1.Reliability and validity test of Decisional Fatigue Scale in medical staffs

Objective:To translate and culturally adjust Decisional Fatigue Scale (DFS) , and test its reliability and validity.Methods:The Chinese version of DFS was formed through the method of double translation, back translation and expert consultation. Using the convenient sampling method, 247 medical staffs from 16 departments in 76 wards of Xiangyang No.1 People's Hospital Affiliated to Hubei University of Medicine were selected as the research objects. Content validity, criterion validity and structure validity were used for validity evaluation, and internal consistency reliability, split half reliability and retest reliability were used for reliability evaluation. A total of 247 questionnaires were sent out in this study and 247 were effectively received, with effective recovery of 100.00%.Results:The total Cronbach's α coefficient of Chinese DFS was 0.933, the half-reliability coefficient was 0.849 and the retest reliability coefficient was 0.838 ( P<0.01) . Both the item content validity index and scale content validity index were 1.00, and the correlation coefficient with the total score of SRF-S was 0.729 ( P<0.01) . One common factor was extracted by exploratory factor analysis and 65.64% of the variation was explained cumulatively, which was consistent with the dimensions of the original scale. Confirmatory factor analysis showed that all fitting indexes were up to standard and the fitting degree was good. Conclusions:The reliability and validity of the Chinese version of DFS in medical staffs is good, which can be used to evaluate the degree of decision-making fatigue of Chinese medical staffs.
2.Establishment of a RT-PCR assay for detection of mRNA expression of KIR2DS1 gene on NK cell surface

Objective:
To establish a real-time PCR (RT-PCR) assay for detecting mRNA expression of killer cell immunoglobulin-like receptor (KIR) 2DS1 gene( KIR2DS1 ) on the surface of natural killer (NK) cells, and evaluate its performance.
Methods:
A total of 57 recipient-donor pairs of allogeneic hematopoietic stem cell transplantation (Allo-HSCT) were enrolled in this study. The specific primers and probe of KIR2DS1 gene were designed for Taqman-MGB fluorescence quantitative PCR detection system. The performance parameters of the detecting system, such as coincidence rate, repeatability, sensitivity, scope of application of the instrument and reproducibility of operation technicians were evaluated and validated.
Results:
The KIR-SSO Genotyping Test was used as the gold standard. The results of 35 samples showed the accuracies of self-built method were all 100% for both of positive and negative KIR2DS1 . Three samples with high, median and low value of Ct values were used to verify the repeatability. The coefficients of variation of intra-assay and inter-assay were ranged from 0.09% to 0.46% and 0.71% to 1.13% respectively. The sensitivity of the established method was up to 10 2 copies/μL at least. The coefficients of variation of the three samples with sensitivity of 10 2 copies/μL were 5.37%, 2.71% and 5.51% in five repeated tests respectively. The regression analysis for the samples measured by ABI-7500 and LC-480 fluorescence quantitative PCR instrument showed regression equation was Y=0.973 6X+0.118 3 (R 2 =0.961 9, R 2 >0.95). The reproducibility of 10 samples with positive KIR2DS1 operated by two technicians showed that the biases were all less than ±5%.
Conclusion
A TaqMan-MGB real-time PCR assay for detection of mRNA expression of KIR2DS1 gene was established successfully with fine performance.
3.An investigation of reproductive health and related influencing factors in female staff in six industries in seven provinces in China

Objective To investigate the reproductive health status in female staff,and to develop labor protection strategies for female staff.Methods From May to November,2015,cluster sampling was performed in the industries of medicine and health,petrochemical engineering,metallurgy,railway,mechanical manufacturing,and electronics in seven provinces (municipalities directly under the Central Government and autonomous regions).A questionnaire surveywas performed for female staff,and the investigation covered general information,occupational history andexposure to occupational hazards,and reproductive health and childbearing history.Results A total of 14 614 usable questionnaires were collected,resulting in a response rate of 96.00%.The meanage of all female staff was 36 years.Of all female staff,80.06% were married and 40.84% were exposed to hazardous factors;28.14% had abnormal menstruation,and the female staff in theelectronics and railway industries had higher rates of abnormal menstruation (32.28% and 32.11%,respectively).The overall rate of abnormal gynecological examination results was 18.29%,and the female staff in therailway industry had the highest rate (29.49%).Of all female staff,49.01% reported gynecological diseases in the past,among which hyperplasia of mammary glands accounted for 27.11% and inflammation in the reproductive system accounted for 36.31%.The medical staff had higher rates of abnormal menstruation (33.11%) and abnormal gynecological examination results (27.46%) compared with the staff engaged in other occupations.With normal sexual life and no contraceptive measures,the 1-,2-,and 3-year infertility rates were 24.26%,11.88%,and 8.88%,respectively.Conclusion The female staff in the industries of medicine and health,railway,and electronics have serious reproductive health issues.Labor protection should be enhanced and working environment should be improved.

5.The evaluation of some nutritional indexes in nutritional status of patients with maintenance hemodialysis

Objective To evaluate the diagnostic value of serum albumin (Alb),total protein (TP),body mass index (BMI),Mini Nutritional Assessment (MNA) and hand strength in the nutritional status in maintenance hemodialysis (MHD) patients.Methods A total of 126 MHD patients were included in this study who had been on MHD for at least 3 months.Depending on the levels of Alb,patients were divided into two groups:normal nutrition group (group A) and malnutrition group (group B).TP,BMI,MNA and hand strength were also detected at the same time.Independent samples t test,Spearman correlation analysis,ROC curve were used to analyze their difference between the two groups and evaluate their diagnostic value in nutritional status in MHD patients.Results Age,sex,height,weight and dialysis ages had no statistical significant difference (P > 0.05) between group A and group B,while Alb,TP,BMI,MNA and hand strength had statistical significant difference (P < 0.05) between two groups.After adjusting for age,sex and hemodialysis age,Alb,TP,BMI,MNA and hand strength were positively correlated with each other (P < 0.05).Since the area under the ROC curve of BMI was the smallest,BMI had the lowest diagnostic value in evaluation of the nutritional status in those patients.Conclusions Alb,TP,MNA and hand strength are good indexes in evaluation of the nutritional status in MHD patients but BMI is not.
6.Application of conventional ultrasound and contrast-enhanced ultrasonography in comparison of the two different artery remodling techniques for kidney transplantation

ObjectiveTo assess long-term effect of two different types of arterial reanastomoses for kidney transplantation [an end-to-side anastomoses (ESA) of renal artery to external iliac artery and an endto-end anastomoses (EEA) of renal artery to internal iliac artery] by comparing conventional ultrasound and contrast-enhanced ultrasonography(CEUS) after five years of renal transplantation.MethodsSixty patients underwent kidney transplatation were included and divided into EEA group or ESA group based on the type of anastomoses. Data of renal blood flow perfusion,hemodynamic parameters,and renal anatomical structure were collected and analyzed in each groups using CEUS,color Doppler flow imaging(CDFI) and B-mode ultrasound.ResultsThe patients of the two arterial reanastomoses had no significantly difference in B-mode ultrasound and CDFI. The ESA patients had a slightly better perfusion than the EEA ones in parameters of CEUS.Conclusions Transplant tissue perfusion imaging was performed by CEUS. The perfusion of ESA grafts was slightly better than that of EEA.
7.Initial application of virtual touch tissue quantification in diagnosis of chronic allograft nephropathy

Objective To investigate the feasibility of virtual touch tissue quantification(VTQ) for the assessment of chronic allograft nephropathy(CAN).Methods 48 patients with normal renal function and 50 patients with CAN were checked by color Doppler and VTQ technique.All the results were compared between two groups.Results Mean VTQ-values were significantly different between the two groups( P < 0.05).ROC curve displayed that VTQ value of 2.51 m/s could be used to diagnose CAN,the sensitivity,specificity,positive predictive value and negative predictive value were 76.0%,52.1%,60.3% and 65.7%,respectively.The diagnostic efficiency was 62.2%.Conclusions Parenchymal stiffness measured by VTQ is stable and repeatable,which can be used to diagnose patients with CAN and to monitor renal function.
8.Twenty-year experience in the diagnosis and treatment of hepatic trauma

Objective To summarize the experience in the diagnosis and treatment of hepatic trauma. Methods The clinical data of 260 patients with hepatic trauma admitted from January 1988 to December 2007 were retrospectively reviewed with regard to degree of trauma, treatment methods, therapeutical effects, complications and SO on. Results One hundred and fifty-three eases were treated by operative management,1 07 cases by nonoperative management.236 cases were cured,24 cases died,and the case fatality rate was 10.2%.There were no death among 139 patients with hepatic trauma grades Ⅰ~Ⅱ,22 death among 119 grades Ⅲ~Ⅴ patients, all death of 2 in grade VI, which demonstrated the correlation between death and hepatic trauma grade was statistically significant. Complications appeared in 82 eases, mainly including Secondary hemorrhage, abdominal infection and so on. Conclusions Nonoperative management is suitable for hepatic trauma with stable hemodynamics. Operative management is rapidly selected when the hemodynamics aren't stable. The cooperation of many specialities can enlarge the application of nonoperative management and decrease complications.
9.Evaluation of blood flow perfusion of different function transplanted kidney with conventional ultrasound and CEUS

Objective To compare blood flow perfusion conditions of renal allograft of different renal function with conventional ultrasound and contrast enhanced ultrasonography (CEUS). Methods Sixty patients underwent kidney transplatation were divided into normal group (SCr≤110 μmol/L) and abnormal group (SCr>110 μmol/L). Renal structure and inner diameter of renal artery were messured with two-dimensional ultrasound. The hemodynamic parameters (PSV, EDV and RI) were messured with CDFI. Data of renal blood flow perfusion (AT, TTP, △I and Increase), data of renal blood flow perfusion (AT, TTP, △I and Increase) were quantitatively analyzed with CEUS combined with time intensity curve. All the data were compared between two groups.Results There was no significantly difference of above indexes for B-mode ultrasound and CDFI between two groups, except EDV of arcuate arteries. CEUS parameters of patients with normal renal function were better than those with abnormal function.Conclusion Microcirculation perfusion changes of transplanted kidney with abnormal function could be detected with CEUS. The quantitative indexes of transplanted kidney with normal function were better than those with abnormal function.
10.Color Doppler ultrasound in the assessment of the breast mass

Objective The aim of the study was to determine the diagnostic value of color Doppler ultrasound in the assessment of the breast mass.Methods The results of ultrasonograph of the breast mass were retrospectively observed in 79 operation and pathological proven breast mass parients,in which 47 patients had proven malignant breast mass and 32 patients had proven benign malignant breast mass.Results Color Doppler ultrasound had a sensitivity of 100%for diagnosing breast mass,accuracy of 93.7%(30/32)and 91.4%(43/47)for diagnosing malignant and benign mass.Conclusion Color Doppler ultrasound has high diagnostie value in distinguishing malignant from benign mass.
