1.Intervention of muscle strength training combined with neuromuscular electrical stimulation on lower limb function and biomechanical changes in patients with patellofemoral pain

BACKGROUND:Lower limb peri-knee muscle strength training and neuromuscular electrical stimulation are generally safe and effective rehabilitation methods for patellofemoral joint pain,but the mechanism of their intervention is still unclear. OBJECTIVE:To determine the effect of muscle strength training combined with neuromuscular electrical stimulation on pain,lower extremity function and biomechanical characteristics in patients with patellofemoral pain. METHODS:Thirty-seven patients with patellofemoral pain were randomly divided into muscle strength training combined with electrical stimulation group(trial group,n=19)and muscle strength training group(control group,n=18).Both groups underwent intervention training for 6 weeks,three times a week.The visual analog scale and anterior knee pain scale were used to evaluate the pain level and functional level of the knee.Kinematic and kinetics data during running were collected by using an infrared motion capture system and a three-dimensional force platform simultaneously.A two-way analysis of variance with repeated measures(group*time)was applied to analyze the data. RESULTS AND CONCLUSION:(1)After the intervention,the visual analog scale scores of the trial group and the control group were significantly decreased(P<0.001),and the anterior knee pain scale scores were significantly increased(Ptrial group<0.001,Pcontrol group=0.001)in the trial group and control group.The anterior knee pain scale scores of the trial group were significantly higher compared to the control group after the intervention(P=0.001).(2)The peak knee flexion angle(P=0.011),peak knee extension moment(P<0.001),the peak knee internal rotation moment(P=0.008),the peak patellofemoral stress(P<0.001)and the peak patellofemoral contact force(P<0.001)were significantly decreased in the trial and control groups during running after the intervention compared with those before the intervention.(3)In conclusion,both muscle strength training and muscle strength training combined with electrical stimulation training are helpful to improve the subjective pain and lower limb function of patellofemoral pain patients,enhance the movement pattern during running and reduce the stress of the patellofemoral joint.Compared with muscle strength training alone,muscle strength training combined with electrical stimulation can improve lower limb function more significantly.
2.Effect of muscle energy technique on dynamic postural control and lumbar neuromuscular function in patients with non-specific low back pain:a randomized controlled trial

Objective To explore the effect of muscle energy technique(MET)on dynamic posture control and lumbar neuromuscu-lar function in patients with non-specific low back pain. Methods From March to June,2022,30 college students with non-specific low back pain from Shandong Sport Universi-ty were randomly divided into control group(n=15)and intervention group(n=15).The control group received health education,and the intervention group received MET,for four weeks.They were assessed with Visual Ana-logue Scale(VAS)of pain,Oswestry Disability Index(ODI),Y-balance test and trunk flexion-relaxation test be-fore and after intervention. Results VAS scores decreased in both groups after intervention(|t|>2.449,P<0.05),and it was less in the intervention group than in the control group(t=-5.068,P<0.001);while ODI score decreased in the intervention group(t=4.785,P<0.001),and it was less in the intervention group than in the control group(t=-2.895,P=0.007);the performance of Y-balance test increased(t=-3.662,P=0.003)in the intervention group,as well as flexion-re-laxation ratio of multifidus(t=-2.460,P=0.029). Conclusion MET is effective on alleviating pain and lumbar dysfunction,improving dynamic posture control and en-hancing the function of the multifidus during flexion in patients with non-specific low back pain.

Professor Qiu Maoliang,in his clinical practice and experience summary of acupuncture-moxibustion in the treatment of febrile diseases,proposes four acupuncture-moxibustion antipyretic methods,namely,releasing the exterior and reducing fever,clear-ing the interior and purging the heat,nourishing the yin and purging the heat,and assisting the yang and reducing fever,which respec-tively correspond to the exterior heat syndrome,interior heat syndrome,yin deficiency fever syndrome,and yang deficiency fever syn-drome.The academic connotation of Professor Qiu Maoliang's acupuncture-moxibustion for fever can be summarized as examining the syndrome and seeking the cause,and classifying fever;coordinating the four methods of acupuncture-moxibustion and operation tech-niques,which reflect Professor Qiu Maoliang's academic characteristics,such as the convergence of Chinese and Western medicine,mutual learning of acupuncture-moxibustion and medicine,and the connection of effect mechanism and theory.Professor Qiu Ma-oliang's academic thought of acupuncture-moxibustion antipyretic method not only helps to provide basis for further application of acu-puncture-moxibustion in contemporary clinical practice,but also enriches the modern biological connotation of acupuncture-moxibus-tion medicine.
4.Effects of Muscle Electrical Stimulation Combined with Muscle Strength Training on Biomechanical Characteristics of Knee Joint in Patients with Patellofemoral Pain

Objective To investigate the effects of electrical stimulation combined with muscle strength training on knee joint biomechanical characteristics in patients with patellofemoral pain(PFP).Methods Forty-six patients with PFP were recruited and randomly assigned to the muscle strength training(MST)and electrical muscle stimulation with strength training(EMS)groups.The intervention was performed three times a week for six weeks.The anterior knee pain scale(AKPS)was used to measure the knee pain degree.Knee kinematics,dynamics,and surface electromyography(sEMG)data were collected using an infrared motion capture system,force platform,and sEMG system during drop jumps before and after the intervention.Two-way analysis of variance with repeated measures was applied to determine the differences between the dependent variables of the two groups before and after the intervention.Results Compared with pre-intervention,the AKPS score,vastus medialis oblique(VMO)activation,VMO/vastus lateralis(VMO/VL)activation,maximum knee flexion angle,and peak knee extension moment increased significantly in the EMS group;the maximum knee abduction,external rotation angle,and peak knee external rotation moment decreased significantly in the EMS group after intervention.Compared with pre-intervention,the AKPS score,maximum knee flexion angle,and peak knee extension moment increased significantly in the MST group after intervention,the peak knee abduction and external rotation moment significantly decreased in the MST group after intervention.Post-hoc comparisons indicated that compared with the MST group,the AKPS score,VMO activation,VMO/VL activation were significantly higher and the maximum knee abduction angle was significantly lower in the EMS group.Conclusions EMS contributes to the better balance muscle activation of the VMO and VL and corrects the excessive knee abduction angle during jump landing,which may be helpful in relieving pain and improving lower limb function in patients with PFP.
5.Inhibition of S1P/S1PR2-mediated pericyte loss alleviates blood-brain bar-rier dysfunction in NPSLE mice

AIM:To investigate the pathological mechanism by which abnormal activation of sphingosine-1-phosphate(S1P)/S1P receptor 2(S1PR2)signaling pathway induces pericyte loss and subsequently affects blood-brain barrier dysfunction in neuropsychiatric systemic lupus erythematosus(NPSLE)mice using MRL/lpr mouse model.METHODS:Behavioral changes in female MRL/lpr mice from 6 weeks of age were assessed using open-field test(OFT),novel object recognition test(NORT)and forced swimming test(FST).The mice with neurobehavioral changes exceeding 20%compared with baseline were defined as NPSLE mice,and were randomly divided into 3 groups,each with 6 mice:NPSLE-S1P antagonist group,NPSLE-S1PR2 blocker group,and NPSLE saline treatment group.Additional 6 mice with no abnormal behavioral changes served as control group.The mice in NPSLE-S1P antagonist group were treated with S1P antagonist FTY720(2 mg/kg,orally),those in NPSLE-S1PR2 blocker group received treatment with S1PR2 blocker JTE-013(8 mg/kg,intraperitoneally),while those in NPSLE saline treatment group and control group received an equivalent volume of saline.Treatments were conducted 3 times per week for 3 weeks.Behavioral changes were reassessed using OFT,NORT and FST.Enzyme-linked immunosorbent assay(ELISA)was used to detect serum levels of S1P,interleukin-6(IL-6)and interferon-α(IFN-α).Blood-brain barrier permeability changes were evaluated by Evans blue staining.He-matoxylin-eosin(HE)staining was employed to observe brain tissue inflammation,Nissl staining was used to examine neu-ronal damage,and immunofluorescence staining was performed to assess the expression of pericyte marker neural/glial an-tigen 2(NG2)and endothelial cell marker CD31 in microvessels.Western blot analysis was conducted to detect the ex-pression levels of S1P,S1PR2,platelet-derived growth factor receptor-β(PDGFR-β)and zonula occludens-1(ZO-1)in brain tissues,while RT-qPCR was utilized to measure S1PR2 and PDGFR-β mRNA levels.RESULTS:Compared with model group,the mice in S1P antagonist and S1PR2 blocker groups exhibited reduced latency and immobility time in the water(P<0.05)and increased central zone exploration distance(P<0.05).There was reduced inflammatory exudation and neuronal damage in the ventricles,along with attenuated retention of pericytes(NG2,green)and endothelial cells(CD31,red).Serum levels of S1P,IL-6 and IFN-α were significantly decreased(P<0.05).The S1PR2 mRNA expres-sion was significantly reduced,whereas PDGFR-β mRNA expression was significantly increased(P<0.05).Protein ex-pression levels of S1P and S1PR2 were significantly decreased,while PDGFR-β and ZO-1 protein levels were significantly increased(P<0.05).CONCLUSION:Inhibition of the S1P/S1PR2 pathway attenuates neurobehavioral symptoms,re-duces blood-brain barrier permeability,and suppresses inflammation in NPSLE mice.
6.Effects and mechanism of mechanical stretch promoting proliferation and inhibiting osteogenic and adipogenic differentiation of skeletal satellite cells in mice

Objective To investigate the effect of mechanical stretch on the proliferation and differen-tiation of skeletal satellite cells in mice,and explore its mechanism.Methods The research was divid-ed into two parts.In the first part,all mice were not given osteogenic or adipogenic induction,and randomly divided into a control group,a mechanical stretch(MS)group,a solvent control group[Di-methyl sulfoxide(DMSO)+mechanical stretch],a Notch inhibitor(KY-02111)+mechanical stretch group and a Wnt inhibitor(FLI-06)+mechanical stretch group,with the aim to evaluate cell prolifera-tion and migration,and the effect of mechanical stretch on the Wnt and Notch signaling pathway.In the second part,all mice were randomly divided into 6 groups:a control group,an induced differentia-tion(ID)group,an induced differentiation+mechanical stretch(IDMS)group,an induced differentia-tion+DMSO+mechanical stretch(IDDMS)group,an induced differentiation+Notch inhibitor(KY-02111)+mechanical stretch(IDNMS)group,and an induced differentiation+Wnt inhibitor(FLI-06)+mechanical stretch(IDWMS)group.All groups except the control group were induced differenti-ation using osteoblastic or adipogenic medium,and the IDMS group was applied tensile stress to the cell basement membrane,with the IDDMS and IDNMS groups given DMSO and KY-02111 or FLI-06 with a final concentration of 5 μM into the culture medium,respectively.On the 7th and 14th days af-ter culture,cell proliferation was detected using CCK-8,while on the 14th day after culture,cell mi-gration was observed using the streak method.Alkaline phosphatase and oil red staining were per-formed on all cells.Moreover,the mRNA expression of osteogenic[alkaline phosphatase(ALP)and Runt-related transcription factor(Runx)-2]and adipogenic[fatty acid-binding protein(FABP)4 and li-poprteinlipase(LPL)]differentiation marker genes were quantitatively detected by using the real-time PCR.Meanwhile,while the expression of c-Myc and Cyclin B(CCNB)1 proteins was measured using Western blotting.Results Cell proliferation increased significantly in MS group compared with the con-trol group(P<0.01),while that of IDMMS and IDNMS groups decreased significantly compared with MS group.As to cell migration,MS group was significantly better than the control group(P<0.01),while there was a significant decrease in both IDMMS and IDNMS groups compared with MS group(P<0.01).According to ALP and oil red staining,a significant increase in ALP activity and lipid droplet secretion was observed in skeletal muscle satellite cells of ID groups compared with the control group,with the increase in IDMMS and IDNMS groups significantly greater than MS group.More-over,the results of real-time PCR showed that the expression of ALP and Runx-2mRNA in the osteo-genic induction group and the FABP4 and LPL mRNA in the adipogenic induction group increased sig-nificantly compared with the control group(P<0.05),with a significant decrease in the above measure-ments of IDMS group compared with ID group,but a significant increase in IDMMS and IDNMS groups compared with IDMS group(P<0.01).Meanwhile,according to Western blotting,the expres-sion of c-Myc and CCNB1proteins increased significantly in the skeletal muscle satellite cells of MS group compared with the control group(P<0.01),while that of IDMMS and IDNMS groups decreased significantly compared with MS group(P<0.01).Conclusion Mechanical stretch can promote the prolif-eration and migration of skeletal satellite cells in mice,but inhibit osteogenic and adipogenic differenti-ation.Its mechanism is related to the activation of Wnt and Notch signal pathways.
7.Risk prevention principle of public health data sharing and its application

Data sharing is the fundamental work for building a " Healthy China" and a technical support for improving China′s healthcare service system. It is of great significance to the government, society, and individuals. In the digital era, public health data could be referred to various data collected, connected, read, and stored on digital platforms for research, monitoring, prevention, and control of diseases, health events, and the operation of health systems. China′s public health data sharing has initially formed a development pattern of " four modernizations", providing basic conditions and practical preparations for vigorously promoting data sharing. In the process of data sharing, the risk prevention principle should be fully considered and established, including establishing a series of measures to balance legal protection and interest protection for different entities and situations, that is, data information typing to limit data access rights, building a unified public health data sharing platform, and continuously improving public health data sharing mechanisms. At the same time, appropriate data use policies and security measures should be formulated to ensure data privacy and security.
8.A multi-center study of biochemical and hotspot gene screening for neonatal genetic metabolic diseases

Objective:To investigate the efficiency of biochemical screening and hotspot gene screening in the detection of neonatal inherited metabolic diseases.Methods:This was a prospective multi-center study.The study was carried out on 21 442 neonatal samples collected from 12 hospitals in 10 provinces from November 2020 to November 2021.The results of biochemical screening and hotspot gene screening were analyzed jointly.Biochemical screening methods included glucose-6-phosphate dehydrogenase deficiency enzyme activity assay and neonatal tandem mass spectrometry.Genetic screening analysis involved 135 genes associated with 75 neonatal diseases.Results:Of all the 21 442 neonates enrolled in the study, 21 205 were subject to biochemical screening.A total of 813 cases were positive in the initial screening, and 0.45% of them (95 cases) were diagnosed after recall.All the 21 442 neonates underwent gene screening.About 168 positive cases were detected in the initial screening, and 0.73% (156 cases) of them were confirmed finally.Biochemical and genetic screening improved the detection sensitivity of such diseases as primary carnitine deficiency, neonatal intrahepatic cholestasis caused by citrin deficiency, and 2-methylbutyrylglycinemia.Moreover, biochemical and genetic screening enabled the detection of more diseases, including the common single-gene genetic diseases such as thalassemia and Wilson disease.Conclusions:In neonatal screening, the combination of biochemical screening and gene screening expands the number of diseases detected and improve screening efficiency.
9.Effect of a space stabilizing device on the capacity for osteogenesis in the bone-grafted area

Objective:
To study the effect of a space stabilizing device on the capacity for osteogenesis in the bone-grafted area.
Methods:
Patients with anterior teeth defects as well as labial alveolar ridge defects were randomly divided into two groups: the experimental group was treated using a space stabilization device after guided bone regeneration whereas the control group received regular guided bone regeneration. The CBCT data was analyzed prior to, immediately and 6 months after procedure to compare the osteogenesis with or without the device.
Results:
Seventeen bone-grafted sites were examined(7 in the experimental group and 10 in the control group). At 2 mm and 4 mm below the alveolar ridge, the horizontal bone resorption rate in the experimental group was significantly lower than the corresponding level in the control group(P=0.001). The horizontal bone resorption rate at 2 mm below the alveolar ridge of each group was significantly higher than that at 4 mm below the alveolar ridge in corresponding groups(P=0.003).
Conclusion
The space stabilizing device used in this study has a positive clinical impact on preserving space stability and minimizing bone resorption.

Colorectal cancer is one of the most common malignant tumors of digestive tract. In 2020, 1.93 million new cases of colorectal cancer were diagnosed globally, ranking third in the global incidence spectrum, and 930 000 new deaths were reported, ranking second in the global cause of death spectrum. Meanwhile, the medical cost of metastatic colorectal cancer is the highest among all stages. A large number of studies have demonstrated that traditional Chinese medicine(TCM) treatment can bring clinical benefits to patients with metastatic colorectal cancer with unique efficacy. In order to further standardize the TCM diagnosis and treatment for metastatic colorectal cancer and improve the level of TCM diagnosis and treatment, Xiyuan Hospital, China Academy of Chinese Medical Sciences, together with other relevant units in China, according to the guideline development process of the World Health Organization Handbook for Guideline Development and the relevant requirements of the Clinical Evidence Grading Criteria on TCM Based on Evidence Body, the Regulations for Group Standards of China Association of Chinese Medicine and others, combined with the characteristics of TCM diagnosis and treatment and the actual situation in China, the Guidelines for TCM Diagnosis and Treatment of Metastatic Colorectal Cancer was developed in accordance with the Catalogue of TCM Diagnosis and Treatment Plans for 105 Diseases in 24 Specialties issued by Department of Medical Administration of National Administration of TCM.
