group, n = 31) or triptorelin acetate plus growth hormone (observation group, n = 31) for 12 successive months. Before and after treatment, bone age difference/chronological age difference (△BA/△CA), body height, body weight, uterine and ovarian volume and sex hormone level were compared between the control and observation groups. Results:△BA/△CA in the observation group was significantly lower than that in the control group [(0.64 ± 0.17) vs. (0.95 ± 0.13), t = 8.065, P < 0.05). Body height and weight in the observation group were (127.32 ± 1.08) cm and (33.42 ± 2.37) kg, respectively, which were significantly higher than those in the control group [(126.34 ± 0.87) cm and (31.01 ± 2.15) kg, t = 3.934 and 4.193, both P < 0.05]. Uterine and ovarian volume in the observation group were (1.68 ± 0.29) cm 3 and (1.26 ± 0.18) cm 3, respectively, which were significantly lower than those in the control group [(2.41 ± 0.46) cm 3 and (1.83 ± 0.26) cm 3, t = 7.474 and 10.036, both P < 0.05). After treatment, there were no significant differences in serum estradiol and luteinizing hormone levels between the two groups (both P > 0.05). Conclusion:Growth hormone combined with triptoreline acetate has a good effect on central precocious puberty in girls because it can improve the growth and development of girls and reduce serum levels of estradiol and luteinizing hormone.
7.Influence of depression on glycemic control in patients with type 2 diabetes mellitus

Objective To investigate the influence of depression on glycemic control in the patients with type 2 diabetes mellitus(T2DM).Methods The Zung self-rating depression scale(SDS) was used to assess depression.A total of 276 cases of T2DM were divided into the group A(SDS standard score ≥53 points) and B(SDS standard score <53points).The levels of HbA1c,FPG,HOMA-IR,etc.were compared between the two groups,and the influencing factors of glycemic control in T2DM patients were analyzed.Results In the patients with T2DM,the SDS standard score was correlated with HbA1c(r=0.26,P<0.05).The multivariate regression analysis showed that the SDS standard score was still correlated with HbA1c (β =0.30,t =5.1,P< 0.05).The HbA1c level in the group A was higher than that in the group B(t=3.685,P<0.05);after correcting the factors of sex,age and education,the HbA1c level in the group A was still higher than that in the group B(F=47.8,P<0.05).Conclusion The depression mood is adverse to glycemic control in T2DM patients.
8.Prospective randomized controlled trial of two ureteroileal anastomosis: split-cuff nipple vs.direct anastomosis

Objective To compare split-cuff nipple and direct ureteroileal anastomosis during ureteroileal anastomosis.Methods Between December,2014 and March,2017,a prospective randomized study was conducted on 70 patients who underwent radical cystectomy and urinary diversion.In every patient,both ureters were randomized to be implanted using an antireflux,split-cuff nipple technique (group A) or a reflux,direct technique (group B).After pelvic lymph node dissection and radical cystectomy,a Mshape orthotopic ileal neobladder was constructed and two ureters were implanted with single-J tubes placed for 10-12 days.For split-cuff nipple technique,a 0.5 cm longitudinal incision in the ureter was made,and the ureteral wall was turned back on itself,construction a nipple.The cuff was stabilized at the corners with sutures.The ureter was then placed into the bowel with 0.5 cm nipple.The ureter was sutured to the full thickness of the bowel wall with interrupted 4-0 PDS.For direct technique,a 0.5 cm incision in the ureter was made,the full thickness of the ureter was sewn to the mucosa of the bowel.Results 70 patients were enrolled in the study,63 males and 7 females,(62.5 ± 10.4) years old.Over a median follow-up of 13.2 months,one patients had bilateral anastomosis stricture 3 months after operation,1 patient in group A had stricture 6 months after operation,2 patients in group B had stricture 6 and 12 months after operation,respectively.Six patients (8.6%) in group A found reflux compared with 21 patients (30.0%) in group B (P =0.004).The reflux pressure was (23.5 ± 9.0) cmH2O and (15.5 ± 4.9) cmH2O in group A and group B (P =0.042),respectively.The GFR of group A was (38.1 ± 7.6) ml/min compared with (38.6 ± 12.9) ml/min in group B at 12 months after operation.One patient in group A and four patients in group B had acute nephropyelitis.Four patients in group A had renal stones formation compared with 1 patients in group B.The time of anastomosis was (8.8 ± 3.5) minutes and (6.7 ± 1.5) minutes (P =0.037) for group A and group B,respectively.The patients in both groups had no urine leakage.Conclusion Compared with direct technique,split-cuff nipple technique had lower reflux rate,higher antireflux pressure and longer anastomosis time than direct technique.
9.Association between HLA-A and HLA-DRB1 allele polymorphisms and susceptibility to tuberculosis in southern Chinese population

Methods The polymorphisms of HLA-A and HLA-DRB1 loci in the PBMCs were analyzed in 294 patients with active tuberculosis using polymerase chain reaction-sequence based typing (PCT-SBT). The allele frequencies in the patients were compared with the data from 644 control southern Chinese subjects obtained from the online database Allele Frequencies in Worldwide Population. Results The frequencies of HLA-A*0101 and HLA-DRB1*1454 alleles in the patient cohort with pulmonary tuberculosis were significantly higher than those in the control group (2.4%vs 0.6%,χ2=10.788, P=0.001, Pc=0.016;7.5%vs 0%,χ2=69.850, P<0.0001);the frequencies of HLA-DRB1*1202 and HLA-DRB1*1401 alleles were significantly lower in this patient cohort than in the control group (10.4%vs 16.1%,χ2=9.845, P=0.002, Pc=0.044;0%vs 3.1%,χ2=18.520, P<0.001). Conclusion The frequencies of HLA-A and HLA-DRB1 alleles are correlated with the susceptibility to active tuberculosis in this southern Chinese population. HLA-A*0101, HLA-DRB1*1454 and the other 3 alleles are likely susceptible genes to tuberculosis, while HLA-DRB1*1202, HLA-DRB1*1401 and the other 4 alleles can be protective genes in this population.
