1.Dupilumab for the treatment of severe asthma:a rapid health technology assessment

OBJECTIVE To evaluate the efficacy, safety and cost-effectiveness of dupilumab in the treatment of severe asthma with rapid health technology assessment (HTA), and to provide evidence-based evidence for clinical treatment. METHODS Retrieved from PubMed,Cochrane Library,CNKI,Wanfang, VIP database, and other related websites of HTA. HTA reports, systematic review/meta-analysis, and economic studies of dupilumab in the treatment of severe asthma were collected. Researchers independently identified literature, extracted data, and assessed the quality of included studies. Qualitative description was performed. RESULTS A total of 15 pieces of literature were included, involving 9 systematic reviews/meta-analyses and 6 economic studies. In terms of effectiveness, dupilumab was significantly better than placebo. Compared with other biological drugs, in the patients with severe asthma aged 12 years and above, for those with eosinophil (EOS) ≥300 cells/μL, dupilumab ranked first in improving forced expiratory volume in one second (FEV1), outperforming tezepelumab, benralizumab, and mepolizumab. It ranked second in reducing acute exacerbations, surpassed only by tezepelumab, while its effect on improving asthma control questionnaire score was relatively lower, being better only than benralizumab. For those with 150 cells/μL ≤EOS<300 cells/μL, dupilumab was superior to mepolizumab in reducing asthma exacerbation, while the effect on FEV1 was weaker than benralizumab and mepolizumab. In terms of safety, there was no significant difference in the incidence of adverse events and serious adverse events between dupilumab and placebo or other biological drugs, while the incidence of injection site reactions of dupilumab was significantly higher than placebo. In terms of cost-effectiveness, the research results of different countries were not consistent, and there was a lack of research data from China. CONCLUSIONS Dupilumab is an effective and safe choice in the treatment of severe asthma, and its cost-effectiveness requires further research based on China’s medical environment to be determined.
2.Comparison of the Prognostic Value of Platelet-Related Indices in Biliary Tract Cancer Undergoing Surgical Resection

Purpose:
Platelet-related indices, including mean platelet volume (MPV) and plateletocrit (PCT), have been reported as new prognostic factors of overall survival (OS) in many cancers, but not yet in biliary tract cancer (BTC). We intended to assess these indices in predicting OS in BTC patients with the aim to build a new prognostic model for patients with BTC after surgical resection.
Materials and Methods:
Survival analysis and time receiver operating characteristic analysis were applied to screen the platelet indices. Univariate and multivariate Cox analyses were used to identify independent prognostic factors and develop a new prognostic model. Harrell’s C-statistics, calibration curves, and decisive curve analysis were used to assess the model.
Results:
MPV and platelet distribution width (PDW)/PCT showed the best prognostic accuracy among the platelet indices. In multivariable analysis, factors predictive of poor OS were presence of nodal involvement, Non-radical surgery, poor tumor differentiation, carbohydrate antigen 19-9 > 100 U/mL, MPV > 8.1 fl, and PDW/PCT > 190. The new model was found to be superior to the TNM staging system and our new staging system showed higher discriminative power.
Conclusion
MPV and PDW/PCT have high prognostic value in BTC patients, and the novel staging system based on these two indices showed good discrimination and accuracy compared with the American Joint Committee on Cancer 7th TNM staging system.
3.Relationships between perioperative depression, anxiety symptoms and fear of cancer recurrence in breast cancer patients

Objective:To investigate the predictive effect of perioperative depression and anxiety symptoms in breast cancer patients on the fear of cancer recurrence.Methods:A total of 194 patients with breast cancer during perioperative period from May 2019 to May 2020 in Qilu Hospital of Shandong University were selected. The depression, anxiety and fear of cancer recurrence were investigated by the general information questionnaire, the Generalized Anxiety Disorder-7 (GAD-7), the Patient Health Questionnaire-9 (PHQ-9) and the Fear of Progression Questionnaire-Short Form (FOR-Q-SF) at admission and two weeks after surgery, respectively.Results:Perioperative depression and anxiety symptoms of breast cancer patients were significant predictors of fear of cancer recurrence ( B value was 2.325, OR value was 10.22, P<0.05; B value was 2.570, OR value was 13.07, P<0.05), and patients with depression and anxiety symptoms after surgery were at higher risk of fear of cancer recurrence ( OR values were 7.653-25.403, P<0.01). Conclusions:For breast cancer patients with negative emotions and fear of disease progression, it is necessary to help them improve their psychological coping ability, encouraging patients to actively cooperate with follow-up treatment, improving the prognosis, and improving their overall quality of life.

5.The effects of pharyngeal pressure feedback training on pharynx constriction caused by brainstem lesions

Objective:To explore the effect of the pharyngeal pressure feedback training on pharyngeal constriction in persons with swallowing disorders caused by brainstem lesions.Methods:Twenty patients with disordered swallowing caused by a brainstem lesion were randomly divided into a control group and an intervention group, each of 10. Both groups received routine swallowing training including oral sensorimotor training, neuromuscular stimulation and balloon catheter dilation, while the intervention group was additionally provided with two weeks of pharyngeal pressure feedback training. Before and after the treatment, both groups were evaluated in terms of the peak pressure of superior and inferior pharyngeal constriction, endoscopically and also using the functional oral intake scale.Results:There was no significant difference between the two groups in any measure before the intervention. Afterward, both groups had improved significantly by all of the measurements, but the average peak upper pharyngeal pressure, the average intake scale score and the endoscopy results of the intervention group were all significantly better than the control group′s averages.Conclusions:Pharyngeal pressure feedback training can significantly relieve swallowing disorders caused by brainstem lesions.
6.Correlation between appearance characteristics and intrinsic quality of Gastrodiae Rhizoma.

Totally 21 batches of Gastrodiae Rhizoma pieces from various habitats with different appearance characteristics were analyzed. Five active components( gastrodin,4-hydroxybenzyl alcohol,parishin B,parishin C,and parishin A) were determined by UPLC. Polysaccharide content was determined by phenol sulfuric acid method. And the content of alcohol extracts was determined. The correlation between appearance characteristics and active components,polysaccharide content and amount of alcohol extracts of Gastrodiae Rhizoma pieces was statistically analyzed by SPSS 19. 0 software. And the five active components in the 21 Gastrodiae Rhizoma pieces were clustered by using the Ward' s method in SPSS 19. 0 software. The study found that inconspicuous keratinous Gastrodiae Rhizoma had a low content of each component,and the degree of keratin was positively correlated with the content of each component and the extract,with no significant correlation with the polysaccharide content. In the cluster analysis,the 21 batches were divided into three groups,except the second group whose gastrodin content was significantly higher than the special conditions of other groups. According to the content of the five active components,19 batches of Gastrodiae Rhizoma pieces were classified into two groups with obvious keratin and no obvious keratin. In terms of color,with the deepening of brown color,the content of each component showed a downward trend. The grading of decoction pieces of Gastrodiae Rhizoma shall give full consideration to the color and texture. And the combination of chemical composition and appearance characteristics is an indicator to evaluate the product specification standards.

7.Quality evaluation of multi-components simultaneous determination and fingerprint of Gardeniae Fructus from different regions.

An analysis method was established by UPLC fingerprint and then applied to simultaneous determination of multiple compounds in Gardeniae Fructus from different areas in China. Samples were separated on a Waters Acquity UPLC BEH C18( 2. 1 mm×50 mm,1. 7 μm) column with 0. 1% formic acid-water and acetonitrile solution as gradient mobile phase at a flow rate of 0. 4 m L·min-1 at various wavelengths. The similarity of samples was over 0. 95 with ″Similarity Evaluation System for Chromatographic Fingerprint of Traditional Chinese Medicine( 2012 edition) ″. The UPLC common fingerprints for 32 batches were established with 19 common peaks identified. The samples were divided into 3 groups analyzed by HCA and PCA. Five components were identified as the main compositions which caused the differences of chemical constituents in the samples from different areas with partial least squares discriminant analysis( PLS-DA). The content of the total components in each area was Zhejiang > Fujian > Jiangxi > Sichuan. This method was accurate and viable,could be used to evaluate the quality of Gardeniae Fructus.

8.Technology optimization of Gardeniae Fructus processed with ginger juice and composition changes after processing.

To optimize the technology of Gardeniae Fructus processed with ginger juice,establish fingerprints and simultaneously determine seven compounds( geniposidic acid,chlorogenic acid,genipin-1-β-D-gentiobioside,geniposide,rutin,crocin Ⅰ,and crocin Ⅱ) by using ultra high performance liquid chromatography( UPLC). Waters ACQUITY UPLC BEH C18( 2. 1 mm×50 mm,1. 7μm) column was used with acetonitrile and 0. 1% formic acid solution as mobile phase for gradient elution at the flow rate of 0. 4 m L·min-1. The data was comprehensively processed and analyzed with similarity evaluation,principal component analysis( PCA) and partial least squares discriminant analysis( PLS-DA) methods. Twenty common peaks were identified in this study,and the similarity of samples was over 0. 97. The results of PCA and PLS-DA showed that there were differences in chemical compositions and contents between the raw Gardeniae Fructus and those processed with ginger juice,with 9 potential differentiated chromatographic peaks. After being processed with ginger juice,the contents of chlorogenic acid,crocin Ⅰ and crocin Ⅱ were less than before and the contents of other four compositions were higher than before. The optimized preparation for Gardeniae Fructus processed with ginger juice was stable and feasible. The methods of UPLC fingerprints and simultaneous determination of seven components can be effectively carried out to distinguish Gardeniae Fructus and Gardeniae Fructus processed with ginger juice.

9.Quality of life and its related factors in caregivers of patients with breast cancer

Objective To exam the quality of life of the caregivers of patients with breast cancer, to explore their related factors, including medical characteristics of the patients, demographic characteristics of the caregivers, relationship with the patients, social support and psychological resilience, and to test the mediating and moderating roles of psychological resilience between social support and quality of life. Methods A total of 179 hospitalized patients diagnosed with breast cancer were recruited in Qilu Hospital of Shandong University. The self-designed demographic questionnaire, Medical Outcome Study 12-Item Short Form Health Survey,Version 2(SF-12V2), Perceived Social Support Scale(PSSS), and 10-item Connor-Davidson Resilience Scale(CD-RISC-10) were used to collect information about demographic characteristics and measure the quality of life, perceived social support and psychological resilience of the caregivers. The factors affecting the quality of life of the caregivers were analyzed by multiple linear regression analysis, and the mediating and regulating effects of mental resilience on social support and mental health were analyzed. Results The age of the caregivers was negatively correlated with the physical health (P=0.010). The score of the body health of the family members of the middle and late stage cancer patients was lower than that of the family members in the early stage (P=0.003). There was a positive correlation between friend support and resilience and mental health (P<0.01). Resilience played a moderating role and mediating role in the relationship between friend support and mental health. Conclusions Attention should be paid to the quality of life of caregivers in patients with breast cancer. The medical staff should provide more social support for the caregivers, and should pay more attention to improve the psychological resilience of the caregivers, guide them to better cope with the burden and pressure of care, and improve their mental health with half the effort.
10.Chemical profiles and metabolite study of raw and processed Polygoni Multiflori Radix in rats by UPLC-LTQ-Orbitrap MS spectrometry.

The raw and processed roots of Plygonum multiflorum Thunb (PM) are used to treat different diseases in clinical practice. In order to clarify the influence of processing, a comparative study of chemical substance analysis was carried out. As the xenobiotics with a high enough exposure in target organs being considered as the potential effective or toxicity components, an in vivo study was also implemented to characterize the constitutes and metabolites, and meanwhile, the factor of compatibility with black bean were also considered. As a result, a total of 148 compounds were detected in PM extracts and more than 40 compounds were only detected in the processed products, which were probably new components produced during the steaming process. In in vivo study, 7 prototype components and 66 metabolites were detected or tentatively identified, 24 of which were reported for the first time. Our results indicated that processing greatly changed the chemical composition of PM and influenced the disposition of the compounds in vivo. To the best of our knowledge, this was the first global comparative study of raw and processed PM. These results expanded our knowledge about the influence of processing of PM and provided the essential data for further efficacy or toxicity studies.
