		                        			Notoginseng Radix et Rhizoma as a traditional Chinese herbal medicine has now been recognized and paid attention to by the pharmaceutical community.Modern phytochemical studies have shown that Panax notoginseng saponin is the main chemical compo-nent of Notoginseng Radix et Rhizoma.Modern pharmacological studies and clinical applications have revealed that it has anti-cancer,antioxidant and cardiovascular disease effects.In this study,we reviewed the research progress of the main chemical components and pharmacological effects of Notoginseng Radix et Rhizoma,with the aim of providing assistance for the clinical application and later stud-ies of Notoginseng Radix et Rhizoma.
		                        		
		                        		
		                        		
		                        	
2.Effect of Ruyi Heibai Power (如意黑白散) on PD-1/PD-L1 Signaling Pathway in Skin Lesion Tissue of Vitiligo Model Mice

		                        			ObjectiveTo investigate the possible mechanism of Ruyi Heibai Power (如意黑白散, RHP) in the treatment of vitiligo. MethodsTwenty-four C57BL/6 mice were randomly divided into blank group, model group, high-dose and low-dose RHP groups, with 6 mice in each group. Model group, high- and low-dose RHP groups were all applied hydroquinone to establish vitiligo animal model. After modeling, High- and low-dose RHP groups were given 7.02 g/kg and 2.34 g/kg of RHP by gavage, respectively, while the blank group and model group were intragastrically given 10ml/kg of normal saline, once a day for 36 days. After administration, the skin lesions were observed with naked eye, and HE staining was used to observe the melanin content of the skin lesions. Immunohistochemistry was used to determine the expression of CD3+ and CD8+ T cells in skin tissue. Immunofluorescence staining was used to measure the expression of programmed death receptor 1 (PD-1) in the skin lesion tissue. RT-PCR was used to detect programmed cell death ligand 1 (PD-L1) mRNA expression. ELISA was used to detect serum superoxide dismutase (SOD), tumor necrosis factor (TNF-α), and tyrosinase (TYR) levels. ResultsCompared to those in the blank group, the skin of the mice in the model group was pale, and the melanin content was significantly reduced under the microscope after HE staining; the rate of excellent and good skin lesions decreased, and the melanin granules in the cells around the epidermis and hair follicles decreased significantly; the expression of CD3+ and CD8+ T cells in skin tissue increased significantly, and the expressions of PD-L1 mRNA and PD-1 decreased; the content of TYR decreased, while the content of SOD and TNF-α increased (P<0.05). Compared to those in the model group, the skin color of high- and low-dose RHP groups were deepened, and the melanin content increased; the rate of excellent and good skin lesions increased, as well as the melanin granules in the spinous cell layer, basal cells and hair follicles; the expression of CD3+ and CD8+ T cells in the skin lesions decreased, while PD-L1 mRNA and PD-1 expression increased; the content of TYR increased, while the content of SOD and TNF-α decreased (P<0.05). Compared to the low-dose RHP group, the high-dose group had a larger pigment recovery area in the modeling area, an increased rate of excellent and good skin lesions, an increase in spinous cell layer, basal cells, and hair follicle melanin granules, a decrease in CD3+ and CD8+ T cells expression, an increase in the expression of PD-L1 mRNA and PD-1, an elevated TYR content, and decreased SOD and TNF-α contents (P<0.05). ConclusionRHP can increase skin melanin content of vitiligo mice.The mechanism of action may be related to activating the PD-1/PD-L1 signaling pathway, and then reducing the destruction of melanocytes by T cell-mediated autoimmunity. 
		                        		
		                        		
		                        		
		                        	
3.Developing curriculum of speciality of education and rehabilitation using WHO rehabilitation competency framework

		                        			ObjectiveTo construct rehabilitation competency framework (RCF) of the speciality of education and rehabilitation (SER-RCF) based on World Health Organization RCF, to innovate the curriculum system. MethodsThe competency for undergraduate level professionals in the speciality of education and rehabilitation were analyzed to develop curriculum using RCF theory and methodology. ResultsTalent training was a kind of competency-based and outcome-based education. SER-RCF integrated the competencies and activities in the areas of professionalism, practice, learning and development, management and leadership, and research required for professional work in education and rehabilitation around core values and beliefs. It also designed the training objectives of the speciality of education and rehabilitation: to be ethical and correct, to be physically and mentally healthy, to have a solid foundation, to be sustainable, to be co-operative and enjoyable, and to be reflective and enquiring. SER-RCF played a central role in developing the curriculum system of rehabilitation, education and psychology, and practice. ConclusionReshaping the undergraduate level curriculum system of education and rehabilitation based on RCF is not only the implementation of the construction concept of new medicine and new liberal arts, but also can effectively connect with the international standard of rehabilitation higher education. 
		                        		
		                        		
		                        		
		                        	
4.Perceived quality of patient-centered practice in surgical services designed for elderly orthopedic patients

		                        			Objective:To investigate the effects on perceived quality of a management assessment system for orthopedic surgery in elderly patients.Methods:Based on the concept of lean management and process nodes in systems, an information management system was established to evaluate the perceived quality of surgical service for elderly orthopedic patients.Four hundred and two elderly orthopedic patients receiving orthopedic surgery after the implementation of the management system were included in the observation group, while 402 elderly patients undergoing orthopedic surgery before the implementation of management system formed the control group.Results:The incidence of hypothermia in patients admitted to the resuscitation room after surgery was lower in the observation group than in the control group(1.7% vs.4.5%, χ2=5.136、 P=0.023). The incidence of shivering during resuscitation was lower and the satisfaction rate of surgical services was higher in the observation group than in the control group(0.7% vs.3.2%) χ2=5.279、 P=0.022, 96.55(95.58, 97.28)分, vs.87.9(83.2, 89.95)分, Z=-4.16, P=0.00). 5.A cross-sectional study on the characteristics of cognitive impairment in middle-aged and elderly patients undergoing maintenance hemodialysis

		                        			Objective:To investigate the clinical features and associated influencing factors of cognitive impairment in middle-aged and elderly Chinese adult patients undergoing maintenance hemodialysis (HD).Methods:A cross-sectional study was conducted among HD patients from 11 centers in Beijing city from April 2017 to June 2017. A neuropsychological battery covering domains of attention/processing speed, executive function, memory, language, and visuospatial function was applied in cognitive function assessment. Patients were classified as normal cognitive function group and cognitive impairment group according to the fifth version of the diagnostic and statistical manual of mental disorders criteria (DSM-V). Multivariate binary logistic regression was used to analyze the independent influencing factors of cognitive impairment. Results:A total of 613 HD patients were included in the study, and the prevalence of cognitive impairment was 80.91% (496/613). Attention impairment (81.05%) and memory impairment (63.51%) were the most common impaired domains, and 79.23% was concomitant impairment across two or more cognitive domains among those with cognitive impairment. Compared with the patients in the normal cognitive function group, the patients in the cognitive impairment group had senior age, longer dialysis vintage, higher proportion of diabetes, hypertension, and stroke, higher level of serum intact parathyroid hormone (iPTH), lower education level, and lower urea clearance index (Kt/V) (all P<0.05). Factors were independently associated with cognitive impairment including increasing age ( OR=1.110, 95% CI 1.072-1.150, P<0.001), education time>12 years (with education time<6 years as reference, OR=0.323, 95% CI 0.115-0.909, P=0.032), history of diabetes ( OR=2.151, 95% CI 1.272-3.636, P=0.004), history of stroke ( OR=2.546, 95% CI 1.244-5.210, P=0.011), increased dialysis vintage ( OR=1.016, 95% CI 1.010-1.022, P<0.001), reduced Kt/V( OR=0.008, 95% CI 0.002-0.035, P<0.001), and increased iPTH level ( OR=1.002, 95% CI 1.002-1.003, P=0.012). Conclusions:The prevalence of cognitive impairment in middle-aged and elderly adult Chinese patients undergoing HD is high. Memory and attention are the most commonly impaired domains. Increasing age, low education level, history of diabetes and stroke, increased dialysis vintage, reduced Kt/V and increased serum iPTH are the independent influencing factors associated with cognitive impairment.
		                        		
		                        		
		                        		
		                        	
6.A prospective cohort study on the association of cognitive impairment and all-cause mortality in the middle and elderly adult patients undergoing maintenance hemodialysis

		                        			Objective:To investigate the association between cognitive impairment and all-cause mortality in middle and elderly adult patients undergoing maintenance hemodialysis (HD).Methods:A prospective cohort study was conducted. Patients from 11 HD centers in Beijing between April and June 2017 were enrolled. Baseline data were collected, and a series of neuropsychological batteries covered 5 domains of cognitive function were applied for the assessment of cognitive function. The patients were then classified as normal and cognitive impairment groups according to the fifth version of the Diagnostic and Statistical Manual of Mental Disorders criteria (DSM-V) and followed-up until June 2018. The clinical characteristics of the two groups of patients were compared. Kaplan-Meier survival analysis was used to compare the difference in the cumulative survival rate between the two groups. Multivariate Cox regression model was used to analyze the independent influencing factors of all-cause mortality, to determine the relationship between cognitive impairment and different cognitive domain impairments and all-cause death.Results:A total of 613 patients were enrolled, of which 496(80.91%) patients had cognitive impairment. Compared with the normal cognitive function group, the patients in the cognitive impairment group tended to be older, longer dialysis vintage, a higher proportion of diabetes, hypertension, and stroke, increased serum iPTH level, and lower education level and urea clearance index (Kt/V) (all P<0.05). After (49.53±8.42) weeks of follow-up, Kaplan-Meier survival analysis showed that the cumulative survival rate of cognitive impairment group was significantly lower than that of cognitive normal group (Log-rank χ2=8.610, P=0.003). Multivariate Cox regression analysis showed that history of diabetes ( HR=2.742, 95% CI 1.598-4.723, P<0.001), coronary heart disease ( HR=1.906, 95% CI 1.169-3.108, P=0.010), dialysis vintage (every increase of 1 month, HR=1.007, 95% CI 1.003-1.011, P=0.001), serum level of albumin (every increase of 1 g/L, HR=0.859, 95% CI 0.809-0.912, P<0.001), cognitive impairment ( HR=2.719, 95% CI 1.088-6.194, P=0.032) were independently associated with all-cause mortality. Multivariate Cox regression analysis on different cognitive domains also indicated that memory impairment ( HR=2.571, 95% CI 1.442-4.584, P<0.001), executive function impairment ( HR=3.311, 95% CI 1.843-5.949, P=0.001) and three, four, five domains combined impairment ( HR=5.746, 95% CI 1.880-17.565, P=0.002; HR=12.420, 95% CI 3.690-41.802, P<0.001; HR=13.478, 95% CI 3.381-53.728, P<0.001) were independently related to all-cause mortality. Conclusions:Cognitive impairment is an independent risk factor of all-cause mortality in middle and elderly adult patients undergoing maintenance hemodialysis, and the risk is significantly increased in patients with the impairment of the domains of memory, executive function, or in the combination of three to five cognitive domains.
		                        		
		                        		
		                        		
		                        	
7.Pharmacoeconomic Evaluation of Magnesium Isoglycyrrhizinate Preventing Liver Damage Induced by Chemo- therapeutic Drugs for Gastric Cancer

		                        			OBJECTIVE:To ev aluate pha rmacoeconomics of magnesium isoglycyrrhizinate preventing liver damage induced by chemotherapeutic drugs for gastric cancer ,and to provide reference for rational use of liver-protecting drugs. METHODS :Totally 200 inpatient medical records were collected from our hospital retrospectively during Jan. 2018-Feb. 2020,and then divided into group A (prophylactic use of magnesium isoglycolate ,50 cases),group B (prophylactic use of magnesium isoglycolate combined with TCM prescriptions ,50 cases),group C (prophylactic use of polyene phosphatidylcholine ,50 cases) and group D (non-prophylactic use of liver-protection drugs ,50 cases). The effects (total response rate )of four plans preventing liver damage were evaluated. Pharmacoeconomic evaluation was analyzed by cost-minimization analysis and cost-effectiveness method , sensitivity analysis was carried out at the same time . RESULTS :Total response rates of group A ,B,C and D were 94.00%, 96.00%,82.00% and 72.00%. The total response rates of group A and B had no statistical significance (P>0.05),but were significantly higher than those of group C and D (P<0.05);total response rate of group C was significantly higher than that of group D (P<0.05). The costs of groups A ,B,C and D were 1 936.70,2 086.96,1 800.91,2 975.42 yuan. The cost-minimization analysis was used to compare the therapeutic plan of group A and B ,and plan of group A was more economical. The cost-effectiveness method was used to compare therapeutic plan between group C and D ,and the plan of group C was more economical. The cost-effectiveness method was used to compare therapeutic plan between group A and C ,and the cost-effectiveness ratio of 2 groups were 2 060.32 and 2 196.2 3,incremental cost-effectiveness ratio was 1 131.58,and the plan of group A was more economical. Above conclusion were supported by the results of sensitivity analysis. CONCLUSIONS :The cost-effectiveness of magnesium isoglycyrrhizinate preventing liver damage induced by chemotherapeutic drugs for gastric cancer is better than magnesium isoglycyrrhizinate combined with TCM prescription , polyene phosphatidylcholine and non-prophylactic use of liver-protecting drugs ,showing economical advantage.
		                        		
		                        		
		                        		
		                        	
8.Clinical diagnostic value of circulating tumor cells and circulating cell-free DNA combined detection in peripheral blood for breast cancer

		                        			Objective To investigate the clinical diagnostic value of circulating tumor cells (CTCs)and circulating cell-free DNA (cfDNA) in peripheral blood samples in breast cancer. Methods From July 2017 to April 2018, 47 patients with BMC (7 in stage Ⅱ, 19 in stage Ⅲ and 21 in stage Ⅳ), 24 patients with benign breast diseases and 28 healthy people were selected. After collecting peripheral blood samples, serum and blood cells were separated. The size-based high-throughput microfluidic chip was used to capture CTCs. The real-time fluorescent quantitative PCR based on Alu sequence was used to detect the length of cfDNA(247 bp, 115 bp)in the serum, and the ratio of amplified products of long and short fragments was used as the index of DNA integrity. The Mann-Whitney U test or Kruskal-Wallis H test was used to compare the differences between the groups and analyze the relationship between CTCs and cfDNA and clinical parameters of breast cancer. The ROC curve was drawn and the area under the curve (AUC) was used to evaluate the feasibility of blood cell CTCs and plasma cfDNA detection as diagnostic criteria. Results The CTCs and cfDNA of 47 BMC patients were analyzed. The CTCs and cfDNA integrity index (Alu 247/115) of BMC patients were significantly higher than those of physical examination patients[(13.98± 12.36)cells / ml vs (1.14 ± 1.35) cells / ml; 0.7687 ± 0.3868 vs 0.5094 ± 0.2456], and the difference was statistically significant(the U value was 126.5,359.0;P<0.001), the area under ROC curve of CTCs was 0.885 (95%CI: 0.805-0.965), cut-off value was 7.68/ml, sensitivity was 80.4%, specificity was 96.4%. The area under ROC curve of Alu 247/115 was 0.727(95%CI: 0.608-0.847), cut-off value was 0.431, sensitivity was 71.7%, specificity was 71.4%. The AUC of CTCs and Alu 247/115 was 0.919 (95%CI 0.854-0.984), which was higher than the single test of each indicator. Conclusions CTCs and cfDNA may be the potential biological indicators for breast cancer diagnosis. 		                        			Exploratory experiment is the key to the experimental physiology,which is universally applicable training of innovative talents.More importantly,it is an important way to cultivate innovative talents of medicine.On the basis of the original multi-subject evaluation system,we have formed a comprehensive evaluation system through refining the scoring item for teachers and students,enriching the forms of student evaluation,and building the student self-assessment.This comprehensive evaluation system is set to promote the development of experimental physiology,especially the exploratory experiments.The practice of this evaluation system effectively improve the objectivity of the evaluation,provide a basis for reform of exploratory experiment,and then promote the training of innovative talents.
		                        		
		                        		
		                        		
		                        	
10.Correlation analysis between psychological capital, career plateau and turnover intention in nurses in a Class Ⅲ Grade A hospital

		                        			Objective To investigate the current status of psychological capital, career plateau and turnover intention in ICU nurses and to analyze the influence of psychological capital and career on turnover intention. Methods A total of 233 ICU nurses from the 2nd Affiliated Hospital of Wenzhou Medical University who met the criteria were selected by convenient sampling from February to April 2017. The nurses were investigated with the Psychological Capital Questionnaire-Chinese (PCQC), Career Plateau Questionnaire (CPQ) and Turnover Intention Scule (TIS). Totally 233 questionnaires were handed out and 224 valid questionnaires were collected. Results The total score of psychological capital in the 224 ICU nurses was (88.53±13.16), and the highest score was found in the dimension of self-efficacy, while the lowest score was in the dimension of hope; the total average score of career plateau was (3.83±0.92); and the score of turnover intention was (16.82±5.01). According to the hierarchical stepwise regression analysis, contractual employment, hope, hierarchical plateau and content plateau were the main influencing factors of ICU nurses' turnover intention, which could account for 39.8% of the total variation. Conclusions ICU nurses show a strong turnover intention, and contractual employment, hope, hierarchical plateau and content plateau may affect their turnover intention. ICU nursing managers should formulate proper and effective improvement strategies for psychological capital and alleviation strategies for career plateau based on the current status of psychological capital and career plateau in nurses and take proper measures to enhance their enthusiasm and job satisfaction of contractually employed nurses, and reinforce the stability of ICU nursing team.
