1.Automated identification and localization of inferior vena cava based on ultrasound images

Objective To explore the automated identification and diameter measurement methods for inferior vena cava (IVC) based on clinical ultrasound images of IVC. Methods An automated identification and localization method based on topology and automatic tracking algorithm was proposed. Tracking algorithm was used for identifying and continuously locating to improve the efficiency and accuracy of measurement. Tests were conducted on 18 sets of ultrasound data collected from 18 patients in intensive care unit (ICU),with clinicians' measurements as the gold standard. Results The recognition accuracy of the automated method was 94.44% (17/18),and the measurement error of IVC diameter was within the range of±1.96s (s was the standard deviation). The automated method could replace the manual method. Conclusion The proposed IVC automated identification and localization algorithm based on topology and automatic tracking algorithm has high recognition success rate and IVC diameter measurement accuracy. It can assist clinicians in identifying and locating IVC,so as to improve the accuracy of IVC measurement.

The incidence of osteoporotic thoracolumbar vertebral fracture (OTLVF) in the elderly is gradually increasing. The kyphotic deformity caused by various factors has become an important characteristic of OTLVF and has received increasing attention. Its clinical manifestations include pain, delayed nerve damage, sagittal imbalance, etc. Currently, the definition and diagnosis of OTLVF with kyphotic deformity in the elderly are still unclear. Although there are many treatment options, they are controversial. Existing guidelines or consensuses pay little attention to this type of fracture with kyphotic deformity. To this end, the Lumbar Education Working Group of the Spine Branch of the Chinese Medicine Education Association and Editorial Committee of Chinese Journal of Trauma organized the experts in the relevant fields to jointly develop Clinical guidelines for the diagnosis and treatment of osteoporotic thoracolumbar vertebral fractures with kyphotic deformity in the elderly ( version 2024), based on evidence-based medical advancements and the principles of scientificity, practicality, and advanced nature, which provided 18 recommendations to standardize the clinical diagnosis and treatment.
3.Application of gene detection in individualized treatment of lipid-lowering drugs

The main clinical manifestation of dyslipidemia is hyperlipidemia, which is an important risk factor leading to the occurrence of cardiovascular and cerebrovascular diseases such as atherosclerosis, coronary heart disease and stroke. In clinical practice, lipid-lowering drugs are mainly used for treatment, but there are issues such as individual differences and genetic effects. Therefore, it is necessary to perform gene detection on patients, so as to guide individualized application of lipid-lowering drugs. This review mainly previews the definition of gene detection and the individualized treatment of lipid-lowering drugs, and introduces the application of gene detection in the individualized treatment of lipid-lowering drugs (statins, fibrates, nicotinic acid and ezetimibe). Among them, the gene polymorphisms of APOE, SLCO1B1 and CYP450 family play a key role in the efficacy and safety of statins; the gene polymorphisms of APOA/B/C family have a significant impact on the efficacy of fenofibrate; the gene polymorphisms of HCAR2 and DGAT2 have an important impact on the efficacy of niacin; the gene polymorphisms of NPC1L1 have a significant impact on the efficacy of ezetimibe. It is suggested to conduct genotype detection for patients with dyslipidemia to select appropriate treatment strategies, so as to provide individualized medication guidance.
4.Optimal exercise combinations for patients with diabetes

Objective:Exercise is a key way to control the blood glucose,which can improve blood lipids and blood pressure,reduce glycated hemoglobin levels,and increase insulin sensitivity.It is of great significance for maintaining blood glucose homeostasis.This study aims to explore the optimal exercise combinations suitable for diabetic patients,and to provide scientific and effective personalized exercise guidance for diabetic patients. Methods:The physical examination data and questionnaire results of were collected from the Third Xiangya Hospital of Central South University.The study was involved in 3 867 diabetes from January,2020 to December,2021.The basic information and living habits were obtained through questionnaires.The fasting blood samples were collected to measure the levels of total cholesterol(TCh),triglyceride,high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),and fasting blood glucose.Physical measurements included height,weight,waistline,hipline,and blood pressure.Categorical variables were tested using chi-square tests,and continuous variables were tested using non-parametric Kruskal-Wallis test.Multiple linear regression model was used to analyze the relationship between exercise behaviors(frequency,duration,and intensity),years of exercise,diet habits,medicine,and fasting blood glucose.The relationship between different exercise behaviors and various indexes(BMI,blood pressure,blood lipids,blood glucose)was analyzed by multiple rising sun rose plots.The polar thermal diagram showed the relationship of exercise behaviors with the best expected effect. Results:Non-parametric Kruskal-Wallis test showed that there were significant differences in other indexes except LDL-C(P=0.681)among groups with different exercise behaviors(all P<0.01).The multiple rising sun rose plots showed that diabetic patients who did moderate to high intensity exercise more than 5 times a week for more than 30 min each time had a healthier BMI,blood lipid levels,blood glucose levels,and blood pressure.Polar thermal diagram showed that low-intensity exercise once or twice a week requires 30 to 60 min or more of exercise to achieve the desired results.Multiple linear regression analysis showed that frequency(β=-0.208,95% CI-0.356 to-0.059,P=0.006),duration(β=-0.227,95% CI-0.387 to-0.066,P=0.006),intensity of exercise(β=-0.110,95% CI-0.218 to-0.002,P=0.046),diet habits(β=0.462,95% CI 0.295 to 0.556,P<0.001)and medicine(β=-0.520,95% CI-0.720 to-0.312,P<0.001)were correlated with fasting blood glucose. Conclusion:Moderate to high intensity exercise for more than 30 min and 5 times a week is the most beneficial combination for diabetes.Low exercise intensity should be combined with higher exercise frequency and longer exercise time to achieve the desired effect.With the increase of exercise intensity,the relationship between low exercise frequency and long exercise time is weakened.
5.Clinical efficacy of hip arthroscopy in the treatment of acetabular labral injury with cysts

Objective:To investigate the clinical efficacy of hip arthroscopy in the treatment of acetabular labral injury combined with cysts.Methods:A total of 14 patients (5 males and 9 females) aged 35.46±12.62 years (range, 26-57 years) with acetabular labral injury complicated with cyst who underwent hip arthroscopy in Department of Sports Injury and Arthroscopy, Tianjin Hospital from January 2017 to April 2022 were retrospectively analyzed. There were 8 cases of left hip and 6 cases of right hip with an average body mass index of 24.35±3.14 kg/m 2 (range, 20.2-28.4 kg/m 2). The duration of symptoms was 6.25±4.39 months (range, 3-11 months). All patients underwent hip arthroscopic cyst cleaning and glenoid labral suturing. The femoral neck α angle, lateral center rim angle, visual analogue score (VAS), Harris hip score (HHS), and international hip outcome tool scores (iHOT-12) were compared before and after operation. Results:All patients successfully completed the operation and were followed up for 10.23±2.45 months (range, 1.5-18 months). The difference between the patients' hip VAS before and after operation was statistically significant ( F=108.47, P<0.001). The VAS score at 18 months after operation was 1.34±0.83, which was significantly lower than that before operation 7.85±1.12 and at 6 weeks after operation 5.03±1.60 ( P<0.05). The difference between the patients' hip HHS scores before and after operation was statistically significant ( F=96.89, P<0.001). The HHS score at 18 months after operation was 85.58±4.65, which was significantly higher than that before operation 54.36±2.31 and 6 weeks after operation 61.12±1.20 ( P<0.05). The differences of iHOT-12 scores before and after operation were statistically significant ( F=117.92, P<0.001). The iHOT-12 score of 18 months after operation was 78.36±2.28 points, which was higher than that before operation 31.39±5.21 points and 6 weeks after operation 47.88±2.20 points, and the difference was statistically significant ( P<0.05). The difference of α angle of femoral neck before and after operation was statistically significant ( F=101.56, P<0.001). The α angle of femoral neck at 12 months after operation was 45.32°±3.16°, which was significantly lower than that before operation 50.86°±8.41° ( P<0.05). The difference of lateral center rim angle before and after operation was statistically significant ( F=100.38, P<0.001). The lateral center rim angle was 28.23°±5.32° at 12 months after operation, which was smaller than that before operation 32.16°±5.13°, and the difference was statistically significant ( P<0.05). Conclusion:Hip arthroscopy in the treatment of acetabular labral injury with cysts can relieve hip pain and improve hip function.
6.Coronary angiographic characteristics of 997 patients with symptomatic recurrence after coronary artery bypass grafting

Objective To explore coronary angiographic characteristics in patients with symptomatic recurrence after coronary artery bypass grafting (CABG). Methods We performed a retrospective study of 997 patients with symptomatic recurrence after CABG in Beijing Anzhen Hospital from 2010 to 2020. There were 762 males and 235 females, with an average age of 62.41±8.70 years. Results There was a high prevalence of risk factors like hypertension, diabetes and a history of smoking. Diseased arterial grafts accounted for 27.44% while saphenous vein graft 54.40%; 240 (24.07%) patients had all patent grafts. The main lesion characteristics of diseased grafts were chronic total occlusion lesions (79.57%). Most patients had more diseased native vessels after CABG than before. The type C coronary artery disease in native vessels relevant to ischemic area occurred in 674 (67.60%) patients; 525 (52.66%) patients with recurrent symptom after CABG had both diseased grafts and diseased native vessels. Conclusion Graft status in patients with symptomatic recurrence after CABG is worse than we expected. The majority have newly developed lesions both in grafts and native vessels. Native vascular lesions will continue to progress after CABG.
7.The observation on the effect of prospective intervention on emergence agitation and postoperative recovery in patients with chronic sinusitis during preoperative visits

Objective:To investigate the effect of prospective intervention on emergence agitation and postoperative recovery in patients with chronic sinusitis during preoperative visits.Methods:A total of 80 patients with chronic sinusitis who underwent general anesthesia in Dayi County People′s Hospital of Chengdu City from December 2019 to October 2020 were selected and randomly divided into group D and group G, with 40 patients in each group. Group D received preoperative visit with conventional methods and group G received preoperative visit with prospective intervention methods. The hemodynamic changes of patients in the two groups at 30 min before the operation (T 1) and 1 (T 2), 5 (T 3), 10 (T 4) and 30 min (T 5) after tracheal tube extraction were recorded. The anxiety and depression scores of patients before the intervention and 1 d after the operation were compared between the two groups. The incidence of emergence agitation after the operation and complications during anesthesia awakening period were observed in the two groups, sino-nasal outcome test-20 (SNOT-20) was used to assess the postoperative recovery. Results:The incidence of emergence agitation in group G was lower than that in group D: 7.5%(3/40) vs. 25.0%(10/40), the difference was statistically significant ( χ2 = 4.50, P<0.05). There was no significant difference in systolic blood pressure, diastolic blood pressure and heart rate between the two groups at T 1 and T 5 ( P>0.05), but the level of above indicators in group G at T 2, T 3 and T 4 were significantly higher than those in group D ( P<0.05). The scores of State-Trait Anxiety Inventory(S-AI) and Self-Rating Depression Scale (SDS) in group G at the first day after the operation were significantly lower than those in group D: (35.45 ± 5.32) scores vs. (39.35 ± 4.91) scores, (35.42 ± 7.82) scores vs. (38.76 ± 5.21) scores, the differences were statistically significant ( P<0.05). The incidence of complications during anesthesia awakening period in group G was slightly lower than that in group D ( P>0.05). After the operation, the scores of sinusitis symptoms and nasal symptoms in the two groups were significantly decreased compared with those before the operation, and the scores of group G were significantly lower than those in group D ( P<0.05). Conclusions:Prospective intervention before anesthesia in patients with chronic sinusitis surgery can reduce stress response, improve bad mood, reduce the incidence of emergence agitation, and promote the postoperative recovery.

With the pace of socialist construction in China, the subject of radiotherapy in Shanxi province has developed from scratch and from small to large for more than 60 years. Remarkable achievements have been made in the establishment of departments, the updating of technical equipment, the increase of employees and clinical scientific research. This article reviews and summarizes the development history of tumor radiotherapy in Shanxi province.
9.Effect of inhaling NO perioperatively on postoperative cardiac and pulmonary functions in infants with congenital ventricular septal defect complicated

Objective To investigate the effect of low-flow inhaling NO for short time on postoperative cardiac and pulmonary functions in infants with congenital ventricular septal defect complicated with severe pulmonary hypertension. Methods Forty-five patients with congenital ventricular septal defect complicated with severe pulmonary hypertension from May 2014 to May 2016 in our hospital were enrolled. There were 19 males and 26 females, whose age ranged from 1 to 22 months (average age: 7.2±14.4 months) and weight ranged from 2.7 to 10.5 kg (average weight: 6.8±3.6 kg). The patients were randomly divided into three groups (n=15 in each): the blank group, the prior inhalation group and the posterior inhalation group. The blank group did not inhale NO, and the prior inhalation group inhalated NO for 10 min after tracheal and intubation. After the opening of the aorta, the posterior inhalation group inhaled NO for 10 min. The concentration of NO was 20 × 10–6. The pressure ratio of pulmonary circulation/systematic circulation, heart index and oxygenation index were calculated and the troponin value of the three groups was monitored 10 min after returning to intensive care unit (ICU) and postoperatively 1 h, 3 h and 24 h. Differences among above indicators between three groups were compared. Results The troponin value of the posterior inhalation group within 3 h increased most, followed by the blank group and the prior inhalation group. Postoperatively 1 h and 3 h, the troponin value of the prior inhalation group was significantly less than that of the blank group and posterior inhalation group (P<0.01) and the value on postoperative 24 h in each group was lower than that on postoperative 3 h. The cardiac index of prior inhalation group was higher than that of the blank group and the posterior inhalation group at each time point. Postoperatively 3 h and 24 h as well as 10 min after returning to ICU, the cardiac index in prior inhalation group was significantly higher than that of the posterior inhalation group (P<0.05). The pressure ratio of pulmonary circulation/systematic circulation of posterior inhalation group increased more than that of blank group; the differences in two groups were significant between postoperative 3 h and 10 min after returning to ICU (P<0.01). There was no statistical significance in the pressure ratio on postoperative 24 h and 10 min after returning to ICU (P>0.05) in three groups. The index of oxygenation of the prior inhalation group was higher than that of the blank group and the posterior inhalation group and statistically different from that of posterior inhalation group (P<0.05). Conclusion Inhaling NO 10 min preoperatively can reduce the injury to the heart and lung function effectively, but the result is the opposite when inhalating NO 10 min after aorta opening.
10.Expression and clinical significance of serum HMGB1 and inflammatory factors in patients with spinal cord injury

Objective To investigate the expression and clinical significance of serum high-mobility group box 1 (HMGB1) in the patients with spinal cord injury (SCI).Methods A total of 87 cases of acute SCI treated in the Aksu Corps First Division Hospital from January 2007 to January 2014 were collected as the SCI group.The SCI severity was evaluated according to the damage grade by the American Spinal Cord Injury Association(ASIA).Venous blood was collected on 1,3,7 d after SCI.Other 87 healthy adults served as the control group.The expression levels of serum HMGB1,inter1eukin-6 (IL-6),IL-1β and tumor necrosis factor alpha (TNF-a) in the two groups were measured by ELISA.Then the correlation between the HMGB1 expression level with IL-1β,IL-6 and TNF-α and the relation between HMGB1 with SCI severity were analyzed.Results The HMGB1,IL-6,IL-1 and TNF-α levels on 1,3,7 d after SCI in the SCI group were higher.The expression levels of serum HMGB1 and IL-6 on 1 d after SCI in the SCI patients were significantly increased,and reached the peak value on 3 d;the levels of IL-1β and TNF-α on 1 d after SCI were highest,and gradually decreased on 3,5 d.The correlation analysis showed that the expression levels of HMGB1 on 1,3 d after SCI were positively correlated with the IL-6,IL-1β and TNF-α expression levels(P<0.05).The serum HMGB1 expression level on 1,3 d after SCI was closely correlated with the SCI severity (P<0.05).The severe the SCI severity,the higher the serum HMGB1 expression level.Conclusion The expression level of serum HMGB1 in the patients with early SCI is significantly increased,and can be used as one of indicators for assessing the severity of early SCI.
