1.Comparison of the control effect of spherical and toric orthokeratology on low-to-moderate myopia with astigmatism in adolescents

AIM: To compare the control effect of spherical and toric orthokeratology on low-to-moderate myopia with astigmatism(-1.00--1.50 DC)in adolescents.METHODS: The clinical data of 119 cases(119 eyes)of low-to-moderate myopia with astigmatism(-1.00--1.50 DC)adolescents who were treated and fitted with orthokeratology in the ophthalmology department of Sichuan Provincial People's Hospital from June 2021 to January 2022 were retrospectively analyzed. They were divided into spherical group, with 65 cases(65 eyes), and toric group, with 54 cases(54 eyes)according to the type of orthokeratology. The changes of uncorrected visual acuity(UCVA), axial length and corneal astigmatism before and after wearing lenses were recorded to evaluate the therapeutic effect.RESULTS: The UCVA of both the groups significantly improved at 1 and 2 a after wearing lenses(all P<0.01); corneal astigmatism decreased, but there was no significant difference(all P>0.05); the axial length was longer than that before wearing lenses(P<0.01). There were no statistical significant differences in the UCVA and corneal astigmatism between the spherical group and the toric group(Fintergroup=0.829,Pintergroup=0.364; Fintergroup=0.997,Pintergroup=0.320); and there were no statistical significant differences in the axial length growth between the spherical group and the toric group after wearing lenses for 1 a(0.18±0.11 mm vs 0.17±0.14 mm), and 2 a(0.17±0.10 mm vs 0.16±0.10 mm; all P>0.05).CONCLUSION: Both orthokeratology lenses can improve the UCVA, reduce corneal astigmatism, and delay axial length growth of adolescents with low-to-moderate myopia with astigmatism(-1.00--1.50 DC), and there are no significant differences in the control effect of spherical design orthokeratology and the toric design orthokeratology on myopia.

Inflammatory pseudotumor-like follicular dendritic cell sarcoma(IPT-like FDCS)is a very rare malignant tumor that is considered to be associated with Epstein-Barr virus.Two patients in this report were generally healthy,and the spleen tumor was found during physical examination.After completing the examination,laparoscopic total splenectomy was performed,and the pathological result showed IPT-like FDCS.Postoperative chemoradiotherapy was not performed in either case.The disease has no characteristic clinical manifestations,and imaging overlaps with sarcoma.Microscopic manifestation showed CD21,CD23 and EBER positive spindle tumor cells in the inflammatory background with matted arrangement.Due to the interwoven distribution of tumor cells and lymphocytes,diagnosis is difficult.In this article,we report this two cases with literature review and summarize their clinical and pathological features to improve diagnostic cognition.
3.Current Status of Outcome Indicators in Randomized Controlled Trials of Traditional Chinese Medicine for Treating Chronic Atrophic Gastritis

ObjectiveThis paper aims to analyze the current status of outcome indicators in randomized controlled trials (RCT) of traditional Chinese medicine (TCM) for treating chronic atrophic gastritis (CAG), so as to provide references for constructing the core outcome set (COS) of TCM in the treatment of CAG. MethodChina National Knowledge Infrastructure (CNKI), Wanfang, VIP, SinoMed, PubMed, Embase, and Cochrane Library databases were searched for RCTs of TCM in the treatment of CAG in the last five years. The risk of bias of included studies was evaluated, and the selection status of outcome indicators was statistically analyzed. ResultA total of 150 RCTs were included, with a sample size of 44-398 cases. 164 outcome indicators were reported, with an application frequency of 1 229 times. The outcome indicators were classified into seven indicator domains according to functional attributes, followed by physical and chemical examination (69.41%), TCM syndrome (12.69%), symptoms and signs (11.15%), safety indicators (5.37%), quality of life (0.65%), long-term prognosis (0.65%), and economic evaluation (0.08%). According to the statistical analysis, there were problems in the selection of outcome indicators in RCTs of TCM for treating CAG, including various indicators, non-standard name reports, unclear primary and secondary indicators, random combination of subjective and objective indicators, neglected patient report outcome indicators, missing long-term prognosis and economic indicators, insufficient reporting of safety indicators, and inconsistent measurement tools and measurement time points. ConclusionIn the past five years, there have been many problems in the selection of outcome indicators in RCTs of TCM for treating CAG. It is necessary to actively promote the construction of the COS of TCM in the treatment of CAG and promote the high-quality development of clinical research of TCM.
4.Effect comparison between autologous tissue breast reconstruction and breast-conserving plastic surgery in breast cancer

Objective:To investigate the differences in survival, recurrence, metastasis and breast reconstruction satisfaction between autologous tissue breast reconstruction and breast-conserving plastic surgery in breast cancer patients.Methods:A retrospective case series analysis and cohort study was conducted. Clinical data of 144 breast cancer patients in Shanxi Province Cancer Hospital from January 2018 to December 2021 were retrospectively analyzed. The patients were divided into breast-conserving plastic surgery group (treated with breast-conserving plastic surgery) and breast reconstruction group [treated with autologous tissue breast reconstruction (transverse rectus abdominis musculocutaneous flap)] according to the surgical methods, and the intervention effect, BREAST-Q scale score and breast aesthetic effect were compared. The patients were followed-up for 1 year, and their survival, local vecurrence and distant metastasis status were recorded.Results:There were 72 patients in the breast-conserving plastic surgery group, aged (45±1) years old, and 72 patients in the breast reconstruction group, aged (45±2) years old. Patients in the breast reconstruction group and breast-conserving plastic surgery group survived 1 year after surgery, and the differences in local recurrence rate [1.39% (1/72) vs. 2.78% (2/72), χ2 = 0.34, P = 0.560] and distant metastasis rate [1.39% (1/72) vs. 1.39% (1/72), χ2 = 0.00, P = 1.000] were not statistically significant. The BREAST-Q scale breast satisfaction score in the breast reconstruction group was higher than that in the breast-conserving plastic surgery group [(87±6) points vs. (67±5) points], and the difference was statistically significant ( t = 14.33, P < 0.001); the differences in scores of psychosocial health, sexual health, physical health of the chest, and the adverse effects of radiotherapy between the two groups were not statistically significant (all P > 0.05). The rate of excellent and good aesthetic outcomes for the breast in the breast reconstruction group was higher than that in the breast-conserving plastic surgery group [95.83% (69/72) vs. 69.44% (50/72)], and the difference was statistically significant ( χ2 = 17.47, P < 0.001). Conclusions:The survival, recurrence and metastasis in breast cancer patients with autologous tissue breast reconstruction and breast-conserving plastic surgery are similar, and there are no differences in psychosocial health, sexual health, physical health of the chest and the adverse effects of radiotherapy, but breast aesthetic effect of autologous tissue breast reconstruction is better, and the patients are more satisfied with their breasts.

This study explored the ideas and methods of acupuncture for Guillain-Barré Syndrome (GBS) with the core principle of “to treat flaccidity, select the yangming (阳明) channel only”. The main pathological mechanism of GBS is deficiency of qi and blood in the yangming channel, malnutrition of all sinews, diminished spleen and stomach function leading to the production of pathogenic damp-heat qi, which obstructs the meridians, and gradually affects the liver and kidneys, consuming essence and damaging blood. Concurrently, dysfunction of the dumai (督脉) pivotal mechanism and lack of moisture in sinews and vessels result in symptoms such as skin numbness, paralysis, and muscle wastage. In clinical diagnosis and treatment, a combination of syndrome and channel differentiation is taken. Treatment primarily focuses on acupoints of yangming channel, aiming to supplement qi and blood, and acupoints of du mai are combined to open the vessel and fill the marrow. Specific acupoints are selected based on syndrome differentiation, providing comprehensive regulation to promote harmonization of qi and blood, relieve meridians, and the smooth generation and circulation of whole body fluids. This, in turn, enhances the strength of muscles and bones, and fosters a robust and freely moving body.
6.Analysis of Review of Anti-tumor Drug Prescriptions Assisted by Information System

Objective To analyze the problems of review of anti-tumor drug prescriptions and medical orders assisted by an information system to improve the review rules,and to provide a reference for improving review quality of anti-tumor drug prescription.Methods The problem with the pre-review of anti-tumor drug prescriptions and medical orders assisted by the information system in Sichuan Cancer Hospital during 2020-2022 were collected.The data came from the MEDICOM PASS system in Sichuan Cancer hospital.Clinical pharmacists made comments on relevant problems and analyzed the results.Results A total of 9 325 antitumor drug pre-approval problems,including 6 279 outpatient prescriptions(67.3%)and 3 046 inpatient orders(32.7%),among which 6 153(66.0%)were unsuitable indications,1 933(20.7%)were drug contraindications,449(4.8%)were problematic routes of administration,345(3.7%)were unsuitable drug compatibility,177(1.9%)were inappropriate drug frequency,133(1.4%)were problematic drug populations,74(0.8%)were unsuitable single doses,39(0.4%)were unacceptable drug interactions,22(0.2%)were unsuitable drug total.The results of clinical pharmacists'comments were 4 459 reasonable cases,with a false positive rate of 47.8%.The false positive problems included 2 264(50.8%)cases of unsuitable indications,1 933(43.3%)cases of drug contraindications,231(5.2%)cases of problematic routes of administration,and 31(0.7%)cases of unsuitable populations.Conclusion The review of anti-tumor drug prescriptions assisted by an information system can effectively intercept irrational drug use and improve the review quality of prescriptions and medical orders.However,the evidence-based medicine date of antitumor drugs is updated quickly.Pharmacists should constantly improve the prescription review rules based on the latest evidence-based medicine data.
7.Surgical procedure coding for common renal replacement therapies in nephrology

The complexity of coding surgical procedures related to renal replacement therapy in nephrology stems from a deficiency in clinical knowledge regarding renal replacement therapies and an incomplete understanding of the classification rules within the ICD-9-CM-3 coding system.This paper delves into the clinical aspects of renal replacement therapy and organizes the corresponding coding classification rules,clarifying the codes for various treatment modalities.For instance,the establishment of dialysis access is coded as 38.95 for hemodialysis venous intubation,39.27 for vascular fistula,and 54.93 for peritoneal dialysis intubation via a cutaneous peritoneal stoma.Maintenance hemodialysis is coded as 39.95,while peritoneal dialysis is coded as 54.98.The removal of dialysis catheter is differentiated into surgical and non-surgical;surgical removal is coded as 86.05,and non-surgical removal as 97.86 or 97.89.For instances of internal fistula stenosis or thrombosis,balloon dilation is coded as 39.50.Stent implantation for stenosis or isolation of a false aneurysm is coded as 39.90 for bare stent,and 00.55 for covered stents.The resection and reconstruction involving stenosis,thrombus segments,or false aneurysms,are coded as 39.42.This classification aims to improve the accuracy of coding for such procedures.
8.Main diagnostic selection and coding quality analysis of 205 obstetric inpatient records

Objective By analyzing the main diagnostic choices of inpatient records in obstetrics and the problems exist-ing in ICD codes,we can improve the quality of the main diagnostic choices and coding in obstetrics.Methods A total of 205 obstetric inpatient medical records were randomly selected from a hospital from January 1 to April 30,2024,and the medical re-cords were checked jointly by three senior coders,and the errors were summarized and analyzed by using the check table.Results Among 205 obstetrical medical records,72(35.12%)were found to have errors in the selection and coding of major diagnoses;among them,there were 55 cases of incorrect selection by clinician and incorrect compilation by coders,13 cases of correct selection by clinician,incorrect compilation or omission by coders,4 cases of omission by clinician and omission by cod-ers.Conclusion The selection and coding of the main diagnosis in obstetrics are very special and difficult,so the training of cli-nicians should be strengthened,the writing of clinicians should be standardized,and the importance of the main diagnosis selec-tion should be strengthened.At the same time,strengthen the management of coders,standardize the coding operation process,strengthen the learning of professional knowledge,improve professional reserves,strengthen the two-way communication with cli-nicians,and jointly improve the main diagnosis selection and coding accuracy rate of obstetrics,so as to ensure the quality of data on the first page of obstetric inpatient medical records.
9.Effect of immune-related plasma proteins under genetic regulation on Parkinson's disease

Objective To explore the connection between immune-related plasma proteins and Parkinson's disease.Methods By analyzing genome-wide association study data of 4907 immune-related plasma proteins,we assessed their direct impact on the risk of Parkinson's disease.Single-nucleus RNA sequencing data were also utilized for protein expression analysis.Results Four im-mune-related proteins,cerebral dopamine neurotrophic factor(CDNF),cathepsin B(CTSB),im-munoglobulin G Fc receptor 2a(FCGR2A),and hemoglobin beta subunit(HBB),were identified to be associated with the risk of Parkinson's disease.Among them,increased expression levels of CDNF,CTSB and HBB were found to decrease the risk(OR=0.871,95%CI:0.779-0.973,P=0.015;OR=0.835,95%CI:0.758-0.920,P=0.001;OR=0.735,95%CI:0.631-0.857,P=0.001),whereas increased level of FCGR2A was associated with a higher risk of PD(OR=1.137,95%CI:1.058-1.223,P=0.001).Singl e-cell sequencing analyzes protein expression and its dis-tribution among different cell types in the brain.CDNF and CTSB exhibit high expression levels in multiple brain cell types,FCGR2A is predominantly expressed in brain microglia and HBB shows minimal expression in the brain.Conclusion There are potential links between the four proteins CDNF,CTSB,FCGR2A and HBB and the risk of Parkinson's disease.Our results emphasize that the genetic risk variants of Parkinson's disease influence the disease's occurrence by modulating the expression of these immune-related proteins.Additionally,single-cell expression data reveal the expression patterns of these target proteins in the brain.

Purpose/Significance To design a comprehensive data platform to meet the needs of collecting and sharing survey data on meteorological sensitive diseases,so as to enhance monitoring capabilities for meteorological sensitive diseases in China.Method/Process Through various methods such as data extraction,data exchange,data import and customized collection,disease data,meteoro-logical data,environmental data and diagnosis and treatment data are processed and integrated into the platform for unified management.Result/Conclusion This platform realizes the functions of data collection,aggregation,visualization display and data sharing,which can provide support for scientific researchers in various bases across the country to better manage and utilize meteorological sensitive disease survey data.
