An adult female patient of internal nuclear inclusion disease(NIID)with a major clinical manifestation of decreased intelligence and recurrent disturbance of consciousness was followed up for nearly 4 years. In particular, the evolution of the video-electroencephalogram(VEEG),the auxiliary diagnosis of VEEG,and the prediction value of VEEG for clinical outcome were summarized. We found that(1)the background rhythm in NIID patients evolved with the progression of the disease;(2)electroencephalogram reactivity could predict the outcome of NIID coma;(3)VEEG helped to determine whether the consciousness disturbance caused by NIID was due to non-convulsive status epilepticus,This case provides a new idea to explore the application of VEEG in NIID.

Objective:To investigate the correlation between different degrees of white matter hyperintensities (WMHs) and outcome after intravenous thrombolysis (IVT) in patients with acute ischemic stroke. Methods:Patients with AIS received intravenous thrombolytic therapy with standard dose of alteplase in the First Hospital of Putian City from January 2019 to June 2022 were retrospectively included. The Fazekas scale was used to score the WMHs shown on MRI, and the patients were divided into without or mild WMH group and moderate-to-severe WMH group. The clinical baseline data and the clinical outcome after 3 months of the two groups were compared. The poor outcome was defined as the modified Rankin scale score >2. Multivariate logistic regression analysis was used to determine the influence of the severity of WMHs on the outcome after intravenous thrombolysis. Results:A total of 103 patients with AIS were included. Their age was 64.85±10.89 years old, and 66 (64.1%) were men. There were 60 patients (58.3%) in the without or mild WMH group, and 43 (41.7%) in the moderate-to-severe WMH group. There were significant differences in age, body mass index, systolic blood pressure, baseline National Institutes of Health Stroke Scale (NIHSS) scores, as well as the proportion of patients with hypertension, smoking, hemorrhagic transformation and poor functional outcome at 3 months after onset in different degrees of WMH groups (all P<0.05). There were 73 patients (70.9%) in the good outcome group and 30 (29.1%) in the poor outcome group. There were significant differences in age, body mass index, baseline NIHSS score, WMH score, as well as the proportion of patients with hypertension, large atherosclerotic stroke and symptomatic intracranial hemorrhage between the two groups ( P<0.05). Multivariate logistic regression analysis showed that after adjusting for confounding factors, moderate-to-severe WMHs were the independent risk factors for the poor outcome at 3 months after intravenous thrombolysis (odds ratio 3.810, 95% confidence interval 1.298-1.124; P=0.015). Conclusion:Moderate-to-severe WMHs are associated with the poor outcome in patients with AIS at 3 months after intravenous thrombolysis.

Objective:
To investigate the relationship between donor chimerism and relapse after allogeneic hematopoietic stem cell transplantation (allo-HSCT) .
Methods:
The clinical data of 105 patients with acute myeloid leukemia (AML) who underwent allo-HSCT and recurrence-free survival>90 days from January 2010 to January 2019 were retrospectively analyzed. The bone marrow samples were collected at 15, 30, 60, 90, 180, 270, 360 days after transplantation. Donor chimerism was detected by single nucleotide polymorphism (SNP) -PCR.
Results:
Of the 105 patients, 43 cases were male and 62 cases were female, with a median age of 38 (16-60) years. Till April 2019, the median follow-up was 843 (94-3 261) days. Ninety days after transplantation, 18 cases relapsed, 33 cases died, and 72 cases survived. The 3-year overall survival (OS) rate was (66.8±5.1) %, and the recurrence-free survival (RFS) rate was (65.1±5.0) %. Pre-transplant disease status, pre-transplant minimal residual disease (MRD) , and 90 day post-transplantation chimerism were independent risk factors related to RFS. The risk of recurrence was significantly increased in patients with a donor chimerism rate ≤97.24% at 90 days after transplantation[

Objective:
To explore the value of superb micro-vascular imaging(SMI) combined with conventional ultrasound in differential diagnosis of polypoid lesions of gallbladder.
Methods:
The ultrasonographic and pathological datas of 67 patients with polypoid lesions of gallbladder (of ≥1 cm) in diameter were analyzed retrospectively. According to the pathological results, the patients were divided into tumorous polyp group and non-tumorous polyp group.Conventional ultrasound, SMI and contrast-enhanced ultrasound (CEUS) were performed in all patients before operation, and the basal width, continuity of cystic wall and internal blood flow morphology of polyps were evaluated. The ROC curve was used to calculate the area under the curve and the optimum boundary value of tumorous polyps, the sensitivity and specificity of SMI combined with conventional ultrasound in the diagnosis of neoplastic polyps were calculated according to the optimal threshold. Kappa consistency test was used to analyze the consistency between microblood flow ability and CEUS shown by SMI technique.
Results:
Of the 67 patients, 22 cases were neoplastic polyps, and 45 cases were non-neoplastic polyps. The polyps were scored quantitatively by SMI combined with conventional ultrasound (0-9 points) and the ROC curve was plotted with, area under curve 0.893(95% CI 0.792-0.994). The sensitivity, specificity and accuracy of diagnosing neoplastic polyps with score (≥4.5) were 77.3%, 93.3% and 88.1%, respectively. Compared with the score of CEUS for microblood flow display (0-4 points), the Kappa values of CDFI, SMI was 0.186, 0.688. SMI and CEUS have good consistency.
Conclusions
SMI combined with conventional ultrasound is helpful in differential diagnosis of polypoid lesions of gallbladder, with a high diagnostic value. SMI and CEUS have good consistency in the display of micro-blood flow. It can provide a new diagnostic basis for differential diagnosis of polypoid lesions of gallbladder.
