1.A multivariate analysis of acute severe cholangitis and the establishment and evaluation of a risk prediction scoring model

		                        			 Objective To investigate the independent risk factors for acute severe cholangitis and related protective factors, and to construct a risk prediction scoring model for acute severe cholangitis. Methods A retrospective analysis was performed for the clinical data of 381 patients with acute cholangitis who were admitted to Department of Hepatobiliary Surgery, The 940th Hospital of Joint Logistics Support Force of Chinese People's Liberation Army, from January 2016 to July 2021, among whom there were 273 patients with non-severe cholangitis and 108 patients with severe cholangitis. Univariate and multivariate logistic regression analyses were used to screen out the independent risk factors for acute severe cholangitis and related protective factors, and then a logistic regression model was established. The receiver operating characteristic (ROC) curve was used to evaluate the discriminatory ability of the model, the calibration curve was used to evaluate the prediction accuracy of the model, and decision curve analysis (DCA) was used to evaluate the clinical value of the model. Moreover, the enhanced Bootstrap method was used to perform internal validation of the model and evaluate the performance of the model in internal validation. The model was visualized by the construction of Web calculator, nomogram, and scoring system. The two-independent-samples t test was used for comparison of normally distributed continuous data between groups, and the Mann-Whitney U test was used for comparison of non-normally distributed continuous data between groups; the chi-square test was used for comparison of categorical data between groups. Results The univariate and multivariate logistic regression analyses showed that total bilirubin (TBil) (odds ratio [ OR ]=1.014, 95% confidence interval [ CI ]: 1.009-1.020, P < 0.001), percentage of neutrophils ( OR =1.128, 95% CI : 1.088-1.175, P < 0.001), and age ( OR =1.053, 95% CI : 1.027-1.082, P < 0.001) were independent risk factors, and albumin (Alb) ( OR =0.871, 95% CI : 0.817-0.924, P < 0.001) was a protective factor. The above independent risk factors and protective factor were included in the logistic regression analysis for model fitting, and the predictive model obtained had an area under the ROC curve (AUC) of 0.925 (95% CI : 0.897-0.952), with a specificity of 0.817 and a sensitivity of 0.935 at the optimal cut-off value of 0.245. The calibration curve showed that the predicted probability of the model was approximately equal to the actual probability, with a Brier value of 0.098, and the decision curve analysis showed that the model had a higher net income within the threshold probability interval of 0.1-0.9. Internal validation showed an AUC internal validation of 0.915 and a Brier value internal verification of 0.106. Conclusion TBil, percentage of neutrophils, and age are independent risk factors for acute severe cholangitis, while Alb is a protective factor. The established risk prediction scoring model has good discriminatory ability, calibration, and clinical value and can identify patients with acute severe cholangitis at an early stage, which provides a reference for subsequent diagnosis and treatment. 
		                        		
		                        		
		                        		
		                        	
2.Summary of best evidence for postoperative rehabilitation training in patients with lumbar disc herniation

		                        			Objective:To retrieve and obtain evidence related to postoperative rehabilitation training of patients with lumbar disc herniation and to summarize the best evidence.Methods:The computer was used to search domestic and foreign guideline networks, evidence-based databases, original research databases and orthopedic related websites for relevant evidence on rehabilitation training for patients with lumbar disc herniation after surgery, including evidence summary, guidelines, expert consensus, systematic review and Meta-analysis. The retrieval time limit was from the establishment of the database to April 30, 2021. The literature quality evaluation, evidence extraction, classification and integration were conducted independently by two researchers, and disagreements were discussed or adjudicated by a third party.Results:A total of 14 articles were included, including 3 evidence summaries, 1 guideline, 6 expert consensuses, and 4 systematic reviews. A total of 13 best evidences of rehabilitation training for patients with lumbar disc herniation after surgery were summarized in 8 categories, including rehabilitation assessment, rehabilitation principle, rehabilitation time, rehabilitation intensity, rehabilitation mode, patient education, rehabilitation psychotherapy and follow-up monitoring.Conclusions:This study can provide evidence-based basis for nursing staff to carry out postoperative rehabilitation guidance, but in clinical practice, physical, psychological and cultural background conditions of patients should be comprehensively assessed, and individualized rehabilitation programs should be formulated for patients based on local medical level and conditions.
		                        		
		                        		
		                        		
		                        	
3.Application of nursing intervention based on individual and family self-management theory in postoperative rehabilitation of patients undergoing percutaneous transforaminal endoscopic surgery

		                        			Objective:To explore the effect of nursing interventions based on individual and family self-management theory (IFSMT) on postoperative rehabilitation of patients undergoing percutaneous transforaminal endoscopic surgery.Methods:A total of 835 patients who underwent percutaneous transforaminal endoscopic surgery in Shanxi Provincial People's Hospital from January 2018 to January 2020 were selected as research objects by the convenient sampling method. According to the random envelope method, patients were divided into the control group ( n=417) and the observation group ( n=418) according to the approximate 1∶1 standard. The control group was given routine nursing intervention, while the observation group was given nursing intervention based on IFSMT on the basis of the control group. The differences in self-management ability, postoperative recovery time, lumbar functional recovery, pain score and postoperative complication rate before and after intervention were compared between the two groups. Results:After intervention, the scores of treatment management, psychological management, life management and knowledge and skill management in the observation group were higher than those before intervention and were higher than those in the control group, and the differences were statistically significant ( P<0.01). The hospitalization days, self-care time and work recovery time of observation group were shorter than those of control group, and the differences were statistically significant ( P<0.01). After intervention, the Oswestry dysfunction index and pain score in the observation group were lower than those before intervention and lower than those in the control group. The treatment score evaluated by the Japanese Orthopedic Association was higher than that before intervention and higher than that of the control group, and the differences were statistically significant ( P<0.01). The incidence of postoperative complications in observation group was lower than that in control group, and the difference was statistically significant ( P<0.05) . Conclusions:IFSMT based nursing intervention for patients undergoing percutaneous foraminal endoscopic surgery is beneficial to postoperative rehabilitation of patients, which is worthy of clinical promotion.
		                        		
		                        		
		                        		
		                        	
4.The role of miR-146a-5p in enriched rehabilitation training′s ability to improve the cognition of stroke survivors

		                        			Objective:To observe the effect of enriched rehabilitation training on cognitive function, plasma mir-146a-5p microRNA precursor levels and inflammatory factors in persons with post-stroke cognitive impairment (PSCI).Methods:Fifty-eight persons with PSCI were randomly divided into an observation group and a control group, each of 29. The observation group was given enriched rehabilitation training, while the control group was provided with conventional cognitive rehabilitation training. The Montreal Cognitive Assessment Scale (MoCA), the Digit Span Test (DST), parts A and B of the Trail Making Test (TMT A-B) and the Modified Barthel Index (MBI) were used to assess the subjects′ cognitive functioning and their ability in the activities of daily living (ADL). Plasma levels of mir-146a-5p, IL-6 and TNF-α were detected before and after the treatment.Results:After treatment, the average MOCA, DST and MBI scores, as well as the average TMT A-B times had improved significantly for both groups. However, the observation group′s averages were significantly better than those of the control group on all three tests. After the treatment, the average plasma expression of miR-146a-5p had increased significantly in both groups, but the increase in the observation group was significantly greater. Plasma IL-6 and TNF-α levels were significantly lower than before the treatment, with the average TNF-α level in the observation group significantly lower than that of the control group.Conclusions:Enriched rehabilitation training can improve the cognition of stroke survivors more effectively than conventional cognitive rehabilitation training. 5.Retrospective analysis of the effectiveness of treating multi-space infection combined with descending necrotizing mediastinitis at oral maxillofacial and cervical region via multidisciplinary team collaboration

		                        			Objective:To explore the strategy and experience for treating maxillofacial and cervical multi-space infection combined with descending necrotizing mediastinitis (DNM) via multidisciplinary team (MDT) collaboration.Methods:A total of 36 patients with maxillofacial and cervical multi-space infection complicated with DNM admitted to the First Affiliated Hospital of Zhengzhou University from July 2011 to July 2019 were included in the study. The clinical data of the patients were retrospectively analyzed, including gender, age, symptoms at admission, source of infection, preoperative and postoperative evaluation indicators, MDT strategy and prognosis.Results:There were 26 males and 10 females with an average age of (51.6±17.6) years (8-80 years). The course of disease before admission was (8.9±8.4) days (2-30 days). All patients were admitted with maxillofacial and neck swelling and pain as the main complaints. Odontogenic infection accounted for 39% (14/36), throat floor swelling and pain accounted for 25% (9/36) and unknown maxillofacial swelling accounted for 36% (13/36). There were 28 cases receiving surgical treatment, 26 cases were cured and discharged (72%), 10 cases died (28%). In the patients treated with multidisciplinary therapy (mainly by surgery), the white blood cell count, neutrophil percentage, C-reactive protein and procalcitonin levels were significantly improved compared with those at admission at each observation time point after operation ( P<0.05). The length of stay was positively correlated with the levels of C-reactive protein ( r=0.545, P<0.05) and procalcitonin ( r=0.504, P<0.05). The prognosis of patients treated with surgery (26/28) was better than that of patients without surgery (0/8) ( P<0.01). Conclusions:The patients with maxillofacial and cervical multi-space infection combined with DNM might be in critical condition. The surgical based MDT strategy has an important impact on the prognosis of patients. 6. Preliminary study on the elimination of artifacts of five kinds of dental prosthetic materials by energy spectrum CT multi-material artifact reduction technology

		                        			Three-unit fixed bridge restorations were fabricated on the first to third molars in pig jaw. Gold alloy, zirconia, cobalt chromium alloy, nickel chromium alloy and pure titanium were used as materials for these fixed bridges. After restoration delivery, the pig jaw was scaned using energy spectrum CT machines. Images in regular 120 kVp scan mode were used as conventional group, and reconstructed single-energy horizontal images of 80, 90, 100, 110, 120, 130 and 140 keV in energy spectrum scanning mode were used as energy spectrum group, and reconstructed images applied MMAR technology in energy spectrum scanning mode were used as energy spectrum MMAR group. Each group was scanned 10 times to measure CT value and noise of muscles around dental prosthetic materials and adjacent non-artifact layers. Artifact index was calculated. Two radiologists scored the image quality of each group subjectively. Kruskal Wallis rank sum test was used to compare the difference of image noise, artifact index and subjective score among the control group and the best keV condition in the energy spectrum group and the energy spectrum MMAR group.

The subjective image quality scores in energy spectrum MMAR group were as follows: 3.0±0.2 for gold alloy, 4.3±0.5 for zirconia, 3.0±0.4 for cobalt chromium alloy, 3.1±0.4 for cobalt chromium alloy, and 4.6± 0.5 for pure titanium. These scores were significantly smaller than those in the conventional group (P<0.05). There was no significant difference in noise between energy spectrum group and energy spectrum MMAR group (P>0.05), and the noise values in energy spectrum group and energy spectrum MMAR group were significantly lower than that in the conventional group (P<0.05). Conclusions Artifacts of pure titanium is minimal. Energy spectrum CT combined with MMAR technology can effectively reduce the artifacts of gold, zirconia, cobalt?chromium alloy, nickel?titanium alloy and pure titanium. 8.Laparoscopic feature and its relationship with clinical manifestations in infertile women with endometriosis

		                        			Objective To investigate the laparoscopic feature and its relationship with clinical manifesta-tions in infertile patients with endometriosis. Methods From Jan.2013 to Jan.2017,infertile patients who received hysteroscopy and hysteroscopy combination surgery in Ganzhou People′s Hospital were enrolled in this ret-rospective study.The basic clinical data and the distribution of endometriotic lesions were recorded and analyzed in patients with endometriosis simultaneously. Analysis was performed about the clinical manifestations and the rAFS stage of the patients. Results 204 patients are totally enrolled in the study. Among them,120 patients(58.8%) had endometriosis. The presence of menorrhea,dyspareunia,tenesmus,chronic pelvic pain,hypermenorrhea, adnexal mass,restricted uterine mobility,tenderness of adnexa,and tenderness nodules of posterior vaginal fornix are clinically statistically significant.In addition,the presence of ovarian endometrioma is also a significant clinical factor that is close related with the rAFS stage of infertile patients with endometriosis. Moreover,uterosacral liga-ment,rectovaginal septum,vagina and bowel were more common infiltrated in stage Ⅲ and stage Ⅳ endometrio-sis. Conclusions For infertile patients,detailed case history,overall bimanual examination and trans-vaginal ultrasound examination are very important to help diagnosis endometriosis preoperatively and provide evidence for the selection of individualized treatment strategies.
		                        		
		                        		
		                        		
		                        	
9.Prevalence and influencing factors for depression in post-esophagectomy patients

		                        			Objective To investigate the prevalence and influencing factors for depression in post-esophagectomy patients. Methods A total of 140 patients with post-esophagectomy who were operated in the First Affiliated Hospital of Xinxiang Medical University from June 2016 to March 2017 were selected in this study using convenient sampling. The prevalence of depression was evaluated using SDS. The related risk factors for post-esophagectomy depression were analyzed using Logistic regression analysis. Results Approximately 30.71% of the participants had depression. The Logistic regression showed that age, education, income, presence of chemoradiotherapy reactions, and postoperative complications were influencing factors for post-esophagectomy depression. Conclusions The risk factors for post-esophagectomy depression include increasing age, presence of chemoradiotherapy reactions, and postoperative complications. 10.Experience of clinical efficacy of renal transplantation from donors of donation after brain death complicated with acute kidney injury

		                        			Objective To summarize the clinical efficacy of renal transplantation from donors of donation after brain death (DBD) complicated with acute kidney injury (AKI). Methods Fifty-nine DBD donors successfully undergoing renal transplantation were recruited in this investigation. According to the Scr level upon admission of intensive care unit (ICU), DBD donors were divided into the AKI group (n=14) and control group (n=45). A total of 101 recipients were assigned into the AKI group (n=23) and control group (n=78) correspondingly. The organ donation conditions of 59 donors were summarized. Main parameters of the donors before organ procurement were statistically compared between two groups. Postoperative kidney function, hospitalization condition and clinical outcomes of the recipients were statistically compared between two groups. Results Among 59 donors, 14 cases (24%) suffered from AKI. Two donors received continuous renal replacement therapy during organ maintenance. Compared with the donors in the control group, the APACHE Ⅱ score of the donors was significantly higher (P<0.05), the incidence of central diabetes insipidus was considerably higher (P<0.01), the Scr levels at admission of ICU and before organ procurement were significantly higher (both P<0.01) and the amount of urine at 24 h before organ procurement was dramatically less in the AKI group (P<0.01).Compared with the recipients in the control group, the Scr levels at postoperative 2 and 3 d were significantly higher (both P<0.05), the length of hospital stay was considerably longer (P<0.01) and the hospitalization expanse was significantly higher in the AKI group (P<0.05). No statistical significance was observed in the postoperative delayed recovery of renal graft function, incidence of acute rejection, infection and rehabilitation dialysis in the recipients between two groups (all P>0.05). At 3 months after transplantation, the recipients in two groups were discharged and the graft survival rate was 100%. Conclusions For renal transplantation from DBD donors complicated with AKI, active measures should be taken to maintain the organ and relieve the AKI, which yields similar clinical efficacy to renal transplantation from non-AKI donors and widens the origin of kidney graft.
