1.The Reality Basis,Development Dilemma and Relief Strategy of China's Health Consumption Upgrading in the New Era

Health consumption upgrading is an important way to benefit residents'livelihood,protect health and promote the construction of healthy China.It analyzes the practical foundation and development dilemma of China's health consumption upgrading in the new period through literature and logical analysis,and puts forward strategies to alleviate the dilemma.It concludes that in the new period,China's health consumption upgrading has the practical foundation of significant effect of health poverty alleviation policies,continuous upgrading of national health consumption concepts,and continuous emergence of health science and technology innovations,but also faces the development dilemmas of unstable health market order,insufficient supply of health products,insufficient application of health science and technology,and lack of deep cultivation of health literacy.Accordingly,the following strategies are proposed to alleviate the difficulties:strengthen top-level design to create a new"environment"for health consumption;promote diversified participation to meet the new"demand"for health consumption;strengthen technology-driven to build a new"industry"for health consumption;and cultivate health literacy.Cultivating health literacy and cultivating new"potential"for healthy consumption.
2.Clinical features and risk factors in 126 patients with anti-MDA5 antibody positive dermatomyositis

Objective:To explore clinical factors of poor prognosis in patients with anti-melanoma differentiation-associated gene 5 andtibody positive dermatomyositis (MDA5-DM).Methods:One hundred and twenty-six enrolled adults with MDA5-DM were divided into the survival group and the deceased group according to the outcomes. Survival time, clinical manifestations, laboratory tests, pulmonary function tests, myositis antibodies and treatments were collected for statistical analysis. Serum concentrations of IL-15, HMGB1, and sCD163 were measured by ELISA in MDA5-DM patients and healthy controls. Mann-Whitney U nonparametric test and Student′s t-test were used to compare the continuous variables between the two groups, and χ2 or Fisher′s exact test were used for comparison of categorical variables. Cox regression analysis was used to assess the survival predictors in MDA5-DM patients. The cumulative survival rate was calculated by Kaplan-Meier curve analysis, and Log-rank tests were used to examine differences in survival curves. P<0.05 was considered statistically significant. Results:Cox multivariate regression analysis revealed that age > 57 years [ HR (95% CI)=3.05 (1.20, 7.80), P=0.020], RP-ILD [ HR (95% CI)=25.07 (5.42, 115.98), P<0.001], and levels of anti-Ro52 antibody [ HR (95% CI)=3.41 (1.36, 8.53), P=0.009] were important prognostic factors independent of multiple clinical parameters. The ELISA test results showed that the levels of serum IL-15[0.91 (0.66, 2.00)pg/ml vs. 0.51(0.39, 0.72)pg/ml, Z=-4.57, P<0.001] and HMGB1 [230.53(90.40, 394.31)ng/ml vs. 32.66 (17.82, 46.21)ng/ml, Z=-6.52, P<0.001] in MDA5-DM patients were significantly higher than those in healthy controls, but there were no significant differences in the level of serum IL-15 [1.21(0.63, 2.12)pg/ml vs. 0.91(0.68, 1.66)pg/ml, Z=-0.30, P=0.766], HMGB1[267.61(167.03, 444.23)ng/ml vs. 228.35(74.74, 344.32)ng/ml, Z=0.82, P=0.413], and sCD163 [112.70(93.45, 148.51)ng/ml vs. 132.72(96.79, 203.18)ng/ml, Z=-0.62, P=0.536] between the survival group and the deceased group. Conclusion:Older age, RP-ILD, and high levels of anti-Ro52 antibody significantly increase the risk of death in MDA5-DM patients. Intensive follow-up of patients with the above factors in the early stages may help to improve the prognosis.
3.3D printing-assisted pre-fenestration and branch stent endovascular repair for the treatment of thoracoabdominal aortic aneurysms

Objective:To evaluate the safety and efficacy of 3D printing-assisted pre-fenestration and branch stent endovascular repair (F/b EVAR) in the treatment of thoracoabdominal aortic aneurysms.Methods:The clinical data of 26 patients treated with 3D printing-assisted F/b EVAR for complicated thoracic and abdominal aortic diseases at the Department of Vascular Surgery, the Ninth People's Hospital,Shanghai Jiaotong University School of Medicine from May 2019 to Sep 2022 were retrospectively analyzed.Results:The success rate in these 26 cases of TAAA with 3D printing combined with F/b EVAR was 97.89%, and the mean follow-up time was (8.03±4.15) months. Four cases had Ⅲc internal leakage and disappeared during the follow-up. One case of type Ⅲ leakage were narrowed during follow-up. Ic type internal leakage occurred in 1 patient and disappeared after the addition of a stent at the distal end. During the follow-up period, aortic CTA indicated that 1 patient had renal artery stent occlusion and smooth blood in other visceral branches. No complications such as organ ischemia, lower limb ischemia and all-cause death occurred during follow-up.Conclusion:3D printing-assisted F/b EVAR minimally invasive repair of TAAA is a feasible, effective and safe technique, with high success rate and low complication rate of visceral branch artery reconstruction.
4.Experimentation of effect of PD-1 inhibitor on myocardial inflammation microenvironment and radiation-induced injury

Objective:To explore the potential mechanism of PD-1 inhibitor P on RIMI from the perspective of immune microenvironment.Methods:To establish a mouse model of radiation-induced myocardial injury (RIMI), twenty C57BL/6 mice were randomly divided into 4 groups, 5 in each group. Group A was the healthy control group; Group B was the PD-1 inhibitor group; Group C was the simple irradiation group, with a heart irradiation of 15 Gy; Group D was the irradiation+ PD-1 inhibitor group. One month after irradiation, the mice were anesthetized and sacrificed. The morphological changes of myocardial tissues were observed by HE staining. The myocardial fibrosis was assessed by Masson staining. CD 3+ , CD 3+ CD 4+ , CD 3+ CD 8 lymphocyte subsets and cytokines (IL-4, IL-6, IL-17A, TNF-α, TGF-β 1 and INF-γ) levels were determined by flow cytometry. The apoptosis rate of myocardial cells was detected by TUNE. Results:One month after irradiation, there was no obvious myocardial fibrosis in group B, and collagen fibers were distributed in the interstitium of myocardial cells in groups C and D. Semi-quantitative analysis results showed that the myocardial collagen volume fraction (CVF) of groups A, B, C and D were (1.97±0.36)%, (2.83±1.03)%, (5.39±0.77)% and (7.72±1.43)%, respectively. The CVF between group A and group B was similar ( P=0.314), and the differences in CVF between the other groups were statistically significant (all P<0.05). Compared with group A, the absolute value and percentage of CD 3+ T lymphocytes were significantly increased in groups B, C and D (all P<0.01). The values in group D were significantly higher than those in group B and group C (all P<0.01); The absolute value and percentage of CD 3+ CD4 T lymphocytes were similar among four groups (all P>0.05); The absolute value and percentage of CD 3+ CD 8 T lymphocytes in group D were significantly higher than those in groups A, B and C (all P<0.001). The expression levels of IL-6, IL-17A, and TGF-β 1 in group D were significantly higher compared with those in groups A, B and C (all P<0.001). The apoptotic index was gradually increased in four groups, and the differences in apoptotic index among four groups were statistically significant (all P<0.001). Conclusion:PD-1 inhibitors can aggravate RIMI by promoting myocardial immune inflammatory response.
5.Real experience of medical and nursing staff in Emergency Department of general hospitals in receiving patients with mental disorders: a qualitative study

Objective:To explore the real experience and feelings of the medical and nursing staff in the Emergency Department of general hospitals in receiving patients with mental disorders, so as to provide reference for formulating corresponding interventions.Methods:From October to November 2021, a semi-structured in-depth interview was conducted with 16 Emergency Department medical and nursing staff from two Class Ⅲ general hospitals in Nanjing by objective sampling. Colaizzi method was used to analyze the data.Results:After analyzing the interview data, three themes were extracted, namely, the emotional experience in treating and nursing patients with mental disorders, the barriers to providing the best management for patients with mental disorders (barriers at the environment/resource level, barriers at the emergency medical and nursing staff level, and barriers at the patient level) , and the educational needs of receiving patients with mental disorders (improving disease awareness and mental health service ability, and changing the way of thinking) .Conclusions:The medical and nursing staff in the Emergency Department of general hospitals have different degrees of negative emotions when receiving patients with mental disorders. There are insufficient knowledge and ability and a series of barriers when providing mental health services for patients. Managers should take various measures to improve the physical and mental health of patients with mental disorders in Emergency Department.
6.Prenatal diagnosis of a novel SOX10 mutation in a patient with syndromic hearing loss.

OBJECTIVE:
To explore the genetic basis for a patient with syndromic hearing loss.
METHODS:
Genomic DNA of the patient was extracted, for which 127 deafness-related genes were enriched with a chip. Following next generation sequencing, pathogenic loci in exonic regions were analyzed through comparison against the databases. Genotype of her fetus for the suspected site was determined by testing the amniotic fluid sample. qPCR method was applied to verify the deletion of a large fragment.
RESULTS:
The proband was diagnosed with Waardenburg syndrome type 2, and had harbored a novel heterozygous deletion of the exons 3 and 4 of the SOX10 gene. Her fetus was found to carry the same deletion and presented with blue eyes and deafness after birth.
CONCLUSION
Waardenburg syndrome type 2 due to SOX10 gene deletion may feature autosomal dominant inheritance with incomplete penetrance. The deletion of exons 3 and 4 of the SOX10 gene probably underlies the disease in this family.

7.Musculoskeletal manifestations of mild osteogenesis imperfecta, a case with ankylosing spondylitis

Objective We reported a case of osteogenesis imperfecta (OI) concurrent with ankylosing spondylitis (AS) and reviewed the literature to investigate the Musculoskeletal manifestations of OI.Methods A 27 years old male patient was admitted to department of rheumatology,Qilu hospital with complaints of left hip and low back pain accompanied with morning stiffness.He had pain and gradual limitation in range of motion of the spine for 2 years and symptomatic exacerbation nearly 8 months.His mother,aunt,daughter and son suffered from OI as well.Clinical examinations revealed blue sclera but hearing and teeth were normal.He had typical AS appearance and severe deformities of left hip joint.His erythrocyte sedimentation rate (ESR) was 32 mm/1 h,C-reactive protein (CRP) was 9.00 mg/L,and human leukocyte antigen (HLA)-B27 was positive.Magnetic resonance imaging (MRI) scanning of sacroiliac joint was consistent with spondyloarthropathies and avascular necrosis of left femoral head.After being treated with daily 99Te methylenediphonate intravenously,oral risedronate 5 mg and diclofenac sodium and sulfasalazine,his condition was improved.Results OI was an autosomal dominant hereditary disease defined by bone frailty due to abnormal synthesis of type Ⅰ collagen in bone matrix.It affects the entire skeleton,predisposing the patient to frequent non-traumatic fractures,causing pain,skeletal deformity,and disability.Conclusion OI patients have many musculoskeletal manifestations,such as arthralgia and deformities and should be considered in the differential diagnosis by rheumatologist.
8.Biliary complications and recovery of liver function after liver transplantation from citizen's deceased donors versus standard criteria donors

Objective To investigate the biliary complications and recovery of liver function after liver transplantation from citizen's deceased donors (DCD) versus standard criteria donors (SCD).Method The clinical data of 269 patients who underwent orthotopic liver transplantation from January 2009 to December 2016 at the Fuzhou General Hospital were collected.197 livers were from SCD and 72 from DCD.Propensity score matching (PSM) was used to compare the biliary complications and recovery of liver function after liver transplantation in the two groups.Results PSM matched 61 pairs of patients.There were 10 (16.4%) and 8 (13.1%) biliary complications in the DCD and the SCD groups,respectively,with no significant difference between them (P > 0.05).The recovery of liver function was significantly delayed in the DCD group when compared with the SCD group.The levels of ALT,AST,GGT and AKP in the DCD group were significant different on the postoperative first,third,fifth,seventh and fourteenth day (P < 0.05).At 30 days after surgery,there was no significant difference in liver function between the two groups.Conclusions Liver grafts from DCD had a significant impact on the recovery of liver function.When compared with the SCD group,the DCD group recovered significantly slower in liver function.There was no significant increase in the incidence of biliary complications.
9.Liver transplantation for treatment of hilar cholangiocarcinoma with a report of three Cases

Objective To investigate the feasibility of liver transplantation in the treatment of inoperable hilar cholangiocarcinoma. Methods The clinical data for 3 patients with unresectable hilar cholangiocarcinoma who underwent liver transplantation in the Department of Hepatobiliary Surgery of Fuzhou General Hospital of People's Liberation Army from January 2006 to December 2012 were retrospectively analyzed. The patients were followed up by phone, outpatient service, and hospitalization. The starting point of the follow-up was the operation date. The patients death was the end point. The clinical and pathological features, postoperative survival, tumor recurrence, and prognostic factors were observed. The follow-up deadline was December 2017. Results All 3 patients underwent classical orthotopic liver transplantation using retrograde perfusion through inferior vena cava and no perioperative deaths occurred. All 3 patients were followed up for 10 to 132 months. During the follow-up period, of 1 patient who died of tumor recurrence, the pathological TNM stage was T4a N1 M0, and both had Union for International Cancer Control stage Ⅳa, and the tumor-free survival time was 3 months, and the survival time was12 months. Of 1 patient who died of other causes, the pathological TNM stage was T3N1 M0, and both had Union for International Cancer Control stage Ⅲ, and the tumor-free survival time was 12 months, and the survival time was12 months. One case as of the end of follow-up, the patient has survived for 132 months, the pathological TNM staging was T2a NOM0, and both had Union for International Cancer Control stage Ⅱ. Conclusions Lymph node positive and high pathological TNM stage were poor prognosis factor for hilar cholangiocarcinoma who underwent liver transplantation. Patients with early hilar cholangiocarcinoma who don't have lymph node metastasis are expected to benefit from liver transplantation.
10.A survey on the AIDS-related knowledge of dental undergraduate students in three universities

Objective To understand the present condition of preclinical dental undergraduate in-terns' AIDS-related knowledge. Methods The questionnaire was designed referring to domestic and foreign relevant questionnaires. The cluster sampling method was used to conduct the questionnaire survey among the undergraduates who started their clinical internship in July 2016 in three domestic colleges, using SPSS 14.0 statistical software to analyze it. Results In this survey, 144 questionnaires were distributed, and 144 ones were filled out and sent back. The survey results suggested that 97.22% (140) of interns held a posi-tive attitude towards aiding HIV-infected patients despite that 96.53% (139) of interns knew that dental medical staff was considered to be highly susceptible with HIV. In spite of the fact that most interns knew AIDS-related knowledge and protection measures, 40.28% (58) of interns were not clear about incubation period of AIDS, and the average awareness of children's AIDS and methods to disinfect HIV-infected dental instruments was less than half in the interns. Meanwhile it was found that 86.81% (125) of interns didn't perform well in terms of complying with the principles of confidentiality in AIDS prevention and treatment. Conclusion Dental clinical interns' attitudes towards AIDS-infected patients should be praised. Further-more, they should accept education about infection control to have a deeper understanding about AIDS-related clinical manifestations and legal knowledge in order to reduce the probability of nosocomial infec-tion of AIDS.
