1.Multi -database analysis and experimental study on the expression of HNF4A in gastric cancer

Objective:
To investigate the expression,prognosis and biological role of hepatocyte nuclear factor 4A (HNF4A) in gastric cancer,and to study its effect on the proliferation of gastric cancer cells.
Methods:
Tumor Immune Estimation Resource 2. 0 (TIMER2. 0) and Gene Expression Profiling Interactive Analysis ( GEPIA2) databases were used to analyze the relative expression levels of HNF4A in gastric cancer and normal tissue,KM Plotter was used to analyze the correlation between the expression level of HNF4A and the survival rate of gastric cancer patients,TISIDB database and R language (4. 1. 2) were used to analyze whether HNF4A was involved in the immune regulation process of gastric cancer.cBioPortal database was used to analyze the mutations of HNF4A in gastric cancer,GSEA 4. 2 was used to analyze the functional enrichment of HNF4A,and LinkedOmics database was used to predict the genes that might be regulated by HNF4A.The relative expression of HNF4A in gastric cancer and adjacent tissues was detected by qRT-PCR , Western blot and immunohistochemistry (IHC) .The proliferation and cell cycle of gastric cancer cells were analyzed by CCK-8,EdU,colony forming assay and flow cytometry.
Results :
The expression of HNF4A increased in gastric cancer tissues (P <0. 05) ,and the overall survival rate of gastric cancer patients with high HNF4A expression was worse (P<0. 001) .HNF4A was mainly missense mutated in gastric cancer.Immune cell infiltration showed that HNF4A was associated with B lymphocytes,CD8 + T cells, neutrophils,macrophages and dendritic cells ( all P <0. 001) . HNF4A was also associated with tumor mutation burden (r = 0. 28,P<0. 0001) and microsatellite instability (r = 0. 13,P<0. 01) .After knockdown of HNF4A, cell proliferation ability was significantly inhibited ,and cell cycle was arrested at G0 / G1 phase.
Conclusion
HNF4A expression significantly increased in gastric cancer tissues,which is associated with poor prognosis ,and may also be involved in immune regulation. Knockdown of HNF4A can inhibit the proliferation of gastric cancer cells.
2. Vitamin D nutritional status and its influencing factors in patients with smear-positive pulmonary tuberculosis in Shenzhen

Objective:
To determine the levels of vitamin D in patients with pulmonary tuberculosis in Shenzhen and identify the influencing factors of vitamin D levels and key groups of vitamin D deficiency, so as to provide a scientific basis for tuberculosis- and nutrition-related health education and promotion in Shenzhen.
Methods:
Patients with smear-positive pulmonary tuberculosis who were diagnosed in 2016 were selected as the research subjects. Their relevant information and blood samples were collected, and the sample pool was established according to the inclusion criteria. One hundred and twenty patients were selected based on simple random sampling, including 84 men (70.0%) and 36 women (30.0%). Blood 25-hydroxyvitamin D [25(OH)D] concentrations were measured using chemiluminescence technology. Vitamin D statuses in patients were statistically described, and vitamin D levels in patients with different characteristics were compared. Multivariate linear regression analysis was performed to identify important factors influencing vitamin D levels in patients.
Results:
Mean serum concentration of 25(OH)D in 120 patients was (40.2±16.0) nmol/L. There were 2 cases of vitamin D sufficiency (1.7%), 28 cases of vitamin D insufficiency (23.3%), and 90 cases of vitamin D deficiency (75.0%), of which 23 cases (19.2%) were of severe deficiency. 25(OH)D concentrations in patients with different lifestyles (indoors; indistinguishable indoors or outdoors; outdoors) were significantly different (35.3 nmol/L

Reactive oxygen species(ROS)and immune response play critical roles in the progression of liver dis-eases.DJ-1,also known as Parkinson disease 7(Park7),is extensively expressed in cells and tissues,where it governs numerous biological functions including chaperone activity,protease function,tran-scriptional and mitochondrial regulation,and ROS modulation.Moreover,we have established that DJ-1 plays a critical role in initiating an inflammatory response by modulating ROS generation.Therefore,DJ-1 may play an important role in the progression of liver diseases by modulating ROS and the immune response.Recently,we have shown that DJ-1 deficiency negatively regulates proliferation of hepatic progenitor cells(HPCs)by impairing the formation of HPC-associated fibrosis and inflammatory niches.Deficiency of DJ-1 ameliorates liver fibrosis by inhibiting hepatic ROS production and inflammation;moreover,in a classic diethylnitrosamine(DEN)-mediated hepatocellular carcinoma(HCC)mouse model,deletion of DJ-1 ameliorates tumorigenesis and HCC cell proliferation by regulating hepatic inflammation and reducing the activity of the interleukin 6/signal transducer and activator of transcription 3(IL-6/STAT3)signaling pathway.Taken together,these data suggest a critical function for,and therapeutic value of,DJ-1 in treatment of liver diseases.The aim of this review is to summarize these functions and the underlying molecular mechanisms of DJ-1 in liver diseases,and to highlight the potential therapeutic value and future research direction of DJ-1 in liver diseases.
4.Clinicopathological characteristics of adenoid cystic carcinoma of the breast: an analysis of 3 cases

Objective To explore the clinicopathological features and diagnosis of adenoid cystic carci noma of the breast (ACCB).Methods Clinical data of 3 cases of ACCB were collected and followed up to observe the clinical pathological features.Immunohistochemistry was used to determine the immune phenotype.Results All the 3 cases were female,aging from 55 to 72 years old (61 years old as the average).The tumors were found in subareolar region well-circumscribed.ACCB was characterized by the presence of a dual cell population of luminal and basaloid cells arranged in cribriform and tubular-trabecular patterns.On immunohistochemical staining,the myoepithelial cells expressed SMA,CK5/6,P63,CD117 and the glandular epithelium cells were mostly positive for CK18,EMA,CK7,while no expression of ER,PR,HER-2,CD10 was observed.Conclusions ACCB is a rare neoplasm with a triple-negative,basal-like phenotype,but exhibits an indolent clinical behavior.The diagnosis can be worked out by evaluation of clinical characterstics,histological and immunohistochemical features.
5.Furazolidone combined with omeprazole and levofloxacin versus tinidazole combined with omeprazole and levofloxacin in eradication rate of helicobacter pylori in elderly patients with Hp positive chronic atrophic gastritis

Objective To compare the eradication rate of helicobacter pylori between furazolidone combined with omeprazole and levofloxacin and tinidazole combined with omeprazole and levofloxacin in elderly patients with Hp positive chronic atrophic gastritis.Methods A total of 96 patients with Hp positive chronic atrophic gastritis were randomly divided into two groups,48 cases in each group.Control group was treated with tinidazole plus omeprazole and levofloxacin,while study group was treated with furazolidone plus with omeprazole and levofloxacin.The gastric function,inflammatory factors and immune function indexes were measured,and the clinical efficacy and the eradication rate of Hp were compared.Results After treatment,symptoms and pathological scores significantly decreased in both groups,G-17,PG-I contents increased significantly,PG-II content decreased significantly,serum IL-6,TNF-α,sIL-2R contents decreased significantly,CD4+ and CD4+/CD8+ contents increased significantly,and CD8+ content decreased significantly(P<0.05).There were significant differences in indexes mentioned above between the two groups(P<0.05).The effective rate and negative rate of Hp in the control group were significantly lower than those in the study group(P<0.05).Conclusion Furazolidone combined with omeprazole and levofloxacin

7.Application of narrow band imaging combined with lugol chromo-endoscopy in the diagnosis of advanced esophageal carcinoma

Objective To investigate the value of narrow band imaging (NBI) and lugol chromo-endoscopy (LCE) in the diagnosis of advanced esophageal carcinoma.Methods The clinical data of 162 patients with advanced esophageal carcinoma who received NBI and LCE at the Cancer Hospital of Peking University from November 2010 to May 2012 were retrospectively analyzed.Esophageal mucosa was first examined using white light imaging (WLI),and then followed by NBI and LCE,and the lengths of the lesions were recorded.Biopsy histology was obtained in all abnormal mucosa which were detected by NBI or LCE.Difference in the length of lesions detected by the NBI/LCE and WLI was calculated.Surgical approach and method of anastomosis were recorded for patients who received surgical treatment,and the final treatment method was recorded for patients who did not receive surgical treatment.Difference in the treatment methods was compared before and after endoscopy.Results The length of the lesions detected by the 3 methods was identical in 121 patients,different in 41 patients.The difference ranged between 1 and 3 cm was observed in 22 patients,>3 cm and ≤5 cm in 8 patients,>5 cm and ≤10 cm in 7 patients,> 10 cm in 4 patients.Of the patients in the above mentioned 4 categories,there were 1,2,2,4 patients in each category received neo-adjuvant therapy,and the rest patients received operation.Superficial cancer contiguous to the primary lesion was found in 41 patients,including squamous cell carcinoma in 31 patients,carcinoma-in-situ in 3 patients and severe dysplasia in 7 patients.Of the 153 patients who received surgery,the surgical plan for 12 patients was modified.Intrathoracic anastomosis was changed to cervical anastomosis in 2 patients,anastomosis under the aortic arch was changed to anastomosis above the aortic arch in 3 patients,trans-abdominal operation was changed to thoraco-abdominal operation in 7 patients.Conclusions The combination of NBI and LCE is more accurate to evaluate the extent of lesions of advanced esophageal carcinoma,and is useful to decide the treatment protocol.Pathological examination of the adjacent abnormal mucosa should be carried out for patients whose lesion length was inconsistent under different observation methods.
8.Effect of bacailin on the angiogenic regulating factor and migration of laryngeal cancer cells

Objective To explore the effects of bacailin on the angiogenic regulating factor and migration activity of laryngeal cancer cells.Methods Hep-2 was cultured in vitro,(1)in the groups adding phorbol ester,gelatin-zymogram and Transwell assay were used to observe the effects of bacailin on the gelatinase activity of matrix metalloproteinase (MMP) and the migration ability of Hep-2,respectively; (2)in the group without phorbol ester,enzyme-linked immunosorbent assay (ELISA) was used to detect the effects of bacailin on the protein level of endothelial growth factor (VEGF) and basic fibroblast growth factor (bFGF).Results Among the control group,the low-dose group,the middle-dose group,and the high-dose group,the activity of MMP-3 of each corresponding group was (100.0 ±0.0) %,(95.0 ±4.9) %,(93.1± 6.2)%,and (90.7 ± 7.3)% with statistically significant difference (t =2.123,2.309,2.412,P <0.05) ; the activity of MMP-9 of each corresponding group was (100.0 ± 0.0) %,(92.3 ± 7.5) %,(89.5 ± 9.3) %,and (85.6 ± 6.1) % with statistically significant difference (t =2.253,2.302,3.708,P <0.05 or P <0.01) ;the protein level of VEGF of each corresponding group was (242.7 ±9.5)ng/L,(230.6 ± 12.7)ng/L,(212.1 ± 15.9)ng/L,and (184.2 ± 23.5)ng/L with statistically significant difference (t =2.408,3.733,4.146,P <0.05 or P <0.01) ; the protein level of bFGF of each corresponding group was (190.7±8.2) ng/L,(181.2±13.0) ng/L,(169.9±17.3)ng/L,and (140.7±9.2)ng/Lwith statistically significant difference (t =2.330,2.922,5.145,P <0.05 or P <0.01) ; Cellular migration rate of each corresponding group was (38.3 ± 5.6) %,(32.9 ± 7.1) %,(27.9 ± 4.4) %,and (23.3 ± 6.0) % with statistically significant difference (t =2.141,3.365,4.027,P < 0.05 or P <0.01).Conclusions Bacailin could suppress the migration of Hep-2 through inhibiting expression of angiogenic regulating factor,which might has clinical value in the treatment of laryngeal cancer.
9.A retrospective study of 512 malignant esophageal strictures treated with covered self-expanding metal stent

Objective To investigate the clinical efficacy of covered self-expanding metal stent in the treatment of malignant esophageal stricture.Methods The clinical data of 512 patients accepted stent were retrospectively analyzed,which included patients' profiles,relief of dysphagia,incidence of complications and survival period.The data were analyzed by t test.Results A total of 596 stents were placed in 512 patients.There was significant difference in mean dysphagia score between before and after stent placement (grade 3.4±0.6 vs grade 1.1±0.8,t=54.09,P<0.01).After the placement,the overall incidence of complications was 14.5% (74/512),of which there were 21 cases of severe retrosternal pain (4.1%),16 cases of bleeding (3.1 %),29 cases of stent displacement (5.7%),six cases of food impaction (1.2%),two cases of fistula without completely blocked (0.4%).The survival period of 238 patients with complete follow-up ranged from 30 to 1445 days,the mean time was 218.9 days.Conclusion Endoscopic covered self-expanding metal stent placement may effectively relieve dysphagia symptoms in patients with malignant esophageal stricture.
10.Clinical Observation on Trigeminal Neuralgia Treated by Acupuncture plus Electrophoresis

Objective: To evaluate the clinical effect of combined acupuncture and electrophoresis in treating trigeminal neuralgia. Methods: Sixty patients were randomly allocated to two groups. The treatment group received acupuncture plus electrophoresis, and the control group took Carbamazepine and Methylcobalamin. The therapeutic effects would be evaluated after 30 d. Results: The total effective rate was 93.3% in the treatment group versus 70.0% in the control group. During the follow-up study (6 months to 1 year), 3 cases got relapsed in the treatment group, occupying 11.1%, and 6 got relapsed in the control group, occupying 28.6%. Conclusion: Combined acupuncture and electrophoresis has a precise effect in treating trigeminal neuralgia, and it bears an advantage of simple operation and maintaining cutaneous sensation.
