1.Observation on the application of polyester filament fiber surgical drape in neurosurgery

		                        			Objectives:To investigate the effect of polyester filament fiber surgical drape in neurosurgery.Methods:A total of 181 neurosurgical surgeries were selected from April to July 2019 in a first-class general hospital. They were randomly divided into 2 groups, cotton group (92 cases) were covered with cotton drape, polyester filament fiber group (89 cases) were covered with polyester filament fiber. The anti-permeation performance, incidence of intraoperative hypothermia, and incidence of postoperative surgical site infection (SSI) between the two groups were compared.Results:At the end of the operation, the wetting rate of the cotton draped was 58.7% (54/92) and that in polyester filament fiber was 15.7% (14/89), with statistically significant differences ( χ2 value was 35.605, P<0.05);The incidence of intraoperative hypothermia was 22.8% (21/92) in the cotton group and 11.2% (10/89) in the polyester filament fiber group, with statistically significant differences ( χ2 value was 4.281, P<0.05). The incidence of SSI in the cotton group was 16.3% (15/92) , while that in the polyester filament fiber group was 6.7% (6/89) , with statistically significant differences ( χ2 value was 4.034, P<0.05). Conclusions:In neurosurgical operations with a long operation time and a large amount of irrigation fluid during the operation, using the polyester filament fiber drape can prevent the irrigation fluid from wetting the surgical drape, protect the surgical incision better, reduce the incidence of SSI and intraoperative hypothermia to some extent.
		                        		
		                        		
		                        		
		                        	
2.Construction of clinical nursing pathway in Post Anesthesia Care Unit based on fast and slow channels

		                        			Objective:To construct a dual-track clinical nursing path based on fast and slow channels in Post Anesthesia Care Unit (PACU) to provide references for the implementation of standardized nursing for patients in the anesthesia recovery period.Methods:On the basis of literature review, evidence-based methods were used to construct the classification management of patients in anesthesia recovery period based on the theoretical framework of fast and slow channels. The research team initially drafted the clinical nursing path table in the PACU from May 2019 to June 2019 and conducted two rounds of Delphi expert letter inquiries from July to October 2019 to further revise and improve the path table.Results:The recovery rates of the two rounds of expert consultation were 90.91% and 100.00%. The expert authority coefficient was 0.89, the variation coefficient of the path table was 0.06, and the Kendall harmony coefficient was 0.25. Finally, the flow chart and three levels of anesthesia recovery based on fast and slow channels were formed.Conclusions:The established clinical nursing path in the PACU has reliable results. Based on the fast and slow channels to assess the risk degree of patients in the anesthesia recovery period, the classified management of patients can be implemented in the PACU, providing a reference for clinical nursing practice.
		                        		
		                        		
		                        		
		                        	
3.Clinical value of serum phospholipase A2 in the screening and diagnosis of benign prostatic hyperplasia

		                        			Objective To investigate the clinical value of serum lipoprotein associated phospholipase A 2 (Lp-PLA2)as a biomarker for benign prostatic hyperplasia(BPH).Methods A total of 65 serum samples of BPH patients were selected as BPH group,64 serum samples of healthy male as control group.The serum lev-el of Lp-PLA2,total prostate specific antigen(tPSA)and free prostate specific antigen(fPSA)in both groups were detected,and the specificity and sensitivity of Lp-PLA2,F/T ratio and combine both were analyzed by ROC curve.Results The serum level of Lp-PLA2,tPSA and fPSA in BPH group were significantly higher than those in control group,the different were significant(P<0.05).ROC curve analyze shown that the area under the curve(AUC)of Lp-PLA2 was 0.763,95% confidence interval(CI)was 0.680-0.833;AUC of F/T ratio was 0.715,95% CI 0.633 -0.795;AUC of Lp-PLA2 combined with F/T ratio was 0.832,95% CI 0.756-0.892.Conclusion The serum level of Lp-PLA2 could be used as a potential diagnostic marker for BPH,and the diagnostic value of Lp-PLA2 combined with F/T ratio was better than ther single indicator. 4.Correlation between ABCB1 Polymorphisms and Synergistic Effects of Wuzhi Capsules on Tacrolimus

		                        			Objective:To investigate the correlation of ABCB1 polymorphisms and the synergistic effect of Wuzhi capsules on ta-crolimus. Methods:The ABCB11236C>T(rs1128503), ABCB12677G>T/A(rs2032582) and ABCB13435C>T(rs1045642) genotypes were determined by restriction fragment length polymorphism ( RFLP) analysis. The whole blood levels of tacrolimus in renal transplant recipients were measured by chemiluminescent microparticle immunoassay. Analysis of covariance ( ANCOVA) was per-formed to determine the difference of tacrolimus C0/D among the various groups. Results: Whether co-administeration of Wuzhi cap-sules and tacrolimus or not, tacrolimus C0/D of ABCB11236C>T, ABCB12677G>T/A and ABCB13435C>T genotype and haplo-type was without significant difference. 5.Study on the Association of Synergistic Effects of Wuzhi Capsules on Tacrolimus with CYP3A5*3 Gene Polymorphism

		                        			OBJECTIVE:To investigate the association of synergistic effects of Wuzhi capsules on tacrolimus with CYP3A5*3 (6986A>G,rs776746) gene polymorphisms. METHODS:One hundred and severty patients underwent renal transplantation receiving tacrolimus maintenance therapy after surgery were selected from our hospital during Jan. 1997-Dec. 2015,and then divided into Wuzhi capsules(+)group(74 cases)and Wuzhi capsules(-)group(96 cases)according to the use of Wuzhi capsules. Both groups received tacrolimus+mycophenolate mofetil+prednisone;Wuzhi capsules (+)group was additionally given Wuzhi capsules,one capsule each time,bid,for more than 12 months. Trough concentration of tacrolimus was detected by CMIA 0,1,3,6,12 months after medica-tion,and the blood concentrations(C0/D)were calculated at different time points after correcting daily dose. CYP3A5*3 gene polymor-phisms was detected by PCR-RFLP. The association of C0/D value with gene polymorphism was investigated by analysis of covariance. RESULTS:Among 170 patients,there were 65 cases of CYP3A5 GG genotype,83 cases of AG genotype and 22 cases of AA geno-type;genotype frequencies were 38.2%,48.8% and 12.9%,which was in line with Hardy-Weinberg balance (P>0.05). There was statistical significance in the distribution frequencies of GG,AG+AA genotype between Wuzhi capsules(+)group and Wuzhi capsules (-)group (P<0.05). After 1 month of medication,C0/D of tacrolimus in GG genotype was significantly higher in Wuzhi capsules (+)group than in Wuzhi capsules(-)group. After 1,3,6,12 months of medication,C0/D of tacrolimus in AG+AA genotype was sig-nificantly higher in Wuzhi capsules(+)group than in Wuzhi capsules(-)group,with statistical significance(P<0.05). There was no statistical significance in C0/D of tacrolimus in GG genotype between 2 groups after 3,6,12 months of treatment(P>0.05). CON-CLUSIONS:Wuzhi capsules can increase C0/D of tacrolimus in CYP3A5*3 AG+AA genotype,but have no significant effect on C0/D of tacrolimus in GG genotype;CYP3A5*3 genotype should be considered when using Wuzhi capsules as synergist of tacrolimus.
		                        		
		                        		
		                        		
		                        	
6.G1986A and G1899A in the pre-C region of HBV promote the serological conversion of e antigen

		                        			Objective To investigate the correlation of the 1896 and 1899 mutations of hepatitis B virus (HBV)with the conversion of e antigen in serum and the progression of the disease. Methods 238 serum samples from the patients with HBsAg positive for over six months and HBV-DNA copy number > 5.0 × 102 IU/mL were collected,and the sequence analysis was used to analyze the nucleotide sequences of the 1896 and 1899 sites in the pre-C region of HBV. At the same time,the relevant clinical data and the expressions of HBeAg were collected,followed by Spearman correlation analysis and chi square test with SPSS 20.0. Results Both 1896 and 1899 sites in the pre-C region of HBV were mutated,and the base G was A,which was closely related to the expression of e antigen(P<0.05). Both G1896A and G1899A promoted the e antigen serological conversion ,and the e antigen serological conversion of G1899A was higher than that of G1896A. G1899A was associated with HBV related disease progression (correlation coefficient 0.280,P < 0.05),especially with the incurrence of HCC. 7.Clinical value of procalcitonin detection and blood culture in early diagnosis of blood stream infection

		                        			Objective To explore the clinical value of procalcitonin (PCT ) and blood culture in the united diagnosis of early blood stream infection .Methods The blood specimens of 625 patients were collected ,the serum level of PCT was detected by EL‐FA ,and the blood culture was accessed at the same time .23 cases of blood culture positive samples were received continuous detec‐tion of PCT ,and the results were analyzed .Results Positive rate of PCT was 41 .01% (0 .05 -2 .58 μg/L)in patients with blood culture negative results (negative group) ,and that in patients with blood culture positive results (positive group) was 80 .77%(0 .05-200 .00 μg/L) .The positive rate of PCT in positive group was significantly higher than negative group (χ2 =65 .12 ,P<0 .01) .Positive rates of PCT in patients with infection of Candida tropicalis ,kinds of bacteria ,Gram‐negative bacilli and Gram‐posi‐tive cocci were 100 .00% ,100 .00% ,92 .11% and 56 .81% ,respectively .Continuous detection of PCT in 23 patients with blood cul‐ture positive results showed that patients with gradually decreased PCT level suggested a good prognosis ,and patients whose PCT levels were higher than 10 μg/L and were maintained at high levels had poor prognosis .Conclusion Simultaneously blood culture and PCT detection was important to the early diagnosis and treatment of blood stream infection .
		                        		
		                        		
		                        		
		                        	
8.Enhanced sensitivity of leukemia cell line KG-1a to activated immune cell-mediated cytolysis after treated with resveratrol.

OBJECTIVETo explore the enhanced sensitivity of leukemia cell line KG-1a to activated immune cell-mediated cytolysis after treated with resveratrol.
METHODSThe value of 50% inhibition concentration (IC₅₀) for KG-1a by resveratrol was analyzed using trypan blue staining. Peripheral blood mononuclear cells were separated, and then activated by interleukin (IL)-2 and IL-15. The sensitivity of KG-1a treated with and without resveratrol to activated immune cell-mediated cytolysis was assayed by lactate dehydrogenase (LDH) -releasing assay. The expression of tumor necrosis factor related apoptosis inducing ligand (TRAIL) on the surface of activated immune cells and its receptors (DR4/5 and DcR1/2) on the surface of KG-1a were detected by flow cytometry.
RESULTSResveratrol could inhibit the proliferation of KG-1a and IC50 at 24 h was 25 mmol/L. At a ratio of 10:1 or 20:1 between effect and target, the cytolytic rates of treated KG-1a by activated immune cells were (55.80 ± 10.88)% and (72.31 ± 13.06)%, significantly higher than (24.96 ± 9.25)% and (37.93 ± 5.21)% of untreated KG-1a (P<0.05). The expression of DR5 on the surface of KG-1a treated with resveratrol was (9.05 ± 3.57)%, significantly higher than (3.11 ± 0.54)% of untreated KG-1a (P<0.05). Conversely, the expression of DcR1 on the surface of treated KG-1a was (13.23 ± 3.56)%, lower than (53.75 ± 10.51)% of KG-1a (P<0.05). When TRAIL pathway on the surface of activated immune cells was blocked, the cytolytic rates of treated KG-1a were (35.97 ± 6.36)% and (49.80 ± 10.68)%, significantly lower than (52.92 ± 6.98)% and (70.73 ± 9.79)% of untreated KG-1a (P<0.05) at the same ratio of effector and target.
CONCLUSIONResveratrol could enhance cytolytic sensitivity of KG-1a by activated immune cells through TRAIL pathway.

9.Value of plasma NT-proBNP assessing prognosis of patients with diastolic heart failure complicated dia-betes mellitus

		                        			Objective:To explore value of plasma N terminal pro B type natriuretic peptide (NT-proBNP)assessing prognosis of patients With diastolic heart failure (DHF)complicated type 2 diabetes mellitus (T2DM).Methods:The folloW-up data of 206 inpatients With DHF and T2DM from our hospital Were retrospectively analyzed.Ac-cording outcome,they Were divided into no event group (n=108)and event group [n=98,With major adverse car-diovascular events (MACE)occurred].Levels of NT-proBNP,glycosylated hemoglobin (HbA1c),total cholesterol (TC)and loW density lipoprotein cholesterol (LDL-C),left ventricular end-diastolic dimension (LVEDd),left ventricular peak early diastolic velocity/peak late diastolic velocity (E/A)and left ventricular ejection fraction (LVEF)during admission Were measured and compared betWeen tWo groups. Results:Compared With no event group,there Were significant increase in NT-proBNP level and LVEDd,and significant decrease in LVEF and E/A in event group (P<0.05 all).Single factor linear analysis indicated that NT-proBNP,HbA1c,LVEDd and E/A Were positively correlated With MACE incidence rate (r=0.075~0.091,P<0.05 all).Mutivariate Logistic regres-sion analysis indicated that NT-proBNP Was an independent predictor for MACE (OR=1.003,P=0.009).Areaunder recieve operating characteristic curve (ROC)Was 0.803 for NT-proBNP predicting mortality of patients during admission (P <0.05),and survival rate of patientsWith plasma NT-proBNP≤4010pg/mlWas significantly higher than that of patients With NT-proBNP>4010 pg/ml (OR=5.028,P <0.05).Conclusion:Plasma NT-proBNP can independently predict prognosis of patients With diastolic heart failure complicated type 2 diabetes mellitus.
		                        		
		                        		
		                        		
		                        	
10.Quality of Life for Tuberculosis Patients after Anterior Debridement, Autograft Bone Fusion and Anterolateral Fixation Operation

		                        			Objective To evaluate the quality of life for tuberculosis patients after anterior debridement, autograft bone fusion and anterolateral fixation operation. Methods 17 cases of thoracolumbar spinal tuberculosis were treated surgically from January 2008 to March 2011. All the cases underwent anterior debridement, autograft bone fusion and anterolateral fixation operation. MOS health survey 36-item Short Form (SF-36), Japanese Orthopaedic Association Scores (JOA) and Visual Analogue Scale (VAS) were used to evaluate the quality of life, spine function and pain symptom before and 1 and 6 months after surgery. Results Compared to the results 1 month after surgery, the scores of physical function (PF), role physical (RP), bodily pain (BP), general health (GH), vitality (VT), social function (SF), role emotional (RE), and mental health (MH), and JOA subjective symptoms, JOA clinical signs, and JOA daily activity limitation, and VAS improved (P<0.05) 6 months after surgery; compared to pretreatment, the scores of PF, BP, GH, VT, SF, and MH, and JOA subjective symptoms, JOA clinical signs, and JOA daily activity limitation, and VAS improved (P<0.05). Conclusion The anterior debridement, autograft bone fusion and anterior fixation operation is effective to improve the quality of life, spine function and pain symptom for tuberculosis patients.
