1.Effect of right stellate ganglion block on acute lung injury induced by hind limb ischemia-reperfusion in rats

Objective:To evaluate the effect of the right stellate ganglion block (SGB) on the acute lung injury induced by hind limb ischemia-reperfusion (I/R) in rats.Methods:Twenty-seven SPF healthy male Sprague-Dawley rats, aged 7-8 weeks, weighing 200-250 g, were divided into 3 groups ( n=9 each) by the random number table method: sham operation group (Sham group), hind limb I/R group (I/R group) and right SGB+ hind limb I/R group (SGB+ I/R group). The right hind limb I/R injury model was developed in anesthetized rats in I/R and SGB+ I/R groups, and the right SGB was performed with 0.2% ropivacaine 0.5 ml before the right hind limb ischemia in SGB+ I/R group. The pathological changes of lung tissues were observed with a light microscope, the wet/dry lung weight ratio, PaO 2 and PaCO 2 were determined, and the oxygenation index was calculated. The concentrations of serum malondialdehyde (MDA) and glutathione peroxidase (GSH-Px) were determined using enzyme-linked immunosorbent assay. Results:Compared with Sham group, the wet/dry lung weight ratio, PaO 2, oxygenation index, and GSH-Px concentrations were significantly decreased, and the concentrations of serum MDA were increased in I/R and SGB+ I/R groups ( P<0.05). Compared with I/R group, the wet/dry lung weight ratio, PaO 2, oxygenation index, and GSH-Px concentrations were significantly increased, the concentrations of serum MDA were decreased ( P<0.05), and the pathological changes of lung tissues were significantly attenuated in SGB+ I/R group. There was no significant difference in PaCO 2 among the 3 groups ( P>0.05). Conclusions:The right SGB can alleviate the acute lung injury induced by hind limb I/R, and the mechanism may be related to the inhibition of oxidative stress responses in rats.

3.Clinical characteristics and diagnostic approach of patients with high-level fall injuries combined with chest trauma: a study utilizing RTS and AIS-ISS scoring systems

Objective:To investigate the clinical characteristics and diagnostic approach of patients with high-level fall injuries combined with chest trauma.Methods:A retrospective analysis was conducted on 137 patients who sought medical treatment at Suzhou Dushu Lake Hospital (Dushu Lake Hospital Affiliated to Soochow University) between December 2020 and June 2023. These patients had sustained injuries from high-level falls with concurrent chest trauma. Among them, there were 87 male individuals with an average age of (40.44±10.23) years and 27 female individuals with an average age of (36.92±10.75) years. Among the patients, 114 met the inclusion criteria, while 23 were excluded. The clinical characteristics and treatment outcomes were analyzed based on the Revised Trauma Score and chest trauma score in order to further investigate the trauma characteristics of patients with high-level fall injuries and enhance the efficiency of diagnosis, treatment, and clinical outcomes.Results:There was a negative linear correlation between the Revised Trauma Score and chest trauma score in patients with high-level fall injuries combined with chest trauma. Chest trauma, requiring comprehensive management throughout the entire course of the disease, emerged as a significant independent factor affecting clinical prognosis. In the RTS severe injury group, the proportion of severe chest injuries or multiple injuries in patients with high-level fall injuries was 59/68 (86.76%), with 47/68 (69.11%) of patients experiencing concomitant head injuries, 25/68 (36.76%) experiencing abdominal organ injuries, 58/68 (85.29%) experiencing limb or spinal fractures, and 26/68 (38.23%) having other types of injuries. In the RTS mild injury group, the proportion of severe chest injuries or multiple injuries was 17/46 (36.95%), with 17/46 (36.95%) of patients experiencing concomitant head injuries, 8/46 (17.39%) experiencing abdominal organ injuries, 17/46 (36.95%) experiencing limb or spinal fractures, and 11/46 (23.91%) having other types of injuries.Conclusion:Patients with high-level fall injuries are prone to multiple chest injuries or injuries in other regions, exhibiting complex and variable injury patterns. Particularly in the RTS severe injury group, the incidence of severe chest injuries or concomitant injuries in the chest or other regions is significantly higher compared to the RTS mild injury group. Therefore, comprehensive management of chest trauma should be prioritized throughout the clinical diagnosis and treatment process to further improve patient prognosis.

5.Relationship between health literacy of primary school teachers and health behavior of students in Baoji city

Objective:
To understand the current situation and the relationship between the health literacy of primary school teachers and health behavior literacy of students, and to provide some inspiration for the reform of health promotion and education in school.
Methods:
Twelve primary schools were selected from Jintai district, Chencang district and Long county,and we used stratified cluster random sampling method to choose 334 primary school teachers and 622 students of grade 4 and 6 to fill out the questionnaire.
Results:
Nearly 46.71% of all the teachers had adequate health literacy. Multivariate Logistic regression analysis showed that the urban and rural (OR=2.30, 95%CI=1.30-4.07) and gender (OR=2.62, 95%CI=1.36-5.05) were statistically significant for the health literacy among teachers; Adequate health behavior literacy was found in 46.78% of all the students,multivariate Logistic regression analysis also showed the urban and rural (OR=2.07, 95%CI=1.33-3.24)and mother’s education (OR=2.01-2.41, 95%CI=1.25-4.76) were statistically significant. Spearman correlation coefficient between health literacy of the teachers and students was(r=0.69, P<0.05).
Conclusion
The relation between health literacy of the teachers and students is positive. To improve the health literacy of primary school students, health literacy of primary school teachers should be improved first in China.
6.Status and influencing factors of health literacy among permanent residents in Shaanxi Province

Objective:
To learn the health literacy level and its influencing factors among permanent residents in Shaanxi Province in order to improve it.
Methods:
From July to November of 2017,multi-stage stratified cluster sampling method was used to select 15-69 year-old permanent residents in Shaanxi Province. A survey was conducted using“Chinese citizen health literacy monitoring questionnaire”issued by National Health Commission. The influencing factors for health literacy were analyzed by the multivariate logistic regression model.
Results:
Totally 2 352 valid questionnaires were collected with a response rate of 98.00%. The health literacy level of permanent residents in Shaanxi Province was 10.54%. The levels of basic knowledge,healthy lifestyle and behaviors,and health related skills were 24.81%,9.33% and 14.52%,respectively. The levels of scientific health views,safety and first aid,health information,infectious diseases prevention,basic medical care and chronic diseases prevention were 42.81%,42.73%,19.11%,18.32%,16.27% and 8.24%,respectively. The results of multivariate logistic regression analysis showed that educational background of junior high school or above(OR:6.660-29.077,95%CI:2.050-97.355)was a promoting factor of health literacy,yet rural residency(OR=0.621,95%CI:0.442-0.872)was a constraining factor.
Conclusion
The heath literacy level of Shaanxi residents in 2017 was 10.54%. Educational background of junior high school or above and rural residency were influencing factors for health literacy.
7.Endoscopic ultrasound guided biliary drainage in patients with biliary obstruction and surgically altered anatomy

Objective To evaluate the efficacy of endoscopic ultrasound guided biliary drainage (EUS-BD) in patients with biliary obstruction and surgically altered anatomies.Methods We collected data from 33 patients with biliary obstruction and surgically altered anatomies from January 2016 to January 2018 in Zhejiang University School of Medicine Affiliated Hangzhou First People's Hospital who underwent EUS-guided biliary drainage after unsuccessful ERCP.The operation success rate,clinical success rate,complication rate,hospital stay were studied.Results Of 33 patients,31 were successfully operated and stented using endoscopic ultrasound puncture:14 patients through the stomach,17 patients through the duodenum;8 patients by the rendezvous approach.The operation success rate was 93.9%.Of the 33 patients,28 had a significant decrease in jaundice,with a clinical success rate of 84.9%.Complications consisted of 2 patients with bleeding and 1 patient with cholangitis.These patients improved after conservative treatment.The complications rate was 9.1%.The hospital stay was (12.4±5.7) d.Conclusion EUS-BD can be the first choice for patients with biliary obstruction and surgically altered anatomy after failed endoscopic retrograde cholangiograohv in centers with exoertise in EUS-BD procedures.
8.Comparison of clinical effects of bare stent and covered metal stent graft in the treatment of complete occlusion of central venography in hemodialysis patients

Objective To evaluate the efficacy of bare mental stent (BMS) and covered stent (CS) in the treatment of complete central venous occlusive disease (CVOD) in hemodialysis patients.Methods A total of 66 cases of CVOD who have been treated by endovascular methods successfully in the First Affiliated Hospital of Sun Yat-sen University from Jan 2015 to Jan 2017 were enrolled in this study.According to the type of stent,the patients were divided into two groups,BMS group (n=46)and CS group (n=20).The demographic data,clinical signs and symptoms,and pre-procedure and post-procedure imaging data were followed up and recorded.The primary patency rates were calculated at 1,3,6,9,and 12 months.Results The related symptoms were improved within 2 day post-procedure.The primary patency rates of BMS group in 1,3,6,9 and 12 months were 97.83%,95.65%,69.56%,41.3%,and 34.78% respectively.The rates of CS group were 100%,100%,95%,65%,and 60%respectively.They did not reached statistical significance for primary patency rates between two groups in 1,3,and 6 months (P > 0.05 respectively).However,from 9 months after procedure,it began to show the significant difference between two groups (P < 0.05).The median patency time of the CS group was (10.30±5.32) months,while BMS group was (8.52±0.49) months.The difference between the two groups was statistically significant (P=0.046).Conclusions Stent implantation for complete occlusion of central venous in hemodialysis patients can get credible effect.The use of CS for CVOD provides superior patency as well as patency time in long period after procedure as compared with BMS.
9.The use of multi-detector computed tomography venography and three dimensional reconstruction image in evaluating the central venous occlusive disease of hemodialysis patients

Objective To evaluate the feasibility and value of multi-detector computed tomography venography (MDCTV) and three dimensional reconstruction image in the assessment of central venous occlusive disease in hemodialysis patients,and in the value of guiding interventional treatments.Methods Sixty hemodialysis patients with swelling of upper limbs were scanned by Toshiba 128-multislice spiral computed tomography (128-MSCT) and totally 80-100 ml non-ionic contrast media was injected into each of the patients via the peripheral veins of the contralateral limb with the rate of 4 ml/s.MSCT scanning was taken by the technique of intelligent triggering after setting scanning triggering threshold,with the monitoring point set in the development of the lumen of inferior vena cava,to detect the position and degree of vascular stenosis.The images were reformed as maximum intensity projection (MIP),volume rendering (VR),curved planar reformation and threedimensional image reconstruction technique.Results MDCTV clearly demonstrated the lesion location in all cases enrolled.Seventy-five occlusive lesions were detected in the total of 60 hemodialysis patients with swelling of upper limbs by MDCTV,of which the lesions of brachiocephalic vein was 47,superior vena cava 15 and subclavian vein 13.Among the 75 stenosis lesions,the number of complete occlusive,severe,moderate and mild stenosis was 31,24,19 and only 1,respectively.MDCTV provided information coincident with that of digital subtraction angiography (DSA),which the correlation index was 0.401,while DSA showed that number of complete occlusive,severe,moderate and mild stenosis was 49,7,14 and 5,respectively.Pereutaneous transluminal angioplasty was performed in 53 patients,and stent placement was done in 40 patients.After interventional treatments,swelling of upper limbs were obviously relieved and vascular accesses got functional recovery to the extent that they could meet the requirement of hemodialysis.Conclusions MDCTV is the first choice to evaluate the condition of central venous occlusive diseases of hemodialysis patients with advantages of non-invasion,high definition and three-dimensional reconstruction.It can provide accurate evaluations of the conditions of occlusive lesions,which can be of great clinical significance to subsequent interventional therapy.
10.Effect of Sorafenib in Increased VEGF in Stimulating Effect of Walker-256 Transplanted Liver Cancer Model in SD Rats

[Objective] To investigate association between the time point of sorafenib administered and suppress effect on tumor growth secondary to the increased expression of vascular endothelial growth factor (VEGF).[Methods] Fifty SD rats were performed intrahepatic implantation using tumor tissues from subcutaneous tumors in nude mice which were administered Walker 256 tumor cells.Ten days after the procedure,MR scans were used to choose forty SD rats with successful hepatic tumor transplantation among fifty experimental animals.Then they were randomly divided into four groups:(A,control group) mere injection of vascular endothelial growth factor (VEGF);(B) administration of sorafenib 72 hours prior to VEGF injection;(C) administration of sorafenib together with VEGF injection;(D) administration of sorafenib 72 hours later to VEGF injection.The tumor growth and median survival time of rodents were observed and compared.After each experimental animal died,immunohistochemical (IHC) methods were applied to detect the expression of VEGF in tumors.[Results] Ten days after the administration of sorafenib,MR showed significant growth of hepatic tumors,the tumor size in experiment group were significiant smaller,than control group (5.4 cm) with statistical significance.Median survival time of four groups were (19.6 ± 1.8) d,(31.2 ± 7.0) d,(27.4 ± 4.9) d,and (26.5 ± 4.6) d,respectively,which indicated that animals in sorafenib groups lived longer than those in control group (P < 0.05).Differences can be obseverd in sorafenib groups with statistical significance existing (P < 0.05).Harvest hepatic tumor tissues from dead animals and HE staining as well as IHC examination were performed.The expression of VEGF in four groups were 88.3 ± 13.6,42.8 ± 8.0,71.9 ± 15.7,and 73.6 ± 13.7.There were statistical significance between control group and sorafenib groups.And further in sorafenib groups,the expressions of VEGF also varied greatly.[Conclusion] Sorafenib can extend the survival time,reduce tumor angiogenesis.And we can conclude that administration of sorafenib before the transient increased expression of VEGF offers survival benefits than that after the evaluation of VEGF levels.
