2.SE-iFISH used in the detection of circulating tumor cells in patients of hepatocellular carcinoma

		                        			Objective To investigate the correlation between patients clinical characteristics and the number and subtype of circulating tumor cells (CTCs) from peripheral blood of perioperative hepatocellular carcinoma (HCC)patients by SE-iFISH.Methods 20 HCC patients undergoing radical resection were enrolled from June 2015 to June 2016.The SE-iFISH technique was used to separate and identify circulating tumor cells.The pathology and clinical data were used to evaluate patients survival in combination with CTCs characteristics.Results A total of 347 CTCs were detected,of which 114 were triploid,64 were tetraploid,and 165 were pentaploid.The number of preoperative CTCs and the number of preoperative triploids was significantly correlated with the presence of vascular tumor emboli (Z1 =-2.080,P =0.037,Z3 =-2.321,P =0.020) and TNM staging(Z2 =-2.148,P =0.032,Z4 =-2.526,P =0.012).Postoperative patients disease-free survival in high CTCs detection group was significantly shorter than that of CTCs low expression group (x2 1 =7.486,P =0.006,x22 =12.056,P =0.001).Conclusion Detection of the number and the specific subtypes of CTCs with SE-iFISH strategy in patients with HCC help predict treatment efficacy and prognosis.
		                        		
		                        		
		                        		
		                        	
3.Exploration of establishing a tree shrew model of chronic gastrointestinal mucosal injury

		                        			Objective To explore the feasibility of establishing a tree shrew model of chronic gastrointestinal mucosal injury. Methods A total of 12 adult male tree shrews were randomly divided into 3 groups. The experimental groups 1 and 2 were administered with intraperitoneal injection of 2 mg/(kg·d)and 1 mg/(kg·d)of 1-methyl-4-phenyl-1,2, 3,6-tetrahydropyridine(MPTP)once every day for 56 days, respectively. The control group was given the same volume of sterile saline at the corresponding time points. Changes in the body weight of the tree shrews were observed. The contents of dopamine in the cerebrospinal fluid were detected. Gastrointestinal morphology was observed by stereoscope and histopathological changes of the gastrointestinal mucosa were examined by HE staining. Results The body weight and the contents of dopamine in the cerebrospinal fluid of the tree shrews in the model group were significantly decreased(P< 0.05 for both). Pathological changes to some extent of the gastric antrum, the gastric body and the duodenum were observed, without obvious differences between the 2 mg/kg group and the 1 mg/kg group. No obvious changes were found in the control group. Conclusions Long-term intraperitoneal injection with a low dose of MPTP is a feasible method for the establishment of a tree shrew model of chronic gastrointestinal mucosal injury. 4.The value of bone mineral density and bone resorption markers around the mini-screw implant in the orthodontic treatment

		                        			Objective To investigate the value of bone mineral density and bone resorption markers around the mini-screw implant in the orthodontic treatment.Methods 178 orthodontic patients were prospectively collected.According to the organization condition around the mini-screw implant loaded,all patients were assigned into stable group (n =160) and loose group (n =18).The levels of interleukin-1 β (IL-1 β) and matrix metalloproteinase-9 of the liquid around the mini-screw implant were detected at 1,2 and 3 months after loading.Moreover,the bone miner al density around the mini-screw implant before and after loading were studied as well.Results Compared with the stable group,the loose groups at 1,2 and 3 months after loading got significantly higher levels of IL-1β [(35.48 ± 4.39)pg/mL vs.(29.48 ±3.92)pg/mL,t =3.348,P =0.004;(41.45 ±5.39)pg/mL vs.(26.29 ±4.12)pg/mL,t =6.493,P =0.000;(54.39 ± 12.82) pg/mL vs.(23.58 ± 3.62) pg/mL,t =11.589,P =0.000].Compared with the stable group,the loose group at 1,2 and 3 months after loading got significantly higher levels of MMP-9[(5.68 ± 3.54) ng/mL vs.(1.74 ± 0.88) ng/mL,t =8.496,P =0.000;(6.84 ± 2.82) ng/mL vs.(1.25 ± 0.62) ng/mL,t =9.835,P =0.000;(9.84 ± 4.39) ng/mL vs.(1.21 ± 0.58) ng/mL,t =12.548,P =0.000].Compared with the sta ble group,the loose group got significantly lower levels of bone mineral density of maxillary bones around the miniscrew implant before and at 3 months after loading[(620.48 ±67.82) HU vs.(694.39 ±84.58)HU,t =2.459,P =0.015;(597.39 ± 58.93) HU vs.(693.59 ± 83.29) HU,t =2.909,P =0.008].Conclusion The decrease of bone mineral density around the mini-screw implant is related to the loosening of the mini-screw implant,and IL-1 β and MMP-9 of the surrounding fluid can reflect the peripheral inflammation.
		                        		
		                        		
		                        		
		                        	
5.Relationship between cytotoxic associated protein A, cytotoxic associated protein E, vacuolating cytotoxin A genotypes of Helicobacter pylori and upper digestive tract diseases

		                        			Objective To explore the relationship between cytotoxin associated protein A (cagA), cytotoxin associated protein E(cage), vacuolating cytotoxin A (vacA) genotypes of Helicobacter pylori (Hp) and the upper digestive tract diseases. Methods A total of 112 patients with upper gastrointestinal diseases were selected. The cagA, cagE, vacA genotypes of Hp strains in gastric mucosa of the patients were detected and analyzed. Results All Hp strains in the gastric mucosa samples of patients were found with both cagA gene and cagE gene expression, and the positive rate was 100%. The positive rates of vacA s1/m2 genotype in the patients were the highest with 54.1% and 60.5% respectively, followed by the vacA s1/mlb genotype and vacA s1/m- phenotypes, and the positive rates were from 13.2% to 21.1%. The differences of positive rates of each genotypes in Hp genes between the patients with peptic ulcer and chronic gastritis were not significant (P>0.05). Conclusion Hp strains in gastric mucosa patients with upper gastrointestinal diseases show the advantage expressions of cagA, cagE, vacA s1/m2 subtypes, which refers that such virulence phenotypes may play important roles in initiation and promotion process of upper gastrointestinal diseases by Hp, but the correlation with the type of upper digestive tract diseases is not proved.

7.Effective components of Semen Ziziphi Spinosae for sedative-hypnotic based on receptor ligand binding assay

		                        			Aim To determine the effective compo-nents of Semen Ziziphi Spinosae for sedative-hypnotic and its mechanism. Methods The extraction of Se-men Ziziphi Spinosae and the rat brain homogenates were prepared. High concentrations of Diazepam com-petitively replaced the ligand compounds of Semen Ziz-iphi Spinosae combining BDZ receptor in brain tissue, and all the compounds with sedative and hypnotic effects were collected and identified by HPLC and LC-MS technique, as the compounds extracted from the brain tissue were administered with Semen Ziziphi Spi-nosae. The brain tissue was administered with Diaze-pam, and with Semen Ziziphi Spinosae and Diazepam. Results The HPLC chromatograms show that the peak time of BDZ receptor ligand compounds was 2. 71 min and 46. 87min, when compared with Diazepam. And the LC-MS chromatograms display the relative molecu-lar weight of the ligand compounds was 274. 28 m/z, 453. 34 m/z,496. 34 m/z and 608. 38 m/z respective-ly. According to the fingerprint of Semen Ziziphi Spi-nosae, these compounds may be fatty acid substances and lupine pill triterpene compounds. Conclusions On the basis of the principle of receptor ligand bind-ing, we established a way to quickly analyze and iden-tify the role of natural products in the same drug target compounds. The method not only can clearly define the effective components of natural products, but also clar-ify the mechanism of action of the compounds. The ac-tive ingredient of calm hypnosis in Semen Ziziphi Spi-nosae may be fatty acid substances Palmitic acid ( C16 H32 O2 ) and lupine pill triterpene compounds Alphitolic acid( C30 H48 O4 ) and Spinosin( C28 H32 O15 ) . They exert their sedative and hypnotic effects by combining with BDZ receptor, and the research has laid a theoretical foundation for the further study about mechanism of Se-men Ziziphi Spinosae.
		                        		
		                        		
		                        		
		                        	
8.Clinical application of precise liver surgery techniques for donor hepatectomy in living donor liver transplantation.

OBJECTIVETo evaluate the effect of techniques of precise liver surgery for donor hepatectomy in living donor liver transplantation.
METHODSEighty-nine donors aged from 19 to 57 years were performed by the same surgical team from June 2006 to December 2013 in Chinese People's Liberation Army General Hospital.Individualized surgical program were developed according to preoperative imaging examination and hepatic functional reserve examination. The evaluation included liver function, liver volume, vascular anatomy and bile duct anatomy. According to the results after the operation, preoperative evaluation accuracy, postoperative donor liver function and postoperative complications were analyzed. ANOVA analysis was used to compare the difference of graft volume by two-dimensional, three-dimensional calculation method and actual postoperative graft weight. Pearson correlation test and linear regression analysis were used to verify the correlation between the estimated graft volume each method and actual graft postoperative weight.
RESULTSAll the 89 cases operation protocol as following, there were 5 cases with left lateral lobe graft, 10 cases with left lobe liver graft, 74 cases with right lobe graft. There were 59 cases with middle hepatic vein (MHV) harvested, and 30 cases without MHV. The mean graft volume by two-dimensional, three-dimensional calculation method and actual postoperative graft weight were (656.2±134.1) ml, (631.7±143.2) ml and (614.5±137.7) ml respectively. ANOVA analysis results showed that there were no statistically significant difference in the three methods (P>0.05). Compared to the actual postoperative graft weight, the average error rate of the two methods were 7.9% and 5.3% respectively. Pearson correlation test showed the graft volume calculated by two-dimensional and three-dimensional methods had a significantly positive correlation with actual graft weight (r=0.821, 0.890, P<0.01) and linear regression analyze showed the R2 were 0.674 and 0.792, respectively. The accuracy rate of preoperative evaluation about portal vein, hepatic vein, hepatic artery and bile duct were 100%, 100%, 97.8% and 95.5%, respectively. The preoperative plan and postoperative practical scheme coincidence rate was 95.5%. Overall donor complication rate was 7.4%. All donors were alive. Sixteen donors received right lobe hepatectomy with gallbladder preserved had a good liver function and gallbladder function.
CONCLUSIONThrough the precise preoperative evaluation, surgical planning, fine operation and excellent postoperative management, precise liver surgery technique can ensure the safety of donor in living donor liver transplantation.

9.Liver transplantation for irresectable hilar cholangiocarcinoma.

OBJECTIVETo evaluate the outcome of patients with irresectable hilar cholangiocarcinoma undergoing orthotopic liver transplantation (OLT) and to identify the prognostic factors that could influence survival.
METHODSThe data of 18 patients who underwent OLT for irresectable hilar cholangiocarcinoma between June 2003 and October 2010 were analyzed retrospectively. There were 12 male and 6 female cases with median of 52 years(range from 34 to 65 years).Fifteen patients underwent modified piggyback liver transplantation, 2 patients underwent classical orthotopic liver transplantation and 1 patient underwent living donor liver transplantation. Data were evaluated regarding tumor size, pathologic stage, overall survival, recurrence rates and prognostic factors.
RESULTSOLT with lymphadenectomy was received by 18 patients with hilar cholangiocarcinoma. Median time until tumor recurrence was 20.5 months(range from 6.0 to 33.0 months). Seventeen patients died during follow-up.Of these, 14 patients died from recurrent or metastatic diseases, 2 patients died from multiple organ dysfunction syndrome during peri-operative period, and one patient died from other cause. The median survival time was 29.5 months(range from 3.0 to 84.0 months). The overall survival rate and recurrence-free survival rate at 1, 3, and 5 year were 16/18, 8/18, 1/18 and 13/18, 2/18, 1/18, respectively.Lymph node metastases had a statistically significant negative impact on overall survival. The 1, 3, and 5 year survival rates were 6/7, 1/7,0 and 10/11, 7/11, 1/11 (P < 0.05) in lymph node-positive and lymph node-negative patients.
CONCLUSIONSAcceptable survival rates can be achieved by OLT for irresectable hilar cholangiocarcinoma without lymph node metastases.Strict patient selection plus multimodal chemoradiation therapy prior to OLT are recommend for patients with lymph node metastases.

10.Optimization of biocontrol agent Burkholderia pyrrocinia strain JK-SH007 fermentation by response surface methodology.

		                        			In order to improve ferment efficiency of biocontrol agent Burkholderia pyrrocinia JK-SH007, the fermentation conditions of this strain were optimized. The optimal fermentation conditions were corn steep liquor (13.88 g/L) and glucose (3.37 g/L) by screening test, steepest ascent experiments and response surface analysis. The results showed that the cell density of JK-SH007 (1.18 x 10(9) CFU/mL) increased 1.35 times than before, and there was a 28.84% increase in antifungal activity.
