1.Quality Evaluation of Clinical Guidelines for Acute Myocardial Infarction Based on AGREE Ⅱ

		                        			ObjectiveThis paper used the AGREE Ⅱ guideline evaluation tool to evaluate the quality of 14 clinical guidelines for acute myocardial infarction,aiming to provide reference for the formulation and improvement of the guidelines. MethodsClinical guidelines and expert consensus related to acute myocardial infarction were searched by web search. The search period ranges from January 1,2019 to November 1,2024 in CNKI,VIP,Wanfang Data,SinoMed,Web of Science,OVID, the International Guidelines Collaboration Network (GIN),the UK National Institute for Health and Clinical Excellence (NICE),Yimaitong, and other platforms. Three researchers independently screened the literature and used AGREE Ⅱ to score the screening results. After ensuring that the researchers have a consistent understanding of each guideline,the quality of the guidelines was evaluated. After that,the ratings were analyzed by layer according to the issuing agency,category,method of formulation,and funding situation and compared longitudinally by rating time. The clinical guidelines and expert consensus were compared in terms of content and evidence. ResultsA total of 14 guidelines and consensus were included. The results of AGREE Ⅱ in the six areas in descending order were scope and purpose (62.82%±10.43%),rigor (62.40%±12.77%),editorial independence (62.11%±22.26%),participants (61.42%±11.65%),clarity of expression (59.98%±9.62%),and application (52.94%±16.90%) . Eleven of the guidelines were at level B, and three were at level A. In the stratified analysis,the score of the guideline formulated by the Chinese Medical Doctor Association was lower. There was little difference between the scores of Chinese/Western and Western medicine guidelines. The average score of the guidelines was higher than the consensus. Funded guidelines and consensus scores were higher. In the longitudinal comparison,the highest number of guidelines were developed in 2020 and 2021,while those developed in 2023 scored the highest. In the differential comparison analysis,the content of the guidelines was more comprehensive, and the evidence level was higher,while the content of the consensus was more novel, and the evidence was less. ConclusionThe AGREE Ⅱ score of the clinical guidelines for acute myocardial infarction is generally moderate,and there is room for improvement in terms of applicability. At the same time,the content quality of expert consensus should be improved,and more efforts should be made to develop and apply Chinese medicine guidelines for complications such as heart failure and microcirculatory obstruction after acute myocardial infarction. 
		                        		
		                        		
		                        		
		                        	
2.Long term follow-up results of pipeline embolization devices showing no in-stent stenosis at short-to-medium term follow-up examination

		                        			Objective To investigate the long-term incidence of in-stent stenosis(ISS)in patients with intracranial aneurysms receiving pipeline embolization device(PED)who showed no ISS at short-to-medium term follow-up examination.Methods The clinical data of patients,who received PED treatment at the Department of Neurointervention,First Affiliated Hospital of Zhengzhou University of China between April 2015 and June 2022,were retrospectively collected.The patients with intracranial aneurysms,who showed no ISS at the initial follow-up with DS A and completed>12 months long-term follow-up check after treatment at the same hospital,were screened out,and their relevant clinical data and imaging materials were collected.The incidence of ISS occurring in postoperative>12 months long-term follow-up was calculated.The ISS was defined as a>25%lumen loss of the parent artery when compared with its lumen size measured immediately after PED implantation.Results A total of 57 patients with 61 aneurysms were enrolled in this study,and a total of 68 PEDs were implanted.Forty-one(67.21%)aneurysms were treated by PED implantation only,and 20(32.79%)aneurysms by PED plus spring coils.The median initial follow-up time was 184.0 days(119.0,212.5).At postoperative>12 months long-term follow-up visit,DSA was employed for 35(57.38%)aneurysms,CTA was adopted for 22(36.07%)aneurysms,and 3D-SPACE sequence MR scan was performed in 4(6.56%)aneurysms.The median follow-up time was 538.0 days(407.5,678.0),and the incidence of ISS was 0%.No ISS-related neurological symptoms occurred in all patients.Conclusion In treating intracranial aneurysms with PED,the postoperative incidence of ISS is low.No ISS is found during the short-term follow-up period,and long-term follow-up results tend to indicate that no ISS events have occurred.
		                        		
		                        		
		                        		
		                        	
3.Relationship between calcitonin receptor expression and clinicopathologic features and prognosis of gastric cancer

		                        			Objective:To explore the relationship between calcitonin receptor (CALCR) expression and clinicopathologic features and prognosis of gastric cancer patients.Methods:A retrospective analysis was performed to collect 108 samples of gastric cancer patients treated in Department of Gastrointestinal Surgery, First Affiliated Hospital of Bengbu Medical College from January 2014 to November 2017. Immunohistochemistry (IHC) was used to analyze the differential expression of calcitonin receptors in gastric cancer and adjacent tissues, and the relationship and significance between their expression and clinical pathological parameters were analyzed K-M and ROC curves were used to assess the K-M and ROC curves were used to evaluate the effect of CALCR expression on patients' postoperative survival and to predict the 5-year postoperative survival rate. The comparison of inter group rates in count data was performed using χ2 test, and the Kaplan Meier method was used to estimate overall survival and progression free survival. The comparison of survival differences was performed using the Log-rank method test. Results:CALCR was mainly expressed in the cytoplasm and its expression in gastric cancer tissues(62.04%(67/108)) was higher than that in paracancerous tissues (13.89% (15/108)), and the differences were statistically significant ( χ2=53.16, P<0.001). The tissue infiltration depth (91.04% (61/67)), lymph node metastasis (73.1% (49/67)), and peripheral blood CA19-9 (32.8% (22/67)) in gastric cancer patients with high expression of CALCR were higher than those in the low expression group of CALCR (63.41% (26/41), 53.7% (22/41), 12.2% (5/41)), and the differences were statistically significant ( χ2 values were 12.40, 4.28, and 4.78; P values were <0.001, 0.038, and 0.016). The results of K-M curve and Log-rank test showed that among gastric cancer patients, the postoperative 5-year survival rate of patients with high expression of CALCR was lower than that of patients with low expression (11.9% (8/67) and 43.9% (18/41), Log-rank χ2=29.92, P<0.001). The results of the ROC curve showed that the area under the curve was 0.772, sensitivity was 87.8%, specificity was 53.7%, indicating that CALCR is of high value in predicting the 5-year survival rate of gastric cancer patients after surgery. Conclusion:CALCR is highly expressed in gastric cancer tissue and is associated with the pathological characteristics of patients. High expression of CALCR suggests poor prognosis, indicating that it has the potential to become an important indicator for evaluating the survival prognosis of gastric cancer patients.
		                        		
		                        		
		                        		
		                        	
4.Predictive efficacy of serum GDF-11 and S100A4 for contrast-induced nephropathy after coronary angiography

		                        			Objective lo investigate the predictive value of serum growth differentiation factor 11(GDF-11)and calprotectin A4(S100A4)for contrast-induced nephropathy(CIN)after coronary angiography(CAG).Methods A total of 528 patients who underwent CAG in the First Affiliated Hospital of Xi'an Jiao-tong University from December 2020 to November 2023 were selected as the research objects.According to whether the patients had CIN,they were divided into non-nephropathy group(472 cases)and nephropathy group(56 cases).Enzyme-linked immunosorbent assay was used to detect serum levels of GDF-11 and S100A4.Multivariate Logistic regression analysis was used to explore the influencing factors of CIN after CAG.Receiver operating characteristic(ROC)curve was used to evaluate the predictive value of serum GDF-11 and S100A4 for CIN after CAG.Results The serum levels of GDF-11 and S100A4 in nephropathy group were higher than those in non-nephropathy group(P<0.05).The contrast agent dose and postoperative ser-um creatinine level in the nephropathy group were higher than those in the non-nephropathy group(P<0.05).Multivariate Logistic regression analysis showed that contrast agent dose ≥131.84 mL(OR=2.158,95%CI 1.284-3.627),postoperative serum creatinine ≥ 87.57 μmol/L(OR=2.445,95%CI 1.533-3.898),GDF-1 1 ≥4 50.84 ng/mL(OR=2.445,95%CI 1.533-3.898)were the influencing factors of CIN af-ter CAG(P<0.05).The area under the curve(95%CI)of serum GDF-11 and S100A4 for predicting CIN af-ter CAG was 0.861(95%CI 0.810-0.912)and 0.798(95%CI 0.747-0.849),respectively.The optimal cut-off values were 450.84 ng/mL and 86.98 μg/mL,the specificity were 65.89%and 57.62%,and the sensi-tivity were 94.74%and 94.74%,respectively.The area under the curve of the combination of the two was 0.906(95%CI 0.856-0.957),the specificity was 87.09%,and the sensitivity was 84.26%.Conclusion The elevated levels of serum GDF-11 and S100A4 are closely related to the occurrence of CIN in patients after CAG surgery,which can be used as biological indicators to evaluate the occurrence of CIN in patients after CAG surgery,and the combined prediction efficiency of GDF-11 and S100A4 is higher.

		                        			Thrombocytopenia is one of the common complications of cirrhotic patients, which can induce an increasing bleeding risk and closely correlate with bleeding following invasive procedures. Consequently, how to respond to thrombocytopenia is crucial for improving the prognosis of patients with cirrhosis. 		                        			Activation of nuclear factor erythroid 2-related factor 2(Nrf2)by Kelch-like ECH-associated protein 1(Keap1)alkylation plays a central role in anti-inflammatory therapy.However,activators of Nrf2 through alkylation of Keap1-Kelch domain have not been identified.Deoxynyboquinone(DNQ)is a natural small molecule discovered from marine actinomycetes.The current study was designed to investigate the anti-inflammatory effects and molecular mechanisms of DNQ via alkylation of Keap1.DNQ exhibited signif-icant anti-inflammatory properties both in vitro and in vivo.The pharmacophore responsible for the anti-inflammatory properties of DNQ was determined to be the α,β-unsaturated amides moieties by a chemical reaction between DNQ and N-acetylcysteine.DNQ exerted anti-inflammatory effects through activation of Nrf2/ARE pathway.Keap1 was demonstrated to be the direct target of DNQ and bound with DNQ through conjugate addition reaction involving alkylation.The specific alkylation site of DNQ on Keap1 for Nrf2 activation was elucidated with a synthesized probe in conjunction with liquid chromatography-tandem mass spectrometry.DNQ triggered the ubiquitination and subsequent degra-dation of Keap1 by alkylation of the cysteine residue 489(Cys489)on Keap1-Kelch domain,ultimately enabling the activation of Nrf2.Our findings revealed that DNQ exhibited potent anti-inflammatory capacity through α,β-unsaturated amides moieties active group which specifically activated Nrf2 signal pathway via alkylation/ubiquitination of Keap1-Kelch domain,suggesting the potential values of targeting Cys489 on Keap1-Kelch domain by DNQ-like small molecules in inflammatory therapies.
		                        		
		                        		
		                        		
		                        	
7.Effect of Jianpi Yichang Powder on NLRP3/ASC/Caspase-l Signaling Pathway in Rats with Ulcerative Colitis

		                        			ObjectiveTo explore the effect of Jianpi Yichang power on the nucleotide-binding oligomerization domain-like receptor protein 3 (NLRP3) inflammasome signaling pathway in a rat model of ulcerative colitis (UC). MethodSixty Sprague-Dawley rats were randomly divided into a normal group (n=10) and an experimental group (n=50). The experimental group received 5% dextran sulfate sodium (DSS) solution freely for 7 days to induce UC, and then they were further randomly divided into model group, sulfasalazine (0.3 g·kg-1) group, and high-, medium-, and low-dose Jianpi Yichang power groups (54.4, 27.2, 13.6 g·kg-1) for continuous treatment of 14 days. The general condition of the rats was observed and recorded daily, and the disease activity index (DAI) was scored before and after treatment. Enzyme-linked immunosorbent assay (ELISA) was used to detect the levels of interleukin-1β (IL-1β) and interleukin-18 (IL-18) in the serum of rats in each group. Hematoxylin-eosin (HE) staining was performed to observe the histopathological changes in the colon tissue. Immunohistochemistry, Western blot, and Real-time polymerase chain reaction (Real-time PCR) were used to detect the positive protein expression, protein expression, and mRNA expression of NLRP3, apoptosis-associated speck-like protein containing a CARD (ASC), and cysteine aspartate-special proteases-1(Caspase-1) in the colon tissue. ResultCompared with the condition in the normal group, the general condition of rats in the model group was relatively poor, with increased DAI scores (P<0.01), pathological changes in the colon, increased levels of IL-1β and IL-18 in the serum (P<0.01), and enhanced positive protein expression, protein expression, and mRNA expression of NLRP3, ASC, and Caspase-1 in the colon tissue (P<0.01). Compared with the condition in the model group, the general condition of rats in the Jianpi Yichang power groups at various doses improved significantly, with reduced DAI scores (P<0.05, P<0.01), alleviated pathological changes in the colon as revealed by HE staining, and reduced protein expression levels of NLRP3 and Caspase-1 in the colon tissue (P<0.05, P<0.01). The serum levels of IL-1β and IL-18, and ASC protein expression in the colon, as well as the mRNA expression levels of NLRP3, ASC, and Caspase-1, decreased in the high- and medium-dose Jianpi Yichang power groups (P<0.05, P<0.01). The positive protein expression levels of NLRP3, ASC, and Caspase-1 were reduced in the high-dose Jianpi Yichang power group (P<0.01). The positive protein expression levels of ASC and Caspase-1 were reduced in the medium-dose Jianpi Yichang power group (P<0.05). The mRNA expression level of ASC was reduced in the low-dose Jianpi Yichang power group (P<0.05). 8.Clinical characteristics and microbiological tests analysis of 18 patients with Talaromyces marneffei infection

		                        			Abstract:Objective To investigate the clinical characteristics and early diagnostic methods of patients with Talaromyces marneffei infection, so as to reduce the mortality of patients. Methods    The clinical characteristics and microbiological analysis data including fungal culture, smear examination and mass spectrometry were collected from 18 patients with Talaromyces marneffei infection in the Department of Respiratory Medicine, Department of Tuberculosis, and Department of Critical Respiratory Medicine in Fuzhou Pulmonary Hospital from January 2017 to December 2021, and descriptive analysis was conducted. Results All the 18 patients were confirmed to be infected with Talaromyces marneffei by conventional culture and matrix-assisted laser desorption/ionization-time of flight (MALDI-TOF) mass spectrometry (MS). The main infection sites of 18 patients with Talaromyces marneffei infection were lungs and lymph nodes, and the patients were accompanied by clinical manifestations such as cough, sputum and fever. The imaging features such as patchy shadows, mediastinal lymph node shadows and nodular shadows were common. Microbiological testing showed a statistically significant difference between smear and culture with a higher positive culture rate (χ2=13.74, P<0.05). The positive rate of blood culture in microbiological test was 60.0% (9/15), the positive rate of bronchial lavage fluid culture was 26.7% (4/15), the positive rate of sputum culture was 5.6% (1/18), one case each of pus, bone marrow, pleural fluid and cerebrospinal fluid was positive for culture and the other cases were negative, one case of sputum and one case of pus were positive for smear and the rest were negative. Colony characteristics showed that the colony morphology was mycelial phase at 25 ℃, producing red pigment, and the branching pattern of the penicillus was seen microscopically as monoverticillate or biverticillate; At 35 ℃, the yeast phase appeared at the initial stage, and then the mycelium phase changed after 5-6 days; the yeast phase was observed at 37 ℃, and yeast-like cells were seen under the microscope. All 18 patients with Talaromyces marneffei infection got better after using antifungal drugs. Compared with non-HIV patients with Talaromyces marneffei infection, leukopenia and anemia were common in HIV patients with Talaromyces marneffei infection, and the differences were statistically significant (P<0.05).  Conclusions    The infection of Talaromyces marneffei can be divided into localized type and disseminated type, which usually invade the lungs, skin, lymph nodes and other places. The main manifestations of patients are fever, cough, phlegm and other atypical symptoms. At present, the diagnosis of Talaromyces marneffei infection is mostly based on the fungal culture test, and the application of MALDI-TOF MS method can effectively shorten the diagnosis time of Talaromycosis marneffei. Clinical characteristics combined with microbiological analysis provide an objective basis for early diagnosis of patients with Talaromyces marneffei infection, and timely use of antifungal therapy can improve the prognosis of patients.
		                        		
		                        		
		                        		
		                        	
9.Preparation and in vitro property evaluation of β-cyclodextrin-daidzein/PEG_(20000)/Carbomer_(940) nanocrystals.

		                        			This study aims to improve the solubility and bioavailability of daidzein by preparing the β-cyclodextrin-daidzein/PEG_(20000)/Carbomer_(940) nanocrystals. Specifically, the nanocrystals were prepared with daidzein as a model drug, PEG_(20000), Carbomer_(940), and NaOH as a plasticizer, a gelling agent, and a crosslinking agent, respectively. A two-step method was employed to prepare the β-cyclodextrin-daidzein/PEG_(20000)/Carbomer_(940) nanocystals. First, the insoluble drug daidzein was embedded in β-cyclodextrin to form inclusion complexes, which were then encapsulated in the PEG_(20000)/Carbomer_(940) nanocrystals. The optimal mass fraction of NaOH was determined as 0.8% by the drug release rate, redispersability, SEM morphology, encapsulation rate, and drug loading. The inclusion status of daidzein nanocrystals was determined by Fourier transform infrared spectroscopy(FTIR), thermogravimetric analysis(TGA), and X-ray diffraction(XRD) analysis to verify the feasibility of the preparation. The prepared nanocrystals showed the average Zeta potential of(-30.77±0.15)mV and(-37.47±0.64)mV and the particle sizes of(333.60±3.81)nm and(544.60±7.66)nm before and after daidzein loading, respectively. The irregular distribution of nanocrystals before and after daidzein loading was observed under SEM. The redispersability experiment showed high dispersion efficiency of the nanocrystals. The in vitro dissolution rate of nanocrystals in intestinal fluid was significantly faster than that of daidzein, and followed the first-order drug release kinetic model. XRD, FTIR, and TGA were employed to determine the polycrystalline properties, drug loading, and thermal stability of the nanocrystals before and after drug loading. The nanocrystals loaded with daidzein demonstrated obvious antibacterial effect. The nanocrystals had more significant inhibitory effects on Staphylococcus aureus, Escherichia coli, and Pseudomonas aeruginosa than daidzein because of the improved solubility of daidzein. The prepared nanocrystals can significantly increase the dissolution rate and oral bioavailability of the insoluble drug daidzein.

10.Effect of acupotomy on the fat infiltration degree of lumbar multifidus muscle in patients with lumbar disc herniation after percutaneous transforaminal endoscopic discectomy.

		                        			OBJECTIVE:
		                        			To observe the effect of acupotomy on the fat infiltration degree of lumbar multifidus muscle (LMM) in patients with lumbar disc herniation after percutaneous transforaminal endoscopic discectomy (PTED).
		                        		
		                        			METHODS:
		                        			A total of 104 patients with lumbar disc herniation treated with PTED were randomly divided into an observation group (52 cases, 3 cases dropped off) and a control group (52 cases, 4 cases dropped off). Patients of both groups received rehabilitation training of two weeks 48 h after PTED treatment. The observation group was treated with acupotomy (L3-L5 Jiaji [EX-B 2]) once within 24 h after PTED. In the two groups, the fat infiltration cross sectional area (CSA) of LMM was compared before and 6 months after PTED, the visual analogue scale (VAS) score and Oswestry disability index (ODI) score were observed before and 1, 6 months after PTED. The correlation between fat infiltration CSA of LMM in each segment and VAS score was analyzed.
		                        		
		                        			RESULTS:
		                        			Six months after PTED, the fat infiltration CSA of LMM in L4/L5 and the total L3-S1 segments of the observation group was lower than that before PTED (P<0.05), and the fat infiltration CSA of LMM in L4/L5 of the observation group was lower than the control group (P<0.01). One month after PTED, the ODI and VAS scores of the two groups were lower than those before PTED (P<0.01), and those in the observation group were lower than the control group (P<0.05). Six months after PTED, the ODI and VAS scores of the two groups were lower than those before PTED and 1 month after PTED (P<0.01), and those in the observation group were lower than the control group (P<0.01). There was a positive correlation between the fat infiltration CSA of LMM in the total L3-S1 segments and VAS scores in the two groups before PTED (r = 0.64, P<0.01). Six months after PTED, there was no correlation between the fat infiltration CSA of LMM in each segment and VAS scores in the two groups (P>0.05).
		                        		
		                        			CONCLUSION
		                        			Acupotomy can improve the fat infiltration degree of LMM, pain symptoms and activities of daily living in patients with lumbar disc herniation after PTED.
