1.Transverse sinus blood flow characteristics of pulsatile tinnitus with dehiscent sigmoid plate based on 4D flow MRI

		                        			Objective:To investigate the hemodynamic characteristics of transverse sinus with sigmoid sinus wall dehiscence (SSWD) of pulsatile tinnitus (PT) based on 4D flow MRI.Methods:Retrospective analysis was performed on all patients admitted to Beijing Friendship Hospital, Capital Medical University from January 2019 to January 2021 for dehiscent sigmoid plate pulsatile tinnitus. A total of 26 patients (sides) who met the criteria and underwent 4D flow MRI were included. A total of 26 subjects (46 sides), matched 1∶1 according to gender and age, were included in the normal healthy control group. Nonparametric rank sum test, Student′s t test, and ANOVA were performed by SPSS 19.0 software. Binary Logistic regression was applied to the data with statistical significance. Results:There were more patients with dominant drainage on the affected side in PT group than in control group (73.1% vs. 42.3%). The incidence of transverse with a focal intraluminal filling defect and tapered stenosis was higher than that in control group (21.7% vs. 69.2%; 17.4% vs. 42.3%). Average through-plane velocity and maximum through-plane velocity in PT group were higher than those in control group [(33.75±13.88) cm/s vs. (15.84±7.21) cm/s; (93.19±33.55) cm/s vs. (40.40±14.40) cm/s]. The middle part and proximal end of Flow avg (ml/s) in PT group were larger than those in control group [4.69 (2.87; 5.62) ml/s vs. 2.76 (1.67; 4.99) ml/s; 3.41 (2.16; 5.47) ml/s vs. 2.67 (1.68; 4.41) ml/s]. In control group, the velocity of transverse sinus changed relatively gently, while in PT group, the velocity of proximal sinus increased significantly. Binary Logistic regression showed that SSWD PT was independently correlated with proximal maximum flow velocity [ OR=1.086(1.029-1.146), P=0.003]. Conclusion:4D flow MRI showed that the dominant drainage and higher velocity at the proximal end of the transverse sinus might be an important hemodynamic characteristics of dehiscent sigmoid plate pulsatile tinnitus.
		                        		
		                        		
		                        		
		                        	
2.Establishment and application of an ORTCC model-based training system for orthopedic subspecialty nurses in the operating room

		                        			Objective:To explore the construction and clinical practice of the training system for nurses in operating room subspecialty based on ORTCC model (including objectives, rules, training, assessment and culture), and to provide theoretical and practical guidance for the development of nursing subspecialty in operating room.Methods:This was a quasi-experimental study. The Union Hospital Affiliated to Tongji Medical College of Huazhong University of Science and Technology was convenient to select 23 operating room nurses who rotated in orthopedic surgery rooms from January 2019 to June 2020 as the control group, and the traditional training model for specialized nurses in operating rooms was adopted. From July 2020 to December 2021, the 24 operating room nurses rotating orthopedic surgery rooms were selected as the experimental group, and ORTCC teaching mode was adopted. The degree of mastery of theoretical knowledge, standardization of operation skills, correct rate of standard preventive implementation and satisfaction rate of doctors of operating room nurses under two different training methods were compared.Results:The theoretical score of operating room nurses increased from (84.71 ± 5.70) points in the control group to (92.68 ± 3.64) points in the experimental group, and the skill score increased from (90.17 ± 4.69) points in the control group to (95.43 ± 2.61) points in the experimental group, with statistical significance ( t = 13.76, 5.64, both P<0.01). The satisfaction rate of surgeons to nurses′ surgical cooperation ability communication, cooperation ability and emergency treatment ability increased from 82.61%(19/23), 73.91%(17/23) and 78.26%(18/23) in the control group to 100.00%(24/24), 95.83% (24/24) and 100.00% (24/24) in the experimental group, respectively, with statistically significant differences ( t = 4.56, 4.45, 5.84, all P<0.05). Conclusions:The training system of operating room subspecialty nurses based on ORTCC model is conducive to improving the theoretical knowledge mastery, standard operation skills and doctor satisfaction rate of operating room nurses to the specialty and subspecialty, providing training basis for the development of operating room subspecialty, and worthy of clinical promotion.
		                        		
		                        		
		                        		
		                        	
3.Lycorine alleviates oxaliplatin-induced chemotherapy induced pain in mice via reducing COX-2 /NLRP3 inflammatory signaling

		                        			To explore the effect and mechanism of lycorine on oxaliplatin ( OXA) induced chemotherapy pain in mice.
		                        		
		                        			Methods  :
		                        			40 mice were randomly divided into 4 groups,10 mice per group,which were respectively divided into control group,model group,administration group,and inhibitor group.A mouse model of chemotherapy induced pain was established by intraperitoneal injection of OXA for 5 consecutive days.Intrathecal administration of lycorine was performed.Behavioral changes and expression levels of inflammatory related proteins were detected .
		                        		
		                        			Results :
		                        			Compared with control group,model group mice exhibited the increased number of spontaneous  flinches,decreased mechanical nociceptive threshold,decreased movement distance and latency,and up-regulated  expression levels of interleukin-1 β  (IL-1 β) ,astrocytic marker glial fibrillary acidic protein  ( GFAP) ,cyclooxygenase-2( COX-2) ,NOD-like receptor protein 3  ( NLRP3 ) ,cysteinyl  aspartate  and  specific proteinase  1  ( Caspase- 1) .Compared with model group,lycorine administration reduced the number of spontaneous flinches,increased  mechanical nociceptive threshold ,enhanced the movement distance and latency ,bound and reduced  COX-2  expression,down-regulated the expression levels of IL-1 β , GFAP ,NLRP3  and  Caspase-1.
		                        		
		                        			Conclusion
		                        			Lycorine  reduces COX-2 expression,inhibits NLRP3 inflammasome activation,suppresses spinal inflammation,consequently  alleviates pain behaviors and improved motor ability of mice.
		                        		
		                        		
		                        		
		                        	
4.Influence factors and family burden analysis of bicycle injury among middle school students in Shanghai, 2017-2018

		                        			To understand the influencing factors and family burden of bicycle injury among middle school students in Shanghai for bicycle injury prevention.
		                        		
		                        			Methods:
		                        			Based on 1∶2 matched case-control design, students who were injured when cycling were selected from Shanghai student injury case reporting system during Sept. 2017 to Dec. 2018 as the case group. The same-sex control was selected from the same grade in the same school with the case. A total of 61 cases and 122 control were selected. Self-reported questionnaires were conducted by parents of children. Univariate and multivariate logistic regression were used to analyze the influencing factors of bicycle injury.
		                        		
		                        			Results:
		                        			Poor eyesight (OR=2.27, 95%CI=1.02-5.07), lots of intersections near the residence (OR=2.00, 95%CI=1.00-3.98), and lots of cyclists near the residence (OR=2.15, 95%CI=1.06-4.36) were positively correlated with bicycle injuries among middle school students. About 34.43% and 8.20% of parents reckon bicycle injuries had a moderate impact on family economic, daily activities and recreational activities. Among the students with bicycle injury, 70.49% of the students were absent from school because of illness, and 36.07% of their parents missed work because of illness. The average absence time for school and working were 10.79 days and 3.03 days respectively. The total economic loss caused by the lack of work was 84 950 yuan, with an average of 1 393 yuan.
		                        		
		                        			Conclusion
		                        			Bicycle injury of middle school students in Shanghai causes certain family burden. Poor eyesight, lots of intersections near the residence, lots of cyclists near the residence are positively correlated with bicycle injuries among middle school students.
		                        		
		                        		
		                        		
		                        	
5.High resolution CT in evaluation of anatomic relationship between labyrinth segment of facial canal and cochlea

		                        			Objective To investigate the value of high resolution CT (HRCT) in displaying the anatomic relationship between labyrinth segment of facial canal and cochlea.Methods Totally 110 patients (220 ears) who underwent HRCT were collected.The original images were transferred to workstation for image processing.MPR images were acquired.The anatomic relationship between labyrinth segment of facial canal and cochlea was observed in oblique coronal MPR images.The bony septum between labyrinth segment of facial canal and cochlea was assessed as definite defect (Type Ⅰ),doubtful defect (Type Ⅱ) or complete (Type Ⅲ),respectively.Results There were 71 ears (71/220,32.27%) of Type Ⅰ,diameters of bone fissure ranged from 0.3-1.3 mm (average diameters [0.64±0.26]mm),86 ears (86/220,39.09%) of Type Ⅱ and 63 ears (63/220,28.64%) of Type Ⅲ,with bony septum thickness ranged from 0.3-1.0 mm (average thickness [0.68±0.15]mm).No statistical difference of rates of the above three types was found between different genders,among age groups and between both side of ears (all P>0.05).Conclusion HRCT is a reliable method to show the anatomic relationship between labyrinth segment of facial canal and cochlea.
		                        		
		                        		
		                        		
		                        	
6.Effects of perioperative body temperature management on intraperitoneal hyperthermic perfusion chemotherapy patients with abdominal malignant tumor

		                        			Objective To study the effects of perioperative body temperature management on intraperitoneal hyperthermic perfusion chemotherapy patients with abdominal malignant tumor. Methods From June to Decmber 2016, a total of 120 intraperitoneal hyperthermic perfusion chemotherapy patients with abdominal malignant tumor who met the inclusion criteria were recruited and randomly divided into control group and observation group, with 60 cases in each. All the subjects were given routine operation nursing. On the basis of that, the observation group were given perioperative body temperature management. The body temperature, heart rate, pulse pressure and the difference of tumor markers of carcineombryonic antigen (CEA) and carbohydrate antigen19-9(CA199) before and after treatment were compared between the two groups. Results The difference value of temperature, heart rate and pulse pressure between 30 minutes after treatment and before treatment in the observation group were (3.7±0.4)℃, (12.4±2.7)bp/min and (5.6±1.9)mmHg respectively. The fluctuation of vital signs was lower than the control group, and the differences were statistically significant (t=6.062, 14.897, 10.392; P<0.01). The observation group patients' CEA was (52.8±2.4)μg/L, and CA199 was (106.1±5.3)U/ml after treatment, which were both lower than the control group, with statistical differences(t=31.415, 33.803; P< 0.05). Conclusions Perioperative body temperature management could significantly stabilize the body temperature, heart rate and pulse pressure, which could significantly promote intraperitoneal hyperthermic perfusion chemotherapy's curative effect for patient with abdominal malignant tumor.
		                        		
		                        		
		                        		
		                        	
7.Application of disposable surgery retractor made of high polymer medical plastic

		                        			Objective To analyze the application effect of disposable surgery retractor which made of high polymer medical plastic.Methods From January 2016 to October 2016,80 patients of HCC who were treated with hepatectomy under general anesthesia in our hospital were randomly divided into the control group and the observation group according to different retractor during surgery.The control group used the traditional abdominal retractor during the sugery.Meanwhile,the observation group used disposable surgery retractor.The application effect between the two groups were compared.Results The satisfaction rate of surgical exposure and effect in surgeon of the observation group were higher than those of the control group.The wound pain time of patients in perioperative period and 1 month after surgery and chronic pain incidence in postoperative 6 months between the two groups were evaluated.It turned out that the difference in average pain score was statistically significant (P<0.05).Conclusion The disposable surgical retractor can improve the satisfaction rate of surgeons with better surgical exposure,and it can significantly reduce the damage of wound tissue,which effectively relieves the postoperative pain,shortens the time of postoperative pain,and reduces the incidence of chronic pain.
		                        		
		                        		
		                        		
		                        	
8.Study on the relationship of platelet specific-autoantibodies with therapeutic outcomes by dexamethasone in immune thrombocytopenia purpura.

OBJECTIVETo investigate the different outcomes by dexamethasone in adults immune thrombocytopenia purpura (ITP) with different types of platelet specific-autoantibodies.
METHODSA total of 185 ITP were enrolled, 61 males and 124 females, with a median age of 42 (18-83) years, including 117 newly diagnosed, 35 persistent, and 33 chronic cases. All the patients received the dexamethasone at an initial dose of 40 mg per day for 4 days and a low dose of 5-10 mg for 3-4 weeks. The platelet specific-autoantibodies were identified by the modified monoclonal antibody-specific immobilization of platelet antigen (MAIPA) assay.
RESULTSAmong the IgG positive patients, the response rates in anti-GPIIb/IIIa antibody, anti-GPIbα antibody, both antibody positive, and both antibody negative were 87.5%, 50.0%, 68.0%, and 72.3% (χ²=11.489, P<0.05), respectively. Among the IgM positive patients, the response rates in the four groups were 82.1%, 71.4%, 61.9%, and 68.9% (χ²=2.719, P=0.437), respectively. Among the GPIbα antibody positive patients, the response rates in IgG alone, IgM alone, both positive, and both negative were 52.4%, 59.1%, 76.1%, and 77.9% (χ²=10.811, P<0.05), respectively. Among the GPIIb/IIIa antibody positive patients, the response rates in the four groups were 73.3%, 71.0%, 78.6%, and 66.3% (χ²=1.374, P=0.719), respectively.
CONCLUSIONITP patients with GPIbα-IgG antibody have worse response to dexamethasone treatment.

9.CK assay with gene testing for screening of Duchenne muscular dystrophy in the newborn infants

		                        			Objective To establish a system using CK activity assessing with follow-up Duchenne muscular dystrophy (DMD) gene testing to newborn screening for DMD.This study provided a pathway to improve the health outcome for individuals with DMD.Methods Tests for CK were performed with Beckman original reagent on a Beckman Coulter AU 5800.Preliminary studies established a population-based range of CK in newborns using 5 892 deidentified anonymous blood samples,which were collected from Shanghai Changning Maternity and Infant Health Hospital between November 2013 and July 2014.Mutation analysis used multiplex PCR-denature high-performance liquid chromatography (PCR-DHPLC) method for screening large duplications and deletions and Sanger DNA sequencing for screening point DMD gene mutation.Results DMD gene mutations (point mutation,exon60,c.9072G > A) were found in 1 of 5 892 newborn subjects,which had CK level > 2 000 U/L large duplications and deletions in DMD gene were not found.Conclusions A system of analysis for newborn screening for DMD has been established.This path for newborn screening fits our health care system and minimizes the false-positive results for predicting DMD gene mutations by use of CK levels in blood
		                        		
		                        		
		                        		
		                        	
10.Sanhuangyinchi decoction pretreatment ameliorates acute hepatic failure in rats by suppressing antioxidant stress and caspase-3 expression.

OBJECTIVETo observe the effects of Sanhuangyinchi decoction (SHYCD) pretreatment on acute hepatic failure (AHF) induced by D-galactosamine and lipopolysaccharide (LPS) in rats and explore the possible mechanisms involving antioxidant stress and cell apoptosis-related protein expression.
METHODSForty-eight SD rats were randomized equally into control group, AHF model group, high-, medium- and low-dose SHYCD groups, and Bicyclol group. Five days after administration of the corresponding drugs, the rats were challenged with peritoneal D-galactosamine (700 mg/kg) plus LPS (10 ug/kg) injections to induce AHF acute hepatic failure except for those in the control group. At 48 h after the injections, blood samples were collected from the rats to detect the levels of ALT, AST, TBIL, PT, INR and FIB, and pathological changes and superoxide dismutase (SOD) and malondialdehyde (MDA) contents in the liver were examined; immunohistochemistry and western blotting were used to detect caspase-3 protein expression in the liver.
RESULTSThe levels of ALT, AST, TBIL, TP and INR in the 3 SHYCD groups and Bicyclol group significantly decreased (P<0.05) while FIB significantly increased in comparison with those in the model group. SHYCD obviously ameliorated the pathological changes, enhanced SOD activity (P<0.05), and decreased MDA levels (P<0.05) and caspase-3 expression (P<0.05) in the liver tissue. SHYCD at the medium dose produced similar effects to Bicyclol (P>0.05) and showed better effects at the high dose than Bicyclol (P<0.05).
CONCLUSIONSHYCD pretreatment can dose-dependently ameliorate AHF in rats possibly by suppressing antioxidant stress and caspase-3 expression to decrease hepatic cell apoptosis.
