1.Effect of Liuwei Dihuangwan on EMT and Expression of CSC Properties in 4T1 Cells by Regulating Myeloid-derived Suppressor Cells

		                        			ObjectiveTo investigate the effect of Liuwei Dihuangwan drug-containing serum (LDP) on epithelial-mesenchymal transition (EMT) and the expression of cancer stem cell (CSC) properties in 4T1 cells from triple-negative breast cancer by intervening myeloid-derived suppressor cells (MDSCs). MethodsSPF-grade female SD rats were randomly divided into three groups, which were given 0.39, 1.94, 3.89 g·kg-1·d-1 suspension of Liuwei Dihuangwan for 7 days, respectively, to prepare low-, medium-, and high-dose LDPs. 4T1 cells were inoculated subcutaneously into the mammary glands of SPF-grade female Balb/c mice to construct a transplantation tumor model. Bone marrow cells were extracted from the tibia and femur and induced into MDSCs in vitro. The cell counting kit-8 (CCK-8) assay was used to detect the viability of 4T1 cells and MDSCs. The number of MDSCs and the expressions of CSC surface markers CD44 and CD24 in 4T1 cells were detected by flow cytometry (FC). The migration, invasion, and proliferation of 4T1 cells were detected by cell scratch assay, Transwell invasion assay, and plate colony-forming assay, respectively. Western blot (WB) was used to detect the protein expression of transforming growth factor-β (TGF-β), nuclear factor-κB (NF-κB), C-X-C motif chemokine ligand 2 (CXCL2), E-cadherin, and N-cadherin. The expression of EMT-related proteins E-cadherin and N-cadherin were detected by immunofluorescence (IF). ResultsCompared with the normal group, LDP showed no significant inhibitory effect on the cell viability of 4T1 cells, but it significantly reduced the viability and number of MDSCs and reduced the number of MDSCs, as well as the expression of TGF-β (P<0.05, P<0.01). The migration, invasion, and proliferation of 4T1 cells were increased after co-culture with MDSCs (P<0.05, P<0.01). The expressions of NF-κB, CXCL2, and N-cadherin and the proportion of CSC (CD44+CD24-) were elevated (P<0.05, P<0.01), while the expression of E-cadherin was decreased (P<0.05). After the intervention of MDSCs with LDP, followed by co-culture with 4T1 cells, the migration, invasion, and proliferation of 4T1 cells were obviously reduced (P<0.01). The expressions of NF-κB, CXCL2, and N-cadherin were decreased (P<0.05, P<0.01), and the expression of E-cadherin was increased (P<0.05, P<0.01). There was no statistical difference in the proportion of CSC (CD44+CD24-) in 4T1 cells. However, the proportion of CSC (CD44+CD24-) was decreased in the co-culture system of 4T1 cells and MDSCs with LDP intervention (P<0.05, P<0.01). 3.Liuwei Dihuang Erzhiwan Combination Regulate Myeloid-derived Suppressor Cells to Inhibit Breast Cancer Lung Metastasis

		                        			ObjectiveTo investigate the mechanism by which Liuwei Dihuang Erzhiwan combination inhibit the lung metastasis of spontaneous breast cancer in mice by regulating the recruitment of myeloid-derived suppressor cells (MDSCs). MethodThree hundred and eighty SPF-grade 10-month-old female breeders of Kunming mouse were palpated at the mammary gland site once every 3 days. Mice that have not had a lump touched after being raised for 6 months are used as control group. After tumor development, the mice were randomized into model, positive control (paclitaxel, intraperitoneal injection at 0.01 g·kg-1 every other day for 22 d), Liuwei Dihuangwan (0.65 g·kg-1·d-1 by gavage), Erzhiwan (5.41 g·kg-1·d-1 by gavage), and Liuwei Dihuang Erzhiwan combination (6.05 g·kg-1·d-1 by gavage) groups. The mice were euthanised when the tumor reached a diameter of about 15 mm, and the tumor and lung tissues were collected. The survival time, tumor mass, and lung metastasis rate of tumor-bearing mice were recorded. Hematoxylin-eosin (HE) staining was used to observe the histopathological and morphological changes of mouse tumor and lung tissues. Immunofluorescence (IF) was used to detect the distribution of MDSCs in tissues of mice in each group by double-staining of MDSCs cells with lymphocyte antigen 6 complex site G6D (Ly6G) and CD11 antigen-like family member B (CD11b). Western blot was employed to determine the protein levels of matrix metalloproteinase-9 (MMP-9), transforming growth factor-β (TGF-β), zinc finger transcription factor 1 (Snail1), and E-cadherin in the tumor tissue and CC motif chemokine 9 (CCL9) and CC motif chemokine receptor 1 (CCR1) in the lung tissue. ResultDuring the modelling period, the paclitaxel group and Chinese medicine intervention groups had longer median number of days of survival and lower tumor weight, lung metastasis rate, and lung nodule than the model group (P<0.05, P<0.01). HE staining showed an increase in tumor cell necrosis in the paclitaxel group and the Liuwei Dihuang Erzhiwan combination group. The paclitaxel group and Chinese medicine intervention groups had lower fluorescence intensity of MDSCs in the tumor tissue than the model group (P<0.05, P<0.01). Compared with the normal control group, the model group showed increased fluorescence intensity of MDSCs in the metastatic lung tissue (P<0.01), which, however, was decreased in the paclitaxel group and Chinese medicine intervention groups (P<0.01). The model group showed higher protein levels of MMP-9, TGF-β, and Snail1 and lower protein level of E-cadherin in the tumor tissue than in the normal control group (P<0.01). Compared with model group, paclitaxel and Chinese medicine interventions downregulated the protein levels of MMP-9, TGF-β, and Snail1 (P<0.05, P<0.01) and upregulated the protein level of E-cadherin in the tumor tissue (P<0.01). Moreover, the Liuwei Dihuang Erzhiwan combination group had lower protein levels of TGF-β and Snail1 than the Liuwei Dihuangwan group and Erzhiwan group (P<0.05). In the metastatic lung tissue, the expression of CCL9 and CCR1 was higher in the model group than in the normal control group, paclitaxel group, and Chinese medicine intervention groups (P<0.05, P<0.01). ConclusionLiuwei Dihuang Erzhiwan combination inhibit tumor growth, prolong survival time, and reduce the occurrence of lung metastasis in the mouse model of spontaneous breast cancer by reducing the recruitment of MDSCs in the tumor and lung tissues and modulating the phenotypes of epithelial-mesenchymal transition (EMT)-related molecules and the expression of CCL9/CCR1.

		                        			Objective:To explore potential predictors of refractory Mycoplasma pneumoniae pneumonia (RMPP) in early stage. Methods:The prospective multicenter study was conducted in Zhejiang, China from May 1 st, 2019 to January 31 st, 2020. A total of 1 428 patients with fever >48 hours to <120 hours were studied. Their clinical data and oral pharyngeal swab samples were collected; Mycoplasma pneumoniae DNA in pharyngeal swab specimens was detected. Patients with positive Mycoplasma pneumoniae DNA results underwent a series of tests, including chest X-ray, complete blood count, C-reactive protein, lactate dehydrogenase (LDH), and procalcitonin. According to the occurrence of RMPP, the patients were divided into two groups, RMPP group and general Mycoplasma pneumoniae pneumonia (GMPP) group. Measurement data between the 2 groups were compared using Mann-Whitney U test. Logistic regression analyses were used to examine the associations between clinical data and RMPP. Receiver operating characteristic (ROC) curves were used to analyse the power of the markers for predicting RMPP. Results:A total of 1 428 patients finished the study, with 801 boys and 627 girls, aged 4.3 (2.7, 6.3) years. Mycoplasma pneumoniae DNA was positive in 534 cases (37.4%), of whom 446 cases (83.5%) were diagnosed with Mycoplasma pneumoniae pneumonia, including 251 boys and 195 girls, aged 5.2 (3.3, 6.9) years. Macrolides-resistant variation was positive in 410 cases (91.9%). Fifty-five cases were with RMPP, 391 cases with GMPP. The peak body temperature before the first visit and LDH levels in RMPP patients were higher than that in GMPP patients (39.6 (39.1, 40.0) vs. 39.2 (38.9, 39.7) ℃, 333 (279, 392) vs. 311 (259, 359) U/L, both P<0.05). Logistic regression showed the prediction probability π=exp (-29.7+0.667×Peak body temperature (℃)+0.004×LDH (U/L))/(1+exp (-29.7+0.667×Peak body temperature (℃)+0.004 × LDH (U/L))), the cut-off value to predict RMPP was 0.12, with a consensus of probability forecast of 0.89, sensitivity of 0.89, and specificity of 0.67; and the area under ROC curve was 0.682 (95% CI 0.593-0.771, P<0.01). Conclusion:In MPP patients with fever over 48 to <120 hours, a prediction probability π of RMPP can be calculated based on the peak body temperature and LDH level before the first visit, which can facilitate early identification of RMPP.
		                        		
		                        		
		                        		
		                        	
5.A comparative study of three palliative surgical approaches for pulmonary atresia with ventricular septal defect

		                        			Objective    To compare and investigate the efficacy and differences of modified B-T shunt, central shunt and right ventricle-pulmonary artery (RV-PA) connection in the treatment of pulmonary atresia with ventricular septal defect (PA/VSD). Methods    A total of 124 children with PA/VSD underwent initial palliative repair in Shanghai Children's Medical Center from September 2014 to August 2019, including 63 males and 61 females, aged 7 days to 15 years. They were divided into in a modified B-T shunt group (55 patients), a central shunt group (22 patients) and a RV-PA connection group (47 patients). The clinical data of these children were retrospectively analyzed. Results    There were 9 early deaths after palliation, with an early mortality rate of 7.3%. The mean follow-up time was 26.5±20.3 months, with 5 patients lost to follow-up, 5 deaths during the follow-up period, and 105 survivors. The 1-year and 5-year survival rates were both 89.7%. The monthly increased Nakata index was 5.2 (–0.2, 12.3) mm2/m2, 9.2 (0.1, 23.6) mm2/m2, 6.3 (1.8, 23.3) mm2/m2 in the modified B-T shunt group, the central shunt group, and the RV-PA connection group, respectively, with no statistical difference among the three groups. The 1-year survival rate was 85.3%, 78.4%, 95.2%, and the 5-year (4-year in the central shunt group) survival rate was 85.3%, 58.8%, 95.2% in the three groups, respectively, with a statistical difference among them (P<0.05). The complete repair rate was 36.5%, 19.0% and 67.4% in the three groups, respectively, with a statistical difference among the three groups (P<0.001). Conclusion    All these three palliative surgical approaches can effectively promote pulmonary vascular development. But compared with systemic-pulmonary shunt, RV-PA connection has a lower perioperative mortality rate and can achieve a higher complete repair rate at a later stage, which is beneficial for long-term prognosis.
		                        		
		                        		
		                        		
		                        	
6.Correlation between Peripheral Blood Monocyte Levels and Postoperative Cognitive Dysfunction in Elderly Patients

		                        			Objective To explore the correlation between peripheral blood monocyte levels and postoperative cognitive dysfunction(POCD)in elderly patients.Methods A total of 96 elderly patients undergoing elective total hip arthroplasty under general anesthesia in the Affiliated Hospital of Xuzhou Medical University from October 2021 to March 2022 were enrolled in the prospective observational stud-y,and the patients were divided into POCD group(26 cases)and Non-POCD group(70 cases)according to the mini-mental state ex-amination(MMSE)score.Peripheral blood monocyte levels and other relevant indicators were recorded.Receiver operating characteristic(ROC)curve were applied to analyze the predictive value of peripheral blood monocyte levels for POCD with total hip arthroplasty in eld-erly patients.Univariate and multivariate Logistic regression analyses were used to evaluate risk factors.Results The optimal cut-off value for preoperative monocyte levels predicting POCD in elderly patients with total hip arthroplasty was 0.55×109/L,and area under the curve value was 0.729.Univariate Logistic regression analysis showed that lower education level,higher American Society of Anesthe-siologists(ASA)grade,and preoperative and postoperative elevated monocyte levels were the risk factors for POCD;multivariate Logistic regression analysis showed that preoperative monocyte levels were independently associated with the occurrence of POCD.Conclusion Preoperative peripheral blood monocyte levels are associated with the occurrence of POCD,and preoperative elevated monocyte level is a risk factor for POCD,helping to predict the potential risk of POCD.
		                        		
		                        		
		                        		
		                        	
7.Role of intestinal flora and 5-HT in depression- and anxiety-like behaviors in mice exposed to PM2.5

		                        			Background Some studies have shown that PM2.5 exposure is closely related to central nervous system diseases that lead to cognitive dysfunction and change the composition of intestinal flora. However, there are few studies on the role of intestinal flora in PM2.5-induced depression- and anxiety-like behaviors in mice. Objective To observe the effects of PM2.5 exposure on depression- and anxiety-like behaviors and the composition of intestinal flora in mice, and to explore the role of intestinal flora in regulating 5-hydroxytryptamine (5-HT) in depression- and anxiety-like behaviors in mice exposed to PM2.5. Methods Eight-week-old male SPF C57BL/6J mice were randomly divided into control group (NS group), probiotic group (LGG group), PM2.5 group (PM group), and combined exposure group (PML group), 6 mice in each group. Mice in the PM group and the PML group were exposed to PM2.5 in a dynamic exposure cabinet for 6 h per day, 6 d a week for 7 consecutive weeks, and the PM2.5 concentrations were approximately 8 times higher than the outdoor concentration. The LGG group and the PML group were orally administered with Lactobacillus rhamnosus while the NS group and the PM group were orally administered with the same amount of saline. Elevated plus maze test and open field test were used to detect depression and anxiety in mice. Fecal samples of mice were collected to evaluate intestinal flora abundance, diversity, and structure between groups using high-throughput sequencing of 16S rRNA. ELISA was employed to detect the levels of 5-HT in serum and hippocampus. Spearman correlation was used to analyze the correlations of differential intestinal flora with 5-HT level in hippocampus and depression- and anxiety-like behavior indicators in mice. Results The percentage of open-arm entry [M(P25, P75)] in the PM group was 0.0% (0.0%, 33.3%), lower than those in the NS group [47.7% (25.0%, 50.8%) ] and the PML group [46.9% (40.0%, 50.0%)], and the differences were statistically significant (P<0.05). 8.Human Umbilical Cord-Derived Mesenchymal Stem Cells Repair SU5416-Injured Emphysema by Inhibiting Apoptosis via Rescuing VEGF-VEGFR2-AKT Pathway in Rats

		                        			 Background and Objectives:
		                        			Chronic obstructive pulmonary disease (COPD) is a common, frequently-occurring disease and poses a major health concern. Unfortunately, there is current no effective treatment for COPD, particularly emphysema. Recently, experimental treatment of COPD using mesenchymal stem cells (MSCs) mainly focused on bone marrow-derived MSCs (BM-MSCs). Human umbilical cord-derived MSCs (hUC-MSCs) have more advantages compared to BM-MSCs. However, studies on the role of hUC-MSCs in management of COPD are limited. This study sought to explore the role of hUC-MSCs and its action mechanisms in a rat model of VEGF receptor blocker SU5416-injured emphysema. 
		                        		
		                        			Methods:
		                        			and Results: hUC-MSCs were characterized by immunophenotype and differentiation analysis. Rats were div-ided into four groups: Control, Control+MSC, SU5416 and SU5416+MSC. Rats in model group were administered with SU5416 for three weeks. At the end of the second week after SU5416 administration, model group were infused with 3×106 hUC-MSCs through tail vein. After 14 days from hUC-MSCs transplantation, rats were euthanized and data were analyzed. HE staining and mean linear intercepts showed that SU5416-treated rats exhibited typical emphysema while emphysematous changes in model rats after hUC-MSCs transplantation disappeared completely and were restored to normal phenotype. Furthermore, hUC-MSCs inhibited apoptosis as shown by TUNEL and Western blotting.ELISA and Western blotting showed hUC-MSCs rescued VEGF-VEGFR2-AKT pathway in emphysematous lungs. 
		                        		
		                        			Conclusions
		                        			The findings show that hUC-MSCs effectively repair the emphysema injury. This study provides the first evidence that hUC-MSCs inhibit apoptosis via rescuing VEGF- VEGFR2-AKT pathway in a rat model of emphysema. 
		                        		
		                        		
		                        		
		                        	
9.HCV genotyping of anti-HCV reactive samples by one ELISA assay and blood donor reentry study

		                        			【Objective】 To investigate the genotype of anti-HCV reactive blood donors by one ELISA assay and provide scientific basis for the reentry of anti-HCV false positive blood donors. 【Methods】 The data of 453 blood donors reactive to antibody to HCV(anti-HCV) with one ELISA assay(third generation) were extracted via the blood donor information system of Shaoguan Central Blood Station from January 1, 2014 to December 31, 2018. The subjects were recalled to the station for the serological retest, using a 4th generation ELISA reagent, and PCR qualitative test. The PCR reactive samples were sent to the genetic testing laboratory for HCV genotyping, in order to guide diagnosis and treatment in the future. Meanwhile, those PCR negative blood donors returned to be eligible again based on the Guidelines for the Return of Reactive Blood Donors for Blood Screening. 【Results】 70.2% (318/453) of the previous anti-HCV-reactive blood donors, using a third-generation ELISA assay responded to the HCV genotyping, of which 83.0%(264/318) were negative, and 17%(54/318) positive. The profile of HCV subtypes in positive donors was HCV2a>1b>3a=6a. A little bit high false positive rate was presented by the third, and former, generation reagent than the four generation(0.41% vs 0.06%), which was confirmed by HCV RNA qualitative and HCV genotyping tests.After two rounds of reentry testing, 98 eligible blood donors returned to the blood donor team, with the return rate at 21.63% (98 / 453). 【Conclusion】 NAT or (and) HCV genotyping for anti-HCV reactive blood donors screened out by the third, and former, generation, should be carried out to permanently shield the true positive donors and reenter the negative ones.

		                        			Objective    To evaluate the effect of off-pump pulmonary valvulotomy for the patients with pulmonary atresia with intact ventricular septum (PA/IVS). Methods    The clinical data of 61 PA/IVS patients who underwent off-pump right ventricular decompression surgery in our hospital from January 2013 to September 2019 were retrospectively analyzed, including 37 males and 24 females, with an average age of 29.7 (2.0-86.0) d and weight of 4.1 (2.5-6.9) kg. Thirty-nine patients received off-pump pulmonary valvulotomy (an open-view valvulotomy group) and 22 patients received balloon valvuloplasty through the right ventricle pulmonary valve (a hybrid therapy group). The postoperative mortality, early re-intervention, and completion of final operation of the two groups were compared. Results    There were 2 deaths in the study with a mortality rate of 3.3% (2/61), and the mortality rate of the two groups was not significantly different (2.6% vs.4.5%, P=0.68). The rate of early re-intervention in the two groups was 5.3% and 19.0%, respectively (P=0.09). There was no statistical difference in intubation time (56.0±25.9 h vs. 62.0±28.9 h, P=0.41), ICU retention time (4.7±2.9 d vs. 5.5±2.2 d, P=0.23) and postoperative hospital stay time (3.9±0.9 d vs. 4.3±1.1 d, P=0.38) between the two groups. The follow-up time was 45.3 (4.0-84.0) months. There were 5 patients lost to follow-up. During the follow-up period, in the open-view valvulotomy group, 17 patients did not need further operation, 13 patients completed the final operation. In the hybrid therapy group, 7 patients did not need further operation, 8 patients completed the final operation. Heart function classification of all patients was in New York Heart Association class Ⅰ-Ⅱ. Conclusion    Compared with the hybrid therapy, off-pump pulmonary valvulotomy for PA/IVS also has the advantages of simple operation, short operation time and high survival rate, and it may be easier to be promoted in clinical application because of its more economic benefits and relatively lower re-intervention rate.
