1.Issues on Blood Pressure Values in Local Residents and Necessity of Community Pharmacist’s Support for Hypertensive Residents

Health support activities for residents were held three times during a period from June, 2016 to November, 2017. The residents registered in order to participate through 11 community pharmacies. Participant’s blood pressure values were measured at the venue of health support activities by pharmacists. The blood pressure values in 66 of the 106 participants were analyzed, excluding 40 participants receiving medical treatment. The rate of participants with hypertension, systolic blood pressure (SBP)≧140 mmHg or diastolic blood pressure (DBP)≧90 mmHg, was 32% (21/66). Of those with hypertension, 43% (9/21) had hypertension of a high risk level, SBP≧160 mmHg or DBP≧100 mmHg. Pharmacists provided lifestyle counseling and encouraged them to make changes in order to achieve tighter blood pressure control. The data of their blood pressure values were provided to community pharmacies for continuing support of their blood pressure control. With the pharmacist’s support, the median value of blood pressure of participants who had hypertension decreased in the second event. In addition, 50% (3/6) of participants who attended the second event with hypertension of high risk level, visited the hospital for a consultation with a doctor after getting pharmacist’s advice. These results indicate that greater access to community pharmacists improved blood pressure control of residents with hypertension. However, there were many residents who consider it difficult to use pharmacies without a prescription. Hence, friendly relationship to residents on their utilizing pharmacies is needed for providing them lifestyle counseling such as preventing hypertension by pharmacists.
2.A Case of Takayasu's Arteritis with Aortic Root Abscess after AVR and during Biologic Drug Administration

A 32-year-old woman was diagnosed with Takayasu's arteritis 5 years ago and underwent aortic valve replacement for aortic regurgitation 1 year ago. She had been taking Prednisolone and Azathioprine for Takayasu's arteritis, but these drugs were switched to subcutaneous Tocilizumab 4 months ago. One month ago, she had dyspnea on exertion, and 2 days ago, chest discomfort appeared, and she came to our hospital. Blood tests showed CRP 0.02 mg/dl, and echocardiography and CT showed perivalvular leakage and aortic root pseudoaneurysm, which led us to suspect aortic root pseudoaneurysm due to Takayasu's arteritis and to perform emergency surgery. Although a circumferential pseudoaneurysm was observed at the aortic root, no destruction of the prosthetic valve was observed. The suture from the previous surgery was attached to the sawing cuff of the prosthetic valve, and the prosthetic valve was not fixed to the aortic annulus and could be easily removed. The Bentall operation was performed using a bioprosthetic valve. The histopathological diagnosis was subacute infective endocarditis, and the patient was diagnosed with a pseudoaneurysm of the aortic root due to infection. The patient had a good postoperative course and was discharged home on the 19th day. We report a case of Takayasu's arteritis with valve annular abscess after AVR, which was treated surgically during biologic drug administration.
3.Shot performance characteristics of badminton players of different skill levels

This study aimed to determine the differences in the shot performance of badminton players of different skill levels. The participants included 10 male college and 10 unskilled badminton players. Each participant was asked to perform backhand drives with varying degrees of effort while seated to avoid any influence of footwork. Each participant completed 50 trials. In each trial, shot performance was evaluated using a three-dimensional motion capture system along with video data. Additionally, shot accuracy, shuttle velocity, impact position, and shuttle launch angle were assessed. Skilled players had more accurate shots with higher shuttle velocities than the unskilled players. Notably, shuttle velocity increased with the subjective effort level in both types of players. Variability of the impact position was also examined in all trials, and no inter-skill level differences were noted. Moreover, shuttle launch angles were calculated in the left-right and vertical directions. No differences were observed between the skilled and unskilled players in the left-right direction. In the vertical direction, unskilled players hit more upward than skilled players; however, no differences were noted in their effort levels. These results suggest that skilled players adjust the shuttle velocity and vertical launch angle with varying degrees of effort to hit accurate shots. In contrast, unskilled players struggle to adjust vertical launch angle, resulting in inaccurate shots.
4.Quantitative Analyses of Geniposide in Ethical Kampo Extract Formulations Containing Gardenia Fruit

Objective: Prolonged use of Kampo medicines containing Gardenia fruit can cause mesenteric phlebosclerosis. A cumulative dose exceeding 5,000 g of Gardenia fruit may trigger this condition. However, the Japanese Pharmacopoeia 18th edition only specifies the minimum geniposide content in Gardenia fruits, making it difficult to assess the risk based solely on the cumulative Gardenia fruit dose. Hence, we evaluated mesenteric phlebosclerosis risk concerning cumulative geniposide intake in the current study. Quantitative analyses were conducted on ethical Kampo extract formulations containing Gardenia fruit. Methods: Ethical Kampo extract formulations containing Gardenia fruit were separately extracted using an H2O/methanol (1:1) solution and the resulting extracts underwent filtration. Each filtrate underwent quantitative, high-performance liquid chromatography analysis. Results: Significant variation was observed in the geniposide quantities among various ethical Kampo extract formulations containing identical amounts of Gardenia fruit in each daily dose. Additionally, the extracts of orengedokuto, kamishoyosan, and bofutsushosan obtained from various companies exhibited maximum differences in geniposide quantities ranging from 1.5- to 2.7-fold. Conclusion: This study highlights the challenges in accurately assessing geniposide levels in ethical Kampo extract formulations containing Gardenia fruit solely based on the daily dose of Gardenia fruit. The potential development of mesenteric phlebosclerosis should be considered based on the cumulative intake of geniposide rather than the cumulative dose of Gardenia fruit. Additionally, the study offers recommendations regarding the optimal geniposide quantities in ethical Kampo extract formulations containing Gardenia fruits.

Methods:
We retrospectively analyzed the clinical records of 134 patients who received ddTC+Bev or ddTC as first-line chemotherapy for stage III–IV ovarian cancer. Progressionfree survival as primary endpoint of this study was compared using the log-rank test. Cox proportional hazards model and propensity score matching (PSM) were used to analyze prognostic factors, and the frequency of adverse events was examined using the χ 2 test.
Results:
We categorized 134 patients in the ddTC+Bev (n=57) and ddTC (n=77) groups who started treatment at four related institutions from November 2013 to December 2017.No patients used poly (ADP-ribose) polymerase inhibitors as the first line maintenance therapy. The progression-free survival (PFS) of the ddTC+Bev group had a significantly better prognosis than that of the ddTC group (hazard ratio [HR]=0.50; 95% confidence interval [CI]=0.32–0.79; p<0.003). Multivariate analysis showed that ddTC+Bev regimen was a prognostic factor. However, intergroup comparison using PSM revealed that the PFS of the ddTC+Bev group had a nonsignificantly better prognosis than that of the ddTC group (HR=0.70; 95% CI=0.41–1.20; p=0.189). Few adverse events above G3 were noted for ddTC+Bev, which were sufficiently tolerable.
Conclusion
This study could not demonstrate that adding Bev to ddTC improves prognosis.Further studies with more cases are warranted.

8.Discrepancy between Clinician-rated and Self-reported Depression Severity is Associated with Adverse Childhood Experience, Autistic-like Traits, and Coping Styles in Mood Disorders

Objective:
This study aimed to determine if the discrepancy between depression severity rated by clinicians and that reported by patients depends on key behavioral/psychological features in patients with mood disorders.
Methods:
Participants included 100 patients with mood disorders. First, we examined correlations and regressions between scores on the Hamilton Depression Rating Scale (HAMD) and Beck Depression Inventory (BDI). Second, we divided the participants into those who provided 1) greater ratings for the BDI compared with the HAMD (BDI relative-overrating, BO) group, 2) comparable ratings for the BDI and HAMD (BDI relatively concordant, BC) group, or 3) less ratings for the BDI (BDI relative-underrating, BU) group. Adverse childhood experiences, autistic-like traits, and coping styles were evaluated with a six-item short version of the Childhood Trauma Questionnaire (CTQ-6), the Social Responsiveness Scale for Adults (SRS-A), and the Ways of Coping Checklist (WCCL), respectively.
Results:
A significant correlation was found between HAMD and BDI scores. Total and emotional abuse subscale scores from the CTQ-6, and the self-blame subscale scores from the WCCL were significantly higher for the BO group compared with the BU group. The BO group also elicited significantly higher SRS-A total scores than did the other groups.
Conclusion
These findings suggest that patients with adverse emotional experiences, autistic-like traits, and self-blame coping styles perceive greater distress than that evaluated objectively by clinicians. The results indicate the need for inclusion of subjective assessments to effectively evaluate depressive symptoms in patients deemed to have these psycho-behavioral concerns.
9.A Case of Redo-Mitral Valve Replacement for a Björk-Shiley Delrin Valve Implanted 47 Years Previously

We report a case of redo mitral valve replacement (MVR) for a Björk-Shiley Delrin valve implanted 47 years previously. A 71-year-old man initially underwent MVR for mitral regurgitation at our hospital at the age of 16 years. Following the operation, follow-up examinations were performed at the outpatient clinic and annual transthoracic echocardiogram findings showed only mild mitral regurgitation, with no adverse events noted. However, a transthoracic echocardiogram examination performed 45 years after the operation revealed mild to moderate mitral regurgitation, while dyspnea with exertion was also noted at that time. As part of a more detailed examination, transesophageal echocardiogram results showed moderate transvalvular leakage. Redo MVR was subsequently performed under the diagnosis of prosthetic valve dysfunction. Analysis of the explanted prosthetic valve revealed wear of the Delrin disk, and widening of the gap between the disk and strut, which were presumed to be the cause of transvalvular leakage. A half century has passed since introduction of the Björk-Shiley valve and the present is a rare case of valve malfunction. Presented here are related details, along with a review of existing literature and results of Björk-Shiley valve use at our hospital.
10.Effect of number of college baseball pitching on movement, performance, physical strength, physiological, and psychological functions of pitchers

This study aimed to examine the effects of college baseball pitching on movement, performance, physical strength, and physiological and psychological functions of pitchers. The participants were 10 right-handed male pitchers from the University baseball team. The number of pitches were 15 per inning and 135 per nine innings. Ball speed and accuracy were measured for each pitching. The grip strength, back muscle strength, and standing long jump were measured before and after all pitches. Blood lactate levels were measured before pitching, at the end of the 5th and 7th innings, and at 3rd, 6th, and 9th minutes after pitching. The participant’s heart rate and subjective exercise intensity were measured at the beginning and end of each inning using the Borg scale measurement. Five high-speed cameras were used to capture the pitching motions. The displacement of the center of gravity, lower and upper limb joint angles, and the speed of each segment were calculated. The baseball speed and accuracy did not change with the increased number of pitches. However, the grip strength decreased. Although blood lactate and heart rate were not altered, subjective exercise intensity was increased. The lower limb kinematics remained unchanged; however, elbow height was reduced in the upper limb. These results suggest that highly competitive pitchers experience subjective fatigue with the increased number of pitches, however, they maintain pitch performance, speed, and accuracy without altering whole-body physiology and lower-body function and form.
