1.Desalination effect on FⅧ components: a compartive study among 5 desalination methods

【Objective】 To compare the desalination effects of five desalination methods and their effects on the components for human coagulation factor Ⅷ(FⅧ), and provide reference for selection of protein desalination methods. 【Methods】 Sephadex G-25 Medium gel, Fractogel EMD BioSEC gel, ultrafiltration, room temperature dialysis and 4℃ dialysis were used to desalt human FⅧ. The desalination effect was evaluated by the removal rate of Na +, citrate ion and glycine. FⅧ protein recovery, FⅧ activity (FⅧ∶C), VWF antigen (VWF∶Ag), VWF activity(VWF∶Ac), VWF polymers and SDS-PAGE analysis before and after desalination were compared to evaluate the effect of desalination on FⅧ components. 【Results】 In terms of desalination effect, the removal rate of Na+ was the lowest in ultrafiltration desalination, while that of Fractogel EMD BioSEC gel was the highest [(97.90±0.06) % vs (99.82±0.07) %]. Except that there was no statistical significance between Sephadex G-25 Medium gel desalination and Fractogel EMD BioSEC gel desalination (P=0.90), the removal rates of the other four methods were statistically significant. The removal rate of glycine was the lowest in ultrafiltration desalination, wihle that of Fractogel EMD BioSEC gel desalination was the highest [(95.78±0.42) % vs (99.81±0.08) %]. Significant difference in glycine removal was noticed in ultrafiltration desalination, but not among the other four desalination methods. There was no significant difference in the removal rate of citrate ions among the five methods (P=0.85). For the effect of FⅧ components, FⅧ∶C, VWF∶Ag, VWF∶Ac and protein recovery rates of ultrafiltration desalination were the highest, with (18.34±1.99) IU/mL, (11.81±0.33) IU/mL, (12.26±0.58) IU/mL and (97.13±1.37) %, respectively. There was no significant change in VWF∶Ac/VWF∶Ag before and after desalination by the five methods. SDS-PAGE and VWF polymer analysis showed that different desalination methods had no significant impact on protein composition. 【Conclusion】 Although different desalination methods had no significant effect on the composition of FⅧ protein, the desalination effect was different. Moreover, different desalination methods had significant effects on protein recovery, FⅧ∶C, VWF∶Ag and VWF∶Ac. 2.Effects of propofol on blood pressure and heart rate in hypertensive patients with hyperacute uncomplicated type B aortic dissection

Objective:To investigate the clinical efficacy of propofol on blood pressure (BP) and heart rate (HR) in hypertensive patients with hyperacute uncomplicated type B aortic dissection (HU-TBAD).Methods:This study was a single-center, double-blind, randomized controlled trial. Totally 96 consecutive hypertensive patients with HU-TBAD admitted to the Department of Emergency in our hospital from July 2020 to March 2023 were enrolled and randomly divided into control and treatment groups ( n=48/group) by envelope method. All patients were treated with nicardipine, remifentanil, esmolol, and basic treatments. Besides, patients in the treatment group were injected with 0.5 mg/kg propofol, followed by 1.0 mg/(kg·h) with continuous micro-pump intravenous infusion; the RASS score was evaluated every 15 minutes to adjust the dosage of propofol to maintain the RASS score at -2-0 points, while the control group was given an equal volume of normal saline. Systolic blood pressure (SBP), diastolic blood pressure (DBP), and HR were analyzed at different time points (TPs). Related indexes between the two groups were compared at 0 (T 0 min) and 60 (T 60 min) minutes. Standard-reaching rate of related indexes, levels of mean nicardipine dose (mND) and urine volume, and adverse effect rates (AERs) were also compared between the two groups. All patients were admitted to the cardiovascular surgical ward to receive proper management and follow-up for 21 d after discharge from the Department of Emergency. Mann-Whitney U test, t-test, χ2 test, or Fisher's test were used to compare the data between the two groups, while the data of two groups at different TPs were compared by using repetitive measurement deviation analysis. Results:No significant differences were observed in general clinical data between the two groups (all P>0.05). There were significant differences in SBP, DBP, and HR levels in different TPs, groups, and interaction of Time and Group (Time×Group) (all P<0.05). For comparison of related indexes at T 0 min and T 60 min, there were statistical differences in oxygenation index levels at different TPs ( P<0.01), but not in different groups and Time×Group (all P>0.05); significant differences in levels of partial pressure of carbon dioxide, respiratory rate, and lactate were observed in different TPs and Time×Group (all P<0.01), but not observed in different groups (all P>0.05). There were significant differences in NRS score in different TPs, groups, and Time×Group (all P<0.05), while not in cardiac troponin I levels in different TPs, groups, and Time×Group (all P>0.05). Compared with the control group, the standard-reaching rate of SBP, DBP, HR, sedation, and analgesia as well as the level of RASS score reduction in the treatment group were significantly increased [54.17% vs. 77.08%, 56.25% vs. 81.25%, 50.00% vs. 72.92%, 47.92% vs. 72.92%, 43.75% vs. 83.33%, 1.00 (0, 2.00) vs. 2.00 (1.00, 3.00), respectively, all P<0.05], while the level of mND was significantly decreased [μg·kg -1·min -1, 2.50 (2.00, 2.50) vs. 2.00 (1.50, 2.50), P<0.01]; there were no statistical differences in both urine volume levels and AERs between the two groups (all P>0.05). Following up for 21 d, the rate of aortic dissection deterioration and ICU admission was significantly lower in the treatment group than in the control group (19.57% vs. 4.26%, 23.91% vs.6.38%, respectively, all P<0.05). Conclusions:Propofol enhances the analgesic effect of remifentanil, synergistically reduces SBP, DBP, and HR, and improves clinical prognosis in hypertensive patients with HU-TBAD.
3.Association between sexual orientation and high risk sexual behaviors among college students in Hubei Province

Objective:
To examine the association between sexual orientation and high risk sexual behaviors and its sex differences among college students, so as to provide a theoretical basis for intervention and controlling of high risk sexual behaviors of the college students.
Methods:
From November 2020 to May 2021, 1 920 college students among four universities in Hubei Province were recruited by multi stage stratified random cluster sampling, and their sexual orientation and the frequency of high risk sexual behaviors were investigated by a self designed questionnaire. And then analyzed the association between sexual orientation and high risk sexual behaviors and its sex differences by using Chi square test and Logistic regression analysis method.
Results:
The prevalence of sexual behaviors was 18.2%, the prevalence of high risk sexual behaviors was 7.2%, and the prevalence of high risk sexual behaviors was higher among non heterosexuals(13.5%) than among heterosexuals (6.7%),the prevalence of high risk sexual behaviors was higher among males (9.2%) than among females(5.3%),and the differences were statistically significant( χ 2=9.40, 10.77, P <0.01). Logistic regression analysis showed that the association between sexual orientation and high risk sexual behaviors was not statistically significant( OR=1.52, 95%CI=0.83-2.80, P >0.05). However, stratified analysis by sex revealed that among male students, the risk of high risk sexual behaviors was 3.57 times higher among non heterosexuals than among heterosexuals( OR= 3.57 , 95%CI=1.52-8.41, P <0.01), while among female students, the association between sexual orientation and high risk sexual behaviors was not statistically significant( OR=0.48, 95%CI=0.15-1.57, P >0.05).
Conclusion
There are sex differences in the relationship between sexual orientation and high risk sexual behaviors among college students. Male non-heterosexuals are at higher risk for high risk sexual behaviors than male heterosexuals. Therefore, prevention and interventions for high risk sexual behaviors among college students should focus on gay and bisexual men so as to reduce the occurance rate of their high risk sexual behaviors.
4.Analgesic effect of intravenous anesthesia induction combined anterior lumbar quadratus block and related hemodynamic changes in patients undergoing partial nephrectomy via retroperitoneal approach

Objective:To investigate the analgesic effect of intravenous anesthesia induction combined with anterior quadratus lumborum block (AQLB)and related hemodynamic changes in patients undergoing laparoscopic retroperitoneal partial nephrectomy (RPN).Methods:A total of 116 patients undergoing elective laparoscopic partial nephrectomy for renal tumors in Jinhua Central Hospital from August 2021 to February 2022 were randomly divided into two groups with 58 cases in each group. Patients in control group received intravenous anesthesia , while those in study group received intravenous anesthesia induction with AQLB. The analgesic effect was evaluated at 1, 6, 12, 24, and 48 h after the operation. The hemodynamics were monitored at the time of entering the operating room (T 0), 3 min after induction of anesthesia (T 1), at the beginning of the operation (T 2), after the operation (T 3), and leaving the operating room (T 4). Microcirculation was assessed at 1, 6, 12, 24, and 48 h after operation. Cognitive function was assessed 30min before anesthesia, 6 h, 24 h, and 72 h after operation. Results:At 1, 6, 12, 24 and 48 h after operation, the visual analogue scale (VAS) of the resting (quiet state) pain in the study group were 3.2±1.2, 2.6±0.3,2.0±0.4, 1.5±0.4 and 0.8±0.2, which were significantly lower than those in control group (4.0±1.7, 3.4±0.7, 2.9±0.5, 1.7±0.5 and 1.2±0.3) ( t=2.93, P=0.004; t=8.00, P<0.001; t=10.07, P<0.001; t=2.38, P=0.019; t=8.45, P<0.001). There was no significant difference in heart rate and mean arterial pressure (MAP) at T 0 between two groups ; no significant difference in the heart rate at T 1, T 2, T 3 and T 4. There were significant differences in MAP levels at T 1, T 2, T 3 and T 4 between study group [(80.0±8.0)mmHg (1 mmHg=0.133 kPa), (84.4±8.4)mmHg, (80.4±5.7)mmHg, (86.4±4.7)mmHg and control group (77.1±7.5)mmHg, (88.0±8.6)mmHg, (83.0±7.7)mmHg, (92.2±6.2) mmHg; t=2.01, P=0.046; t=2.28, P=0.024; t=2.07, P=0.041; t=5.68, P<0.001]. At 6, 12, 24 and 48 h after operation, the morphological scores of tube loops in the study group were 1.0±0.2, 0.8±0.2, 0.7±0.1 and 0.7±0.1, which were lower than those in the control group (1.1 ±0.2, 0.9±0.2, 0.8±0.2 and 0.8±0.1; t=2.69, P=0.008; t=2.69, P=0.008; t=3.41, P=0.001; t=5.39 , P < 0.001). The blood flow status scores of the study group were 1.1±0.2, 0.9±0.2, 0.8±0.2 and 0.6±0.1, which were lower than those of the control group (1.2±0.2, 1.0±0.2, 0.9±0.2 and 0.7±0.1; t=2.69, P=0.008; t=2.69, P=0.008; t=2.69, P=0.008; t=5.39, P<0.001). The cognitive function scores of the study group and the control group were 24.4±1.0, 27.1±0.9 and 23.5±0.9, 26.7±0.9 at 6 h and 24 h after operation ( t=5.10, P<0.001; t=2.39, P=0.018); while there were no significant at 72 h after operation between two groups (28.2±0.9 vs. 28.1±0.8, t=0.63, P=0.529). Conclusion:Intravenous anesthesia induction combined with anterior quadratus lumborum block has a good analgesic effect in patients undergoing RPN, with stable hemodynamics and microcirculation, and not affecting cognitive function of patients.
5.Effect of a dietary and exercise intervention on children s cognition and behaviors:the mediating role of family support

Objective:
To evaluate the effect of a dietary and exercise intervention on cognition and behaviors among primary school students, and to further explore the mediating role of family support, in order to provide scientific evidence for future effective intervention strategies.
Methods:
A dietary and exercise intervention program for childhood obesity prevention was carried out in 24 primary schools in Beijing, Changzhi and Urumqi from 2018 to 2019, and 1 392 children in grade four as well as their parents were included. Family support and children s cognition and behaviors were collected through questionnaire. To carry out diet and exercise behavior intervention on the three levels of school, family and individual in the intervention group,and children s dietary and exercise knowledge, eating habits, physical activities, as well as sedentary and screen behaviors were evaluated. The linear mixed model was used to analyze the effect of intervention on children s cognition and behaviors, and the mediation model was used to explore the role of family support.
Results:
The proportion of children with higher score of dietary and exercise knowledge ( OR= 2.34 , 95%CI =1.71-3.21), eating habits ( OR=2.58, 95%CI =1.75-3.82), and sedentary and screen behaviors ( OR=1.91, 95%CI =1.35-2.68) increased in the intervention group after one year intervention ( P <0.01), compared with the control group, respectively. The intervention also increased the proportion of children s family support in the intervention group compared with the control group ( OR=3.45, 95%CI =2.19-5.45), and the support from children s fathers ( OR=2.70, 95%CI =1.68-4.35), mothers ( OR=3.71, 95%CI =2.28-6.04), paternal grandmothers ( OR=1.65, 95%CI =1.00-2.70), and maternal grandmothers ( OR= 2.14, 95%CI =2.12-2.16) increased significantly ( P <0.05). The mediation analysis showed that family support played a mediating role in association between comprehensive intervention and children s eating habits as well as sedentary and screen behaviors.
Conclusion
The dietary and exercise intervention effectively promoted children s cognition and behaviors, and family support played an important mediating role.
6.Research progress on non-drug intervention for patients with perimenopausal depression

This article gives a review on domestic and foreign researches on the non-drug intervention for patients with perimenopausal depression published from 2012 to 2022, so as to provide a reference for clinical treatment and nursing. Currently, pharmacotherapy is the mainstay of treatment for perimenopausal depression, while many existing problems remain to be solved such as the variation in treatment effect and the difficulty in maintaining emotional stability. however,high safety and relatively low cost are the characteristics of non-drug intervention, so this article expounds on the non-drug intervention measures for perimenopausal depression, such as music therapy, dietary therapy, exercise therapy, acupuncture therapy and repetitive transcranial magnetic stimulation (rTMS).

Polyhydroxyalkanoates (PHAs) are polymers obtained by esterification of hydroxy fatty acid monomers. Due to similar mechanical characteristics of traditional petroleum-based plastics, 100% biodegradability and biocompatibility, PHAs are considered to be one of the most potential green materials. However, the application and promotion of PHAs as a green and environmentally friendly material are difficult because of the high production costs. This article focuses on the current methods to reduce production cost of PHAs effectively, such as cell morphology regulation, metabolic pathway construction, economic carbon source utilization and open fermentation technology development. Despite most research results are still limited in laboratory, the research methods and directions provide theoretical guidance for the industrial production of economic PHAs.

8.Thromboprophylaxis of 31 patients with COVID-2019

Objective:To investigate thromboprophylaxis of patients with COVID-19.Methods:Clinical data of 31 patients with COVID-19 in Zhongnan Hospital of Wuhan University was analyzed retrospectively, including basic information, clinical characteristics, examination results, therapy and Padua scores. 5G remote ultrasound diagnosis system was used in the ultrasonography of lower limb vein, inferior vena cava and the heart. According to the patient′s status, the thromboprophylaxis was established, and the incidence of venous thromboembolism (VTE) was observed.Results:There were 20 cases of general types and 11cases of severe types. All patients underwent dynamic assessment and thromboprophylaxis. Basic prevention was applied in 28 cases and mechanical prevention was used in 3 cases. There was no VTE event in all patients during hospitalization.Conclusions:Prevention and monitoring of VTE is an important part of the treatment of patients with COVID-19, which can effectively reduce the incidence of lower extremity deep vein thrombosis and fatal pulmonary embolism in isolation ward.
9.Efficiency comparison of Kwak and ACR ( 2017 ) Thyroid Imaging Reporting and Data System ( TI‐RADS) classification :a polycentric retrospective study

Objective To evaluate the diagnostic efficacy of Kwak and ACR( 2017 ) thyroid imaging reporting and data systems ( T I‐RADS ) for thyroid nodules . Methods Cases of thyroid nodule who underwent surgery from January 2015 to M arch 2018 in 15 hospitals in Sichuan province were collected and the ultrasonographic features of thyroid nodules were retrospectively analyzed by trained senior ultrasound physicians using Kwak and ACR T I‐RADS classification methods . Totally ,12 712 thyroid nodules were observed ,7 023 thyroid nodules in 7 023 cases with complete ultrasound and surgical and pathological data were eventually enrolled in the study . T hyroid nodules with solid ,hypoechoic or very hypoechoic ,tall/wide ratio ≥ 1 , margin ill‐defined and microcalcification were classified as malignant signs of ultrasound . M alignant percentage was calculated and diagnostic tests were performed . Results ① T here was a statistical difference between the benign and malignant nodules in the two types of T I‐RADS classification ( P<0 .01) . ② T he area under ROC curve of Kwak and ACR in the diagnosis of malignant nodules were 0 .89 and 0 .84 ,respectively . T he Youden index of Kwak and ACR were 0 .66 and 0 .57 ,respectively . ③Taking Kwak T I4B and ACR T R4 as critical points for malignancy ,the sensitivity ,specificity ,positive predictive value and negative predictive value of Kwak T I 4B were 75 .0% ,90 .9% ,83 .2% ,and 85 .9% , respectively . T he accuracy of Kwak T I4B was 84 .9% ; T he sensitivity ,specificity ,positive predictive value and negative predictive value of ACR T R4 were 88 .2% ,68 .9% ,62 .9% ,and 90 .8% ,respectively . T he accuracy of ACR T R4 was 76 .2% . T he Kappa value of Kwak TI4B and ACR T R4 was 0 .52 . T he χ2 value of Kwak T I4B and ACR T R4 was 2 174 .6 ( P < 0 .01 ) . Conclusions T he diagnostic values of two T I‐RADS classification methods for thyroid malignant nodules are high . T he overall efficiency of Kwak T I‐RADS classification method is better than that of ACR TI‐RADS classification method .
10.Evalution of the combination of dexmedetomidine and fentanyl in sedation during awake nasotracheal intu-bation

Objective To evalute the combination of dexmedetomidine and fentanyl in sedation during awake nasotracheal fiberoptic intubation. Methods One hundred and twenty ASAⅠ or Ⅱ patients scheduled to receive general anesthesia were randomly divided into 3 groups (n = 40 in each group). Patients in group L received an infusion of 1 μg/kg dexmedetomidine,patients in group H received an infusion of 2 μg/kg dexmedeto-midine ,and patients in group DF received an infusion of 1 μ g/kg dexmedetomidine added to 1 μ g/kg fentanyl. Nasotracheal intubation was performed after complete topical anesthesia. HR and MAP were recorded before anes-thesia(baseline,T0),before intubation(T1)and immediately after intubation(T2),respectively. The intubation score(vocal cord movement,coughing and limb movement),fiberoptic intubation score,nasotracheal intubation score and airway obstraction score were assessed in all aptients. On the first post-operative day,recall,adverse events and satisfaction score were also assessed. Results HR and MAP at T1 in three groups were significantly lower than those at T0(P < 0.05,respectively ). HR and MAP at T2 in group L were significantly higher than those in group H and DF(P<0.05,respectively). More incidence of vocal cord closed,severe cough,severe limb movement,heavy grimacing,defensive movement of head and hands after nasotracheal intubation were observed in group L than those in the other two groups. The incidence of airway obstraction and bradycardia in group H were higher than those in group L and DF. Patients in group L had lower postoperative satisfaction scores. Conclusion Adding 1 μg/kg fentanyl to 1 μg/kg dexmedetomidine is a good method for awake nasotracheal fiberoptic intuba-tion,which can prevent the risk of airway obstruction associated with the increase of dexmedetomidine dose,with the achievement of the same favorable sedation.
