1.Construction and drug resistance evaluation of cell strains which stably express FIP1L1-PDGFRA protein and its mutants

Objective To construct mouse BaF3-FIP1L1-PDGFRA(F/P),BaF3-F/P-T674I and BAF3-F/P-D842V pre-B cell strains which stably express F/P fusion protein and its T674I and D842V mutants in order to e-valuate their activity by checking their responses to tyrosine kinase inhibitors(TKIs).Methods Lentivirus infected technique was used to transfect the target gene into BaF3 cells.RT-qPCR was used to detect mRNA expression,and CCK-8 method was used to detect the inhibitory effect of TKIs on the proliferation of stable cell strains.Results The constructed BaF3-F/P,BaF3-F/P-T674I and BAF3-F/P-D842V cell strains all transcripted FIP1L1 and PDGFRA mRNA.They exhibited malignant phenotypic characteristics of proliferation independent of IL-3 and sen-sitivity to corresponding TKIs.Conclusions The pre-B-cell strains stably expressing F/P and its T674I and D842V mutants are successfully constructed,which provide a good cell model for the development of compounds targeting at those molecules.
2.Research progress on relationship between cellular senescence and scar fibrosis

Cellular senescence and fibrosis are two biological processes that are closely related to the development of many diseases.Cellular senescence can occur through mechanisms such as telomere shortening,DNA damage,and oxidative stress,leading to degradation of cell function and decreased ability to repair damage.More and more studies have shown that fibrosis and cell senescence are closely related,and cell senescence has been confirmed to be involved in the occurrence and development of scar fibrosis diseases.An in-depth understand-ing of the relationship between cellular senescence and scar fibrosis is helpful to find new therapeutic strategies and develop targeted drugs to reduce the process of scar fibrosis.
3.Observation of the clinical efficacy of external application of piyan formula in treating EGFR-TKIs-related rash

Objective To observe the clinical efficacy and safety of external application of Piyan Formula in treating epidermal growth factor receptor tyrosine kinase inhibitor(EGFR-TKIs)-related rashes.Methods Sixty cases of EGFR-TKIs-related rash patients were randomly allocated into either a treatment group or a control group.The treatment group received external application of Piyan Formula to the rash area twice daily for 14 days.The control group received external application of fucidic acid cream to the rash area twice daily for 14 days.Changes in rash grading,itching grading,quality of life scores and adverse event were observed and recorded in both groups.At the same time,levels of hypersensitive C-reactive protein,interleukin(IL)-6,and IL-1β were measured before treatment and 24 hours after treatment.Results After treatment,the rash severity,itching severity,and quality of life scores were notably lower in the treatment group compared to the control group(P<0.05).The levels of hypersensitive C-reactive protein,IL-6,and IL-1β exhibited a significant decrease compared to their pre-treatment values.(P<0.05).Compared with the control group,the levels of hypersensitive C-reactive protein,IL-6,and IL-1β decreased in the treatment group,with statistically significant differences(P<0.05).No adverse events related to Piyan Formula or fucidic acid cream occurred during the treatment process.Conclusion External application of Piyan Formula in treating EGFR-TKIs-related rashes shows significant clinical efficacy,can effectively reduce the levels of inflammatory factors,and has high safety,thus warranting clinical promotion.
4.Practice and exploration of interdisciplinary codes in standardized management of biomedical engineering graduate students

The standardized use of interdisciplinary code classification system in the field of higher education management is critical to standardize talent cultivation and improve management efficiency. However, the phenomenon of "multiple codes for the same discipline" within interdisciplinary fields has complicated the standardized management of graduate students at colleges and universities. This paper focused on the cross-discipline of biomedical engineering, compared the status quo of the management of interdisciplinary codes in China and the United States of America, and analyzed the rules of cross-discipline management from the perspective of the development of the application of discipline codes. This study examined the causes of the multiple codes in the discipline of biomedical engineering and analyzed the complex situations of these codes in student recruitment, cultivation, and degree conferment. Additionally, the setting and management of biomedical engineering codes were preliminarily discussed. Measures for the improvement of the classification of interdisciplinary codes and new ideas for standardizing talent cultivation were proposed, with the purpose to enhance the capacity construction for medical education management.
5.Familial aggregation of human hookworm infections in Sichuan Province

Objective To investigate the family aggregation of human hookworm infections in Sichuan Province and to identify its influencing factors, so as to provide insights into management of hookworm infections. Methods Three to four counties (districts) were sampled from basins, hilly regions and mountainous regions around the basins in Sichuan Province from 2017 to 2022 as fixed survey sites, and 17 to 30 counties (districts) were selected as mobile survey sites. At least 1 000 permanent residents at ages of 3 years and older were sampled from each survey site, and hookworm eggs were detected in human stool samples using the Kato-Katz technique. Subjects with 2 and more family members and at least 2 individuals diagnosed with hookworm infections in the county (district) where they lived were selected, and the familial aggregation of hookworm infections was analyzed using the test of goodness of fit for binomial distribution. In addition, the knowledge and practice of hookworm disease control were investigated among residents in Hejiang County and Wutongqiao District, Leshan City, Sichuan Province in 2021 and 2022, and the difference in the knowledge and practice of hookworm disease control was compared between members with and without familial aggregation of hookworm infections. Results A total of 66 812 residents from 25 196 households were sampled from main endemic areas of hookworm diseases in Sichuan Province from 2017 to 2022 for detection of hookworm infections, and 4 403 infections were identified (6.59% prevalence). The distribution of human hookworm infections in Sichuan Province did not fit the binomial distribution, and showed family aggregations (χ2 = 2 116.759, P < 0.001). Family aggregation of human hookworm infections was found in endemic areas with 1% and higher prevalence of human hookworm infections (χ2 = 136.006 to 428.738, all P values < 0.001), and family aggregation of human hookworm infections was identified in different years (χ2 = 87.615 to 471.838, all P values < 0.001) and in different terrains of endemic areas (χ2 = 8.423 to 1 144.176, all P values < 0.001). The members with hookworm infections had median eggs per gram of 180 (interquartile range, 780) in aggregated families and 72 (102) in non-aggregated families (Z = −2.686, P < 0.05). The proportion of members in families with aggregation of hookworm infections who knew the preventive measures of hookworm disease was significantly lower than in non-aggregated families (24.49% vs. 51.72%; χ2 = 10.262, P < 0.05), and the proportion of members in families with aggregation of hookworm infections who often worked barefoot on the ground was significantly higher than in non-aggregated families (30.61% vs. 13.25%; χ2 = 6.289, P < 0.05). Conclusions There is a familial aggregation of human hookworm infections in Sichuan Province, and awareness of preventive measures for hookworm disease and frequent working barefoot on the ground are associated with familial aggregation of hookworm infections.
6.Detection of Brucella abortus using an electrochemical immunosensor modified with PB-MWCNTs-GNPs

Objective To prepare a high performance electrochemical immunosensor for detecting Brucella abortus(B.abortus).Methods Prussian blue(PB),multi walled carbon nanotubes(MWCNTs)and gold nanoparticles(GNPs)(PB-MWCNTs-GNPs)nanocomposites were prepared,and appropriate antibody was used to construct the immunosensor for detecting B.abortus samples.The optimal conditions were clarified by examining the key factors in sensor construction,and then the performance of the sensor was evaluated.Results The optimal construction conditions were determined as follows:the ratio of MWCNTs-PB was 1∶5,the drying temperature was 37 ℃,the pH value of buffer system was 7.5,and the incubation time of antibody and sample was 1 h and 30 min,respectively.B.abortus exhibited a good linear relationship,when ranging from 10 to 1 × 105 CFU/mL.The sensor had good anti-interference ability,repeatability,stability and high accuracy.Conclusion Our prepared PB-MWCNTs GNPs nanomaterials modified electrochemical immunosensor for detecting B.abortus is easy to prepare,has good performance,and can provide reference for the early clinical diagnosis of brucellosis.
7.Preoperative assessment of tricuspid valve replacement with LuX-Valve by CT: our initial exploration

Objective:To evaluate the value of CT in preoperative assessment of tricuspid valve replacement with LuX-Valve.Methods:145 consecutive patients with severe tricuspid regurgitation from October 2020 to April 2021 were selected. The multi-slice spiral CT (256 slice and above) scans were used to measure related indicators. Then analyses were made to determine whether the patients were suitable for valves replacement and to select the appropriate valve model.Results:49 cases were excluded after CT measurements, including 8 cases because of the values of the valve annulus and the remaining 41 due to comprehensive indicators includes: valve annulus size and shape, fixation method and operational safety. A total of 96 TTVR operations were performed, of which 6 were treated with thoracic surgery, 1 had moderate valve regurgitation, and the rest had no or mild regurgitation or paravalvular leaks. The operations showed a high success rate of 92.7%. After comprehensive analysis, it was found that among the 89 successful tricuspid valve replacements with LuX-Valve, 26 cases had annulus models consistent with the measurements of the diameter from tricuspid annular circumferences, while the remaining 63 had valve models larger than the diameter measurements. The results indicate that the size of right atrium was statistically significant to the enlargement of the valve size.Conclusion:CT has important application value in the preoperative screening of TTVR. For non-radial force LuX-Valve, the annulus size is not the only decisive factor, and the valve model should be comprehensively judged considering all the measurement indicators.
8.Long-term efficacy of endoscopic transluminal drainage for acute pancreatitis complicated with walled-off necrosis or pancreatic pseudocyst

Objective:To evaluate the long-term efficacy of endoscopic transluminal drainage(ETD) for acute pancreatitis complicated with walled-off necrosis (WON) or pancreatic pseudocyst (PPC).Methods:A total of 79 patients who were diagnosed as having WON or PPC by abdominal CT or ultrasound and treated with ETD in Nanjing Drum Tower Hospital were enrolled. Past medical records and follow-up by phone call after discharge were analyzed for long-term outcomes including endocrine and exocrine functions and long-term quality of life.Results:A total of 50 patients were enrolled, including 31 patients with infected WON/PPC and 19 patients with uninfected WON/PPC. Seventeen patients (54.84%) in the infected WON/PPC group and 11 patients (57.89%) in the uninfected WON/PPC group lost 5% or more of their weight. There were no significant differences in the proportion of cases of weight loss of 5% or more ( P=0.833), or the weight loss between the two groups (12.59±8.89 kg VS 10.91±2.47 kg, P=0.522). Only one patient in the infected WON/PPC group had chronic abdominal pain. There was no significant difference in the Izbicki score between the two groups (23.79±6.74 VS 22.03±3.21, P=0.295). None of the patients developed steatorrhea after discharge. Five patients (16.67%, 5/30) in the infected WON/PPC group and 6 patients (40.00%, 6/15) in the uninfected WON/PPC group developed endocrine insufficiency with no significant difference ( P=0.140). Greater risk of secondary diabetes resulted from higher low-density lipoprotein cholesterol ( HR=1.9, 95% CI: 1.0-3.4, P=0.044)and triglycerides ( HR=1.2, 95% CI: 1.0-1.3, P =0.029). Conclusion:ETD is safe and effective for WON and PPC. But there is possibility that patients develop secondary diabetes. Additionally, greater risk of secondary diabetes results from higher low-density lipoprotein cholesterol and triglycerides.
9.3D printing of bioinspired compartmentalized capsular structure for controlled drug release.

Drug delivery with customized combinations of drugs, controllable drug dosage, and on-demand release kinetics is critical for personalized medicine. In this study, inspired by successive opening of layered structures and compartmentalized structures in plants, we designed a multiple compartmentalized capsular structure for controlled drug delivery. The structure was designed as a series of compartments, defined by the gradient thickness of their external walls and internal divisions. Based on the careful choice and optimization of bioinks composed of gelatin, starch, and alginate, the capsular structures were successfully manufactured by fused deposition modeling three-dimensional (3D) printing. The capsules showed fusion and firm contact between printed layers, forming complete structures without significant defects on the external walls and internal joints. Internal cavities with different volumes were achieved for different drug loading as designed. In vitro swelling demonstrated a successive dissolving and opening of external walls of different capsule compartments, allowing successive drug pulses from the capsules, resulting in the sustained release for about 410 min. The drug release was significantly prolonged compared to a single burst release from a traditional capsular design. The bioinspired design and manufacture of multiple compartmentalized capsules enable customized drug release in a controllable fashion with combinations of different drugs, drug doses, and release kinetics, and have potential for use in personalized medicine.
10.Comparison of the efficacy and safety of endoscopic submucosal excavation and full-thickness resection for intraluminal gastric stromal tumors

Objective:To compare the efficacy and safety of endoscopic submucosal excavation (ESE) and endoscopic full-thickness resection (EFR) for intraluminal gastric stromal tumors.Methods:Data of 441 patients diagnosed as having gastric stromal tumors in Nanjing Drum Tower Hospital from June 2009 to June 2020 were retrospectively analyzed. A total of 241 patients underwent ESE (ESE group) and 200 EFR (EFR group). Epidemiological data (gender, age and body mass index), tumor size, procedure related parameters, complications, hospital stay, cost and follow-up were compared between the two groups.Results:There were no significant differences between the two groups in gender, age, body mass index, tumor size, National Institutes of Health risk grade, complete resection rate, or block resection rate ( P>0.05). Compared with the EFR group, the ESE group required less titanium clips to close defects in the stomach wall [6.0 (4.0, 6.0) VS 6.0 (5.0, 8.0), U=18 424.0. P<0.001], shorter time of first postoperative fluid intake [2.0 (1.0, 2.0) days VS 2.0 (2.0, 3.0) days, U=17 420.0, P<0.001] and hospital stay [6.0 (5.0, 8.0) days VS 7.0 (6.0, 9.0) days, U=18 906.0, P<0.001], and lower total cost [18.9 (16.4, 21.4) thousand yuan VS 20.9 (18.1, 23.8) thousand yuan, U=17 956.0, P<0.001]. Moreover, the total incidence of complications of the ESE group was lower than that of the EFR group [5.8% (14/241) VS 11.5% (23/200), χ2=4.605, P=0.032]. Patients were followed up with the median period of 45.0 months. The disease recurrence rate was 0.45% (2/441), and there were no disease-related deaths. Conclusion:The efficacy is comparable between ESE and EFR for treating intraluminal gastric stromal tumors, but ESE shows a lower incidence of complications, and requires a shorter hospital stay and lower cost.
