1.Protein Modification Omics: A Novel Approach Investigating Mechanisms of Chinese Medicines

Traditional Chinese medicine (TCM), as an invaluable scientific legacy and cultural patrimony of China, encapsulates the cumulative wisdom of millennia from the Chinese civilization. The deeply rooted medical theories and extensive empirical practices of TCM hold an indispensible central role within China's healthcare framework. Facing the challenges of modern medicine and health needs, it is particularly urgent to explore the mechanisms of Chinese medicines. Post-translational modification (PTM), as a core mechanism to regulate the complex and diverse protein functions, is a bridge between environmental stimuli and physiological responses. Different PTMs can interact with each other to form a complex regulatory network, which is in line with the multi-component and multi-target action of Chinese medicines. In recent years, protein modification omics has emerged as a powerful means to probe into PTMs, and studies have employed protein modification omics to investigate the mechanisms of Chinese medicines. However, the research in this field is still in the initial stage. This article summarizes the basic theory of PTM and the basic research process of protein modification omics, reviews the application status of protein modification omics in the research on the mechanisms of Chinese medicines, and analyzes the existing research limitations. This study aims to explore an innovative research paradigm for analyzing the mechanisms of Chinese medicines from the perspective of PTM and provide a theoretical basis and practical guidance for understanding the mechanisms of Chinese medicines in the future.
2.Predictive value of isoform -2 proprostate-specific antigen and its derived indexes for prostate cancer patients with PSA 4-20 ng/mL

【Objective】 To evaluate the predictive value of isoform [-2] proprostate-specific antigen, p2 PSA (p2PSA) and its derived indexes for prostate cancer in a Chinese cohort with PSA 4-20 ng/mL. 【Methods】 A total of 139 males scheduled for biopsy were enrolled in the prospective study from Nov.2021 to Jun.2022. The total PSA (tPSA), free PSA (fPSA), fPSA/tPSA (f/t) and p2PSA were collected, and the percentage of p2PSA(%p2PSA) and prostate health index(PHI) were calculated. The predictive value of p2PSA and its derived indexes were compared with traditional indexes with receiver operating characteristic (ROC) curve and Logistic analysis. 【Results】 Prostate cancer was found in 54 cases (38.8%). There were significant statistical differences in tPSA(10.68 vs.8.14, P=0.021), f/t(0.13 vs.0.16, P=0.006), p2PSA(30.25 vs.19.81, P<0.001), %p2PSA(21.52 vs.13.15, P<0.001) and PHI(64.3vs.38.2, P<0.001) between prostate cancer patients and non-prostate cancer patients. The area under the ROC curve (AUC) of tPSA, fPSA, %fPSA, p2PSA, %p2PSA and PHI were 0.63, 0.51, 0.63, 0.71, 0.73, and 0.80, respectively. Claudin18.2(CLDN18.2)is a tight junction protein that is overexpressed in a variety of solid tumors such as gastrointestinal cancer and oesophageal cancer.It has been identified as a promising target and a potential biomarker to diagnose tumor,evaluate efficacy,and determine patient prognosis.TST001 is a recombinant humanized CLDN18.2 antibody that selectively binds to the extracellular loop of human Claudin18.2.In this study,we constructed a solid target radionuclide zirconium-89(89Zr)labled-TST001 to detect the expression of in the human stomach cancer BGC823CLDN18.2 cell lines.The[89Zr]Zr-des-ferrioxamine(DFO)-TST001 showed high radiochemical purity(RCP,>99%)and specific activity(24.15±1.34 GBq/μmol),and was stable in 5%human serum albumin,and phosphate buffer saline(>85%RCP at 96 h).The EC50 values of TST001 and DFO-TST001 were as high as 0.413±0.055 and 0.361±0.058 nM(P>0.05),respectively.The radiotracer had a significantly higher average standard uptake values in CLDN18.2-positive tumors than in CLDN18.2-negative tumors(1.11±0.02 vs.0.49±0.03,P=0.0016)2 days post injection(p.i.).BGC823CLDN18.2 mice models showed high tumor/muscle ratios 96 h p.i.with[89Zr]Zr-DFO-TST001 was much higher than those of the other imaging groups.Immunohistochemistry results showed that BGC823CLDN18.2 tumors were highly positive(+++)for CLDN18.2,while those in the BGC823 group did not express CLDN18.2(-).The results of ex vivo biodistribution studies showed that there was a higher distribution in the BGC823CLDN18.2 tumor bearing mice(2.05±0.16%ID/g)than BGC823 mice(0.69±0.02%ID/g)and blocking group(0.72±0.02%ID/g).A dosimetry estimation study showed that the effective dose of[89Zr]Zr-DFO-TST001 was 0.0705 mSv/MBq,which is within the range of acceptable doses for nuclear medicine research.Taken together,these re-sults suggest that Good Manufacturing Practices produced by this immuno-positron emission tomog-raphy probe can detect CLDN18.2-overexpressing tumors.
4.Construction of peacetime and wartime integrated training indicator system for nursing staff in Armed Police Force hospitals

Objective:To construct a peacetime and wartime integrated training indicator system for nursing staff in Armed Police Force hospitals, so as to provide scientific basis for reform of nursing staff training in the Armed Police Force.Methods:Using a mixed research design, from October 2021 to May 2022, based on the literature review and semi-structured interviews, the first draft of peacetime and wartime integrated training indicator system for nursing staff in Armed Police Forces hospitals was formed. The Delphi method was used to consult 15 experts for 2 rounds to determine the final version of the system.Results:The recovery rate of 2 rounds of expert inquiries was 100%, and the authority coefficients of the experts were 0.82 and 0.79. The Kendall′s rank-order correlation coefficients of 2 rounds of expert inquiries were 0.49, 0.51 for level-1 items, 0.27,0.31 for level-2 items ( all P<0.05). The final established peacetime and wartime integrated training indicator system for nursing staff in Armed Police Force hospitals included 5 level-1 items with 36 level-2 items, which were organizational management, teacher resources, training content, training forms and training assessment. The weights of all 5 level-1 items were from 0.135 to 0.221. Conclusions:The peacetime and wartime integrated training indicator system for nursing staff in Armed Police Force hospitals is scientific and reliable, which provides a reference for nurse training in Armed Police Force hospitals.
5.Preliminary Study on China ’s Insurance Model for ADR Injury Compensation under MAH System

OBJECTIVE:To construct China ’s Insurance Mode for ADR injury compensation model under marketing authorization holder (MAH)system,and to provide reference for reducing the medical disputes caused by ADR damage and protecting the rights and interests of patients. METHODS :Using the methods of recommendation and “snowball”,the interviewers were invited to conduct semi-structured interviews to obtain qualitative data ,and the results were analyzed by thematic analysis. According to the results of subject analysis ,the ADR injury compensation model was designed from three aspects of insurance type setting,insurance level division and compulsory insurance protection objects ,and the Fuzzy comprehensive evaluation method based on AHP was used to evaluate the model ;questionnaire and multiple factor Logistic regression analysis was adopted to analyze the influential factors of whether drug related institutions were willing to establish ADR injury compensation system. RESULTS:A total of 40 experts were invited and 34 of them completed the interview ,with a response rate of 85.0%. At present , the problems of ADR injury compensation in China mainly included the difficulties of third-party identification and adducing evidence of ADR injury ,the difficulty of timely and fair settlement of medical disputes caused by ADR injury ,the high cost and long time of handling ADR injury disputes ,and the uncertainty of compensation mode. More than half of the experts interviewed agreed with the compensation mode of insurance . The overall score of established model was 89.50. A total of 640 questionnaires were distributed and 559 valid questionnaires were recovered (204 drug manufacturers ,172 drug distributors and 183 medical institutions). The effective rate of the questionnaire was 87.3%. Multiple Logistic regression analysis showed that the factors positively correlated with the approval of establishing ADR injury compensation system after the implementation of MAH system in China included the familiarity of ADR staff in drug manufacturing enterprises with ADR related policies (β=1.697,P<0.05),the types of drugs operated by pharmaceutical companies (β=2.987, P<0.05),the cognitionlevel of medical staff on ADR (β= 2.800,P<0.05). The working years of medical staff were negatively correlated with the approval of establishing ADR injury compensation system after the implementation of MAH system ( β =- 2.544,P<0.05). CONCLUSIONS :ADR injury compensation model designed in this study is feasible. It has reference value for reducing the economic burden of ADR patients , treating patients ’health problems caused by ADR injury in time ,and promoting the development of China ’s drug insurance. Main influential factors for the establishment of ADR injury compensation model under MAH system contain the familiarity of ADR staff in drug manufacturing enterprises with ADR related policies ,the types of drugs operated by pharmaceutical companies ,the cognitionlevel of medical staff on ADR ,the types of drugs operated by pharmaceutical companies ,the cognitionlevel of medical staff on ADR.
6.The application value of dual-layer detector spectral CT angiography in evaluation of brain perfusion in patients with acute ischemic stroke

Objective:To explore the clinical application value of the dual-layer detector spectral CTA in evaluation of brain perfusion impairment in patients with acute ischemic stroke.Methods:Clinical and imaging data of 35 patients with acute ischemic stroke in Weihai Central Hospital from March 2020 to October 2020 were reviewed retrospectively. All patients underwent head and neck spectral CTA examination and dynamic cerebral perfusion CT examination with dual-layer detector spectral CT. The iodine density map and effective atomic number map were reconstructed using CTA data, and the iodine density and effective atomic number, as well as the cerebral blood volume (CBV) and cerebral blood flow (CBF) values of the hypoperfusion area and the contralateral side were measured and compared; the areas of brain hypoperfusion regions were measured. Pearson′s correlation coefficient was used to analyze the correlation between iodine density values and CBV values, iodine density values and CBF values, effective atomic number values and CBV values, effective atomic number values and CBF values, as well as hypoperfusion area shown on CTA images and displayed on CTP-CBF map.Results:Of all the 35 patients, the iodine density value [(0.22±0.07) mg/ml], effective atomic number value (7.38±0.05), CBV value [(1.9±0.7) ml/100 g] and CBF value [(15.1±5.9) ml/(100 g·min)] of the hypoperfusion area were significantly lower than those of the healthy side [iodine density value (0.44±0.10) mg/ml, effective atomic number value (7.52±0.06), CBV value (3.4±0.7) ml/100 g, CBF value (57±27) ml/(100 g·min); t values were -14.7, -14.5, -11.2, -9.7, respectively, all P<0.001]. No significant difference was found between the hypoperfusion area shown on spectral CTA [(2 292±1 393) mm 2] and shown on CTP-CBF map [(2 290±1 359) mm 2] ( t=-0.076, P=0.944). There was a positive correlation between iodine density value and CBV (affected side: r=0.350, P=0.039, healthy side: r=0.551, P=0.001); a positive correlation was also found between effective atomic number value and CBV (affected side: r=0.488, P=0.003, healthy side: r=0.552, P=0.001); and there was a strong positive correlation between the hypoperfusion area on CTA and that on CTP-CBF ( r=0.993, P<0.001). Conclusion:Dual-layer detector spectral CTA can provide the “one-stop” assessement including head and neck vascular evaluation, as well as the hypoperfution area measument, which can be an alternative rapid method for evaluation of patients with acute ischemic stroke.
7.Analysis of Influential Factors for the Construction of ADR Damage Relief System under MAH System :Based on the Perspec- tive of Pharmaceutical Manufacturers in Shaanxi Province

OBJECTIVE: To provide reference for constructing ADR damage relief system in China. METHODS: Questionnaire survey was conducted among 217 pharmaceutical manufacturers in Shaanxi province by census method. Fisher’s exact test, Pearson χ2 test and multivariate Logistic regression analysis were used to analyze the factors effecting construction of ADR damage relief systerm unter marketing authorization holder (MAH) system, and the interaction among various factors. RESULTS: A total of 217 drug manufacturers were investigated in this study (217 questionnaires were sent out). 209 questionnaires were collected and 204 valid questionnaires were screened out with recovery rate of 94.01%. Totally 9 independent influential factors screened by Fisher’s exact test and Pearson χ2 test had a significant impact on the attitude of building ADR damage relief system under MAH system, i.e. the last year’s earnings of enterprises, the proportion of pharmaceutical technicians, whether to set up R&D departments, whether to obtain new drugs/generic drugs in recent 3 years, whether to actively carry out post-marketing safety evaluation of drugs, whether ADR commissioners are familiar with ADR-related policies in MAH system, cognition about whether MAH system promotes the development of pharmaceutical industry, the development direction of enterprises after the full implementation of MAH, cognition about the lack of legal basis for ADR damage relief (P<0.05). Multivariate Logistic regression analysis showed that whether the ADR specialist was familiar with the ADR-related policies in MAH system was the main influential factor for constructing ADR damage relief system under the MAH system (P=0.044). It was positively correlated with the attitude of approving the construction of ADR damage relief system under MAH system (B=1.697). CONCLUSIONS: The pharmaceutical manufacturers, in which ADR specialists who are familiar with the ADR-related policies in the MAH system, are more inclined to agree to constructing ADR damage relief system under MAH system.
8. An iliac-crest-preserving iliac wing bone graft harvesting technique

Objective:
To evaluate the clinical application effects of an iliac-crest-preserving iliac wing bone graft harvesting technique and the bone regenerative ability of the donor site after harvesting.
Methods:
From January 2016 to June 2017, a series of 39 patients including 28 men and 11 women aged between 16 and 59 y (35±13 y in average) were prospectively collected to treat bone defects using the iliac-crest-preserving iliac wing bone graft harvesting technique. The patient cohort included 38 ipsilateral and 1 bilateral bone harvests taken from 15 femoral fracture non-unions, 8 tibial fracture non-unions, 11 femoral head necroses (hip preserving surgery), 1 tuberculosis deriving hip arthritis with bone defect (total hip arthroplasty), 2 hip prosthesis loosenings (revision), and 2 proximal femoral benign tumors. All patients were treated through comprehensive surgeries containing autologous iliac bone grafting. The surgical time, blood loss, bone graft volume, 24 h post-operative visual analogue scale (VSA) at the iliac surgical site, complications, and bone regeneration of the donor site were documented and evaluated.
Results:
In the 39 patients (40 sides), the average surgical time was 25±4 min, average blood loss was 79±23 ml, average bone graft volume was 27±6 cm3. The average 24 h post-operative VAS at the iliac surgical site was 1.8±0.7 points. The VSA at the 6 week later and thereafter were 0 in all patients. The iliac incisions in 38 patients (39 sides) were healed uneventfully. However, seroma at the iliac surgical site emerged in 1 patient at the 6th post-operative day in the manner of serous exudation and was treated successfully with non-operative measures. None infections and lateral femoral cutaneous nerve injuries took place. Iatrogenic non-displaced iliac crest fractures happened in the very first 2 patients when taking the crest as a pivot to pry up the graft, which united without special cure at the 3 month post-operative. This complication was completely avoided when shifting the pivot to the anterior pillar containing the anterior iliac spines. None pelvic fractures and heterotopic ossifications took place. Post-operative radiographical examinations revealed that the bone defects at the donor site shrank through bone regeneration, that mamillary or canine-tooth-shaped bone formation occurred in some of the cases, and that none complete bone regeneration took place to eliminate the bone defect at the donor. There were 12 patients in whom pre- and post-operative computed tomographic scans necessitated by the disease were prescribed, which facilitated the measuring of the bone defect at the donor site. The measurement of 13 sides revealed that the bone defects were decreased more or less: the average immediate post-operative bone defect was 25.7±6.5 cm2, the average 12 month post-operative bone defect was 12.7±5.3 cm2.
Conclusion
The iliac wing bone graft harvesting technique suggested here is safe and less invasive, in the premise of preserving the iliac crest and retaining the figure of the surgical site, it can harvest a large amount of iliac wing bone graft and reserve the bone regenerative ability of the donor site

Multivariate logistic regression analysis was performed to estimate the associated factors of PAD. Results The age?and sex?standardized prevalence of PAD was 3.84% (200/5 208, 95%CI 3.32%-4.36%). There was no significant difference between male and female (3.83%(102/2 664 ,95%CI 3.10%-4.56%) vs. 3.85% (98/2 544,95% CI 3.10%-4.60%), P=0.965). The prevalence of PAD in urban was higher than that in rural (4.34% (163/3 755,95%CI 3.69%-4.99%) vs. 2.55% (37/1 453,95%CI 1.74%-3.36%), P=0.001). Furthermore, the prevalence of PAD increased with age (Ptrend<0.01), and the difference between genders did not change with ageing (all P>0.05). In addition, age (OR=1.03, 95%CI 1.01-1.04), urban (OR=1.52, 95%CI 1.08-2.12), smoking ( OR=1.83, 95%CI 1.29-2.59), hypertension ( OR=1.61, 95%CI 1.17-2.22) and diabetes (OR=1.44, 95%CI 1.08-1.93) were related with increased risk of PAD in logistic regression analysis models. Conclusions The prevalence of PAD increases with age in Beijing and there are significant difference between urban and rural on prevalence of PAD. Age, urban, smoking, hypertension and diabetes are related with increased risk of PAD.
