1.Short-term efficacy of oblique lateral interbody fusion combined with lateral plate fixation in treatment of single-level lumbar degenerative disease

		                        			BACKGROUND:Stand-alone oblique lateral interbody fusion has a high rate of complications of fusion segment sink.Oblique lateral interbody fusion with posterior fixation can provide stable support,but intraoperative position changes and double incisions weaken the advantages of this technique.Oblique lateral interbody fusion combined with lateral plate fixation can achieve one-stage decompression in the same incision,while the lateral internal fixation provides stable support. OBJECTIVE:To analyze the short-term efficacy of oblique lateral interbody fusion combined with lateral plate fixation in the treatment of single-level lumbar degenerative disease. METHODS:The clinical data of 34 patients with single-level lumbar degenerative disease treated with oblique lateral interbody fusion combined with lateral plate fixation were collected from May 2020 to October 2022.Among them,14 were males and 20 were females aged from 41 to 72 years at the mean age of(58.6±9.9)years.There were 11 cases of lumbar spondylolisthesis(Ⅰ°),7 cases of lumbar disc herniation with segmental instability,and 16 cases of lumbar spinal stenosis.Operation time,blood loss,and complications were recorded.Visual analog scale scores of lumbago,radiative pain of both lower limbs,and Oswestry disability index scores were evaluated before surgery,3 months after surgery,and the last follow-up.Dural sac cross-sectional area,intervertebral height,and intervertebral fusion were measured and observed. RESULTS AND CONCLUSION:(1)The 34 patients were followed up for 14-36 months,with an average of(21.3±5.2)months.(2)The operation time ranged from 50 to 92 minutes,with an average of(68.5±11.1)minutes.Intraoperative blood loss was 50-170 mL,with an average of(71.6±25.3)mL.(3)Compared with the preoperative results,the visual analog scale scores and Oswestry disability index scores were significantly decreased at 3 months after surgery and at the last follow-up(P<0.001),and the maximum Oswestry disability index scores were improved by nearly 50%.(4)Bone fusion was achieved in all patients during half-year follow-up.The overall complication rate was 21%(7/34),including 1 case of plate displacement,3 cases of cage subsidence,1 case of psoas weakness,and 2 cases of anterior thigh pain.(5)It is concluded that oblique lateral interbody fusion combined with lateral plate fixation for the treatment of lumbar degenerative diseases has the characteristics of less blood loss,short operation time,rapid postoperative recovery,and significant short-term clinical efficacy with the stable support to a certain extent.The long-term curative effect needs further follow-up observation.
		                        		
		                        		
		                        		
		                        	
2.In vivo and in Vitro Component Identification and Pharmacokinetic Analysis of Houpo Wenzhongtang Based on Rats with Deficiency-cold of Spleen and Stomach

		                        			ObjectiveTo identify the chemical components of Houpo Wenzhongtang in vivo and in vitro and to analyze the pharmacokinetic properties of the index components in rats with deficiency-cold of spleen and stomach. MethodThe chemical components of Houpo Wenzhongtang was analyzed and identified by ultra performance liquid chromatography-quadrupole-time-of-flight tandem mass spectrometry(UPLC-Q-TOF-MS/MS). Six rats were randomly selected from 18 SD rats as the blank group, and the remaining rats were given lard and cold vinegar for a long time to construct a rat model with deficiency-cold of spleen and stomach. After successful modeling, the rats were randomly divided into the model group and Houpu Wenzhongtang group(13.5 g·kg-1, calculated as crude drug). The administration group was given the corresponding dose of Houpu Wenzhongtang by gavage, and the blank group and the model group were given the same amount of distilled water by gavage. Enzyme-linked immunosorbent assay(ELISA) were used to measure gastrin(GAS) and motilin(MTL) levels in each group. At the same time, plasma samples were collected at different time points after administration, and blood-entry prototype components and metabolites of Houpo Wenzhongtang were analyzed by UPLC-Q-TOF-MS/MS. On this basis, plasma concentrations of magnolol, honokiol, alpinetin and hesperidin in Houpo Wenzhongtang were determined by ultra performance liquid chromatography coupled with triple quadrupole/linear ion trap mass spectrometry(UPLC-QTRAP-MS/MS), and the pharmacokinetic parameters were calculated using DAS 2.0 software. ResultA total of 79 chemical components, including 44 flavonoids and 11 lignans, were identified in Houpo Wenzhongtang. Meanwhile, 18 blood-entry prototype components and 27 metabolites were identified, the main metabolic pathways of metabolites were glucuronidation, sulfation, oxidation and hydrolysis, and phase Ⅰ and phase Ⅱ were the two primary forms of metabolism. Pharmacokinetic results showed that among the four index components, the time to peak(tmax) values of magnolol and honokiol were consistent and exhibited similar drug metabolism characteristics, the tmax of alpinetin was the shortest, and the absorption rate was the fastest, which had the earliest peak plasma concentration levels, and hesperidin had the shortest mean residence time(MRT0-t) and the highest metabolic rate in rats. ConclusionThis study clarifies the blood-entry prototype components and their metabolites of Houpo Wenzhongtang in the rat model of deficiency-cold of spleen and stomach, and reveals the pharmacokinetic characteristics of the main active ingredients, which can provide a scientific basis for the study of pharmacodynamic material basis of this formula and its clinical application in treating the syndrome of deficiency-cold of spleen and stomach. 
		                        		
		                        		
		                        		
		                        	
3.Biomechanical Evaluation of Oblique Lateral Interbody Fusion Combined with Different Internal Fixation Methods for Treating Degenerative Lumbar Scoliosis:A Finite Element Analysis

		                        			 Objective To verify the biomechanical stability of oblique lateral interbody fusion ( OLIF) combinedwith different fixation methods for treating degenerative lumbar scoliosis (DLS) by three-dimensional (3D) finite element analysis. Methods The L1-S1 3D finite element DLS model ( Model 1) was established, and then the OLIF (L2-5) at 3 contiguous levels of fusion and its combination with different internal fixation methods were simulated, namely, stand-alone OLIF model ( Model 2), vertebral screw fixation model ( Model 3), unilateral pedicle screw fixation model (Model 4) and bilateral pedicle screw fixation model (Model 5) were established,respectively. Under upright, flexion, extension, lateral bending and axial rotation states, range of motion (ROM) of fusion segments, as well as cage stress, internal fixation stress, and stress distribution were recorded and analyzed. Results Under six motion states, the overall ROM of fusion segments in Models 2-5 was smaller than that of Model 1. Compared with Model 1, the overall ROM reduction of Model 3 and Model 4 was larger than that of Model 2 and smaller than that of Model 5. Under flexion and extension, the overall ROM reduction of Model 4 and Model 5 was basically equal. Under left and right lateral bending, the overall ROM reduction of Model 3 and Model 5 was basically equal. Under all motion states, the peak stress of Model 3 and Model 4 fusion cage was larger than that of Model 5 and smaller than that of Model 2. The peak stresses of L2-3, L3-4 and L4-5 fusion cages in Model 3 increased by 5. 52% , 10. 96% and 7. 99% respectively compared with Model 5 under left lateral bending, and the peak stresses of L2-3, L3-4 and L4-5 fusion cages in Model 4 increased by 8. 70% , 7. 00% and 6. 99% respectively under flexion. Under all motion states, the peak stress of screw rod in Model 5 was smaller than that of Model 3 and Model 4, and the peak stresses of screw rod in Models 3-5 were the smallest in upright state. Conclusions The OLIF with unilateral pedicle screw fixation or vertebral screw fixation can provide favorable biomechanical stability of the fusion segment. 4.Study on the Mass Spectrometry Fragmentation Regularity and Rapid Identification of Macrocyclic Polyamine Alkaloids from Tripterygium wilfordii

		                        			OBJECTIVE:To stud y the mass spec trometry fragmentation regularity and establish rapid identification method of macrocyclic polyamine alkaloids from Tripterygium wilfordii. METHODS :The determination was performed on Hypersil GOLDTM C18 column with 0.1% formic acid solution (A)-methanol(B)as mobile phase (gradient elution )at the flow rate of 0.25 mL/min. The column temperature was 40 ℃,and sample size was 2 μL. The mass spectrometric condition included ESI,positive ion mode , breakdown voltage of 100 V,collision energy of 35 eV,and scanning range of m/z 100-2 000. UPLC-Q-TOF-MS/MS combined with PeakView 1.2 software were used to analyze mass spectrum of 4 macrocyclic polyamine alkaloids control as celafurine , celabenzine,celacarfurin,celacinnine and total extract of T. wilfordii ,and summarized the mass fragmentation regularity of alkaloids. According to retention time ,accurate molecular weight and secondary mass fragment ion information ,referring to the above mass spectrometry fragmentation regularity ,macrocyclic polyamine alkaloids in the total extract of T. wilfordii were quickly identified,and whether it was a new compound was determined by consulting Scifinder database and literature comparison. RESULTS:There were three characteristic cleavage fragments of macrocyclic polyamine alkaloids ,i.e. m/z 160.11(base peak ),m/z 188.10 and m/z 100.07 in positive ion mode. According to the precise molecular weight and mass spectrometry fragmentation regularity,10 macrocyclic polyamine alkaloids were identified rapidly ,and compound 1-6,8 were 2-phenyl-1,5,9-triazacy- clotridecan-4-one, 9-acetyl-2-phenyl-1,5,9-triazacyclotridecan-4-one, 9-nicotinoyl-2-phenyl-1, 5, 9-triazacyclotridecan-4-one, celafurine,celabenzine,celacarfurine,celacinnine,respectively. Compounds 3,7,9 and 10 might be new compounds. CONCLUSIONS:The macrocyclic polyamine alkaloids canform three common characteristic fragment ions ,m/z 160.11(base peak ),m/z 188.10 and m/z 100.07,which can be used  for the rapid identification of these active components.
		                        		
		                        		
		                        		
		                        	
5.A clinical study of preoperative endoscopic assessment of the invasion depth of colorectal laterally spreading tumor

		                        			Objective To compare the diagnostic accuracy of magnifying chromoendoscopy (MCE) and endoscopic ultrasonography (EUS) for preoperative endoscopic assessment of the invasion depth of colorectal laterally spreading tumour(LST).Methods Data of 104 cases of colorectal LST were included.With the final pathological diagnosis as the golden standard,the accuracies of MCE and EUS for preoperative assessment of the invasion depth of colorectal LST were compared.Results The diagnostic accuracies of MCE and EUS for evaluating the invasion depth of LST were 89.4% (93/104) and 73.1% (76/104),respectively(P<0.05).The lesion size and the endoscopist could affect the accuracy of the EUS evaluation (P=0.017,OR=3.561;P=0.035,OR =1.399).The accuracy of EUS seemed to show a downward trend for colorectal LST of larger diameters.Conclusion Both MCE and EUS are effective for evaluating the invasion depth of colorectal LST,but the accuracy of MCE may be higher than that of EUS.Large diameter of the lesion and the doctor's experience inadequacy may be the risk factors for the accuracy of EUS.
		                        		
		                        		
		                        		
		                        	
6.Early and mid-term outcomes of aortic root enlargement with supraannular prostheses replacement for adult patients with aortic stenosis and small aortic root

		                        			Objective To summarize the short and mid-term outcome of adult patient suffered with aortic stenosis and small aortic root treated by aortic root enlargement with supraannular prostheses replacement or supraannular prostheses replace-ment.Methods From January 2005 to January 2017, 223 patients with aortic stenosis and small aortic root who underwent i-solated aortic valve replacement(AVR) were included in this retrospective study cohort.Patients with aortic insufficiency who underwent isolated AVR or those who underwent combined valve replacement were excluded from the study cohort .Aortic root enlargement with supraannular prostheses replacement was performed in 98 patients(ARE), and supraannular prostheses re-placement was performed in the remaining 125 patients as a control group(SP).The mean age and other baseline characteristics were compared between the two group, except that body surface area(BSA) in ARE were higher than that in SP[(1.62 ± 0.04)m2 vs(1.61 ±0.04)m2, P=0.015].Results Operative mortality occurred in 6 patients(2.7%), the cause of death including low cardiac output syndrome(LCOS, 3 patients), multiple organ failure(MOF, 2 patients) and stroke(1 patient). Reoperation for bleeding occurred in 5 patients and acute renal failure in 9 patients, pneumonia in 5 patients.The other nonfa-tal operative complications included wound complication(8 patients), temporary pacing therapy(24 patients), and new onset of acute mitral regurgitation(1 patient).The operative mortality and nonfatal complication were not statistically different be-tween the two groups.Patients in ARE received more bioprotheses and iEOA was higher than those in SP .Transvalvular pres-sure gradients and incidence of patient-prostheses mismatch were lower in ARE.At the 2 years of follow-up, transvalvular pres-sure gradients and left ventricular mass index were statistically lower in ARE compared with SP .The iEOA of ARE was higher than that in SP(1.22 ±0.13 vs 0.87 ±0.13, P<0.01).However, during the mid-term follow up(mean duration of follow-up was 6.31years), the overall survival rate was not statistically different between the two groups.Conclusion The strategy of aortic root enlargement with supraannular prostheses to treat adult patients with aortic stenosis and small aortic root can provide more optimal hemodynamic effect , effectively avoid PPM and was not associated with increased risk of mortality or adverse event when compared with strategy of supraannular prostheses replacement.However, the mid-term survival rate was not statistically different between the two strategies.
		                        		
		                        		
		                        		
		                        	
7.Construction of finite element model of left atrial diverticulum based on computed tomography and reverse engineering softwares.

		                        			This paper aims to explore the feasibility of building a finite element model of left atrial diverticulum (LAD) using reverse engineering software based on computed tomography (CT) images. The study was based on a three-dimensional cardiac CT images of a atrial fibrillation patient with LAD. The left atrium and LAD anatomical features were accurately reproduced by using Geomagic Studio 12 and Mimics 15 reverse engineering software. In addition, one left atrial model with LAD and one without LAD were created with ANSYS finite element analysis software, and the validity of the two models were verified. The results show that it is feasible to establish the LAD finite element model based on cardiac three-dimensional CT images using reverse engineering software. 8.Application and Rationality Evaluation of Hemostatic Drugs in the Patients with Subarachnoid Hemorrhage

		                        			Objective:To investigate the current use situation of hemostatics in preventing re-bleeding in the patients with sub-arachnoid hemorrhage. Methods:The records of patients with subarachnoid hemorrhage were selected from a hospital in 2016 and the types,dosage,course and drug adverse reactions of hemostatics were recorded and evaluated according to the drug information leaflets/related guideline,drug utilization researches and adverse drug actions monitoring. Results:Totally 81 cases with hemostatics involved 3 varieties of hemostatic drugs,the combination proportion was 16.05%,the reasonable use rate was 86.42%,the reasonable rate of administration route was 100%,the reasonable solvent rate was 98.77% and the course no more than 72 h was 88.89%. The drug uti-lization index of etamsylate injection and tranexamic acid injection was greater than one,indicating that the dosage was unreasonable. There were no adverse reactions in the 81 cases.Conclusion:The use of hemostatics in the patients with subarachnoid hemorrhage is common,and there is still unreasonable use (such as dosage and course etc) that we should pay attention to.
		                        		
		                        		
		                        		
		                        	
9.Ischemic mitral valve reconstruction and biological valve replacement in elderly patients: comparison of long-term survival and complications

		                        			Objective This study reviews the 198 consecutive mitral valve operations for elderly patients(> 65 y) with ischemic mitral insufficiency performed at Anzhen Hospital between January 2000 and june 2016.The results for mitral valve reconstruction are compared with those for biological mitral valve replacement.Methods From January 2000 to June 2016,198 elderly patients with ischemic mitral insufficiency underwent mitral valve reconstruction (n =150) or biological mitral valve replacement(n =48).All of them coronary artery bypass gafting was performed at the same time.Preoperative clinical characteristics,procedural characteristics,major and minor complications after surgery,preoperative and postoperative left ventricular ejection fraction (LVEF) by echocardiography,and outcome (survivor or death,mitral regurgitation,NYHA degree) were assessed.Results There was no significant difference between the two groups in the rate of mortality during hospitalization and early postoperative cardiac function.The proportion of severe MR in 1,3,and 5 years after MVP were 5.1% 、6.3%、7.7% respectively.There was no MVR due to the recurrent moderate-to-severe reflux.There were 2 SBE but no reflux appeared after MVR.There was no significant difference in cardiac function and mortality between the two groups within 5 years.Conclusion There is no significant difference between CABG + MVP and CABG + MVR(BV) in cardiac function and mortality in the treatment of elderly patients with IMR.,There was a possibility of MR recurrence after CABG + MVP.There was no MR after CABG + MVR(BV),but the risk of SBE is higher than MVP.We can select operation mode personalized according to the type of mitral valve disease and cardiac function.
		                        		
		                        		
		                        		
		                        	
10.The development of eco-migrant children's extroversive behavior and its relation with personality and family environment

		                        			Objective To explore the developmental tendency of eco-migrant children's extroversive behavior and the relationship between extroversive behavior and personality and family environment.Methods 856 eco-migrant children(aged 6-16)participated in the present longitudinal study.At first time the Child behavior checklist(CBCL),Eysenck personality questionnaire (EPQ) and Family environment scale (FES) were used to assess their behavioral problems,personality and family environment.Participants' parents subsequently completed measures assessing behavioral problems every nine months for 27 months.Data were analyzed using hierarchical linear modeling analyses.Results The scores of extroversive behavior in eco-migrant children were(10.09±7.11) at first time,(7.66±7.56) at the second wave,(8.54±7.49)at the third wave and(8.11±7.33) at the last time.During the longitudinal period,the descending trend of eco-migrant children's extroversive behavior was significant (β=-0.51,P<0.05).The scores of psychoticism,neuroticism,family conflict,organization,cohesion and cultural factors were differently correlated with children's extroversive behavior (β=-0.67-0.32,P<0.05).Family conflict predicted the developmental trend of children's externalizing (β=-0.46,P<0.05).Conclusion During the longitudinal period,the level of eco-migrant children's extroversive behavior decreased,and personality and family environment have significant influences on it.
