1.Imaging features and clinical analysis of posttransplantation lymphoproliferative disorders after liver transplantation in children

Objective:To investigate the imaging and clinical characteristics of posttransplantation lymphoproliferative disorders (PTLD) after liver transplantation in children.Methods:From February 2017 to November 2020, the imaging and clinical data of 17 children with PTLD after liver transplantation confirmed by pathology or clinical diagnosis were retrospectively analyzed in Shanghai Children′s Medical Center, School of Medicine, Shanghai Jiao Tong University. The site, range, density/signal/echo of the lesions were observed.Results:The mean age at transplantation was 8 (7, 11) months, and 14 patients were younger than 1 year old. The interval between liver transplantation and PTLD diagnosis was 22 (10, 34) months, ranging from 3 to 54 months. The interval was less than 1 year in 6 patients (early onset) and equal or greater than 1 year in 11 patients (late onset). Fifteen patients had Epstein-Barr virus infection. Among the 12 pathologically confirmed PTLD cases, 8 cases were diffuse large B-cell lymphoma, 3 cases were Burkitt lymphoma, and 1 case was reactive plasma cell hyperplasia. Among the 17 children with PTLD, 8 cases demonstrated involvement of lymph nodes and 16 cases had extranodal involvement. The latter included 15 cases of abdominal involvement. Abdominal sites involved included small intestine in 14 cases, colon in 7 cases, mesentery in 4 cases, kidney in 3 cases, liver in 2 cases, abdominal lymph nodes in 2 cases, peritoneum in 1 case, and stomach in 1 case. The sites of extra-abdominal involvement included lymph nodes in 7 cases, lung in 3 cases, skull in 1 case, brain in 1 case, pleura in 1 case, chest wall in 1 case, and nasopharynx in 1 case. The most common abdominal imaging abnormalities were thickening of the intestinal wall, eccentric mass and dilation of the lumen. Both small intestines and colons could be involved, and the former more commonly. Multiple masses were found in patients with liver and kidney involvement. The most common imaging manifestation of PTLD outside the abdomen was lymph node enlargement, which was found in 7 cases, and the most common was in the neck. The manifestation was shorter diameter of lymph nodes>10 mm, uniform density and signal, with mild enhancement.Conclusions:PTLD can occur months to years after liver transplantation in children, which can affect many parts of the whole body. Extranodal lesions are more than intranodal lesions. Abdominal involvement is most common in PTLD, and the infection rate of EB virus is high. Combined with medical history, EB virus infection status and imaging examination are helpful for early diagnosis.
2. Radiographic manifestations and clinical relevance to central nervous system complications of leukemia in children

Objective:
To investigate the radiographic manifestations and clinical relevance to central nervous system complications of leukemia (CNSCL)in children.
Methods:
The CT and magnetic resonance imaging(MRI) fin-dings and clinical features of 49 pediatric patients with CNCSL in Shanghai Children′s Medical Center Affiliated to Shanghai Jiaotong University School of Medicine from May 2010 to June 2018 were retrospectively analyzed.
Results:
(1) Cerebrovascular abnormalities in 23 cases included hemorrhage(20 cases), infarction(2 cases) and sinus thrombosis(1 case). One case of epidural hematoma and 19 cases of intracerebral multiple bleeding were seen in the hemorrhage group, which demonstrated high-density on CT and different signal on MRI as time went by.Microhe-morrhage displayed as low signal on susceptibility weighted imaging.(2) Among 23 cases of leukemic infiltration, the dura and/or skull were involved in 18 cases, which presented as fusiform or mass, with high density on CT, low signal on T1WI, intermediate signal on T2WI and strong enhancement; 6 leptomeningeal infiltration demonstrated as meningeal thickening and enhancement; 2 parenchymal involvement manifested with high-density mass; 2 oculomotor nerve and 1 optic nerve infiltration demonstrated thickening and enhancement.(3)White matter disease was seen in 2 cases, with hyper-intensity on T2WI.(4) One case of secondary tumor was glial tumor in the brainstem.
Conclusions
The radiographic manifestations of CNCSL in children are various.CT and MRI are of important diagnostic values.Choosing the best imaging examination method and sequence according to clinical symptoms and test results can provide more valuable information for clinical diagnosis and treatment.
3.Influence of special nursing intervention for the quality of life of patients with chronic pelvic inflammatory disease

Objective To discuss the influence of special nursing intervention for the quality of life of outpatients with chronic pelvic inflammatory disease. Methods Divided 80 patients into the observation group and the control group randomly, there were 40 cases in each group. Routine nursing, treatment and disease-related lecture were used in both the groups, while the combined therapy of Chinese and Western medicine was used in the observation group in addition. Evaluated the quality of life and the effects of treatment by the World Health Organization′s quality of surviving chart between the two groups after 3-month intertention and 6-month follow-up. Results The scores of quality of environment field, physiology field ,psychological field, sociological field, independence field and mental field in the observation group was (70.33+16.85 ) (70.38+18.80), (71.43+18.28),(70.13+19.08), (73.10+17.70) and (75.38+7.30) respectively, while in the control group was (61.38+16.17), (56.70+17.69), (43.13+19.77), (55.00+19.08), (53.75+16.45) and (49.80+17.26) respectively, the difference between the two groups was significant, the t values were 2.424-8.631, P < 0.05. The effective rate of treatment in the observation group was 95.00%(38/40), while in the control group was 82.5%(33/40), the difference was significant, U=2.507, P < 0.05. Conclusions Special nursing intervention can promote the quality of life ,effects of treatment and the satisfaction of treatment in outpatients with chronic pelvic inflammatory disease, improve their psychological state and the compliance.
4.Curriculum setting of higher vocational geriatric nursing based on working process systematization

Objective To explore the influences of the teaching mode based on working process systematization on teaching results of geriatric nursing. Methods A totally of 209 students of 2013 higher vocational nursing major in four classes were selected and divided into the experimental group and the control group by simple random methods, among which the experimental group contains 103 students who were taught in teaching mode based on working process systematization while the control group contains 106 students who were taught in the traditional teaching mode. The new teaching mode′s design and development linked closely to working situations and tasks; adopt the teaching mode combining teaching, learning and practicing together aimed at the position requirements of old-age nursing service system at the core of the true clinic old-age nursing work-post-skills, which were based on nurse practice qualification. Students′ performance achievement between two groups after teaching were compared. The thinking ability of students was evaluated by judging thought scale, and the students′ evaluation of the new teaching method in the experimental group was accessed by a self-designed questionnaire based on working process systematization.Results After the implementation of teaching, there were significant differences between students in two groups in their performance in theory examination and practice examination based on statistics tests ( P<0. 05 ); after the intervention, there were statistically significance in the differences in students′critical thinking ability between two groups by comparison in term of each aspect such as reasoning, agree on the assumptions, explanation, deduction, discoursing ( t=7. 452, 6. 964,10.220,4.771,4.765;P<0.05);the students in the experimental group made a positive evaluation on the teaching method based on working process systematization.Conclusions The teaching method based on working process systematization is available for the teaching of geriatric nursing in higher vocational institutions, and is helpful for students to master professional skills, cultivate their spirit of innovation, strengthen their team spirit, communication skills, critical thinking ability and other qualities. This new method is worth learning in the teaching of geriatric nursing course in higher vocational colleges.
5.Research in correlation of hardiness personality and professional commitment among nursing students

Objective To explore the correlation between nursing students' hardiness personality and professional commitment.Methods 1 115 undergraduate nursing students from 3 medical universities in Guangzhou were investigated with the scales of hardiness personality and nursing professional commitment.Results The scores of hardiness personality and professional commitment were (2.55 ± 0.44) and (2.70 ± 0.43),respectively,and both of them were at the middle level.Correlation analysis revealed that hardiness personality was positively related to professional commitment (P<0.01),and the challenge dimension of hardiness personality had positive predictive effect on professional commitment.Conclusions Nursing educators should take measures to improve students' hardiness personality and professional commitment.It would help maintain the stability of the whole nursing team.
6.Quantitative research and qualitative research in the application of health education work

The health education as part of the public health, largely using the research methods of social science, thus, the qualitative research and the quantitative research inevitably become the main research methods. With the develop-ment of the statistics and the wide application of statistical software, the quantitative research is becoming main part of health education survey work, and the qualitative research is often overlooked. In this paper, by comparing the charac-teristics of the quantitative research and qualitative research, using health education diagnosis, planning, intervention and evaluation of system thinking process, discusses the two methods of research the application at all stages of the health education work, it is concluded that health education work needs to be combined with the qualitative research and the quantitative research, multiple research methods, analyze and solve problems comprehensively in health educa-tion work, which provide reference to carry out health education for the future working.
7.MRI assessment of fetal autosomal recessive polycystic kidney disease

Objective To explore the value of MRI on fetal autosomal recessive polycystic kidney disease (ARPKD).Methods Sixteen pregnant women,aged from 28 to 38 years (average 30 years) and with gestation age from 22 to 36 weeks (average 25 weeks) underwent MR scanning with a 1.5 T MR unit within 24 to 48 hours after ultrasound examinations.The imaging sequences included steady-state free-precession (SSFP) sequence,single-shot turbo spin echo (SSTSE) sequence and T1-weighted fast imaging sequence.Prenatal US and MR imaging findings were compared with autopsy or pathological results.Results A total of 16 cases of ARPKD showed bilateral markedly enlarged kidneys and diffuse high signal small cysts in renal medulla on SSTSE sequence.Among the 16 cases,11 cases were with oligohydramnios,1 1 cases were with pulmonary hypoplasia,and 6 cases were with hepatic fibrosis.Eleven cases of pulmonary hypoplasia and 6 cases of hepatic fibrosis were all missed by US.For the diagnosis of the renal anomalies,US missed one case.MRI diagnosis was correct in all these cases.Conclusions MRI shows great advantages on the diagnosis of fetal ARPKD,and it is not affected by the amount of amniotic fluid.It can be used to evaluate kidney and lung abnormalities accurately.
8.Evaluation of effect of community health education on type 2 diabetes mellitus patients

Objective To probe into the effect of community health education in comprehensive treatment of type 2 diabetes mellltus(T2DM)patients.Methods 120 cases of T2DM patients were claasified randomly into the intervention group(55 cases)and the control group (65 cases).After systematic diabetes knowledge education was given to the intervention group,individualized intensive education was accepted by each patient of the intervention group.General diabetic education was taken by the control group.The levels of plasma glucose,urinary glucoee,glycosylated hemoglobin and blood-fat were tested before and 3,6 months after education.Results Six months after education,the level of self care ability,self care technique,self care sense of responsibility and health knowledge were higher than those before education in both groups.And six months after education the levels of HbAlc,TC and TG in the intervention group and the control group improved obviously.All indicators in the intervention group were better than those in the control group.Conctusions Community health education can effectively improve self care ability,self care technique,self care sense of responsibility and health knowledge of diabetes patients,and can improve their glucolipide metabolic level as well.
9.Nursing of patients with intractable postpartum hemorrhage undergoing interventional treatment

Objective To investigate the effect of nursing intervention in interventional treatment of patients With intractable postpartum hemorrhage.Methods 90 patients with intractable postpartum hemorrhage in our hospital from May 2008 to May 2011 adopted interventional treatment approach.They were divided into the control group and the observation group with 45 patients in each group.The control group received routine care,while the observation group received pertinent nursing.The treatment effect was compared between two groups.Results The mean operation time,hemostasis time,hospitalization time were significantly shorter than those of the contrtol group.The cure rate was sigrnificantly better than the control group.No serious complications occurred.Conclusions During interventional treatment for patients with intractable postpartum hemorrhage,pertinent nursing can reduce the incidence of complications,stop bleeding quickly and safely,keep the uterus and its physiological function.It can obtain satisfactory results and improve life quality of patients.
10.Influence of nursing intervention on medication compliance of patients with climacteric dysfunctional uterine bleeding

ObjectiveTo investigate the effect of nursing intervention on medication compliance of patients with climacteric dysfunctional uterine bleeding. Methods84 patients with climacteric dysfunctional uterine bleeding in our hospital from April 2009 to April 2011 were treated,they were randomly divided into the control group and the observation group with 42 cases in each group.The control group was given routine care during the course of medication,the observation group was given pertinent nursing intervention based on the routine nursing.The medication compliance and treatment effect were retrospectively analyzed. ResultsAmong 42 cases in the observation group,40 cases (95.2%) followed the doctor's advice,41 cases (97.6%) visited on time,3 cases (7.1%) failed the treatment.In the control group,30 cases (71.4%) followed the doctor's advice,25 cases(59.5%) visited on time,14 cases(33.3%) failed the treatment.The observation group was better than the control group. ConclusionsHormone treatment for patients with climacteric dysfunctional uterine bleeding can obtain desired therapeutic effect,but method of medication was more complicated.Nursing intervention during medication course can significantly improve medication compliance and treatment effect,which is of great clinical significance.
