1.Two-sample bidirectional Mendelian randomization to analyze the causal relationship between blood metabolites and keloids

Objective:To explore the causal relationship between blood metabolites and keloids.Methods:The study was a two-sample bidirectional Mendelian randomization (MR) analysis-based study. Blood metabolites of 7 824 adult volunteers and 8 299 participants and data related to 481 912 keloid patients were obtained from the genome-wide association studies (GWAS) Catalog database. Single nucleotide polymorphisms (SNPs) significantly associated with blood metabolites and keloids were screened for inclusion as instrumental variables in the MR analysis by setting a significance threshold of P<1.0×10 -5, chain imbalance analysis [ r2 = 0.001, kilobase pairs (kb) = 10, 000)], and the F statistic ( F≥10) . Five method of MR analysis, i.e., inverse variance weighting (IVW) as the main method and MR-Egger regression, weighted median, simple modeling, and weighted modeling as auxiliary method, were used to analyze the causal relationship between blood metabolites (exposure factors) and keloids (outcome variables) . Sensitivity analyses were performed on eligible blood metabolite SNPs to assess the reliability and stability of the findings: heterogeneity was assessed by Cochran Q-test and MR-Egger regression test, MR Egger intercept test to rule out horizontal pleiotropy, leave-one-out test to determine if the presence of a single SNP significantly affected the result of the MR analyses, MR-PRESSO method was used to test for outliers of SNPs, which were corrected by false discovery rate (FDR) (FDR <0.2) to control the false positive rate. Reverse MR analysis was performed with keloid as the exposure factor, and blood metabolites screened by the aforementioned MR analysis were used as outcome variables for effect analysis and sensitivity analysis. The data were analyzed using R 4.3.2 software and the TwoSampleMR program package therein, and the causal effect values of the MR analysis were expressed as the ratio ( OR) and 95% CI, with P<0.05 being considered as a statistically significant difference, i. e., the evidence of a potential causal effect was substantial. Forest plots, funnel plots, and scatter plots were constructed to visualize the result of MR analysis and sensitivity analysis. Results:A total of 1 400 blood metabolites with 34 843 SNPs were obtained from the GWAS Catalog database, all of which were consistent with the hypothesis that genetic variants are closely associated with exposure factors; a total of 24 197 210 SNPs were obtained from the keloid dataset. IVW analysis revealed that one blood metabolite, succinyl taurine (16 ∶ 1n-7), had 28 SNPs with keloid with a causal relationship ( OR=1.13, 95% CI 1.06-1.19, P<0.001, FDR=0.070) ; MR-Egger regression method ( OR = 1.11, 95% CI 1.04-1.19, P=0.005), weighted median method ( OR = 1.11, 95% CI 1.02-1.20, P=0.014) and weighted modeling method ( OR=1.12, 95% CI 1.04 to 1.20, P=0.004) analyses also showed that succinyl taurine (16 ∶ 1n-7) was a risk factor for keloid disease; the result of the simple modeling method only showed that the causal relationship between succinyl taurine (16 ∶ 1n-7) and keloid disease was not significant ( OR=1.10, 95% CI 0.85-1.41, P=0.485) . MR overall analysis showed a significant positive causal relationship between succinyl taurine (16 ∶ 1n-7) and keloid, i.e., elevated levels of succinyl taurine (16 ∶ 1n-7) were associated with an increased risk of keloid disease. Cochran Q-test ( Q = 26.98, P=0.465), MR-Egger regression test ( Q = 26.65, P = 0.428), MR-Egger intercept test ( P = 0.574), and MR-PRESSO composite test ( P=0.569) showed that there was no heterogeneity and horizontal pleiotropy among SNPs ( P>0.05) ; the leave-one-out test confirmed that individual SNPs did not have a significant effect on the overall result, indicating that the result had reliability and stability. The inverse MR analysis suggested that there was no causal relationship between keloid on succinyl taurine (16 ∶ 1n-7) (IVW: OR=0.98, 95% CI 0.93-1.04, P=0.490) . Conclusions:There is a significant positive causal relationship between the blood metabolite succinyl taurine (16 ∶ 1n-7) and keloids, and succinyl taurine (16 ∶ 1n-7) is a risk factor for keloid disease.

3.Development of review indicators and analysis of obstructive and facilitating factors for gastrointestinal function recovery in postoperative patients with gynecologic malignancies

Objective:To implement evidence-based nursing practices for gastrointestinal function recovery in postoperative patients with gynecologic malignancies, develop review indicators based on the best available evidence, analyze their obstructive and facilitating factors, and provide references for applying evidence in clinical practice.Methods:Based on the 6S Pyramid Model, a comprehensive computer-based search was conducted across Chinese and international databases for evidence related to gastrointestinal function recovery in patients with gynecologic malignancies. The search included clinical decisions, guidelines, evidence summaries, systematic reviews, expert consensus, and randomized controlled trials up to July 10, 2023. The knowledge-to-action framework (KTA) was used to systematically retrieve, evaluate, and summarize the best evidence to develop an evidence-based nursing review plan for postoperative gastrointestinal function recovery in gynecologic malignancy patients. A baseline review was conducted, and influencing factors were analyzed to formulate action strategies.Results:A total of 16 articles were included, and 17 review indicators were developed. One indicator had an execution rate of 100%, one indicator had an execution rate of 95%, and the remaining 15 indicators had execution rates between 0 and 60%. Obstructive factors included ingrained previous work patterns, lack of awareness regarding evidence translation, increased clinical workload leading to resistance among medical staff, lack of necessary systems, processes, and equipment for practice change, and poor communication between multidisciplinary teams. Facilitating factors included Qilu Hospital of Shandong University (Qingdao) being an evidence-based nursing evidence application base in Shandong Province, with the Department being a key discipline in the hospital and having the necessary talent for practice change. In addition, the Department had high cooperation among medical staff and strong leadership with a focus on evidence-based clinical translation.Conclusions:There is a significant gap between clinical practice and best evidence in the gastrointestinal function recovery of postoperative patients with gynecologic malignancies. Precisely analyzing obstructive and facilitating factors and developing scientific action strategies are essential for driving effective evidence translation.
4.Application of retroauricular sulcus incision in the operation of benign tumors in the deep lobe of parotid gland

Objective:To investigate the application of retroauricular groove incision in the resection of benign tumors in the deep lobe of parotid.Methods:From January 2017 to January 2022, 19 patients (11 males and 8 females, age ranged from 17 to 69 years, with a median age of 48) with benign tumor in the deep lobe of parotid gland underwent parotidectomy through retroauricular sulcus incision in Linyi People′s Hospital. Among them, 17 cases with tumor diameter≤4.0 cm underwent simple retroauricular groove incision, and 2 cases were dumbbell type with tumor diameter>4.0 cm on the medial side of mandible protruding into the parapharyngeal space, in which the deep lobe and tumor of parotid gland were resected through retroauricular sulcus incision combined with intraoral incision.Results:Tumors were completely removed through retroauricular sulcus incision in 17 cases, and dumbbell type tumors were removed through retroauricular sulcus incision combined with intraoral incision in 2 cases. Postoperative pathological examinations showed pleomorphic adenoma in 13 cases, basal cell adenoma in 4 cases and Warthin′s tumor in 2 cases. Temporary mandibular marginal branch paralysis occurred in 2 patients and returned to normal 3 weeks after operation. All incisions healed in Phase I. By following-up of 1-5 years with a median follow-up time of 3.1 years, none of the patients had Frey syndrome, salivary fistula, other complications and tumor recurrence. The patients and their families were satisfied with the postoperative facial appearances.Conclusion:The retroauricular groove approach can not only preserve the function of parotid superficial lobe and facial nerve, but also has less trauma, less tissue defect and hidden scar. As the advantages of less complication, low recurrence rate and good cosmetic effect, the incision is worthy of clinical application.
5.Application of retroauricular sulcus incision in surgery of parotid gland tumor

Objective:To investigate the application of retroauricular sulcus incision in surgery for benign tumor in superficial lobe of parotid gland.Methods:A total of 20 patients were reviewed who underwent the resection of benign tumor in the superficial lobe of parotid gland with retroauricular sulcus incision at the Department of Otorhinolaryngology, the Linyi City People Hospital between May 2015 and March 2017. There were 12 males and 8 females with an age range of 12-66 years old, including 15 cases with mixed tumor and 5 cases with adenolymphoma. Surgical procedure included facial never dissection from the main trunk to facial never cervical trunk, temporal trunk and peripheral branches. After resection of tumor and superficial lobe of parotid gland, and sternocleidomastoid muscle flap was used to fill the defect in parotid area. All patients were postoperatively followed up after surgery.Results:Surgeries were successfully operated in all patients. Follow-up for 2 years showed there was no presence of recurrence, salivary fistula, Frey syndrome, scar affecting facial appearance, or significant deformity in the surgical area.Conclusion:Surgery for superficial lobe of parotid gland and facial nerve dissection through retroauricular sulcus incision have some advantages such as incision concealment, simple procedure for facial nerve dissection, simultaneous repair of tissue defects with sternocleidomastoid muscle flap, and less complications.
6.Effects of propofol and midazolam on the prognosis of patients treated with noninvasive positive pressure ventilation

Objective To compare the effects of propofol and midazolam on the prognosis of patients treated with noninvasive positive pressure ventilation.Methods A prospective,single-blind,randomized controlled trial (RCT) was conducted in 90 patients who were treated with noninvasive ventilation for acute dyspnea in the ICU of the Sixth People's Hospital Affiliated to Shanghai Jiaotong University from October 2014 to December 2016.They were randomly divided into three groups according to the digital table,with 30 cases in each group.The control group was not given sedation treatment.The propofol group was given propofol 0.5 ~ 1 mg/kg,and then administered by intravenous infusion of 1 mg · kg-1 · h-1 with a micropump.The midazolam group was given midazolam 0.05-O.1 mg/kg,and then with intravenous infusion of 0.05-0.1 mg · kg-1 · h-1 maintaining the patients'sedation goals(Ramsay score of 2).The vital signs and blood gas analysis indicators were recorded.The incidence of tracheal intubation,the incidence of hospital infection,length of ICU and hospital stay,mortality and sedation-related complications were compared.Results The tracheal intubation rate in the propofol group was similar to that in the midazolam group (20.0% vs.23.3%,x2 =2.65,P > 0.05),while the tracheal intubation rate (46.7%) in the control group was significantly higher (x2 =4.21,4.17,all P < 0.05).The length of ICU and hospital stay in the pmpofol group [(7 ± 3)d and (15 ± 5) d] and midazolam treatment group[(8 ± 4) d and (16 ± 4) d] were significantly shorter than those in the control group[(13 ± 4) d and (20 ± 6) d] (t =2.384,2.371,2.392,2.389,all P < 0.05).The mortality rates of 30d (20.0%,6/30) and 90d (30.0%,9/30) in the control group were higher than those in the propofol group(10.0%,3/30;20.0%,6/30),and the midazolam group (13.3%,4/30;23.3%,7/30),but the differences were not statistically significant(P > 0.05).The incidence rates of hospital infection in the pmpofol group and midazolam group were 6.6% (2 cases) and 10.0% (3 cases),which were significantly lower than 33.3% (10 cases) in the control group (x2 =4.32,4.23,all P < 0.05).Conclusion The use of mild sedation in patients of acute dyspnea treated with noninvasive positive pressure ventilation can improve the patients' tolerance rate,reduce the rate of tracheal intubation and the incidence of hospital infection,and decrease the length of ICU and hospital stay,without significant adverse reactions.There was no significant difference between propofol and midazolam.

Objective To understand the prevalence,treatment and influence factors of hypertension in maintenance hemodialysis (MHD) patients in Anhui Province.Methods A total of 2724 adult patients on MHD from January 1st 2014 to March 31st 2014 in 26 hospitals of southern,northern and central Anhui Province were investigated.Their demographic characteristics,primary disease,complications,medications,dialysis and laboratory examination were explored.The prevalence treatment rate and control rate of hypertension were analyzed.Associated factors for controlling hypertension [systolic blood pressure (SBP) < 140 mmHg and diastolic blood pressure (DBP) < 90 mmHg] were assessed by logistic regression analysis.Results (1) The prevalence of hypertension in the hemodialysis patients was 87.0%.Their treatment rate and control rate were 93.2% and 23.9% respectively.The average of SBP was (145.90±21.18) mmHg,and the DBP on average was (83.60± 12.21) mmHg.The most commonly used anti-hypertensive drug is calcium channel blocker (88.2%).Over one third (45.7%) of patients were treated with two kinds of anti-hypertensive drug,26.2% with 1 kind,21.7% with 3 kinds,and 6.4% with 4 kinds or more.(2) Compared with non-hypertension patients,patients with hypertension have older age,higher body mass index (BMI),phosphorus,SBP and DBP,as well as lower hemoglobin and Kt/V (all P < 0.05).(3) The multivariate logistic regression analysis showed that Ca > 2.50 mmol/L (OR=2.084,95%CI 1.008-4.307,P=0.047) positively correlated with controlling hypertension,while smoke (OR=0.594,95%CI 0.356-0.911,P=0.046) and BMI 18.5 ~ 23.9 kg/m2 (OR=0.516,95%CI 0.293-0.907,P=0.022) negatively correlated with it.Conclusions High prevalence yet low control rate of hypertension in MHD patients in Anhui Province were observed.Hypocalcemia may be a protective factor for hypertension control,while smoke and BMI may be risk factors for it.
8.Effect of one-lung ventilation on cerebral oxygen balance and energy metabolism during total intravenous anesthesia for thoracoscopic surgery

Objective To observe the effect of one-lung ventilation (OLV) on cerebral oxygen balance and energy metabolism during total intravenous anesthesia for thoracoscopic surgery.Methods Thirty patients scheduled for thoracoscopic surgery were selected.After inducing and intubating,patients were assigned to maintenance of anesthesia with propofol by target controlled infusion in order to maintain a bispectral index(BIS) between 40 and 60,and end-tidal partial pressure of carbon dioxide (PETCO2) between 30mmHg and 35mmHg.Mean arterial pressure (MAP),heart rate (HR),SpO2,PetCO2,cerebral blood flow velocity (CBFv),BIS value and nasopharyngeal temperature(NPT) were measured,always with the patients in the lateral position,in four phases:10min after beginning twolung ventilation (TLV),15 min after beginning OLV (OLV + 15),30min after beginning OLV (OLV + 30) and 60 min after beginning OLV(OLV + 60).Blood samples were drawn simultaneously and analyzed within 5min.The Da-jvO2,CERO2,CMRO2,Da-jvLac and Da-jvGlu at each phase were calculated.Results In all patients,a decrease in PaO2 [(172±85) vs (428±42);(162±54) vs (428±42);(185±61) vs (428±42)] and MAP [(70±10) vs (81 ±11) ; (71 ± 12) vs (81 ± 11)] occurred during OLV (t =15.02,13.14,23.25,20.16,18.02,all P < 0.05).SjvO2 at the phase:OLV + 15 and OLV + 30 were significantly lower than those at TLV [(54.0 ± 1.2) % vs (65.0 ± 0.8) % ;(55.0±1.5)% vs (65.0 ±0.8)%] (t =3.12,2.14,all P<0.05).Ca-jvO2[(50 ± 12)% vs(40 ± 12)% ;(54±11)% vs (40 ± 12)%],CMRO2 [(186 ±40) vs (162 ± 35);(191 ±24) vs (162 ±35)]and CERO2 [(36 ± 12) vs (30 ± 1 1) ; (35 ± 10) vs (30 ± 11)] atthephase:OLV + 15 andOLV + 30weresignificantlyhigher than those at TLV (t =5.23,4.28,1.86,2.01,8.21,10.11,all P < 0.05).After OLV,Da-jvGlu [(0.45 ± 0.10) vs (0.22 ± 0.30) ; (0.52 ± 0.20) vs (0.22 ± 0.30) ; (0.40 ± 0.20) vs (0.22 ± 0.30)] significantly increased (t =6.45,12.03,15.10,all P < 0.05).The differences of Da-jvLac and CBFv at every phase were not significant (P >0.05).Conclusion During total intravenous anesthesia,OLV resulted in an increase of consumption of cerebral oxygen and energy.It may be not good for cerebral oxygen balance and energy metabolism.The efficient prevention is necessary clinically.
9.Investigation on disturbing of adipocytic differentiation of rBMSCs by loaded titanium particles.

Our previous investigations demonstrated that varying sizes of loaded titanium particles could inhibit proliferaion, adhension and osteoblastic differentiation of rat bone marrow mesenchymal stem cells (rBMSCs). The present study aims to validate the hypothesis that particled-shaped wear debris from prosthetic implants influence the adipocytic differentiation of rBMSCs. The effects of different sizes of loaded titanium particles (6.9 microm, 2.7 microm and 0.9 microm) on the adipocytic differentiation of rBMSCs were studied by observing lipid droplet formation, assaying the expression of lipoprotein lipase (LPL) mRNA by RT-PCR and Triglycerides (TG) secretion. The loaded titanium particles were found to influence adipogenesis of rBMSCs, but had different effects, depending upon particle size, concentration and loading time duration. 2.7 microm and 0.9 microm titanium particles promoted lipid droplet formation, LPL mRNA expression and TG secretion, while at a higher concentration of titanium particles and a longer loading duration, 6.9 microm titanium particles gradually inhibited adipogenic differentiation of rBMSCs. Three sizes of loading titanium particles obviously disturbed the adipocytic differentiation capability of rBMSCs: the smaller particles promoted but the larger inhibited the adipogenic differentiation of rBMSCs.

10.Influence of titanium particles loading on releases of cytokines of osteoblasts.

Aseptic loosening is mainly mediated by bone resorption cytokines in the surrounding area of orthopaedic implants. Our previous investigation demonstrtated that different-sized titanium particles loading can inbibit the osteoblastic differentiation and mineraliztion. In order to investigate the hypothesis that particulate wear debris derived from prosthetic biomaterials affects the release of bone resorption related cytokines, we studied the influence of different-sized titanium particles loading on the osteoblastic cytokines by assaying the secretion of IL-6, IL-10 with use of ABC-quantitative sandwich enzyme-linked immunosorbent assay, and on the expression of osteoclast differentiation factors (ODF) by RT-PCR. The results showed that the 0.9 microm titanium particles promoted osteoblasts producing bone resorption cytokines (IL-6, ODF), and simultaneously secreted bone absorption restraining factor (IL-10) quickly and transitorily. In comparison, the 2.7 microm and 6.9 microm titanium particles,especially the latter primarily promoted osteoblasts secreting bone absorption promoting factors powerfully and slowly. The results suggested that there was a biphasic response appearing in titanium particles loaded-osteoblastic cultures, the level of which varied according to the different size and the loading time of titanium particles. This in vitro experimental result showed that attentaion to the inhibition of bone resorption cytokines stimulated by wear debris and to the screen potential favourable biomaterials for implants must be taken.
