1.Risk Factor and Risk Prediction Modeling of Rectal Neuroendocrine Tumors

Objective To analyze the risk factors associated with the occurrence of rectal neuroendocrine tumors (RNETs) and construct a risk prediction model. Methods Clinical data of patients who underwent electronic colonoscopy were collected. The clinical information on patients with and without RNETs were compared, and potential risk factors for RNETs were identified. Binary logistic regression was performed to analyze the relevant risk factors and construct a risk prediction model. Results Among 164 patients, 66 were diagnosed with RNETs, and 98 who did not have such a condition were randomly selected. Univariate logistic regression analysis revealed that age, fatty liver, anxiety and depression, total cholesterol, triglyceride levels, and carcinoembryonic antigen (CEA) were significant factors influencing the occurrence of RNETs (P<0.05). Multivariate logistic regression analysis identified age (P=0.015), anxiety and depression (P=0.031), cholesterol level (P=0.009), fatty liver (P=0.001), and CEA (P<0.001) as independent risk factors for RNETs. The participants were randomly divided into training and test sets at a 7:3 ratio. The training set was used to construct a nomogram-based risk prediction model, and the testing set was used for internal validation. The area under the curve values for the training and testing sets were 0.843 and 0.772, respectively (P>0.05). These findings indicate a good discriminative performance. The calibration curves for the training and testing sets were in good agreement with the 45° standard line, which suggests that the predicted probabilities were consistent with the actual outcomes. Decision curve analysis showed that the model provided a high net benefit within a threshold range of 0.2 to 0.7 for clinical decision making. Conclusion Young age, fatty liver, high CEA levels, high cholesterol levels, and anxiety and depression are independent risk factors for RNETs. The nomogram model constructed based on these risk factors exhibits a strong capability to predict the occurrence of RNETs, and clinical intervention can be considered based on the predicted probability values.
2.Improvement effects of Tongxie yaofang on irritable bowel syndrome with diarrhea by regulating colonic TPH1,SERT and intestinal flora

OBJECTIVE To investigate the effects of Tongxie yaofang (TXYF) on the symptoms of rats with irritable bowel syndrome with diarrhea (IBS-D) by regulating colonic tryptophan hydroxylase 1 (TPH1), serotonin transporter (SERT) and intestinal flora. METHODS Forty-two SD rats were randomly divided into control group (7 rats) and modeling group (35 rats). In modeling group, rat model of IBS-D was established by intragastrical administration of 0.45 g/L senna leaf solution [10 mL/(kg·d)] combined with chronic unpredictable stimulation. Thirty-five successfully modeled rats were randomly divided into model group, pinaverium bromide group [15 mg/(kg·d)] and TXYF low-dose, medium-dose and high-dose groups [3.75、7.5、15 g/(kg·d), calculated by crude drug], with 7 rats in each group. Each administration group was orally administered the corresponding drug, once a day, for 10 consecutive days. The general condition and weight changes of each group of rats were compared before modeling, after modeling and before administration, after the last drug intervention; the diarrhea index and visceral sensitivity were detected, and pathological changes of colon tissue were observed after modeling and before administration, after the last drug intervention. The level and expression of 5-hydroxytryptamine (5-HT), protein and mRNA expressions of TPH1 and SERT were determined in colon tissue. The diversity and structural changes of fecal intestinal flora of rats were analyzed. RESULTS There was no significant change in colon histopathology in each group. Compared with model group, the general condition of rats in each medication group improved. The daily body weight gain of rats was significantly increased, while diarrhea index, visceral sensitivity, the expressions of 5-HT and TPH1 in colon tissue were significantly decreased; SERT expression of colon tissue was significantly increased in TXYF medium-dose and high-dose groups (P<0.05 or P<0.01). The diarrhea index, colon TPH1 protein expression and colon 5-HT protein positive rate in the TXYF low-dose group decreased while the mRNA expression of SERT increased significantly (P<0.05). There was a dose- dependent trend in the effect of TXYF. Compared with model group, Chao1 index and Shannon index of the rats in TXYF high- dose group were significantly decreased (P<0.05 or P<0.01), the beneficial bacteria such as Firmicutes and Lactobacillus increased significantly, while the pathogenic bacteria such as Proteobacteria, Escherichia-Shigella and Rikenellaceae_RC9_gut_ group decreased significantly (P<0.05 or P<0.01). CONCLUSIONS TXYF can decrease the level of 5-HT and improve intestinal flora disorder by inhibiting the expression of TPH1 and up-regulating the expression of SERT in colon tissue, thus promoting the symptoms of IBS-D rats.
3.Quantitative analysis of cervical vertebral maturation in Chinese adolescents based on three-dimensional morphology of cervi-cal vertebrae

Objective To investigate associations between three-dimensional(3D)morphology of cervical vertebrae and skeletal mat-uration by cone-beam computed tomography(CBCT)and establish corresponding regression models for quantitatively evaluating cervical vertebral maturation(CVM).Methods The analyzed sample consisted of 358 CBCT images(175 male,183 female),of which 277 images were randomly selected as the model development group and 81 as the performance test group.Twenty-one 3D morphological pa-rameters were defined and measured,incorporating all parts of the cervical vertebrae,including the cervical vertebral bodies,transverse processes,spinous processes,pedicles,lamina,and articular processes.The cervical vertebral maturation index(CVMI)was determined by experienced orthodontists as reference standard.Spearman's rank correlation coefficient and multivariable stepwise regression analysis were used to identify the associations and build regression models.The performance test group was employed to ex-amine each model's reliability.Paired-samples Wilcoxon signed-rank test compared the CVMI of the model prediction with the reference standard.Results Three-dimensional morphological changes in various parts of the cervical vertebrae correlated with CVMI(P<0.05).Six 3D morphometric parameters were each recognized for male and female models,three of which were identical.The adjusted R2 was 0.899 for males and 0.902 for females,with corresponding accuracies of 85.0%and 85.4%,respectively.These models showed no difference as compared with the reference standard(P>0.05).Con-clusion New associations were found between 3D morphology of cer-vical vertebrae and skeletal maturation.The 3D-driven morphometric CVM assessment method and corresponding regression models exhibited good credibility and high consistency with experts.
4.Clinical decision of centric relation

Centric relation(CR)was used in the complete dentures for edentulous jaws firstly.Then,CR was gradual-ly applied in the clinical diagnosis and treatment of dentate jaws.Regarding the application of CR in dentate jaws,there are many doubts about its rationality.For instance,should CR be the core of diagnosis and treatment of temporomandibu-lar disorders?Should CR be used as the jaw position of establishing occlusion in occlusal reconstruction?Should CR serve as the target jaw position in orthodontic treatment?Meanwhile,varieties of CR clinical applications are objective.According to the existing evidence,this paper summarizes the applicable conditions of several main clinical situations with little controversy.We preliminarily put forward the decision tree for the clinical application of CR,which can be used as a reference in clinical practice.
5.Application effect of multidisciplinary team nursing model in enteral nutrition support of ICU patients

Objective:To explore the application effect of multidisciplinary team (MDT) nursing model in enteral nutrition support for patients in intensive care unit (ICU) .Methods:The convenient sampling method was used to select 76 patients undergoing enteral nutrition in ICU of Taizhou Hospital in Zhejiang Province from August 2020 to August 2022 as the research objects. A total of 37 patients admitted from August 2020 to August 2021 were set as the control group, and 39 patients admitted from September 2021 to August 2022 were set as the intervention group. Patients in the control group received routine nursing, while patients in the intervention group received MDT nursing model. The start time of enteral nutrition after admission to ICU, the compliance rate of enteral nutrition at 7 days after feeding, the rate of feeding interruption and the incidence of complications were compared between the two groups.Results:After intervention, the start time of enteral nutrition after admission to ICU in the intervention group was shorter than that in the control group, the compliance rate of enteral nutrition at 7 days after feeding was higher than that in the control group, and the feeding interruption rate and incidence of aspiration, nausea, vomiting and diarrhea were lower than those in the control group ( P<0.05) . Conclusions:The MDT nursing model can improve the effect of enteral nutrition support for ICU patients and reduce the incidence of enteral nutrition complications.
6.Development of a fast Monte Carlo dose verification module for helical tomotherapy

Objective To develop a GPU-based Monte Carlo dose calculation module for helical tomotherapy(TOMO),and integrate it into the commercial software ArcherQA to achieve fast and accurate dose verification in clinic.Methods The TOMO treatment head was modeled using TOPAS to obtain phase space files,and a fast weight tuning algorithm was used to simulate particle transport in multi-leaf collimator for improving computational efficiency,and finally,GPU-based Monte Carlo algorithms in ArcherQA were used to simulate particle transport in patients.To verify the model accuracy,the ArcherQA calculated results in water tank were compared with measured data for different open fields.In addition,multiple comparisons among ArcherQA results,TPS results and ArcCHECK results were conducted on 15 clinical cases(5 cases in the head and neck,5 cases in the chest and abdomen,and 5 cases in the whole body).Results In the water tank tests for 40 cm×5.0 cm,40 cm×2.5 cm and 40 cm× 1.0 cm radiation fields,the average global relative errors of the percentage depth dose,transverse dose distribution,and longitudinal dose distribution calculated by ArcherQA with the corresponding measured values were 0.72%,0.66%,and 0.54%,respectively.Over 98%of the voxels had a global relative error of less than 1%.As for 15 clinical cases,in 2%/2 mm criteria,the mean Gamma passing rate was 98.1%between ArcherQA and TPS,99.1%between TPS and ArcCHECK,and 99.4%between ArcherQA and ArcCHECK.The uncertainty of the simulation maintained less than 1%,and the average time taken for calculation based on patient CT vs ArcCHECK phantom was 87 s vs 64 s.Conclusion ArcherQA can be used for independent dose validation for TOMO plans for it can provide fast and accurate dose calculations.
7.Preliminary clinical application of magnetic resonance-guided fractionated stereotactic radiation in the treatment of brain tumors

Objective:To evaluate the dosimetric characteristics, safety and effectiveness of magnetic resonance-guided fractionated stereotactic radiotherapy (FSRT) for brain tumors.Methods:Clinical data of 8 brain tumor patients treated with magnetic resonance-guided FSRT in the Radiotherapy Department of the First Medical Center of the PLA General Hospital from July 2023 to February 2024 were retrospectively analyzed. Online adaptive radiotherapy was adopted for all patients. Adapt-to-position (ATP) or adapt-to-shape (ATS) radiotherapy was chosen by radiologists. Each adaptation was initiated after the radiotherapy plan was re-examined. The radiotherapy fractionation plan was 21-30 Gy/3-5 F. Clinical characteristics, radiotherapy plans and plan parameters were analyzed by statistical description. Median ( Q1, Q3) was used to describe continuous data and percentage was used to describe categorical data. Results:In this study, 9 lesions were treated a total of 41 times, including 20 times (49%) of ATP plan and 21 times (51%) of ATS plan. The median target area coverage rate was 95.1% (95%, 99.8%), the median target area maximum dose rate was 1.15 (1.07, 1.31), the median conformity index (CI) was 0.75 (0.69, 0.86), the median homogeneity index (HI) was 1.09 (1.06, 1.21), and the median gradient index (GI) was 4.73 (3.36, 8.45), respectively. After ATS plan, the median reduction in gross target volume (GTV) was 8.22 cm3 (1.2, 10.1 cm3), and the median reduction in brain tissue V12 Gy was 30.46 cm3 (8.34, 31.13 cm3).The median follow-up was 3.2 months (1.4, 6.1 months). No radiation necrosis was found in any patient. There were 2 cases of acute brain edema during radiotherapy (both were mild). Except for 1 case who died due to systemic disease progression, the remaining patients had no local recurrence, and achieved good quality of life. Conclusions:The parameters of the treatment plan of magnetic resonance-guided FSRT are generally acceptable. The adaptive plan can effectively reduce the dose of normal brain tissues. It is safe and feasible to use the magnetic resonance-guided FSRT for brain tumors.
8.Gut microbiota: a new insight into neurological diseases.

In the last decade, it has become increasingly recognized that a balanced gut microbiota plays an important role in maintaining the health of the host. Numerous clinical and preclinical studies have shown that changes in gut microbiota composition are associated with a variety of neurological diseases, e.g., Parkinson's disease, Alzheimer's disease, and myasthenia gravis. However, the underlying molecular mechanisms are complex and remain unclear. Behavioral phenotypes can be transmitted from humans to animals through gut microbiota transplantation, indicating that the gut microbiota may be an important regulator of neurological diseases. However, further research is required to determine whether animal-based findings can be extended to humans and to elucidate the relevant potential mechanisms by which the gut microbiota regulates neurological diseases. Such investigations may aid in the development of new microbiota-based strategies for diagnosis and treatment and improve the clinical management of neurological disorders. In this review, we describe the dysbiosis of gut microbiota and the corresponding mechanisms in common neurological diseases, and discuss the potential roles that the intestinal microbiome may play in the diagnosis and treatment of neurological disorders.

9.The relationship of serum NLRP3 and PTEN expression with sex hormone level and semen quality in male infertile patients

Objective To investigate the relationship of the expression of nucleotide-binding oligomerization domain-like receptor protein 3(NLRP3)and phosphatase and tensin homolog deleted on chromosome 10(PTEN)in the serum of male infertility patients with the levels of sex hormones and semen quality.Methods A total of 80 male infertile patients admitted to our hospital from May 2021 to April 2023 were enrolled as the research subjects,who were divided into oligozoospermia group(n = 47)and asthenozoospermia group(n = 33)based on semen quality.Meanwhile,50 male healthy patients who underwent health examinations in our hospital during the same period were recruited as the control group.The level of serum NLRP3 and PTEN were determined by ELISA,and their relations with the sex hormone levels and semen quality were analyzed.Results Compared with the control group,the semen quality as well as the levels of testosterone(T)and serum PTEN in the patients were significantly reduced(P<0.05),while the levels of follicle-stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E2),prolactin(PRL)and serum NLRP3 were significantly increased(P<0.05).Compared with the oligospermia group,the sperm concentration,semen volume,and the levels of FSH,T,serum NLRP3,and PTEN in the asthenospermia group were significantly increased(P<0.05),the sperm motility and forward motility sperm rate(PR)were significantly decreased(P<0.05),and there were no significant differences in the levels of LH,E2,and PRL(P>0.05).Through Pearson correlation analysis,the level of serum NLRP3 was negatively correlated with the semen volume,sperm motility,and the levels of PR and T,and significantly positively correlated with the levels of FSH,LH,E2,and PRL(P<0.05);The PTEN expression was significantly positively correlated with the sperm concentration,semen volume,sperm motility and the levels of PR and T,and negatively correlated with the levels of FSH,LH,E2,and PRL(P<0.05).Conclusion The level of serum NLRP3 is decreased and the PTEN level is increased in male infertile patients.Both of them are correlated with sex hormones and semen quality.
10.Simultaneous implantation and tooth preparation technology guided by 3D-printed guide.

Using digital technologies in concurrently performing missing tooth implantation and preparation of remaining teeth is a solution to reduce the number of visits and improve efficiency. This paper proposes a digital process for simultaneously implanting and preparing teeth. It integrates implant surgical guide and 3D-printed tooth preparation guide into a single guide and completes guided implant placement and precise tooth preparation. Based on "repair-oriented" virtual implant planning, the implant surgical guide can improve the efficiency and predictability of implant placement, and its linear accuracy is about 1 mm. The tooth preparation guide precisely guides tooth preparation and restoration space visualization, ensuring the quality of the tooth preparation. The two guides have different design accuracy requirements, and thus their combination improves the overall guiding accuracy requirements. The concurrent application of the two guides minimizes the clinical operation time, number of visits, and economic burden of patients.
