1.Meta-analysis of the efficacy and safety of PD-1/PD-L1 inhibitors combined with bevacizumab in the treatment of advanced non-small cell lung cancer

OBJECTIVE To evaluate the efficacy and safety of programmed death-1/programmed death-ligand 1 (PD-1/PD-L1) inhibitors combined with bevacizumab in the treatment of advanced non-small cell lung cancer (NSCLC) based on platinum- containing dual therapy. METHODS Retrieved from CNKI, Wanfang, VIP, Web of Science, PubMed and other Chinese and English databases, cohort studies or randomized controlled trial studies on the treatment of advanced NSCLC with platinum- containing double agents in combination with PD-1/PD-L1 inhibitors and bevacizumab (trial group) versus platinum-containing double agents with or without PD-1/PD-L1 inhibitor or bevacizumab (control group) were collected from the inception to April 25, 2024. After screening literature, extracting data and evaluating quality, meta-analysis and sensitivity analysis were performed by using RevMan 5.4.1 software. RESULTS A total of 15 pieces of literature were included, involving 13 clinical studies with a total of 3 282 patients. Compared with the control group, partial response rate [RR=0.75,95%CI(0.68,0.82),P<0.000 01], complete response rate [RR=0.47,95%CI(0.29,0.76),P=0.002], progressive disease rate [RR=1.23,95%CI(1.11,1.37),P<0.000 1], objective response rate (ORR) [RR=0.72,95%CI(0.67,0.79),P<0.000 01] and disease control rate (DCR) [RR=0.85, 95%CI (0.77,0.95),P=0.003] were higher in the trial group. There was no statistically significant difference in the stable disease rate [RR=1.25, 95%CI (0.86, 1.83), P=0.25] or overall adverse drug reaction incidence rate [RR=0.95, 95%CI (0.90, 1.00), P= 0.07] between the two groups of patients. Sensitivity analyses showed robust and reliable results for all outcome indicators. CONCLUSIONS PD-1/PD-L1 inhibitors combined with bevacizumab based on platinum-containing dual therapy in the treatment of advanced NSCLC can improve patients’ clinical benefits, such as ORR and DCR, without increasing the risk of adverse drug reaction.
2.Signal intensity changes of dentate nucleus on plain MR T1WI in nasopharyngeal carcinoma patients after radiotherapy and multiple injections of gadolinium-base contrast agent

Objective To observe changes of plain MR T1WI signal intensity of dentate nucleus in nasopharyngeal carcinoma patients after radiotherapy and multiple times of intravenous injection of gadolinium-based contrast agent(GBCA).Methods Fifty patients with pathologically confirmed nasopharyngeal carcinoma and received intensity-modulated radiotherapy were retrospectively enrolled as the nasopharyngeal carcinoma group,and 50 patients with other malignant tumors and without history of brain radiotherapy were retrospectively enrolled as the control group.All patients received yearly GBCA enhanced MR examinations for the nasopharynx or the head.T1WI signal intensities of the dentate nucleus and the pons on same plane were measured based on images in the year of confirmed diagnosis(recorded as the first year)and in the second to the fifth years.T1WI signal intensity ratio of year i(ranging from 1 to 5)was calculated with values of dentate nucleus divided by values of the pons(△SIi),while the percentage of relative changes of year j(ranging from 2 to 51 was calculated with △SIj compared to △SIi(Rchangej).The values of these two parameters were compared,and the correlation of △SI and GBCA injection year-time was evaluated within each group.Results No significant difference of gender,age nor △SI1 was found between groups(all P>0.05).The second to the fifth year △SI and Rchange in nasopharyngeal carcinoma group were all higher than those in control group(all P<0.05).Within both groups,△SI was positively correlated with GBCA injection year-time(both P<0.05).Conclusion Patients with nasopharyngeal carcinoma who underwent radiotherapy and multiple times of intravenous injection of GBCA tended to be found with gradually worsening GBCA deposition in dentate nucleus,for which radiotherapy might be a risk factor.
3.Clinical application of modified radical neck dissection by gasless unilateral axillary approach in papillary thyroid cancer

Objective:To investigate the effectiveness, safety, and advantages of modified radical neck dissection by gasless unilateral axillary approach (GUA-MRND) in the surgical management of selected patients with papillary thyroid cancer.Methods:We retrospectively analyzed patients with papillary thyroid cancer who underwent GUA-MRND (endoscopic group, n=16) versus unilateral open modified radical neck dissection (MRND) (open group, n=32) during the period from Jan. 2019 to Jun. 2021, including the differences in surgical efficiency, complication rate, and incisional satisfaction.Results:Compared MRND with GUA-MRND, the patients were younger ( P<0.05) , operative time and postoperative drainage anterior ( P<0.01) were slightly inferior in the latter, but it had obvious advantages in cervical swallowing discomfort and incision satisfaction evaluation ( P<0.05) . There was no significant difference in the incidence of temporary recurrent laryngeal nerve injury, intraoperative and postoperative bleeding, hematoma, infection, lymphatic or chylous leakage and supraclavicular numbness after surgery ( P>0.05) . The number of dissected lymph nodes in area II in the GUA-MRND was lower ( P<0.05) , but it was significantly higher ( P<0.01) in area III. And the average regional cleaning efficiency in the GUA-MRND was level Ⅲ (35.5%) , level Ⅵ (28.59%) , level Ⅳ (23.21%) , level Ⅱ (7.18%) and level Ⅴ (7.12%) , suggested that GUA-MRND had higher efficacy for level III, level Ⅵ and Level IV. Conclusion:GUA-MRND is safe, effective, and has high cosmetic satisfaction in the treatment of selected patients with lateral cervical lymph node metastases from papillary thyroid cancer.
4.Anti-tumor Effect of Alkaloids of Chinese Medicine: A Review

Malignant tumors, with the increasing crude morbidity and mortality year by year, have become the major diseases threatening human health. The conventional therapeutic drugs against tumors have serious adverse reactions, which can cause a heavy burden on patients. The active components of Chinese medicine can effectively inhibit tumor growth, improve the quality of life of patients, and have few toxic and side effects. Alkaloids of Chinese medicine are natural organic compounds widely existing in a variety of Chinese herbal medicines. In recent years, they have attracted more and more attention because of their anti-tumor effect. The anti-tumor mechanisms of alkaloids of Chinese medicine mainly include the induction of apoptosis, inhibition of tumor cell migration and invasion, suppression of proliferation, induction of autophagy of tumor cells, cell cycle arrest, inhibition of tumor angiogenesis, regulation of microRNA, and modulation of immunity. In addition, Chinese medicine alkaloids can also reverse tumor drug resistance and reduce the stemness of tumor stem cells. Alkaloids of Chinese medicine can regulate the phosphatidylinositol 3-kinase/protein kinase B (PI3K/Akt), c-Jun N-terminal kinase (JNK), p38 mitogen-activated protein kinase (p38 MAPK), mammalian target of rapamycin (mTOR), Notch, Hedgehog, Wnt/β-catenin, and other signaling pathways to participate in the processes of tumor proliferation, invasion and metastasis, autophagy and apoptosis, and affect the occurrence and development of tumors in multiple links and ways. The derivatives and nano-preparations of alkaloids can improve the solubility, utilization, and anti-tumor activity of alkaloids, bringing a broader prospect for the clinical application of alkaloids. This review summarized the recent anti-tumor research on alkaloids, their representative derivatives, and nano-preparations to provide references for the in-depth research on the anti-tumor effect of alkaloids.
5.Herbal textural of Taraxaci Herba

There are many kinds o f Taraxacum plants,which are widely distributed . The dried whole plants of many kinds of plants can be used as medicinal materials ,and the origin is confused . This study finds that Taraxaci Herba first appears as “Pugoncao”in the Liu Juanzi Guiyifang of Jin Dynasty after consulting ancient herbal books ,medical books ,prescription books and modern literature . According to characteristics ,morphology,color and sound of the plant ,it has “Huanghualang”, “Gounoucao”,“Fugongying”,“Pugongying”,“Huanghuadiding”and other aliases . Taraxaci Herba originates from the dried whole plant of Compositae family Taraxacum mongolicum Hand.-Mazz.,T. borealisinense Kitam. or several plants of the same genus . Similar to it ,Sonchus oleraceus Linn.(also known as “Kujucai”)cannot replace Taraxaci Herba as medicine . Because of its characteristics of rapid reproduction and vigorous growth ,Taraxaci Herba is widely distributed in most areas of China .“Absolutely no in Lingnan ”stated in Bencao Gangmu is inconsistent with the actual situation after consulting the relevant works . Ancient physicians take growth height of Taraxaci Herba as their quality standard . Now,those with many leaves ,gray green color and long roots are preferred . Ancient doctors pointed out that Taraxaci Herba were mostly harvested in April and May of the lunar calendar , and modern research believes that it should be harvested in the near flowering period . In ancient times ,Taraxaci Herba is processed by many methods ,such as purification ,charcoal-frying,honey-frying and wine -frying. In modern times ,purification is the main method. This paper provides theoretical support for the value development and rational utilization of Taraxaci Herba by systematically researching its name ,origin,place of production ,harvesting and processing .
6. Clinical characteristics and standardized treatment of restless legs syndrome

Restless legs syndrome is a common motor-sensory disease of the nervous system. The main manifestations are an urge to move the legs, usually accompanied by or thought to be caused by uncomfortable and unpleasant sensations in the legs. These symptoms may begin or worsen during periods of rest or inactivity such as lying down or sitting and be partially or totally relieved by movement, such as walking or stretching, at least as long as the activity continues. This article will systematically review the pathogenesis, clinical manifestations, diagnostic criteria, differential diagnosis and treatment of restless legs syndrome.
7.The treatment of butterfly-shape ear combine with cup ear

Objective:To investigate the surgical regimen of butterfly-shape ear combine with cup ear and restore the appearance of auricle.Methods:From January 2012 to December 2019, 13 patients with butterfly-shape ear combined with cup ear deformity were treated in the Department of Plastic Surgery, Union Hospital, Huazhong University of Science and Technology. There were 6 males and 7 females, with an average age of 9.2 years (ranged from 6 to 15 years old). All patients were treated by folding cartilaginous flap to correct cup ear deformity, pushing forward the lobe flap and auricle composite tissue flap to repair the cleft on the ear. A piece of conchal cartilage was harvested and sutured to the outer edge of the cleft as the internal support for the cleft. The physiognomic ear length and breadth of two ears in the patients were measured immediately post operation. The results were evaluated after 6 months follow-up.Results:The appearance of all of the ears was significantly improved, no upper auricle hanging . A slight hypertrophic scar was found on the surface of one ear. The scar resolved after 6 months of ointment application and prosthesis. In all patients with unilateral ear deformity, the reconstructed ear length on the affected side was smaller than that on the contralateral side. In one patient, the difference between two sides ranged from 0.5 cm to 0.7 cm. For other patients, the difference between two sides was less than 0.5 cm. The difference between the reconstructed ear breadth of the affected ear and that of the reverse ear was less than 0.3 cm.Conclusions:This method can be applied to correct both the butterfly-shape ear deformity and cup ear malformation. The result was satisfactory.

9.Serological feature and molecular mechanism for a case with A307 subgroup.

OBJECTIVE:
To explore the serological feature and molecular mechanism for a case with A307 subgroup of the ABO blood group system.
METHODS:
Serological assay was carried out to determine the ABO blood group of the proband and his family members. Genotypes for exons 1 to 7 of the ABO gene were determined with sequence-specific primer polymerase chain reaction (SSP-PCR) and direct sequencing. The impact of the variant on the stability of alpha-1,3-N-acetylgalactosaminyltransferase (GTA) was predicted through construction of a 3D molecular model.
RESULTS:
The proband, his brother and daughter were diagnosed with Aend phenotype by serological analysis. Their ABO genotype was determined as A307/O02, with heterozygous c.467C>T (p.P156L) and c.745C>T (p.R249W) variants identified in exon 7 of the ABO gene. Molecular modeling suggested that the p.R249W variant may alter the number of hydrogen bonds between the amino acids. The protein was predicted to have a decreased Δ Δ G value of thermodynamic stability.
CONCLUSION
The p.R249W variant may give rise to the A307 subgroup by reducing the stability of the GTA enzyme, leading to serological features of Aend phenotype.
10.Expression of KCNA2 in the dorsal root ganglia of rats with osteoarthritis pain induced by monoiodoacetate.

OBJECTIVE:
To investigate the changes in the expression of voltage-gated potassium channel subunit KCNA2 in the dorsal root ganglion (DRG) neurons of rats with osteoarthritis (OA) pain induced by sodium monoiodoacetate and explore the mechanism.
METHODS:
A total of 156 adult male Sprague-Dawley rats were randomly divided into blank control group, saline group and intra-articular monoiodoacetate injection-induced OA group. The paw withdrawal mechanical threshold (PWMT) was measured before and at 1, 2, 4, and 6 weeks after monoiodoacetate injection. At 4 weeks after the injection, the pathological changes in the knee joints were analyzed using HE staining and Safranin O-Fast Green staining, and the expression of activating transcription factor 3 (ATF-3) and inducible nitric oxide synthase (iNOS) in the DRG neurons were detected by immunofluorescence staining. The expression of mRNA in the DRG neurons was detected by RT-qPCR at 1, 2, 4 and 6 weeks after the injection. The expression of KCNA2 in the DRG was measured by Western blotting, and the methylation level of promoter region was measured by MSPCR at 4 weeks after the injection.
RESULTS:
The PWMT of the rats in OA group was significantly decreased at 2, 4, and 6 weeks after the injection as compared with the baseline ( < 0.05 or < 0.001) as well as the control group ( < 0.05 or < 0.001). Four weeks after the intra-articular injection, fractures and defects on the surface of the articular cartilage, bone hyperplasia, and blurred tidal line were observed in the rats in OA group, but no obvious pathological changes were detected in the control or saline groups. Compared with those in the control group, the expressions of ATF-3 and iNOS were significantly increased ( < 0.01) at 4 weeks after injection; the expression of mRNA at 2, 4 and 6 weeks and the expression of KCNA2 protein at 4 weeks were all significantly decreased ( < 0.05 or < 0.01), and the methylation level of gene was significantly increased at 4 weeks after the injection in OA group ( < 0.01).
CONCLUSIONS
The expression of KCNA2 is decreased in the DRG neurons of rats with OA pain likely as a result of enhanced methylation of promoter region.
