1.Cross-Sectional and Longitudinal Associations between Forearm Bone Mineral Density and Anthropometry in Adult Japanese Men and Women

		                        			 Background:
		                        			No consensus exists regarding which anthropometric measurements are related to bone mineral density (BMD), and this relationship may vary according to sex and age. A large Japanese cohort was analyzed to provide an understanding of the relationship between BMD and anthropometry while adjusting for known confounding factors.  
		                        		
		                        			Methods:
		                        			Our cohort included 10,827 participants who underwent multiple medical checkups including distal forearm BMD scans. Participants were stratified into four groups according to age (≥50 years or <50 years) and sex. The BMD values were adjusted for confounding factors, after which single and partial correlation analyses were performed. The prevalence of osteopenia was plotted for each weight index (weight or body mass index [BMI]) class.  
		                        		
		                        			Results:
		                        			Cross-sectional studies revealed that weight was more favorably correlated than BMI in the older group (R=0.278 and 0.212 in men and R=0.304 and 0.220 in women, respectively), whereas weight and BMI were weakly correlated in the younger age groups. The prevalence of osteopenia exhibited a negative linear relationship with weight among older women ≥50 years of age, and an accelerated increase was observed with decreasing weight in older men weighing <50 kg and younger women weighing <60 kg. When weight was replaced with BMI, the prevalence was low in most subgroups classified by weight.  
		                        		
		                        			Conclusions
		                        			Weight, rather than BMI, was the most important indicator of osteopenia but it might not be predictive of future bone loss. 
		                        		
		                        		
		                        		
		                        	
2.The Details of Inpatient Cancer Rehabilitation Provided by Designated Cancer Hospitals in Japan

		                        			Objective: This study aimed to clarify the details of inpatient cancer rehabilitation interventions provided by designated cancer hospitals in Japan. Methods: This questionnaire-based survey asked specialists regarding the outline of their facilities’ inpatient cancer rehabilitation, Dietz classification, disease, and intervention details. Results: Restorative interventions were the most common, and the most common cancer was lung cancer followed by colorectal cancer; hematologic malignancy; gastric cancer; and liver, gallbladder, and pancreatic cancer. Intervention proportions for colorectal and gastric cancer were significantly higher in general hospitals than in university hospitals and cancer centers; in contrast, those for hematological malignancy were significantly higher in university hospitals than in general hospitals. For bone and soft tissue sarcomas, intervention proportions in cancer centers were significantly higher than those in university and general hospitals; and for oral, pharyngeal, and laryngeal cancers, they were significantly higher in university hospitals and cancer centers than in general hospitals. The most common intervention was walking training, followed by resistance training, basic motor training, activities of daily living training, and respiratory rehabilitation. Respiratory rehabilitation was performed significantly more frequently in university and general hospitals than in cancer centers.Conclusion: The diseases had differed according to the characteristics of the facilities, and the interventions were considered accordingly. In future, it will be necessary to verify the effectiveness of inpatient cancer rehabilitation according to facility characteristics and to disseminate information on inpatient cancer rehabilitation.
		                        		
		                        		
		                        		
		                        	
3.Chronic lymphocytic leukemia and concurrent seminoma in the same testis

		                        			 A 59-year-old man presented with a painless testicular mass and underwent a radical orchiectomy. The resected specimen showed a 5-cm-sized, white-yellow and homogenous solid mass in the testicular parenchyma. Histologically, the central part of the tumor exhibited typical features of seminoma. The peripheral part of the tumor exhibited diffuse infiltration of small, monotonous lymphoid cells involving the tunica albuginea. The monotonous lymphoid cells were immunoreactive for CD20, CD79a, CD5, and CD23, and negative for CD3, CD10, and cyclin D1. Kappa light chain restriction was detected on flow cytometry using the resected specimen. Considering the circulating lymphoid cell count of >5.0×103/µL, we diagnosed the peripheral component of the tumor as an infiltration of chronic lymphocytic leukemia. This extremely rare combination of seminoma and lymphoid neoplasm should be considered in the differential diagnosis of classic seminoma with extensive lymphoid reaction in tumors arising in elderly patients. 
		                        		
		                        		
		                        		
		                        	
4.Survey Targeting Community Pharmacists regarding Consultation Requests from Patients Owing to Media Reports on "Dangers of Drugs"

		                        			Objective: In our previous study, we revealed that articles on "dangerous drugs" in weekly magazines are inadequate and incomplete with respect to scientific validity, and that there may be many aspects that need to be improved. Next, the extent to which medical professionals, including doctors, pharmacists, and nurses, received consultations by patients owing to such media reports and what countermeasures are implemented requires clarification. In the present study, as a first step, we performed a questionnaire survey of community pharmacists to clarify the occurrence of such consultations.Methods: A questionnaire survey was conducted among 698 community pharmacists. The survey was conducted over 10 days from December 8 to 17, 2021.Results: Out of the 698 community pharmacists, 545 responded (response rate: 78.1%). Of these, 323 (59.3%) had experienced consultations from patients owing to media reports on the dangers of drugs. Of the 323 respondents, 215 (66.6%) had experienced these consultations less than five times. In contrast, 83 respondents (25.7%) reported more than 10 such consultations. Of the 545 respondents, 190 (34.9%) responded that they routinely perform measures to deal with media reports on the dangers of drugs. The most common routine measure was the "regular checking of various media sources (including preparation of responses when receiving consultation requests)."Conclusion: Our results indicated that more than half of the community pharmacists had experienced receiving consultations from patients, although only approximately 30% were adopting routine measures. This suggests that community pharmacists have been affected to some extent by these media reports, and that further countermeasures need to be implemented.
		                        		
		                        		
		                        		
		                        	
5.Organizing Information on “Dangerous Drugs” Published in Weekly Magazines and Evaluating Its Relevance

		                        			Objective: Recently, special features on “dangerous” prescription drugs have been frequently published in few mass media platforms, such as weekly magazines. However, to our knowledge, there have been no prior studies, systematically organizing and evaluating the contents of these articles (e.g., drugs and their side effects that are reported). In this study, we aimed to evaluate the relevance of the information on “dangerous drugs” that was published in weekly magazines using the modified “Media Doctor” instrument.Methods: We extracted articles on “dangerous” prescription drugs from 10 weekly magazines for which the table of contents of previous issues are available on their official websites. Information regarding the target drugs and their classifications was compiled and organized. The relevance of the extracted news articles was assessed by two independent evaluators. The evaluation index was based on the modified “Media Doctor” instrument, and 9 evaluation criteria were used to assign the following categories: satisfactory, unsatisfactory, or not applicable.Results: A total of 1,064 articles were screened, and 19 articles were selected for further evaluation. A total of 179 drugs (belonging to 34 drug classes) were listed. The most frequently mentioned class of drugs included hypnotics and sedatives, and the most frequently listed was triazolam. Of the 19 articles, 11 had zero items that were judged satisfactory by both the evaluators. The number of unsatisfactory items was widely distributed, but in 11 of the 19 articles, five and more items were judged unsatisfactory by both the evaluators.Conclusion: We revealed that a wide variety of drugs have been termed as “dangerous” by weekly magazines. Additionally, we found that these articles were inadequate and incomplete with respect to scientific validity, and that there are many aspects that require further improvement.
		                        		
		                        		
		                        		
		                        	
6.Efficacy of Underwater Endoscopic Mucosal Resection for Superficial Non-Ampullary Duodenal Epithelial Tumor

		                        			Background/Aims:
		                        			Endoscopic resection (ER) for superficial non-ampullary duodenal epithelial tumors (SNADETs) is challenging. Conventional endoscopic mucosal resection (CEMR) is also problematic due to the anatomical features of the duodenum. We compared the safety and efficacy of underwater endoscopic mucosal resection (UEMR) with those of CEMR through a retrospective analysis. 
		                        		
		                        			Methods:
		                        			Altogether, 44 consecutive patients with 46 SNADETs underwent ER (18 CEMR cases and 28 UEMR cases) between January 2016 and October 2019. We investigated the proportions of en bloc resection, R0 resection, complications, resection time, and total procedure time and compared the outcomes of patients from the CEMR group with those of patients from the UEMR group. 
		                        		
		                        			Results:
		                        			The median tumor size was 8.0 mm (range, 2.0–20.0 mm). The UEMR group showed a higher proportion of en bloc resection (96.4% vs. 72.2%, p<0.05) and significantly lower median resection time and total procedure time (4 min vs. 9.5 min, p<0.05 and 13 min vs. 19 min, p<0.05; respectively) than the CEMR group. No complications were observed. However, two patients treated with piecemeal resection in the CEMR group had residual tumors. 
		                        		
		                        			Conclusions
		                        			UEMR is a feasible therapeutic option for SNADETs. It can be recommended as a standard treatment.
		                        		
		                        		
		                        		
		                        	
7.Anti-lipopolysaccharide antibody mitigates ruminal lipopolysaccharide release without acute-phase inflammation or liver transcriptomic responses in Holstein bulls

		                        			Anti-lipopolysaccharide (LPS) antibody administration has the potential benefits of neutralizing and consequently controlling rumen-derived LPS during subacute ruminal acidosis. Four Holstein bulls were used in this crossover study with a 2-week wash-out period. Anti-LPS antibody (0 or 4 g) was administered once daily for 14 days. Significantly lower ruminal LPS and higher 1-h mean ruminal pH were identified in the 4 g group. However, blood metabolites, acute-phase proteins, cytokines, and hepatic transcriptomes were not different between the two groups. 		                        			Objective: Combination therapy with glecaprevir and pibrentasvir (G/P) has been shown to provide a sustained virologic response (SVR) rate of >97% in patients with chronic hepatitis C virus (HCV) infection in the first published real-world Japanese data. However, a recently published study showed that the treatment was often discontinued in patients ≥75 years old, resulting in low SVR in intention-to-treat (ITT) analysis. Thus, our aim was to evaluate real-world data for G/P therapy in patients ≥75 years of age, the population density of which is high in “rural” regions.Patients and Methods: We conducted a multicenter study to assess the efficacy and safety of G/P therapy for chronic HCV infection, in the North Kanto area in Japan.Results: Of the 308 patients enrolled, 294 (95.5%) completed the treatment according to the protocol. In ITT and per-protocol analyses, the overall SVR12 rate was 97.1% and 99.7%, respectively. The old-aged patients group consisted of 59 participants, 56 of whom (94.9%) completed the scheduled protocol. Although old-aged patients tended to have non-SVR factors such as liver cirrhosis, history of HCC, and prior DAA therapies, the SVR12 rates in old-aged patients were 98.3% and 100% in the ITT and PP analyses, respectively. Of 308 patients enrolled, adverse events were observed in 74 patients (24.0%), with grade ≥3 events in 8 patients (2.6%). There was no significant difference in any grade and grade ≥3 adverse events between the old-aged group and the rest of the study participants. Only one patient discontinued the treatment because of adverse events.Conclusion: G/P therapy is effective and safe for old-aged patients.
