1.Clinical analysis of 8 cases of adrenal hemolymphangioma and literature review

		                        			Objective: To investigate the clinicopathological characteristics and surgical outcomes of adrenal hemolymphangioma,so as to enhance the understanding of this disease. Methods: Clinical and pathological data of 8 patients with adrenal hemolymphangioma admitted to the Department of Urology of our hospital during Jan.2013 and Dec.2022 were retrospectively analyzed,and relevant literature was reviewed. Results: The patients included 5 males and 3 female,median age 54(25-75) years.Adrenal hemolymphangioma was detected in all patients in physical examinations without obvious symptoms.And 4 of the patients had a history of hypertension.Adrenal function test results showed no abnormalities.A total of 9 tumors were identified by imaging examination,including 1 unilateral multiple and 7 unilateral solitary tumors,with a median diameter of 3.6(1.0-5.4) cm.Posterior laparoscopic adrenal tumor resection was performed in 7 cases and robot-assisted laparoscopic adrenal tumor resection in 1 case;all of the tumors were completely removed.The median operation time was 77(53-115) min,median intraoperative blood loss 7.5(2.0-20.0) mL,and median postoperative hospital stay 4(1-7) d.Postoperative pathology showed interwoven deformed and dilated blood vessels and lymphatic vessels in the cystic tumors,with a large amount of lymphoid fluid,lymphocytes and red blood cells.Chronic lymphocyte infiltration was visible between the tube walls.The cystic cavity was partially connected,with flat endothelial cells lining.The pathological diagnosis was adrenal hemolymphangioma.During the median follow-up of 53.5(12.0-106.8) months,all patients recovered well,with stable blood pressure and no tumor recurrence or metastasis. Conclusion: Adrenal hemolymphangioma has no specific clinical symptoms.As adrenal functional tests show no obvious abnormalities,the diagnosis depends on pathological examinations.Popsterior laparoscopic or robot-assisted laparoscopic resection has good efficacy and prognosis.

		                        			Artificial intelligence (AI) and population pharmacokinetics (PPK) technologies have demonstrated significant potential in the personalized medication of immunosuppressants after organ transplantation, enabling precise prediction of drug dosages. This article provides a comprehensive review of the application status of AI and PPK in the individualized administration of immunosuppressants after organ transplantation, focuses on monitoring blood drug concentration, predicting efficacy/adverse reactions, and establishing individualized dosing models for organ transplant recipients after immunosuppressant administration, and analyzes and compares the application characteristics of different methods in different organ transplant patients as well as the integration and future development of AI and PPK technologies. AI and PPK technologies can not only significantly reduce the dependence on human resources, but also greatly improve the level of individualized treatment of immunosuppressants after organ transplantation, and reduce the discomfort and burden caused by frequent blood concentration monitoring to patients.
		                        		
		                        		
		                        		
		                        	
3.Pinoresinol diglucoside activates the Wnt/beta-catenin signaling pathway to protect osteoblasts

		                        			BACKGROUND:Pinoresinol diglucoside promotes bone formation and bone matrix synthesis and accelerates bone tissue repair.However,the mechanism of action and effects of this compound in osteoblasts need to be further explored. OBJECTIVE:To investigate the effect and mechanism of action of pinoresinol diglucoside on dexamethasone-treated osteoblasts based on the Wnt/β-catenin signaling pathway. METHODS:Different concentrations of dexamethasone groups and pinoresinol diglucoside groups were set to treat osteoblasts for 24 hours,and the optimal intervention concentrations were screened.Osteoblasts were treated with dexamethasone,pinoresinol diglucoside and inhibitor XAV-939.Then,control group,dexamethasone group,XVA-939 group,pinoresinol diglucoside group,pinoresinol diglucoside+XVA-939 group were set up.Cell counting kit-8 assay was used to detect cell activity.Alkaline phosphatase activity and caspase3/7 enzyme activity in cells were detected.Annexin V/PI staining and EdU assay were used to detect cell apoptosis and proliferation.Real-time qPCR and western blot were used to detect the mRNA and protein expression levels of Wnt3a,β-catenin,c-myc,osteocalcin,and type I collagen,respectively. RESULTS AND CONCLUSION:After dexamethasone and pinoresinol diglucoside intervened in osteoblasts for 24 hours,10 μmol/L dexamethasone was found to be the optimal intervention concentration for cell inhibition,and cell proliferation was most pronounced at a concentration of pinoresinol diglucoside of 100 μmol/L.Compared with the dexamethasone group,alkaline phosphatase activity was significantly enhanced(P<0.05)and caspase3/7 enzyme activity was significantly reduced(P<0.05)in the pinoresinol diglucoside group.Annexin V/PI staining and cell proliferation assay by EdU method showed that pinoresinol diglucoside inhibited apoptosis and promoted proliferation of osteoblasts after dexamethasone intervention.The mRNA and protein expression levels of Wnt3a,β-catenin,c-myc,osteocalcin,and type I collagen were significantly higher in the pinoresinol diglucoside group and pinoresinol diglucoside+XVA-939 group compared with the dexamethasone and XVA-939 groups(P<0.05).To conclude,pinoresinol diglucoside can inhibit osteoblast apoptosis after dexamethasone intervention,protect osteoblast activity and promote osteoblast proliferation by activating the Wnt/β-catenin signaling pathway,which may play a role in delaying steroid-induced osteonecrosis of the femoral head.
		                        		
		                        		
		                        		
		                        	
4.Research hotspots and trends of medical humanities in China since the new era

		                        			ObjectiveTo analyze the development trajectory, research hotspots, and trends in medical humanities research in China since the new era. MethodsA search was conducted on the CNKI (China National Knowledge Infrastructure) advanced search page using the themes“medical humanities”or “humanistic medicine,”retrieving a total of 5,758 articles. After applying specific screening criteria, 5,095 articles were included in the analysis. Citespace6.1.R6 was used to visualize and analyze the authors, institutions, and keywords of the 5,095 articles. ResultsSince the new era, the volume of publications on medical humanities in China has shown an overall upward trend, with limited collaboration between core institutions and core authors. The research content of medical humanities has evolved from broad to specific, from abstract to concrete, and from theoretical to practical. ConclusionThe development of medical humanities research in China has generally gone through three stages: defining related concepts, integrating medical humanistic spirit into clinical practice, and applying empirical methods. 5.Research progress on the lipid-lowering mechanisms and clinical application of GLP-1 receptor agonists

		                        			Glucagon-like peptide-1 (GLP-1) receptor agonists are a novel class of antidiabetic drugs that also possess lipid- lowering and cardiovascular protective effects, with liraglutide and semaglutide being their representative medications. Based on a systematic literature search, this review summarizes the lipid-lowering mechanisms by which liraglutide and semaglutide exert direct effects on the liver and kidney (regulating autophagy, key lipid metabolism pathways, reverse cholesterol transport, etc.), direct actions on adipose tissue (affecting adipocyte proliferation and differentiation, expression of lipid metabolism proteins, and gene transcription), activation of sympathetic pathways through the central nervous system, and modulation of the gut microbiota. Additionally, it summarizes the clinical evidence of their lipid-lowering effects in populations with type 2 diabetes mellitus, overweight individuals, and others. These findings indicate that GLP-1 receptor agonists exert lipid-lowering effects by acting on multiple tissues or systems, providing crucial evidence for further elucidating the molecular mechanisms of these drugs in lipid regulation and exploring potential new ideas for their clinical applications.
		                        		
		                        		
		                        		
		                        	
6.Immunohistochemical Detection of Treatment-Related Biomarkers in Urothelial Carcinoma

		                        			Immune checkpoint inhibitors and antibody–drug conjugates are increasingly incorporated into the clinical management of advanced urothelial carcinoma. The immunohistochemical detection of relevant biomarkers facilitates the selection of individualized precision therapies and supports the prediction of therapeutic efficacy. 7.Effect of Shenfu Yixin Granules on Mitochondrial Dynamics in Rats with Heart Failure After Myocardial Infarction

		                        			ObjectiveTo prepare a rat model of heart failure after myocardial infarction by ligation of the anterior descending branch of the left coronary artery, and to observe the effect of Shenfu Yixin granules on the mitochondrial dynamics of rats with heart failure. MethodFifty SD male rats were randomly taken ten as the sham operation group and the rest as modeling group. The rat model of heart failure after myocardial infarction was prepared by ligation of anterior descending branch of left coronary artery. According to the left ventricular ejection fraction(LVEF) on the 28th day after operation, the model rats were randomly divided into the model group, Shenfu Yixin granule low-dose and high-dose groups(3.011, 15.055 g·kg-1) and sacubitril valsartan sodium group(20.83 mg·kg-1). Each administration group was gavaged daily with the corresponding dose of drug solution, while the sham operation group and model group were given the same amount of normal saline once a day for 28 days, with 6 rats in each group. Ultrasound was used to detect the cardiac function parameters, rat heart mass and body mass were weighed to calculate the cardiac mass index, enzyme linked immunosorbent assay(ELISA) was used to detect serum brain natriuretic peptide(BNP) and soluble growth stimulation expressed gene 2 protein(sST2) levels. Hematoxylin-eosin(HE) staining was used to observe the pathological morphology of the myocardium. Real-time fluorescence quantitative polymerase chain reaction(Real-time PCR) and Western blot were used to detect the mRNA and protein expression of mitochondrial fusion protein 1/2(Mfn1/2), optic atrophy protein 1(Opa1), dynamin-related protein 1(Drp1) and fission protein 1(Fis1). ResultCompared with the sham operation group, the mRNA and protein expression of LVEF, Mfn1, Mfn2, Opal in the model group decreased(P<0.05), while BNP, sST2, cardiac mass index, Drp1, Fis1 mRNA and protein levels increased(P<0.05). Compared with the model group, the expression of LVEF, Mfn1, Mfn2, Opal mRNA and protein increased in Shenfu Yixin granule high-dose and sacubitril valsartan sodium groups(P<0.05), while BNP, sST2, cardiac mass index, Drp1, Fis1 mRNA and protein levels decreased(P<0.05). Pathological observation showed that compared with the sham operation group, the model group had disordered arrangement of myocardial cells, inflammatory cell infiltration and myocardial fibrosis. Compared with the model group, the degree of inflammatory cell infiltration, myocardial or interstitial fibrosis was improved and alleviated in all administered groups. ConclusionShenfu Yixin granules can resist heart failure, reduce cardiac mass index, decrease BNP and sST2 contents, and improve cardiac function. 9.Several key and difficult issues related to the grading of urothelial carcinoma of urinary bladder

		                        			Grading and staging are the most important prognostic factors for both non-invasive and invasive urothelial carcinomas, and are also one of the most common difficulties encountered by pathologists in the daily diagnostic practice of urothelial carcinoma. Recently, the International Society of Urological Pathology organized a survey and questionnaire conference on various issues related to the diagnosis, grading, and staging of urothelial carcinoma, and ultimately formed a series of consensus opinions. This article briefly summarizes the consensus opinions of this series, and combines them with the current pathological diagnosis status of urothelial carcinoma in China. 10.The efficacy and safety of protein A immunoadsorption combined with rituximab treatment for highly sensitized patients undergoing haplo-hematopoietic stem cell transplantation

		                        			Objective:To investigate the efficacy and safety of protein A immunoadsorption (PAIA) combined with rituximab (RTX) in highly sensitized patients who underwent haplo-hematopoietic stem cell transplantation (haplo-HSCT) .Methods:The clinical data of 56 highly sensitized patients treated with PAIA and RTX before haplo-HSCT at the First Affiliated Hospital of Soochow University and Soochow Hopes Hematonosis Hospital between March 2021 and June 2023 were retrospectively analyzed. The number of human leukocyte antigen (HLA) antibody types and the mean fluorescence intensity (MFI), humoral immunity, adverse reactions during adsorption, and survival within 100 days before and after adsorption were measured.Results:After receiving the PAIA treatment, the median MFI of patients containing only HLA Ⅰ antibodies decreased from 7 859 (3 209-12 444) to 3 719 (0-8 275) ( P<0.001), and the median MFI of HLA Ⅰ+Ⅱ antibodies decreased from 5 476 (1 977-12 382) to 3 714 (0-11 074) ( P=0.035). The median MFI of patients with positive anti-donor-specific antibodies decreased from 8 779 (2 697-18 659) to 4 524 (0–15 989) ( P<0.001). The number of HLA-A, B, C, DR, and DQ antibodies in all patients decreased after the PAIA treatment, and the differences were statistically significant (A, B, C, DR: P<0.001, DQ: P<0.01). The humoral immune monitoring before and after the PAIA treatment showed a significant decrease in the number of IgG and complement C3 ( P<0.001 and P=0.002, respectively). Forty-four patients underwent HLA antibody monitoring after transplantation, and the overall MFI and number of antibody types decreased. However, five patients developed new antibodies with low MFI, and nine patients continued to have high MFI. The overall survival, disease-free survival, non-recurrent mortality, and cumulative recurrence rates at 100 days post-transplantation were 83.8%, 80.2%, 16.1%, and 4.5%, respectively. Conclusions:The combination of PAIA and RTX has a certain therapeutic effect and good safety in the desensitization treatment of highly sensitive patients before haplo-HSCT.
