1.Content determination of ephedrine hydrochloride and pseudoephedrine hydrochloride in Shexiang zhuanggu plaster and its quality evaluation

OBJECTIVE To establish the method for the content determination of ephedrine hydrochloride and pseudoephedrine hydrochloride in Shexiang zhuanggu plaster ,and to evaluate the quality of 222 batches of Shexiang zhuanggu plaster from 41 manufacturers. METHODS HPLC method was established. The determination was performed on Shimadzu Shim-pack GIS-C 18 column with mobile phase consisted of acetonitrile- 0.1% phosphoric acid (3∶97,V/V)at the flow rate of 0.9 mL/min. The column temperature was set at 30 ℃,the detection wavelength was 210 nm,the sample size was 10 μL. Taking the content data of 222 batches of samples as index ,the cluster heatmap was drawn by Hiplot biomedical data visualization and analysis platform. RESULTS The results of the methodological investigation were in line with the requirements of the general principles stated in 2020 edition of Chinese Pharmacopoeia (part Ⅳ ). The total contents of ephedrine hydrochloride and pseudoephedrine hydrochloride in 222 batches of samples from 41 manufacturers were 0.646-6.325 μg/cm2. Results of cluster heatmap analysis showed that these samples of 41 manufacturers could be divided into 3 categories. The contents of components in samples from different manufacturers varied greatly ,and the contents of components in different batches of samples from the same manufacturers also varied greatly. If the proposed limit of this product was that the total amount of ephedrine hydrochloride and pseudoephedrine hydrochloride per 1 cm2 would not be less than 1.00 μg,23 of the 222 batches of samples were unqualified. CONCLUSIONS The established method for the content determination of ephedrine hydrochloride and pseudoephedrine hydrochloride in Shexiang zhuanggu plaster is accurate and precise ,and can be used for the quality control. Some manufacturers should pay attention to optimizing production process and strengthening quality control.

Osteoporotic vertebral compression fracture (OVCF) can lead to lower back pain and may be even accompanied by scoliosis, neurological dysfunction and other complications, which will affect the daily activities and life quality of patients. Vertebral augmentation is an effective treatment method for OVCF, but it cannot correct unbalance of bone metabolism or improve the osteoporotic status, causing complications like lower back pain, limited spinal activities and vertebral refracture. The post-operative systematic and standardized rehabilitation treatments can improve curative effect and therapeutic efficacy of anti-osteoporosis, reduce risk of vertebral refracture, increase patient compliance and improve quality of life. Since there still lack relevant clinical treatment guidelines for postoperative rehabilitation treatments following vertebral augmentation for OVCF, the current treatments are varied with uneven therapeutic effect. In order to standardize the postoperative rehabilitation treatment, the Spine Trauma Group of the Orthopedic Branch of Chinese Medical Doctor Association organized relevant experts to refer to relevant literature and develop the "Guideline for postoperative rehabilitation treatment following vertebral augmentation for osteoporotic vertebral compression fracture (2022 version)" based on the clinical guidelines published by the American Academy of Orthopedic Surgeons (AAOS) as well as on the principles of scientificity, practicality and advancement. The guideline provided evidence-based recommendations on 10 important issues related to postoperative rehabilitation treatments of OVCF.
3.C-arm hip fluoroscopy at primary position in the internal fixation of femoral neck fracture

Objective:To evaluate the hip C-arm fluoroscopy at primary position in internal fixation of femoral neck fracture.Methods:A retrospective study was conducted of the 69 patients with femoral neck fracture who had been treated from August 2015 to August 2018.They were divided into a primary position group ( n=35) subjected to C-arm hip fluoroscopy at primary position under symmetry traction of bilateral lower limbs for reduction of femoral neck fracture and a control group ( n=34) subjected to C-arm hip fluoroscopy at frog’s position on the healthy side for traction and reduction of the injured hip. The 2 groups were compared in terms of fluoroscopy frequency, times of resetting guide pin, intraoperative blood loss and total operation time. Results:The 2 groups were comparable due to insignificant significances between them in the preoperative general data like gender, age, fracture type, injury cause, or interval from injury to operation ( P>0.05). Compared with the control group, the primary position group showed less intraoperative fluoroscopy (7.2 times ± 0.5 times versus 16.1 times ± 1.2 times), fewer times of resetting guide pin (2.1 times ± 0.31 times versus 4.7 times ± 0.8 times), less intraoperative blood loss (96.8 mL ± 18.6 mL versus 198.1 mL ± 13.2 mL), and shorter total operation time (1.2 h ± 0.2 h versus 1.6 h ± 0.3 h). All the differences were statistically significant ( P<0.05). Conclusions:C-arm hip fluoroscopy at primary position may protect the stability after fracture reduction because bilateral lower limbs are under symmetrical traction and the hip position needs no alteration. Operation of C-arm fluoroscopy is easy and convenient and produces clear images. Therefore, this new mode of fluoroscopy has advantages of less intraoperative fluoroscopy and guide pin resetting, leading to significantly reduced intraoperative blood loss, ineffective operation time and anesthesia time.
4.Identification of Chemical Constituents in Fengliaoxing Fengshi Dieda Wine

OBJECTIVE:To identify t he chemical constituents of Fengliaoxing fengshi dieda wine. METHODS :An ultra-high performance liquid chromatography-quadrupole-electrostatic field orbital trap high-resolution mass spectrometry (UPLC-Q-Exactive- MS)technique was used for identifying chemical constituents of Fengliaoxing fengshi dieda wine. The determination was performed on Thermo Accucore aQ RP 18 column with mobile phase consisted of 0.1% formic acid-methanol (gradient elution )at the flow rate of 0.3 mL/min. The column temperature was 30 ℃,and the sample size was 2 μL. HESI source was adopted,ion mode was positive and negative ion mode ,nitrogen as sheath gas and auxiliary gas. The positive ion mode had spray voltage of 3.5 kV, capillary heating temperature of 350 ℃,sheath gas pressure of 35 psi,auxiliary gas pressure of 15 arb,and ion source heating temperature of 320 ℃. The negative ion mode had spray voltage of 3.2 kV,capillary heating temperature of 350 ℃, sheath gas pressure of 35 psi,auxiliary gas pressure of 15 arb,and ion source heating temperature of 300 ℃. The mass axis was calibrated by external standard method (mass error less than 5 ppm). The scanning range of the first mass spectrometry was m/z 80.0-1 200.0 (the resolution was 70 000),the scanning range of secondary mass spectrometry was m/z 80.0-1 200.0(the resolution was 17 500),and the collision voltage was 20,40,60 eV. Retrieved from CNKI ,VIP,PubMed and other database ,the chemical constituents information of each Chinese traditional medicine in Fengliaoxing fengshi dieda wine were collected to establish chemical constituents database. The structure of the compounds was identified on the basis of above constituents database ,the relevant literature ,retention time of reference substance and MS fragmentation regularity. RESULTS & CONCLUSIONS : Fifty-nine compounds were identified in Fengliaoxing fengshi dieda wine ,including 12 flavones(e.g. neoeriocitrin ,hesperidin.),8 alkaloids (e.g. baogongteng C or erycibellin , ephedrine,pseudoephedrine), 9 organic acids (e.g. chlorogenic acid , cryptochlorogenic acid ,neochlorogenic acid ),7 coumarins(e.g. xanthotoxol), 4 esters, 4 amino acids , and 15 other categories(including volatile oils ,terpenes,amides). UPLC- 5103YX11SF37(17)] Q-Exactive-MS technology can quickly and accurately identify the chemical constituents in Fengliaoxing fengshi dieda wine.
5.Research on the relationship between mucoepidermoid carcinoma and human papillomavirus infection

OBJECTIVE To find out the relationship between mucoepidermoid carcinoma and Human papillomavirus (HPV) infection.METHODS Nested polymerase chain reaction (Nested PCR) was adapted to detect HPV DNA from 51 tissues of mucoepidermoid carcinoma using consensus primers, 9 tissues of chronic parotitis were as control.RESULTS HPV DNA was detected in 37 of total 51 mucoepidermoid carcinoma tissues, the detection rate was up to 72.5%, comparatively, it was 22.2% (2 of 9) in chronic parotitis tissues. The HPV DNA detection rate of mucoepidermoid carcinoma tissues was statistically higher than that of chronic parotitis tissues (χ2=8.517, P=0.004).CONCLUSION The results suggested that the HPV infection may cause mucoepidermoid carcinoma.
6.Association between single nucleotide polymorphism of adenylyl cyclase 3 and essential hypertension

Objective To explore the association between the tag single nucleotide polymorphism (tag SNP) of the adenylyl cyclase 3 (ADCY3) and the essential hypertension (EH).Methods From April to July 2013,a total of 1 061 subjects diagnosed with EH and 1 218 control subjects were recruited from Ningbo,Zhejiang Province.Information was collected by face-to-face interview.Twelve tag SNPs were detected by ligase detection reaction technique.Results After adjusted for age,gender,body mass index and other related factors,logistic regression analysis showed that 3 loci (rs11689546,rs7593130,rs2241759) were associated with EH.AG genotype of rs11689546 was associated with 0.494 times lower risk of EH (OR =0.494,95% CI0.246-0.993;compared with AA genotype).CT genotype of rs7593130 was associated with 1.596 times higher risk of EH (OR =1.596,95% CI 1.009-2.524;compared with TT genotype),and CT/CC genotype of rs7593130 was associated with 1.627 times higher risk of EH (OR =1.627,95% CI 1.034-2.559;compared with TT genotype).AG genotype of rs2241759 was associated with 0.669 times lower risk of EH (OR =0.669,95% CI 0.503-0.891;compared with AA genotype),and CT/CC genotype of rs2241759 was associated with 0.687 times lower risk of EH (OR =0.687,95% CI 0.518-0.911;compared with TT genotype).Conclusion The polymorphisms of ADCY3 are associated with lower (G allele of the rs11689546 locus and G allele of the rs2241759 locus) or higher (C allele of the rs7593130 locus) risk of essential hypertension.
7.Function of AFP and DCP in evaluating the prognosis of hepatocarcinoma

Objective To evaluate the prognosis in patients with hepatocarcinoma by examining the expression of α-fetoprotein (AFP) and des γ prothrombin (DCP).Methods Retrospectively analyzed the expression of AFP and DCP in 105 patients received curative hepatectomy.Divided the patients into three groups as follow:the tumor markers were both negative pre-and post-operation (Group N) ; the tumor markers decreased to normal after operation (Group D) ; the tumor markers kept positive or decreased but still higher positive after operation (Group P).Then combined the two markers and divided the patients into 4 groups:two markers both negative(AFP + DCP +)(Group A) ; AFP + DCP-(Group B) ; AFP-DCP + (Group C) ; AFP-DCP-(Group D).Results High AFP and DCP levels were significantly associated with poor tumor-free and overall survival.The presence of large size and advanced stage were significantly associated with Group P.Overall survival in the AFP-N group was significantly better than that of other groups and overall survival in DCP-N group were significantly better than that of the P group.After the combination,Group A had the best overall and tumor free survival rate while the D group had the worst.The differences between B and C group were not significant.Conclusions AFP and DCP can be both used solely as tumor markers and the expressions of them are associated with the prognosis.The combination of two markers can be used for better prediction of hepatocarcinoma.
8.Outpatient traffic analysis and management study of the hospital

Objective To investigate the pattern of outpatients traffic,orchestra service resources,and optimize service provision,for a harmonized outpatient service.Methods A baseline survey was made at the outpatient department in 30 days(six hours per day,7:00-11:00,and 13:00-15:00).On such basis,data of the hospital's HIS system within the last five years were called into play for statistics analysis and simulation test,for the purpose of shift scheduling and resources allocation optimization.Results This study discovered each January-February as the valley of clinic visits and each July-August as the peak.52-week data analysis of a year found that the days of daily registrations between 4000-5000 are up to 163 days,those over 5000 are up to 86 days,of which Mondays with over 5000 registrations are up to 35 days.Each Monday is found to be a peak of outpatient visits,with significant drop during Saturdays and Sundays; daily registrations peak between 7:00-10:00 in the morning,while the traffic rises again during 13:00-14:00 in the afternoon,yet up to 26 %of the morning peak only.Conclusion Statistics analysis of the outpatient service and emergency service at the outpatient department can help identify their patterns of operations,for effective improvement of service provision,alleviation of peak hour workload,shorten patient waiting time and better patient satisfaction.
9.The application of the single cell fluid in detecting HER-2 gene or protein

ObjectiveUsing the single cell fluid to establish a kind of practical experimental method,to detect the HER-2 gene and protein of breast cancer,and to provide reliable basis for pathology diagnosis.MethodsThe single cell fluid of breast cancer was preparaed by Medimachine system.Sucking a little of the single cell fluid spreaded on the slide and to be examined by immunohistochemistry using antibodies to HER-2 and fluorescence in situ hybridization.ResultsThe cell membrane of breast cancer was positive for HER-2 by immunohistochemistry according to American society of clinical oncology of American pathologists guideline recommendations for HER-2 testing in breast cancer and the HER-2 gene of the single cell amplified obviously by fluorescence in situ hybridization which the ratio of the average of HER-2 and the average of CEP17 exceeded 2.2.ConclusionThe single cell fluid could be used as positive control when we detected the HER-2 gene or protein of breast cancer,which guaranteed the correction of the results of the sample to be detected.
10.Research on apoptosis changes of tensor veli palatini in patients with obstructive sleep apnea-hypopnea syndrome

Objective:To explore apoptosis changes of dilator muscles in the upper airway by detecting the expression of Bax, Bcl-2 in tensor veli palatini in patients with OSAHS.Method:The expression of Bax and Bcl-2 were detected in tensor veli palatini in 30 cases with OSAHS and 10 cases chronic tonsillitis without OSAHS by immunohistochemistry and image analytical system, and the results were analyzed.Result:①The expression levels of Bax in the OSAHS group increased significantly compared to control group(P<0.05), but there were no significant differences of Bcl-2 expression between two groups, the ratio of Bax/Bcl-2 increased significantly(P<0.05). ②There were positive correlations between AHI and the expression levels of Bax(r=0.697,P<0.01) respectively in the test group.Conclusion:The results indicate that apoptosis occurred in tensor veli palatini in patients with OSAHS, and the more severity of OSAHS , the more apoptosis.
