1.Impact of measurement of joint space width on effectiveness and prognosis of medial unicondylar knee arthroplasty

BACKGROUND:Compared with total knee arthroplasty,unicondylar knee arthroplasty has such advantages as quick recovery,low cost and good proprioception,but its high revision rate after operation is also a problem that cannot be ignored.At present,the reasons for the high revision rate after unicondylar knee arthroplasty are not completely clear.Therefore,preoperative strict control of surgical indication may be crucial to improve postoperative outcome and reduce revision rate after unicondylar knee arthroplasty.As an index commonly used in the measurement of imaging,the evaluation of joint space width may have important clinical reference value in the selection of indications for unicondylar knee arthroplasty. OBJECTIVE:To review the measurement of joint space width and its effect on the curative effect and outcome of medial unicondylar knee arthroplasty. METHODS:WanFang and PubMed were used to search the relevant literature published from January 1,1950 to June 1,2023 on the evaluation factors of curative effect of unicondylar knee arthroplasty and the influence of joint space width on the curative effect of unicondylar knee arthroplasty.By summarizing and analyzing the literature,the measurement methods of joint space width,the influence of preoperative medial compartment joint space width on the curative effect of medial compartment joint space width,and the influence of postoperative lateral compartment joint space width on the outcome were reviewed. RESULTS AND CONCLUSION:(1)Although many methods have been used to study and measure knee joint space width,X-ray measurement of joint space width under weight-bearing position is still a common method in daily orthopedic practice to assess the progression of osteoarthritis,and it can moderately reflect the thickness of cartilage.(2)Preoperative medial compartment joint space width of knee joint can affect the efficacy of medial movable platform after unicondylar knee arthroplasty.Patients with knee osteoarthritis whose medial joint space width/lateral joint space width ratio is<40%or medial joint space width≤2 mm may be more suitable for medial movable platform unicondylar knee arthroplasty.(3)Changes in lateral compartment joint space width after medial unicondylar knee arthroplasty also have a certain impact on outcome.Improving the joint matching degree of lateral compartment after surgery can delay the progression of lateral compartment joint degeneration and reduce the prosthesis revision rate.However,relevant clinical studies are still lacking.In conclusion,the imaging measurement of joint space width has important clinical reference value for evaluating the postoperative efficacy and prognosis of medial unicondylar knee arthroplasty.
2.Treatment of Paroxysmal Sympathetic Hyperactivity by the Method of “Returning Fire to Its Origin”

This paper summarized the clinical experience of using the method of “returning fire to its origin” for treatment of paroxysmal sympathetic hyperactivity (PSH). According to the causes and clinical characteristics of PSH, the author believes that the deficiency of kidney qi, and the loss of yin and yang are the basis of the pathogenesis of PSH. Fright causes qi to be chaotic as the triggering mechanism of PSH. The key mechanism of PSH is that the deficiency yang with upper manifestation, and the fire does not return to its origin. The treatment should be nourishing yin and astringing yang, by taking modified Yinhuo Decoction (引火汤) internally, and receiving warm moxibustion as the first choice externally with selected acupoints Guanyuan (CV 4), Mingmen (GV 4), and bilateral Yongquan (KI 1); For prevention, attention should be paid to take care of stomach qi, support healthy qi, and cultivate original qi.
3.Factors influencing vicarious posttraumatic growth of emergency nurses based on structural equation modeling

Objective:To explore the current status of vicarious posttraumatic growth (VPTG) among emergency nurses, and analyze its influencing factors and pathways of action.Methods:From March to May 2022, convenience sampling was used to select 532 emergency nurses from 20 public general hospitals in Anhui Province as the research subject. A survey was conducted on emergency nurses using the General Information Questionnaire, Posttraumatic Growth Inventory, Chinese Version of the Interpersonal Reactivity Index, Connor-Davidson Resilience Scale, and the Multidimensional Scale of Perceived Social Support. AMOS 24.0 software was used to establish the structural equation model and analyze the path relationships between variables.Results:A total of 532 questionnaires were distributed, and 480 valid questionnaires were collected, with an effective response rate of 90.23% (480/532). The score of the Posttraumatic Growth Inventory for 480 emergency nurses was (49.35±15.82). Empathy, psychological resilience, and perceived social support all had a positive impact on VPTG, with total effect values of 0.503, 0.261, and 0.163, respectively. Empathy directly or indirectly affected VPTG through perceived social support and psychological resilience. Perceived social support indirectly affected VPTG through psychological resilience, and psychological resilience directly affected VPTG.Conclusions:VPTG of emergency nurses is directly and/or indirectly positively influenced by empathy, psychological resilience, and perceived social support. Nursing managers should pay attention to the interaction between factors, take effective measures based on the association between factors, and promote the positive transformation of emergency nurses after trauma.
4.Latent classes and influencing factors of vicarious posttraumatic growth among emergency nurses

Objective:To investigate the latent classes and influencing factors of vicarious posttraumatic growth among emergency nurses.Methods:Convenience sampling method was used to select the 498 emergency nurses of 20 public general hospitals in Anhui province as the survey objects. The general information questionnaire and Posttraumatic Growth Inventory were used to investigate. Latent class analysis was used to classify vicarious posttraumatic growth of emergency nurses, and the influencing factors of latent classes were analyzed by binary logistic regression.Results:There were two latent classes of vicarious posttraumatic growth of emergency nurses including high growth-high fluctuation group (24.1%, 120/498) and low growth group (75.9%, 378/498). Binary logistic regression analysis showed that professional title, average monthly income and self-rated health status were the factors influencing the latent classes (all P<0.05). Conclusions:There are obvious classification characteristics of vicarious posttraumatic growth among emergency nurses. Nursing managers should formulate and implement targeted intervention measures according to the classification characteristics and influencing factors to improve their vicarious posttraumatic growth level.
5.Research progress of frailty in elderly patients with trauma

Frailty and trauma are closely related, and together affect the health outcomes of elderly trauma patients. However, insufficient attention has been paid to the frailty of elderly trauma patients in China. This paper reviews the relationship between frailty and trauma, assessment tools and management of frailty in elderly trauma patients, in order to provide a new perspective of nursing care for elderly trauma patients in China and to provide references for carrying out research on frailty in elderly trauma patients.
6.Summary of the best evidence for environment management to prevent delirium in ICU patients

Objective:To summarize the relevant evidence of environmental management for preventing delirium in ICU patients, so as to provide references for delirium management in ICU patients.Methods:Evidence-based questions were constructed according to the PIPOST model, and relevant clinical decisions, guidelines, expert consensus, systematic reviews and evidence summaries published by various guideline websites, evidence-based databases and comprehensive databases at home and abroad were systematically searched according to the evidence resource "6S" model. The search period was from the establishment of databases to September 3, 2022. Two researchers independently conducted literature quality evaluation using corresponding literature quality evaluation tools. The Joanna Briggs Institute (JBI) Evidence Pre-grading and Evidence Recommendation Grading System (2014 Edition) was used to grade and recommend included evidence.Results:A total of 30 articles were included, including 5 guidelines, 2 expert consensus, 1 clinical recommended practice, 4 evidence summaries and 18 systematic reviews. A total of 36 best evidences were summarized from 5 aspects, including environment optimization, sleep promotion, cognitive stimulation, positioning and orientation function, and safety protection.Conclusions:Medical staff should manage the ICU environment based on evidence-based evidence to prevent or reduce the occurrence of delirium in patients.
7.Design and application of female infant diapers

Objective:To design and manufacture the female infant diaper, and to explore the application effect of the diaper in routine urine collection of female infants.Methods:Female infants hospitalized in department of Orthopedic Rehabilitation of Children′s Hospital of Zhejiang University School of Medicine from July 2020 to June 2021 were selected as the research objects. According to the chronological order, 148 female infants admitted from July to December 2020 were selected as the control group, and 154 female infants admitted from January 2021 to June 2021 were selected as the experimental group. In the experimental group, urine routine was collected by female infants diapers, while in the control group, urine routine was collected by traditional disposable urine collection bag. The time required for routine urine collection, the number of urine collection bags, the incidence of perineal skin injury and the satisfaction of parents were observed.Results:The time required for routine urine collection was (16.86 ± 12.58) h in the experimental group and (28.76 ± 22.20) h in the control group. The difference between the two groups was statistically significant ( t=5.70, P<0.05). The number of female infant diapers in experimental group was (1.69 ± 0.67); The number of disposable urine collection bags used in the control group was (2.76 ± 1.32), and the difference between the two groups was statistically significant ( t=8.81, P<0.05). In the experimental group, the incidence of perineal skin injury was 0. In the control group, the incidence of perineal skin injury was 24.8%(42/148). The difference between the two groups was statistically significant ( Z=-9.78, P<0.01). The satisfaction of parents in the experimental group was 98.7% (152/154), witch higher than that in the control group was 79.7% (118/148), and the difference was statistically significant ( Z=-9.68, P<0.01). Conclusions:Compared with the traditional disposable urine collection bag, the diaper collection for female infants can shorten the time required for routine urine collection, reduce the number of urine collection tools, reduce the incidence of perineal lesions in children, improve the satisfaction of parents of children, reflect the people-oriented nursing concept, and provide greater clinical convenience.
8.Comparison of EB virus infection between short term and long term use of mycophenolate mofetil for prophylaxis of graft versus host disease after haploidentical hematopoietic stem cell transplantation

Objective:To investigate the role of short-term use of mycophenolate mofetil (MMF) in EB viral infection and acute graft-versus host disease (GVHD) in patients receiving haploidentical hematopoietic stem cell transplantation (haplo-HSCT) .Method:Adult patients (≥14 years) who were diagnosed with hematological malignancies received haplo-HSCT in Peking University Institute of Hematology from May 2016 to December 2017 were retrospectively reviewed. The median age was 30 (14-60) years old. A total of 498 patients including 277 males and 221 females were enrolled. Donors' median age was 38 (8-66) years old. All patients were classified into long-term use of MMF ( n=199), which was defined as 500 mg every 12 hours from day 9 pre-transplant to 250 mg every 12 hours from day 30 after transplant then withdrawal on day 45 to 60 after transplant, and short-term use of MMF ( n=299), which was defined as 500 mg every 12 hour from day 9 pre-transplant then withdrawal till neutrophil engraftment. Kaplan-Meier model was used to analyze the cumulative incidence of EBV infection, and the Cox proportional regression model for multivariate analysis. Result:Characteristics including sex, age, disease types, mismatched HLA loci, donor-recipient relationship, donor-recipient blood type, donor age, and donor sex were comparable between two groups (all P>0.05). According to once, the incidence of EBV viremia, defined as EBV>10 3 copies/ml at least once, in short-term group and long-term group was 19.4% (58/299) and 27.6% (55/199) respectively ( P=0.046).Donor age and the duration of MMF prophylaxis (short-term group as reference) were associated with EBV viremia according to multivariate analysis [ HR=1.022(95% CI 1.006-1.038),1.600(95% CI 1.059-2.418); P=0.006 and 0.026, respectively]. The incidence of grade Ⅱ-Ⅳ and Ⅲ/Ⅳ acute GVHD in long-term and short-term group was 32.2% (64/199) versus 20.7% (62/299)( P=0.005) and 10.1% (20/199) versus 8.0% (24/299) ( P=0.427), respectively. Donor sex (female as reference) and duration of MMF prophylaxis (short-term group as reference) were associated with grade Ⅱ-Ⅳ acute GVHD [ HR=1.908(95% CI 1.079-3.373),1.752(95% CI 1.161-2.643); P=0.026 and 0.008, respectively].There were no statistical differences in the incidence of CMV viremia, refractory CMV viremia and hemorrhagic cystitis (all P>0.05) between the two groups. Conclusion:Short-term use of MMF can reduce EBV viremia without increasing the development of acute GVHD in haplo-HSCT patients.
9.Combining scalp acupuncture with hyperbaric oxygen therapy improves functional recovery after cerebral infarction

Objective:To observe the effect of combining scalp acupuncture with hyperbaric oxygen on serum homocysteine and highly-sensitive c-reactive protein levels and functional recovery after cerebral infarction.Methods:A total of 101 survivors of cerebral infarction were divided randomly into a scalp acupuncture group ( n=33), a hyperbaric oxygen group ( n=34) and a combined treatment group ( n=34). All received routine treatment plus the appropriate supplementary treatment once a day for 10 days. The subjects were evaluated before the experiment as well as 10 and 90 days afterward. The National Institutes of Health′s stroke scale (NIHSS) was used to quantify neurological deficits and the Barthel Index quantified ability in the activities of daily living. Ninety days after the treatment, modified Rankin scale scores were also assigned. The levels of serum homocysteine (Hcy) and highly-sensitive c-reactive protein (hs-CRP) before and after 10 days of treatment were also compared among the 3 groups. Results:The average NIHSS and Barthel Index scores of all three groups had improved significantly after 10 days of treatment and the improvements persisted at the follow-up 3 months later. Both results were significantly better in the combined treatment group than in the scalp acupuncture group at the 90-day follow-up evaluation. The average Rankin score of the combined treatment group was lower at the last follow-up. Compared with before the intervention, the average Hcy of the scalp acupuncture group, the average hs-CRP of the hyperbaric oxygen group, as well as the average Hcy and hs-CRP of the combined treatment group were significantly lower after 10 days of treatment. Compared with the scalp acupuncture group, the average Hcy [(11.68±2.6) μmol/L] of the combined treatment group was significantly lower after the 10 days of treatment.Conclusions:Supplementing scalp acupuncture with hyperbaric oxygen therapy improves the long-term outcomes of cerebral infarction, reducing the level of serum Hcy.
10.Qualitative study on working experience of COVID-19 care nurses.

OBJECTIVE:
To study the working experience of COVID-19 care nurses.
METHODS:
Twenty two nurses taking care of COVID-19 patients were interviewed by means of descriptive phenomenology. All the data were transcribed and recorded, and then processed into WORD documents. The Colaizzi 7 footwork was used to classify, encode, establish nodes and extract themes based on Nvivo11.0 software.
RESULTS:
Two main themes were extracted: one is the positive feelings of nurses, including the sense of professional mission and pride, the sense of achievement and happiness, the improvement of self-worth and ability, the powerful support system and the power of role models; the other is the negative experience of nurses, including the worry and anxiety at work, the lack of experience and trust, the difficulty of work, and the inconvenience of isolating life.
CONCLUSIONS
s While fully affirming the work value of nurses, it is necessary for the society, hospitals and patients to give extensive and continuous support, care and respect to nurses, so as to stimulate their working enthusiasm and sense of professional achievement. Hospital managers need to implement all kinds of security work, meet the safety needs of nurses, pay attention to the physical and mental health of nurses, strengthen the training of nursing talents for critical and severe diseases and infectious diseases, improve the allocation of human resources, and enhance the ability of material allocation and reserve for major health events, so as to make adequate preparations for coping with public health events in the future.
