1.Effects of low-dose esketamine on analgesia and inflammatory factors after thoracoscopic surgery

OBJECTIVE To observe the effects of low-dose esketamine on analgesia and inflammatory factors after thoracoscopic surgery. METHODS Totally 120 patients who underwent thoracoscopic lobectomy in our hospital from October 2021 to March 2022 were selected and randomly divided into low-dose group (group A), conventional-dose group (group B), normal saline group (group C) by using the random number table method, with 40 patients in each group. All the patients were anesthetized with traditional general intravenous anesthesia, group A and B were anesthetized with low dose or normal dose (0.2 or 0.5 mg/kg) of Esketamine hydrochloride injection, and group C was given normal saline intravenously. The visual analog scale (VAS) score 0, 6, 24, 48 h after operation and the consumption of sufentanil 24 h after operation were compared among 3 groups. The levels of white blood cell count (WBC), neutrophil percentage, C-reactive protein (CRP), tumor necrosis factor-α (TNF-α), interleukin-1 (IL-1) and IL-6 were compared 30 min before and 24 h after surgery. The Pittsburgh sleep quality index (PSQI) score and the Beck depression inventory (BDI) score were compared before operation and 0, 1, 2 d after operation, and the occurrence of adverse reaction was also recorded. RESULTS At 0, 6 and 24 h after operation, VAS scores and the consumption of sufentanil within 24 h after operation in group A and B were significantly lower than group C; VAS score 6 h after operation in group A was significantly lower than that in group B (P<0.05). The levels of WBC, neutrophil percentage, CRP, TNF-α, IL-1 and IL-6 in the 3 groups 24 h after surgery were significantly increased, compared with 30 min before surgery; the levels of above indexes in group A and B were significantly lower than those in group C (P<0.05). PSQI score and BDI score 0, 1 and 2 days after operation in group A and B were significantly lower than those in group C, while BDI score 0 day after operation in group A was significantly lower than that in group B (P<0.05). The total incidence of adverse reaction in group A, B and C were 5.0%, 10.0% and 17.5%, without statistical significance (P>0.05). CONCLUSIONS The low-dose esketamine has significant analgesic effect after thoracoscopic surgery, can reduce the levels of inflammatory factors after surgery and improve sleep quality and depression, with good safety.
2.Application Status and Research Progress of Palliative Care in Patients with End-stage Renal Disease

End-stage renal disease is the final stage of chronic kidney disease, and research on palliative care for end-stage renal disease patients in China is still in its infancy. The research content of palliative care for end-stage renal disease at home and abroad mainly includes identification and management of symptoms, advance care planning, psychosocial and spiritual support, and ethical issues in dialysis decision-making. However, practical experience is still insufficient. By focusing on the overview, development status, patient needs, as well as implementation forms and models of palliative care for endstage renal disease patients, this paper summarized the research progress and application status of related research, with a view to providing references for future domestic research and clinical practice in this field.
3.Effect of nursing checklist in posterior surgery for patients with thoracolumbar fracture with general anesthesia under prone position

Objective:To explore the value of nursing checklist in posterior surgery for thoracolumbar fracture with general anesthesia under prone position.Methods:A retrospective case series study was conducted to analyze the clinical data of 106 patients with thoracolumbar fracture admitted to Sir Run Run Shaw Hospital,Zhejiang University School of Medicine from June 2018 to May 2020. There were 80 males and 26 females,with age range of 25-57 years[(48.6 ± 11.9)years]. Segments of injury were located at T 11 in 18 patients,at T 12 in 26,at L 1 in 21,at L 2 in 25 and at L 3 in 16. All patients were treated with thoracolumbar posterior screw fixation under general anesthesia. Of all,51 patients received conventional postural nursing from June 2018 to May 2019(control group),and 55 patients received prone position nursing scheme for general anesthesia on the basis of conventional postural nursing from June 2019 to May 2020(verification group). The incidence of postoperative complications including stress injury,brachial plexus injury,ulnar nerve injury and ocular discomfort as well as length of hospital stay and patients’ satisfaction were compared between the two groups. Visual analogue scale(VAS)and Oswestry disability index(ODI)were also used to measures outcome at postoperative 3 months. Results:All patients were followed up for 8-12 months[(10.5±0.9)months]. Verification group and control group showed significant differences in the incidence of stress injury(4%∶29%),brachial plexus injury(4%∶16%)and ocular discomfort consisiting of tears(2%∶12%),foreign body sensation(0%∶4%)and dryness(4%∶16%)( P < 0.05),not in ulnar nerve injury and blurred vision. Length of hospitalization in verification group was(7.0±1.3)days,significantly shorter than that in control group[(9.9±1.9)days]( P < 0.05). Satisfaction of patients in verification group and control group was 85%(47/55)and 69%(35/51),respectively( P < 0.05). At 3 months postoperatively,VAS in verification group[(1.9 ± 0.8)points]was significantly lower than that in control group[(3.5±1.1)points]( P < 0.05),and ODI was similar between the two groups( P > 0.05). Conclusions For patients with thoracolumbar fracture treated by posterior surgery with general anaesthesia under prone position,nursing checklist helps reduce occurrence of the related complication,shorten length of hospital stay,improve patient satisfaction,reduce postoperative pain and promote rehabilitation.
4.Qualitative Study on Survival Stressors of 15 Community-dwelling People with Mental Illness.

To understand the survival stress of community-dwelling people with mental disorder. Fifteen cases were selected by purposive sampling and received semi-structured individualized interviews.The data were analyzed by Colaizzi framework and themes were extracted. Four themes were extracted:physiological stress due to psychiatric symptoms and side effects of drugs;psychological stress due to the outcome of mental illness and to conflict of roles in daily life;social and environmental stress such as social discrimination,lack of job opportunities,and difficulty in obtaining social welfare resources;and interpersonal stress caused by discrimination and deteriorating family relations. Community-dwelling people with mental illness have a higher level survival stress after returning to their families and society,with the stressors including symptoms of illness,social discrimination,and interpersonal relationship.Eliminating self-discrimination of the patients,improving social support and social welfare system,and increasing individualized community mental rehabilitation activities may reduce the survival stress of these patients and promote their rehabilitation.

5.Correlation between stigma and posttraumatic growth in female infertility patients

Objective:To explore the current situation of stigma and posttraumatic growth in female infertility patients, and to analyze the correlation between them.Methods:From January to May 2019, this study selected 110 female infertility patients of reproductive medicine center in a ClassⅢ Grade A hospital of Zhejiang Province as subjects by convenience sampling. Those infertility patients were investigated with the general information questionnaire, Infertility Stigma Scale (ISS) and the Post-traumatic Growth Inventory-Chinese version (PTGI-C) . A total of 110 questionnaire were sent out and 104 valid of them were collected.Results:Among 104 female infertility patients, the score of ISS was (67.14±14.33) with the intermediate level; the score of PTGI-C was (63.92±11.76) also with the intermediate level. There were statistical differences in the scores of post-traumatic growth among female infertility patients with different education levels, characteristics and whether being the only child ( P<0.05) . The scores of ISS of female infertility patients had statistically negative correlations with the total score and dimension scores of PTGI-C ( P<0.01) . Hierarchical regression analysis showed that the main influencing factors of posttraumatic growth in female infertility patients were the education levels, years of infertility, total score of stigma and scores of emotion and behavior with statistical differences ( P<0.05) . Conclusions:Stigma of female infertility patients is negatively correlated with the posttraumatic growth, which points that nurses could positively reduce the stigma of those patients so as to improve their posttraumatic growth.
6.Olaparib induced senescence under P16 or P53 dependent manner in ovarian cancer

OBJECTIVE: Poly (ADP-ribose) polymerase (PARP) is an important molecule in the early stress response of DNA damage, which is involved in DNA damage repair and cellular senescence. Olaparib, as PARP inhibitor, has an anti-tumor effect on high grade serous ovarian cancer, but its effects on cellular senescence have not been reported. This study intends to explore the role of olaparib in the regulation of senescence in ovarian cancer cells. METHODS: The effects of olaparib on the senescence of ovarian cancer cells were detected by using the senescence-associated β-galactosidase (SA-β-Gal) and senescence-associated heterochromatin aggregation (SAHF). Quantitative real-time polymerase chain reaction was used to analyze the senescence-associated secretory phenotype (SASP). Cell cycle and apoptosis were detected by flow cytometry. The effect of olaparib on tumor growth was analyzed in a nude mouse xenograft transplantation model. RESULTS: Long-term (6 days) treatment with olaparib (5 μM) significantly inhibited the growth of ovarian cancer cells, leading to arrest the cell cycle at G0/G1 phase, significant increase the number of positive SA-β-Gal stained cells and positive SAHF cells. The expression of P16 and retinoblastoma protein (p-RB) were significantly enhanced in SKOV3 cells under olaparib treated, meanwhile, the expression of P53 and p-RB were upregulated in A2780 cells. In OVCAR-3 cells, the expression of P53 was downregulated and p-RB was upregulated. Mice with SKOV3 xenograft transplantation was given olaparib (10 mg/kg/day) via abdominal cavity administration, the tumor volume was reduced (p < 0.01). CONCLUSION: Continuous low dosage administration of olaparib induced senescence under P16 or P53 dependent manner in ovarian cancer.

7.Shenshao decoction improves myocardial inflammatory injury in diabetic rats by regulation of TLR4/MyD88 pathway

Objective To investigate the improving effects of Shenshao decoction on myocardial structure and function in diabetic cardiomyopathy,and its effect on expression of TLR4/MyD88-dependent pathway signal in diabetic cardiomyopathy.Methods Diabetes mellitus was induced in 8-week-old male Wistar rats by a single intraperitoneal injection of streptozotocin.The changes of plasma myocardial enzyme (CK, LDH) and high sensitive C reactive protein (hsCRP) were measured.Cardiac function was measured by left ventricular intubation.Hematoxylin and eosin staining and electron microscopy were used to observe the changes of myocardial morphology and ultrastructure in rats.Expression of Toll-like receptor 4(TLR4), myeloid differentiation protein 88(MyD88), and nuclear factor-κB P65(NF-κB P65) were tested by immunohistochemistry.Results After 6 weeks of treatment, the left ventricular diastolic and systolic functions were obviously improved;the degrees of myocardial fibers and mitochondrial damage were obviously relieved;the content of CK, LDH and hsCRP decreased (P< 0.05), and hsCPR was positively correlated with CK and LDH (r=0.823,r=0.819,P < 0.05).The expressions of TLR4, MyD88, NF-κB P65 were significantly decreased (P< 0.05).There was no significant difference in the above mentioned indicators between the control group and control+Shenshao decoction group (P> 0.05).Conclusions Shenshao decoction can reduce the myocardial injury in diabetic cardiomyopathy and improve cardiac diastolic and systolic functions.The mechanism may be related to attenuated inflammation by TLR4/MyD88 dependent signaling pathway.
8.Perception of hemopurification clinical nurse specialists effects and barriers

Objective To explore hemopurification clinical nurse specialist's effects and barriers,in order to provide evidences to define their value,to remove their barriers,to promote their effects.Methods A qualitative study was adopted in this study.In-depth interviews were conducted on 3 charge nurses in hemopurification and 10 hemopurification clinical nurse specialists.Results The effects of hemopurification clinical nurse specialist can be concluded as effects to patients,to nursing,to medical systems and to hemopurification clinical nurse specialist themselves.Barriers of hemopurification clinical nurse specialist can be concluded as medical environments,the process of nursing developing,ambiguous role of clinical nurse specialist.Conclusions The practice of hemopurification clinical nurse specialists have played various effects,but there were some barriers in their practice.The roles of clinical nurse specialist should be clarified,and guarantees in managements and regulations should be provided for their practice.
9.Application of tandem mass spectrometry in early-stage diagnosis of growth retardation in children

Objective To develop the clinical application of tandem mass spectrometry in earlystage diagnosis of patients with growth retardation.Methods Tandem mass spectrometry was used to quantitatively detect blood amino acids and acyl carnitine in 155 cases of children with growth retardation.The tandem mass spectrometry results,clinical symptoms,and treatment tracking results were analyzed.Results Ten patients with inborn metabolic absence of amino acids and acyl carnitine were confirmed including four cases of methylmalonic acidemia,one case of propionic acidemia,one case of medium-chain acyl-coenzyme A dehydrogenase deficiency,one case of ornithine transcarbamylase deficiency,and three cases of phenylketonuria.Conclusions Tandem mass spectrometry was helpful for some patients in etiologic diagnosis and therapeutic effect assessment of cerebral developmental retardation.
10.Characterization of the ultrastructure and cytoskeleton of bone marrow-derived mesenchymal stem cells in patients with systemic lupus erythematosus

Objective To explore ultrastructure and cytoskeleton characteristics of bone marrow-deftved mesenchymal stem cells (MSCs) in patients with systemic lupus erythematosus (SLE).Methods MSCs were isolated from bone marrow of 2 SLE patients and 2 healthy controls.Their ultrastrnctures were examined by transmission electron microscope (TEM).The expression pattern of actin and vinculin was assessed by laser confocal microscopy (LCM).Results MSCs in patients with SLE presented with signs of ageing and lots of autophagosome could be found in most of the cells.F-actin was aggregated and condensed at the:border of cytoplasm.Vinculin was arranged disorderly and condensed in the cytoplasm.Conclusion The change of uhrastructure and cytoskeleton patterns of bone marrow derived mesenchymal cells of SLE patients may play an important role in the abnormal proliferation of these cells in vitro.
