1.Cost analysis of medical consumables for patients undergoing coronary stent implantation surgery before and after volume procurement in the context of diagnosis related groups payment

Objective:To study the changes of patient-related costs and the use of stents and other consumables before and after the centralized procurement of coronary stents.Methods:The inpatient medical insurance settlement case data of 1,973 patients with coronary stent implantation admitted to Daping Hospital of Army Medical University from December 2019 to October 2021 were selected.Among them,the data of 1,317 cases of percutaneous cardiovascular surgery and coronary stent implantation with serious or complications and accompanying disease group were slected according to disease diagnosis related groups(DRG),which were divided into the pre-centralized procurement group(667 cases)and the post-centralized procurement group(650 cases)according to the centralized procurement of coronary stents before and after.The costs of patients'medical consumables with the consumption of patients'medical consumables and the impact of the use of consumables such as coronary stents on the costs of medical consumables were compared.Results:There was no significant statistical difference in the hospitalization days and the average number of stents used in patients undergoing percutaneous cardiovascular procedures and coronary stent implantation with centralized procurement of coronary stents.There was a statistically significant difference in the total diagnosis and treatment cost,medical consumables cost,medicines and consumables cost and medicines cost between the pre-centralized procurement group and the post-centralized procurement group(Z=-22.316,-23.546,-22.917,-5.724,P<0.05).The cost of stents[16 260(13 300,32 272)yuan],the number of catheter guidewire balloon sheaths consumables[5(4,8)sets(pieces)],and the cost of catheter guidewire balloon sheaths consumables[8 719(5 805,15 372)yuan]in the pre-collection group were collected.There were statistically significant differences in the stent cost[1 059(590,1 770)yuan],the number of catheter guidewire balloon sheaths consumables[8(7,12)sets(pieces)],and the cost of catheter guidewire balloon sheaths consumables[5 708(3 392,12 871)yuan]between the two groups(Z=-30.452,16.582,-7.670,P<0.05).There was a statistical correlation between the cost of coronary stents and the cost of catheter guidewire balloon sheaths before and after centralized volume procurement on the cost of medical consumables for patients(r=0.903,0.473,0.785,0.953,P<0.05).The correlation coefficient between the cost of coronary stents and the cost of medical consumables for patients in the post-centralized procurement group decreased compared to the pre-centralized procurement group,the correlation coefficient between the cost of catheter guidewire balloon sheath and the cost of medical consumables for patients increased.Conclusion:The centralized procurement of coronary stents has a significant cost control effect on patients in the disease groups,and affects the cost structure of medical consumables.Combined with DRG reform,it can continuously improve the standardization and scientificity of clinical use of medical consumables.
2.Molluscicidal effect of spraying different formulations of niclosamide ethanolamine salt with drones against Oncomelania hupensis in ditches

Objective To evaluate the molluscicidal effect of spraying different formulations of niclosamide ethanolamine salt with drones against Oncomelania hupensis in ditches. Methods A semi-dry and semi-wet ditch with O. hupensis snails was selected in the second branch field of Jiangbei Farm, Jiangling County, Hubei Province in May 2023, and divided into 4 experimental areas, named groups A1, A2, B1 and B2. Environmental cleaning was performed in groups A1 and B2, and was not conducted in groups A2 or B2. Then, 50% wettable powder of niclosamide ethanolamine salt was sprayed with drones at an effective dose of 2 g/m2 in groups A1 and A2, and 5% niclosamide ethanolamine salt granule was sprayed with drones at an effective dose of 2 g/m2 in groups B1 and B2. O. hupensis snails were surveyed using the systematic sampling method 1, 3, 5, 7, 14 days after spraying, and the natural mortality and corrected mortality of O. hupensis snails were calculated. Results The occurrence of frames with living snails, mean density of living snails and natural mortality of snails were 97.50% (117/120), 6.30 snails/0.1 m2 and 1.18% (9/765) in the test ditch before spraying, respectively. There were significant differences in the mortality of snails among four groups 1, 3, 5, 7 and 14 days after spraying niclosamide formulations with drones (χ2 = 17.230, 51.707, 65.184, 204.050 and 34.435, all P values < 0.01). The overall mortality rates of snails were 94.51% (1 051/1 112), 79.44% (908/1 143), 96.54% (977/1 012) and 88.55% (1 021/1 153) in groups A1, A2, B1 and B2 (χ2 = 207.773, P < 0.05), respectively. In addition, there was no significant difference in the overall snail mortality between groups A1 and B1 (P > 0.05), and the snail mortality in groups A1 and B1 were both statistically different from that in groups A2 and B2 (all P values < 0.05). Conclusion Both 50% wettlable powder of niclosamide ethanolamine salt and 5% niclosamide ethanolamine salt granule sprayed with drones are active against O. hupensis snails in ditches, and environmental cleaning may improve the molluscicidal effect.
3.New agents-based induction chemotherapy followed by autologous stem cell transplantation and maintenance treatment strategy for multiple myeloma: a single center retrospective study of 300 cases

Objective:To examine the survival and influential factors of an integrated approach of novel agents, autologous hematopoietic stem cell (auto-HSCT) , and maintenance therapy in patients with multiple myeloma (MM) patients from a single center over the past 15 years.Methods:In our center, 300 MM patients who received an integrated strategy of new agents, auto-HSCT, and maintenance therapy over 15 years were retrospectively and prospectively analyzed.Results:The complete remission rates (CR) and ≥very good partial remission rates (VGPR) following induction therapy, transplantation, and maintenance therapy were respectively 35.3% and 55.2% , 72.4% and 80.0% , 89.2% , and 93.4% . When compared to patients receiving double-drug induction, the ≥VGPR and ORR of patients receiving triple-drug induction were improved. No difference existed in CR, ≥VGPR, and ORR between the PAD (bortezomib + liposome doxorubicin+ dexamethasone) and RAD (lenalidomide + liposome doxorubicin + dexamethasone) regimens, but the benefits speed differed. The negative rate of flow minimal residual disease following induction, transplantation, and maintenance was 18.8% (54 cases) , 41.4% (109 cases) , and 58.7% (142 cases) , respectively. The median time to progress (TTP) was 78.7 months and the median overall survival (OS) was 109 months. The median TTP for RISS-Ⅰ-Ⅲ patients were 111.8 months, 77.4 months, and 30.6 months, and the median OS was 118.8 months, 91.4 months, and 48.5 months, respectively. At various points during treatment, the TTP and OS of patients obtaining CR and MRD negative were longer than those of patients who did not obtain CR and MRD negative. TTP was noticeably shorter in high-risk cytogenetic patients compared to standard-risk patients even when CR was acquired during induction. There was no difference in TTP between patients with high-risk cytogenetics and those with standard-risk cytogenetics if MRD negative was acquired during induction. According to a multivariate analysis, the R-ISS stage was a poor predictor of TTP and OS at various treatment intervals. Therapeutic effectiveness was a newly independent prognostic factor following treatment.Conclusion:A median survival of almost 10 years is possible for MM patients who receive an integrated strategy of induction regimens followed by auto-HSCT and maintenance therapy, which significantly improves prognosis. However, this approach did not significantly benefit high-risk cytogenetic MM patients.
4.Differences in the autistic severity and developmental level of children with autism spectrum disorder of different gender and age

Objective:To analyze the autistic severity and developmental level of children with autism spectrum disorder (ASD) of different gender and age.Methods:From March 2018 to February 2019, a total of 286 ASD children aged 18-96 months were enrolled.Severity of ASD was evaluated by the autism behavior checklist (ABC) and the childhood autism rating scale (CARS), while the Griffiths mental development scales-Chinese (GDS-C) was used to evaluate the children's mental development.According to gender and age, ASD severity and developmental levels of different subgroups were analyzed.SPSS 23.0 statistical software was used to analyze the data.According to the non-normal distrbution of variables, Mann-Whitney U test was used for comparison between the two groups, and Kruskal-Wallis H test was used for comparison between multiple groups and Spearman correlation analysis was used to analyze the correlation between two variables. Results:No gender difference was found in the total scores of ABC (52(40, 62), 57(36, 67), Z=0.661, P=0.509) and CARS (31(28, 35), 33(27, 36), Z=0.672, P=0.502) between the ASD boys and ASD girls.No gender difference was found in the total scores of ABC (43(33, 53), 52(34, 58), Z=0.717, P=0.473) and CARS (29(26, 32), 27(26, 30), Z=0.212, P=0.832) between the ASD boys and ASD girls when the general quotient of the GDS-C were all greater than or equal to 70 points.Also, no gender difference was found in the total ABC (55(44, 67), 59(40, 67), Z=0.067, P=0.947) and CARS (32(30, 35), 34(30, 36), Z=0.657, P=0.511) between the ASD boys and ASD girls when the general quotient of the GDS-C were all less than 70 points.The eye-hand coordination (64(52, 77), 60(43, 72), Z=2.138, P=0.033), performance (68(51, 86), 59(43, 68), Z=3.270, P=0.001), and practical reasoning (68(55, 91), 51(33, 58), Z=2.686, P=0.007) quotients of the ASD boys were significantly higher than those of the ASD girls.Compared with boys, a markedly higher proportion of developmental delay (35(80%), 124(51%), χ 2=12.083, P=0.001) was recorded for girls in relation to the performance subscale.There were significant differences in the total scores of ABC and CARS, the locomotor and eye-hand coordination quotients in GDS-C among ASD children of different ages(all P<0.05). The older the first diagnosis age, the lower the scores.The age of initial diagnosis was negatively correlated with the total scores of ABC ( r=-0.259), CARS ( r=-0.268), general quotient of GDS-C ( r=-0.127), locomotor ( r=-0.275), eye-hand coordination ( r=-0.213), performance ( r=-0.160) and practical reasoning ( r=-0.307) (all P<0.05), while positively correlated with hearing and language development quotient ( r=0.143) ( P<0.05). Conclusion:No significant gender difference was found on the severity of ASD in children aged 18 to 96 months, but the developmental level of some areas in boys was better than that of girls with ASD.ASD severity and developmental level vary depending on the age at which their ASD was diagnosed.The age of initial diagnosis is related to the severity and developmental level of ASD.
5.Effect evaluation of video teaching method based on information training platform on improving the quality of cardiopulmonary resuscitation of new nurses

Objective:To study the effect of video teaching combined with offline training in cardiopulmonary resuscitation training for new nurses outside hospital.Methods:From August 2017 to August 2018, 125 new nurses from Shanghai Changzheng Hospital were selected. New nurses in August 2017 were set as control group ( n=65). In August 2018, the new nurses were set as the observation group ( n=60). The control group used the traditional offline training mode to train cardiopulmonary resuscitation outside the hospital. The observation group used information platform video teaching combined with offline theoretical training to carry out cardiopulmonary resuscitation training. After systematic training, the results of cardiopulmonary resuscitation after training and the satisfaction of teaching were compared between the two groups. Results:Before training, there was no significant difference in the assessment results of cardiopulmonary resuscitation theory and operation between the two groups ( t values were 0.12, 1.23, P>0.05). After training the scores of cardiopulmonary resuscitation theory and operation examination in the observation group were 85.41±3.20, 92.40±2.50, and 82.52±4.50, 85.0±3.60 in the control group ( t value was 1.04,4.24, all P<0.05). Encouraging and enhancing the learning interest of new nurses, cultivating mutual supervision and mutual guidance, integrating theoretical knowledge with practical operation, improving and adjusting self-motivation, overall evaluation of teaching methods and total score in the observation group were 3.61±0.34, 3.54±0.20, 3.61±0.28, 3.87±0.20, 3.32±0.21, 17.95±0.26, and 2.60±0.41, 2.41±0.16, 2.55±0.35, 2.41±0.46, 2.58±0.20,12.55±0.32 in the control group ( t value was 2.187-2.452, P<0.05). Conclusion:Video teaching combined with offline training can improve the performance of new nurses in the training of high-quality cardiopulmonary resuscitation, stimulate the learning interest of nurses, and enhance the effect of high-quality cardiopulmonary resuscitation training.
6.A clinical study of 24-hour pumping of continuous feeding in very low birth weight infants

Objective:To investigate the effect of 24-hour continuous feeding by pump on gastrointestinal dysfunction and early growth of very low birth weight infants (VLBWI).Methods:Eighty VLBWI fed by tube in the Neonatal Intensive Care Unit (NICU) of Fuyang Women and Children′s Hospital, Anhui Province from January 2017 to June 2019, were retrospectively analyzed.They were randomly divided into the observation group and the control group according to the simple randomization method, 40 cases in each group.The observation group included 23 males and 17 females, with the gestational age of 28 + 2-33 + 5 weeks, and birth weight of 1.04-1.47 kg.The control group included 21 males and 19 females, with the gestational age of 28 + 1-33 + 5 weeks, and birth weight of 1.05 -1.47 kg.On the basis of giving both groups routine treatment, nursing, and non-nutritional sucking, the control group was treated with intermittent tube feeding and infusion of formula milk, while the observation group was microfed 24 h continuously by pump.The general situation, meconium discharge time, frequency of daily defecation, daily milk volume, gastrointestinal dysfunction, increase of body weight after birth, and neonatal behavioral neurological assessment (NBNA) at 40 weeks of gestational age were compared between the two groups.The increase of body mass after birth and the determination of neonatal behavioral neurological assessment (NBNA) in neonatal behavioral nerve at 40 weeks of gestational age were statistically analyzed by χ2 test and t test. Results:(1)There were no significant differences in sex, gestational age, birth weight, head circumference and body length between the 2 groups (all P>0.05). (2) Compared with the control group, the premature infants in the observation group had increased daily milk volume [(12.95±2.34) g/d vs.(10.08±2.08) g/d], shortened meconium discharge time [(3.90±0.84) d vs.(5.02±0.86) d], and higher frequency of daily defecation[(3.85±1.07) times/d vs.(3.00±0.81) times/d], shortened extubation time[(14.75±3.20) d vs.(16.80±3.08) d], shortened time of weight to loss to minimum[(3.38±0.86) d vs.(4.22±0.89) d], faster growth of the daily average body weight[(15.25±2.29) g/(kg·d) vs.(11.55±1.56) g/(kg·d)], shortened average hospitalization time [(35.58±8.29) d vs.(39.42±8.29) d], and faster increase of the head circumference[(35.23±2.40) cm vs.(33.74±1.63) cm]. The differences were statistically significant ( t=5.805, -5.907, 3.981, -2.918, -2.825, 3.890, -2.060, 3.233, all P<0.05). (3)The incidence of vomiting, abdominal distension, hypoglycemia and hyperbilirubinemia in the observation group was significantly lower than that in the control group ( χ2=5.000, 16.050, 5.165, 14.528, all P<0.05). (4) There was no significant difference in necrotizing enterocolitis (NEC) and dyspnea between the two groups ( χ2=1.438, 10.251, all P>0.05). The total scores of behavioral ability, passive muscle tension, active muscle tension, primitive reflex, general evaluation and NBNA in the observation group were higher than those in the control group, the difference was statistically significant ( t=5.026, 3.207, 3.000, 2.421, 3.150, 6.141, all P<0.05). Conclusions:Twenty-four-hour continuous feeding by pump combined with non-nutritious sucking can significantly reduce the occurrence of gastrointestinal dysfunction in VLBWI, promote the development and maturation of gastrointestinal function, improve the nutritional status, accelerate the physical growth, and thus facilitate the development of behavioral nerve.
7. Bortezomib-based induction chemotherapy followed by autologous hematopoietic stem cell transplantation and maintenance in 200 patients with multiple myeloma: long-term follow-up results from single center

Objective:
To study the efficacy, safety and long-term outcomes of integrated strategy of bortezomib-based induction regimens followed by autologous hematopoietic stem cell (ASCT) and maintenance therapy in Chinese multiple myeloma (MM) patients.
Methods:
200 MM patients receiving integrated strategy of bortezomib--based induction regimens followed by ASCT and maintenance therapy were retrospectively and prospectively analyzed from December 1. 2006 to April 30. 2018.
Results:
The complete remission rates (CR) and better than very good partial remission rates (VGPR) after induction therapy, transplantation and maintenance therapy were respectively 31% and 75.5%, 51.8% and 87.7%,73.6% and 93.4%. There was no difference between 4 cycles and more than 5 cycles induction chemotherapy. The negative rate of MRD detection by flow cytometry was 17.6% and 38.2% respectively after induction and 3 months after transplantation. The negative rate of MRD gradually increased during the maintenance therapy. The success rate of high dose CTX combined with G-CSF mobilization was 95.5% and transplantation related mortality (TRM) was zero. The median time to progress (TTP) was 75.3 months and the median overall survival (OS) was 99.5 months. TTP of patients obtaining CR and negative MRD after induction were longer that those of no CR and positive MRD. TTP and OS of patients receiving triple-drug induction and ASCT in early stage were longer than those of double-drug induction and ASCT in late stage. LDH≥240 U/L, high risk cytogenetics, ISS II+III stage and HBsAg positive were prognostic factors at diagnosis. However, only MRD and high risk cytogenetics were independent prognostic factors after transplantation and maintenance therapy. The clinical characteristics of patients of TTP ≥6 years were listed below: light-chain type M protein, ISS I stage, normal level of hemoglobin and platelet, normal LDH, HBsAg negative, chromosome 17p-negative, good response and sustained good response.
Conclusions
Integrated strategy of bortezomib-based induction regimens followed by ASCT and maintenance therapy can significantly improve the short-term and long-term efficacy. The prognostic factors of TTP in different disease stages were different. Response to treatment, especially MRD, played a more important role in prognostic factors.
8.Effects of low-dose interleukin-2 on peripheral regulatory T cells counts of patients with psoriatic arthritis and its short-term clinical efficacy

Objective To study the expression of peripheral blood lymphocyte subsets in psoriatic arthritis (PsA) patients and the short-term efficacy of low doses of interleukin-2 (IL-2). Methods Ninety-five patients with PsA were enrolled as study subjects and 106 healthy people as control group. On the basis of conventional treatment, a total of 22 PsA patients were randomly selected and treated with low dose IL-2 (5 ×105 U/d, continuously used for 5 days, IH), and the disease condition and lymphocyte changes were observed. Flow cytometry was used to detect the absolute count of T subsets dominated by regulatory T cell(Treg) and T helper cell 17(Th17). Wilcoxon rank sum test, χ2 test and Spearman correlation analysis were used for statistical analysis. Results The absolute number of Th17 cells of PsA patients [7.2 (4.0, 12.8) cells/μl] was higher than that of the control group [5.9(4.0, 8.6) cells/μl] (Z=-1.997, P=0.046), the number of Treg cells [25 (17, 36) cells/μl] decreased compared with the control group [30 (23, 40) cells/μl] (Z=-2.957, Z=0.003), T/Treg [50 (36), 76)], B/Treg [7.6 (5.4, 11.5)], CD4+T/Treg [27 (21, 42)], CD8+T/Treg [20 (12, 30)], Th17/Treg [0.34(0.13, 0.51)], Th1/Treg [4.4(2.3, 7.2)], Th2/Treg [0.34(0.20, 0.53)], compared with control group T/Treg [40 (32, 54)], B/Treg [6.5 (4.2, 8.1)], CD4+T/Treg [20 (17, 25)], CD8+T/Treg [16 (11, 24)], Th17/Treg [0.19 (0.13, 0.31)], Th1/Treg [3.5 (1.8, 5.8)], Th2/Treg [0.24 (0.15, 0.39)] were significantly increased (Z=-3.365, -3.217,-5.285, -2.097, -1.69, -1.482, -2.304, P<0.05). Treg cells were negatively correlated with disease activity indexes TJC (r=-0.213, P=0.038), VAS (r=-0.299, P=0.003), PHGA (r=-0.287, P=0.005), DLQI (r=-0.208, P=0.043). Th17 cells increased from [6.3 (2.3, 11.5) cells/μl] to [11 (7, 20) cells/μl, Z=-2.808, P=0.005] after low-dose IL-2 treatment, Treg cells increased from [27 (15, 30) cells/μl] to [71 (37, 98) cells/μl, Z=-3.945, P<0.01]. Because the growth rate of Treg was much higher than Th17, Th17/Treg [before IL-2 treatment: 0.26 (0.11, 0.44), after IL-2 treatment: 0.14 (0.1, 0.35)] returned back to the normal range. After the treatment with IL-2, the patient's activity indicators were significantly improved, and there were no reversible adverse reactions. Conclusion The reduction of Treg cells may be involved in the occurrence and devel-opment of PsA. Low-dose IL-2 treatment can effectively promote the proliferation of Treg cells and the recovery of Th17/Treg balance, which is conducive to the improvement of the condition and good safety.
9. High dose melphalan (HDM) is superior to cyclophosphamide plus etoposide and busulfan (CVB) as the conditioning regimen in autologous stem cell transplantation for multiple myeloma

Objective:
To compare the efficacy, response and survival between high-dose melphalan (HDM) and cyclophosphamide+ etoposide+ busulfan (CVB) as the conditioning regimen in autologous stem cell transplantation (ASCT) for newly diagnosed multiple myeloma (NDMM) .
Methods:
Retrospectively enrolled 123 consecutive NDMM patients who had received PAD induction with subsequent ASCT from Jan 2011 to Aug 2017. The CVB group and HDM group had 82 and 41 patients respectively.
Results:
①No differences existed between these 2 groups in non-hematological side effects. ②Patients of CVB group had faster neutrophil and platelet engraftment time, with the median neutrophil engraftment time of 10 (9-35) day
10.Gender dependence of metabolomic phenotypes for human saliva using ultra-high performance liquid chromatography with quadrupole time-of-flight mass spectrometry

Objective · To investigate the gender-related metabolomic differences in human saliva. Methods · The saliva metabolomic profiles of 5 male and 5 female healthy volunteers with matched age, body mass index (BMI), living and tooth-brushing condition were acquired using ultra-high performance liquid chromatography with quadrupole time-of-flight mass spectrometry. Metabolites were identified using publicly accessible databases and further confirmed individually with standard compounds. Both multivariant and univariate statistics were conducted to find inter-gender differences.Results · Forty-eight metabolites in human saliva were identified including 13 amino acids, 6 choline metabolites, 15 carnitines, 4 sphinganine metabolites,7 lysophosphatidylcholine and 3 organic acids. Levels of phenylalanine, acetylcarnitine, propionylcarnitine, butyrylcarnitine, isobutyrylcarnitine,isovalerylcarnitine and sphinganine were higher in the saliva of females than that of males.
