1.Research on the extraction process of standard decoction of Longan seed based on antioxidant activity

		                        			OBJECTIVE To optimize the extraction process of standard decoction of Longan seed. METHODS The amount of water added, extraction time and extraction times as the influencing factors, using the extract yield, gallic acid content, corilagin content, ellagic acid content, half clearance concentration (IC50) of 1,1-diphenyl-2-picryl-hydrazyl (DPPH) free radical and IC50 of 2,2′-azino-bis (3-ethylbenzothiazoline-6-sulfonic acid) ammonium salt (ABTS) free radical as the evaluation indexes, the analytic hierarchy process (AHP), criteria importarue though inter-criteria correlation (CRITIC) and AHP-CRITIC method were used to calculate the weight values of each index; the extraction process of Longan seed standard decoction was optimized by single factor test combined with orthogonal experiment; validation test was also performed. RESULTS According to the AHP-CRITIC method, the weights of the above indicators were determined to be 9.224%, 11.784%, 19.320%, 11.206%, 20.597%, 27.869%, respectively. The best process of standard decoction of longan seed was to add 14 times water for the first extraction and extract for 30 minutes; and 12 times water for the second and third extractions, and extract for 20 minutes. Average comprehensive score of the 3 times validation experiments was 97.96 and the RSD was 0.97% (n=3). CONCLUSIONS The process is simple to operate, stable and feasible, which can provide a experimental basis for the further development and utilization of standard decoction of longan seed.
		                        		
		                        		
		                        		
		                        	
2.Network meta-analysis of Insulin degludec and liraglutide injection versus Insulin glargine and lixisenatide injection in the treatment of type 2 diabetes mellitus

		                        			OBJECTIVE To systematically evaluate the efficacy and safety of Insulin degludec and liraglutide injection (IDegLira) and Insulin glargine and lixisenatide injection(iGlarLixi) in the treatment of type 2 diabetes mellitus(T2DM), and provide an evidence-based basis for the clinical treatment of T2DM. METHODS Computerized searches of PubMed, Embase, the Cochrane Library, CNKI, Wanfang data and VIP were conducted with a time frame from the inception to August 2024. Randomized controlled trials(RCTs) were rigorously screened according to inclusion and exclusion criteria, from which information was extracted and included studies were evaluated for risk of bias. Network meta-analysis was performed using Stata 14.0 software. RESULTS A total of 15 RCTs, including 9 513 patients, were included, involving four treatment regimens: IDegLira, iGlarLixi, insulin degludec(IDeg), and insulin glargine(iGlar). The differences between IDegLira and iGlarLixi were not statistically significant(P>0.05) for the outcome indexes of glycosylated hemoglobin(HbA1c), fasting blood glucose, body weight, and the incidence of adverse events(P>0.05); for the outcome index of the incidence of hypoglycemic events, IDegLira was significantly superior to iGlarLixi [OR=0.41,95%CI(0.18,0.91),P<0.05]. Surface under the cumulative ranking curve(SUCRA) results showed that iGlarLixi(84.5%)>IDegLira(81.7%) in lowering HbA1c; IDegLira(71.3%)>iGlarLixi(20.0%) in lowering fasting blood glucose; IDegLira(90.7%)>iGlarLixi(61.8%) in lowering body weight; IDegLira(95.5%)>iGlarLixi(9.7%) in reducing the incidence of hypoglycemic events; and IDegLira(27.1%)>iGlarLixi(14.5%) in reducing the incidence of adverse events. CONCLUSIONS iGlarLixi has better therapeutic efficacy in reducing HbA1c; IDegLira has better therapeutic efficacy in reducing fasting blood glucose and body weight. 3.Analysis of subjective visual vertical test results in patients with benign paroxysmal positional vertigo at different head deflection angles

		                        			Objective To analyze the clinical significance of subjective visual vertical (SVV) tests at different head deflection angles in assessing utricle function in patients with benign paroxysmal positional vertigo (BPPV). Methods A total of 61 BPPV patients who were treated at the Hearing Impairment and Vertigo Diagnosis and Treatment Center of Otolaryngology Head and Neck Surgery, Xinhua Hospital, Shanghai Jiao Tong University School of Medicine from August 2022 to May 2023 were retrospectively included, and 29 healthy adults were selected as controls. SVV tests were performed on all research subjects at different head deflection angles: upright head (0°), left head 45° (L45°), right head 45° (R45°). The test results between the two groups were compared. Results SVV absolute value at R45° in BPPV group was lower than that in the control group (P=0.003); there was no significant difference in SVV values at 0° and L45° between the two groups. There was no statistical difference in SVV values at different head deflection angles between the control group and the left BPPV group. SVV absolute value at R45° in right BPPV group was lower than that in the control group (P<0.001); there was no statistical difference in SVV values at 0° and L45° between the two groups. Conclusions SVV test can provide subjective information about the utricle, and SVV tests at different head deflection angles can fine-tune evaluate the function of the utricle in BPPV patients.
		                        		
		                        		
		                        		
		                        	
4.Potassium dehydroandrographolide succinate regulates the MyD88/CDH13 signaling pathway to enhance vascular injury-induced pathological vascular remodeling.

		                        			Pathological vascular remodeling is a hallmark of various vascular diseases. Previous research has established the significance of andrographolide in maintaining gastric vascular homeostasis and its pivotal role in modulating endothelial barrier dysfunction, which leads to pathological vascular remodeling. Potassium dehydroandrographolide succinate (PDA), a derivative of andrographolide, has been clinically utilized in the treatment of inflammatory diseases precipitated by viral infections. This study investigates the potential of PDA in regulating pathological vascular remodeling. The effect of PDA on vascular remodeling was assessed through the complete ligation of the carotid artery in C57BL/6 mice. Experimental approaches, including rat aortic primary smooth muscle cell culture, flow cytometry, bromodeoxyuridine (BrdU) incorporation assay, Boyden chamber cell migration assay, spheroid sprouting assay, and Matrigel-based tube formation assay, were employed to evaluate the influence of PDA on the proliferation and motility of smooth muscle cells (SMCs). Molecular docking simulations and co-immunoprecipitation assays were conducted to examine protein interactions. The results revealed that PDA exacerbates vascular injury-induced pathological remodeling, as evidenced by enhanced neointima formation. PDA treatment significantly increased the proliferation and migration of SMCs. Further mechanistic studies disclosed that PDA upregulated myeloid differentiation factor 88 (MyD88) expression in SMCs and interacted with T-cadherin (CDH13). This interaction augmented proliferation, migration, and extracellular matrix deposition, culminating in pathological vascular remodeling. Our findings underscore the critical role of PDA in the regulation of pathological vascular remodeling, mediated through the MyD88/CDH13 signaling pathway.

5.Therapeutic effect of PD-1 monoclonal antibody combined with cisplatin or gemcitabine chemotherapy in a mouse xenograft model with KRAS mutant non-small cell lung cancer A549 cells

		                        			[摘  要]  目的:探讨程序性死亡受体-1(PD-1)单抗联合顺铂或吉西他滨在KRAS基因突变非小细胞肺癌(NSCLC)A549细胞移植瘤小鼠模型治疗中的作用。方法:构建免疫系统-肿瘤双人源化A549细胞小鼠移植瘤模型,将60只小鼠按随机数字表法分成6组(10只/组),分别为对照组(200 μL/kg PBS)、PD-1单抗组(20 mg/kg PD-1单抗)、顺铂组(3 mg/kg顺铂)、PD-1单抗+顺铂组(20 mg/kg PD-1单抗+3 mg/kg顺铂)、吉西他滨组(30 mg/kg吉西他滨)和PD-1单抗+吉西他滨组(20 mg/kg PD-1单抗+30 mg/kg吉西他滨)。TUNEL和DAPI双染色法检测移植瘤组织中细胞凋亡水平,测量移植瘤体积和质量并计算肿瘤生长抑制率,免疫组化法检测移植瘤微血管密度(MVD)。结果:成功构建免疫系统-肿瘤双人源化NSCLC A549细胞小鼠移植瘤模型,PD-1单抗+顺铂组移植瘤的细胞凋亡率、肿瘤生长抑制率均最高,移植瘤体积、质量和MVD均最小,与其他5组小鼠比较差异均有统计学意义(均P<0.05)。结论:顺铂与PD-1单抗具有协同活性,而吉西他滨拮抗PD-1单抗的治疗作用。提示PD-1单抗联合顺铂对KRAS突变NSCLC A549细胞移植瘤小鼠的疗效更好。

		                        			Abstract: Inflammatory bowel disease (IBD), whose pathogenesis remains elusive, is a group of autoimmune diseases characterized by chronic, progressive, and lifelong inflammation of the digestive tract. The pathogenesis of IBD remains elusive. Although a number of drugs have been developed to treat IBD, their effects are merely anti-inflammatory. In addition, current treatments for IBD are easily susceptible to resistance in clinical practice. Mesenchymal stem cells (MSCs) have been reported to have the ability to migrate to the site of inflammation, with potent immunoregulatory effects, and to rebalance the immune microenvironment and restore the integrity of the epithelial barrier with significant value of application, particularly for patients who are refractory to classic medicines. In this paper, we reviewed the clinical applications, mechanisms and engineerable properties of MSC products and their exosomes to provide some reference for the use of MSCs and their exosomes in the treatment of IBD.
		                        		
		                        		
		                        		
		                        	
7.The feasibility and safety of simultaneous bilateral adrenal vein sampling using 4F-MPA1 catheter via right elbow vein access:preliminary results in 51 patients

		                        			Objective To discuss the feasibility and safety of simultaneous bilateral adrenal vein sampling(AVS)using two 4F-MPA1 catheters via right elbow vein access.Methods A total of 51 consecutive patients with primary aldosteronism,who received simultaneous bilateral AVS using two 4F-MPA1 catheters(one of the two catheters was shaped into pig tail figure)via right elbow vein access at Xiangyang Municipal Central Hospital between October 2021 and October 2022,were enrolled in this study.The used catheter,the success rate of simultaneous bilateral AVS,and the incidence of complications rate were calculated.Results The 4F-MPA1 catheter was used for all of the right AVS,while a specially shaped 4F-MPA1 catheter was used for the main trunk vein AVS of the left adrenal gland and the central vein AVS of the left adrenal gland.The success rate of simultaneous bilateral AVS was 92.2%(47/51).Adrenal hematoma occurred in one patient(1.96%).Conclusion The technique of simultaneous bilateral AVS using two 4F-MPA1 catheters via right elbow vein access is simple to operate,less traumatic,and clinically safe and feasible.However,due to the small sample used in this study,the clinical value of this technique still needs further investigation and verification.

		                        			Objective The performance characteristics of statistical quality control procedures in Westgard sigma rule are analyzed by using the power function graph.Methods According to the drawing principle of power function graph,the internal quality control program design software of clinical laboratory quantitative measurement was developed.and the Westgard sigma rule quality control procedures were simulated with power function graph.The performance of the quality control procedures recommended by Westgard sigma was analyzed by combining different sigma quality levels.Results The rules recommended by Westgard sigma were relatively simple rules that can meet the requirements of error detection probability and false rejection probability.Conclusion The internal quality control procedure design software for quantitative clinical testing has verified the performance of Westgard sigma recommendation rules.The Westgard sigma rule diagram is intuitive and convenient,the laboratory can use the Westgard sigma recommendation rules for internal quality control.
		                        		
		                        		
		                        		
		                        	
9.Transcriptomic analysis of chondroprogenitors in response to flow fluid shear stress

		                        			Objective:To investigate the response of mandibular condylar chondroprogenitors to flow fluid shear stress(FFSS).Methods:Chondroprogenitors were in vitro cultured and stimulated with FFSS that can cause cell degeneration,and treated with sec-ond-generation high-throughput RNA sequencing.Differential gene expression was screened using DESeq2 software for gene ontology(GO)functional enrichment analysis,kyoto encyclopedia of genes and genomes(KEGG)pathway enrichment analysis and protein-protein interaction(PPI)network analysis.qRT-PCR was performed to validate the core genes screened by PPI.Results:A total of 1996 differentially expressed genes were obtained,mainly including inflammatory response and cell cycle related molecules.Among them,Actal,Atf3,Ccl2,116,Nfkbia,Ret and Vcaml were identified as the core genes.Conclusion:FFSS stimulation affects chondroprogenitor function by acting on inflammatory responses and cell cycle-related signaling pathways in chondroprogenitors.
		                        		
		                        		
		                        		
		                        	
10.Design and application of an enhanced postoperative recovery checklist for perioperative patients after laryngeal cancer surgery

		                        			Objective To design an enhanced postoperative recovery checklist for the patients who had surgery for laryngeal cancer,hence to instruct the patient with a standard procedure in rehabilitation.Methods A total of 72 patients who had surgery for laryngeal cancer in our department from April 2020 and April 2021 were enrolled as study subjects for a controlled study before and after surgery of different patients.A total of 37 patients who had surgery between April and September 2020 were assigned in the control group,while another 37 patients who had the surgery between October 2020 and April 2021 were assigned in the trial group.Patients in the control group were managed with a routine enhanced perioperative recovery plan,and those in the trial group were managed with the checklist,which was drafted by modification and improvement of the routine enhanced perioperative recovery plan through literature reviews and expert consultations.The two groups were compared in terms of the standardised rate of enhanced recovery compliance,medical staff satisfaction,average hospital-stay and patient satisfaction.Results The trial group showed significant improvement in comparison with the control group in terms of the standardised rate of enhanced recovery compliance(93.28%versus 84.08%),score of satisfaction level[(95.57±2.25)versus(92.60±2.49)],score of doctor satisfaction[(35.75±0.89)versus(32.88±1.50)],and the score of nurse satisfaction[(35.45±1.10)versus(33.00±1.30)](all P<0.05).The average hospital-stay in the trial group was significantly shorter than that in the control group[(9.89±3.32)days versus(12.60±4.33)days](P<0.05).Conclusion The designed checklist can improve the standardised rate of enhanced recovery compliance,the satisfaction of both medical staff and patients and facilitate the recovery of patient.
