1.Practice and exploration of the " point to point" management mode of scientific research project application

Objective:To establish a supportive and effective management mode of scientific research project application, promote the capacity building of scientific research in the hospital.Methods:Retrospective analysis was conducted on the national and provincial scientific research projects of Xinhua Hospital Affiliated to Shanghai Jiaotong University Medical School from 2010 to 2019, and the practical effect of " point to point" management mode of scientific research project application was evaluated.Results:The funding rate of Xinhua Hospital, especially national scientific research projects and provincial talents projects, was greatly improved by adoption of the multi-dimensional " point to point" management mode of scientific research project application. The number of national scientific research projects increased from 34 (26.02 million) in 2010 to 72 (51.0851 million) in 2019.The number of provincial talents projects increased from 5 (1.05 million) in 2010 to 26 (6.5 million) in 2019.Conclusions:The " point to point" management mode of scientific research project application plays an important role in promoting the overall funding rate. Enhancement of comprehensive capacity of hospital scientific research can be achieved by further improvement of this management mode, early initiation and arrangement of funding application depending on the " close partner" entity, emphasizing scientific research talents cultivation.
2.Xenogenic bone with chitosan / norvancomycin sustained-release system for treatment of infectious bone defects in rabbits

Objective To investigate the effects of xenogenic bone with chitosan/norvancomycin sustained-release biomaterials in treating infectious bone defects in rabbits.Methods Xenogenic bone with chitosan/norvancomycin sustained-release biomaterials was made by electrospinning technique.Rabbit infectious bone defect models were made by Methicillin-resistant Staphylococcus aureus.A successful model was evaluated with the standard of more than three points in Norden score assessment.All models were divided into two groups by random number table method,with eight models in each.The control group was treated with surgical debridement,and the experimental group was implanted with bone particles of xenogenic bone with chitosan/norvancomycin sustained-release system after debridement.Postoperatively,general conditions,X-ray,histological results of HE staining,and bacteriological examination results of the rabbits were observed.Results X-ray showed significant bone defects,sequestration,periosteal reaction,and soft tissue swelling after one month of modeling,with Norden score of (3.84 ± 0.52) points.The general conditions were good and the sinus tracts were healed in experimental group after two months of treatment.The control group demonstrated generally poor conditions with swollen sinus and purulent discharge.Two rabbits were died of sepsis.The pathological scores of tibial were (0.41 ± 0.08) points in experimental group,and (3.27 ± 0.26) points in control group by gross observation.The pathological score of experimental group was significantly lower than control group(P < 0.05).The bone defects were basically repaired in experimental group.The longest diameter of bone defect in experimental group was (0.11 ± 0.02)cm,significantly smaller than (0.48 ± 0.06) cm in control group (P < 0.05).There were no obvious signs of osteomyelitis and the bone defects were well repaired in experimental group.Periosteal reaction,soft tissue swelling,a substantial number of bone destruction,and sequestration were observed in control group.The Norden score was (1.32 ± 0.23) points in experimental group,lower than (5.21 ± 0.48) points in control group(P < 0.05).HE staining showed a large amount of trabecular bone formation,bone cell formation,and fibrous hyperplasia in experimental group,with no obvious signs of infection.On the other hand,infiltration of inflammatory cells,necrotic tissue,and sequestration were observed in control group.The histological score was(0.61 ± 0.10) points in experimental group,lower than (4.21 ± 0.41) points in control group (P <0.05).The negative rate of bacterial culture in experimental group was 33%,lower than 100% in control group (P < 0.05).Conclusion Xenogenic bone with ehitosan/norvancomycin sustained-release biomaterials has excellent effect in infection clearance and bone defect reparation in treatment of infectious bone defects in rabbits.
3.Research on potential interaction between mitochondrial DNA copy number and related factors on risk of hypertension in coal miners

Objective To investigate the effects of mitochondrial DNA (mtDNA) copy number in peripheral blood and related factors on the risk of hypertension in coal miners.Methods A case-control study was conducted in 378 coal miners with hypertension and 325 healthy coal miners recruited from Datong Coal Mine Group.A standard questionnaire was used to collect their general information,such as demographic characteristics,habits and occupational history.Fluorescence quantitative PCR was performed to detect the copy number of mtDNA.Logistic regression model was applied for identifying the related risk factors of hypertension and analyzing the interaction between mtDNA copy number and risk factors.Results The prevalence of hypertension of high mtDNA copy number was lower than mtDNA copy numberin 0-5.67 group,but the difference was not statistically significant (P=0.414).Alcohol drinking (OR=1.80,95%CI:1.26-2.56),family history of hypertension (OR=1.74,95% CI:1.20-2.50),work shifts (OR=0.69,95% CI:0.48-0.99),education level (P=0.012) and family monthly income level (P=0.001) were related to the prevalence of hypertension.There were potential interactions between mtDNA copy number and alcohol drinking,family monthly income level,family history of hypertension,respectively.Alcohol drinking was a risk factor for hypertension [1.77(1.25-2.50)].Potential interactions between mtDNA copy number and alcohol drinking reduced the risk of hypertension (OR=1.20,95%CI:1.07-1.35).Family history of hypertension was a risk factor for hypertension [1.81(1.26-2.59)].Potential interactions between mtDNA copy number and family history of hypertension reduced the risk of hypertension (OR=1.24,95% CI:1.09-1.41).Family monthly income level was a protect factor for hypertension [0.55(0.46-0.66)].Potential interactions between mtDNA copy number and family monthly income level increased the protection role of hypertension (OR=0.90,95%CI:0.86-0.94).Conclusion mtDNA copy number variation was not significantly associated with the prevalence of hypertension in coal miners,but mtDNA copy number shewed multiplication interaction on the prevalence of hypertension with alcohol drinking,family monthly income level as well as family history of hypertension and made their influences weaken.
4.Effect of Notch signaling and autophagy on MTA induced differentiation of human dental pulp cells in vitro

Objective:
The aim of this study is to investigate the roles of Notch signaling and autophagy on mineral trioxide aggregate (MTA) induced differentiation of human dental pulp cells (hDPCs).
Methods :
Third molars from healthy human were collected and hDPCs were isolated by a combined digestion of collagenase Ⅰ and dispaseⅡ. Real time PCR were used to test the mRNA expression levels of alkaline phosphatase (ALP), runt-related transcription factor 2 (Runx2) and dentin sialophoprotein (DSPP) in MTA treated hDPCs in different time (24 h, 3 d and 7 d). The mineralization nodules formed by hDPCs with or without MTA treatment were detected by Von Kossa staining. Expressions of Notch1, Jagged1, Hes1, LC3Ⅱ/LC3 Ⅰand p62 in wild type and MTA treated hDPCs were detected by western blotting.
Results:
MTA extracted in a concentration of 0.1 mg/mL could promote the differentiation of hDPCs. Compared with that of wild type hDPCs, the expressions of Notch1, Hes1, or Jagged1 and p62 (P<0.01) in MTA treated hDPCs were significantly increased. MTA treatment showed inhibition effects on autophagy flux similar to Bafilomycin A1, a specific inhibitor of fusion between autophagosomes and lysosomes.
Conclusion
MTA could promote hDPCs differentiation with highly relevant in stimulating Notch1-Jagged1-Hes1 signaling and inhibition of autophagy flux.
5.Study on the use of antimicrobial agents in hospitalized trauma patients based on medical big data

Objective To investigate the relationship between the trauma severity and the usage of antibacterial drugs and to provide reference for standard protocol of proper antibiotic use in wound care.Methods ICD-10 and AIS were used to set up the relationship and to analyze the use of antibiotics in patients with different trauma score.Results 25 035 trauma patients were enrolled in this study.Those patients were divided into five groups according to the AIS score with least severe as group 1 to most severe as group 5.The patient percentage in group 1 to 5 was 21.92%,67.73%,8.86%,0.97% and 0.52% respectively.The five most frequently used antibiotic classes are second generation cephalosporins,third generation cephalosporins,first generation cephalosporins,fluoroquinolones and penicillin/beta lactamase inhibitor combination, accounted for 29.69%,22.57%,20.33%,4.66% and 4.47% of total DDDs of antibacterial drugs.Individually, the top 10 antibiotics are cefuroxime (12.21%), cefazolin (8.31%), ceftriaxone (7.74%), cefathiamidine (7.34%), cefotiam (4.87%), ceftazidime (3.68%), amoxicillin/clavulanic acid (3.63%), levofloxacin (3.59%), cefoxitin (3.56%), flucloxacillin (3.52%);gentamicin (2.27%), ornidazole (2.00%) and cefoperazone/tazobactam (1.44%) were used most in their categories respectively.The variety and quantity of antibacterial drugs used for different trauma patients were different.Conclusion The trauma score based on ICD-AIS can reflect the severity of trauma.The use of antibiotics in patients with different trauma score can provide reference for the clinical applications of antibiotics in wound care.
6.Comparison of maximum slope and deconvolution algorithms in multi-slice CT hepatic perfusion measurement

Objective To evaluate the reliability of CT perfusion parameter values of the normal hepatic segments and neoplasms, obtained with deconvolution (DC) and maximum slope (MS) algorithms. Methods Perfusion parameter values of 111 ROIs in 62 normal hepatic segments and 49 neoplasms derived from 62 CT perfusion studies performed with 320 multi-slice CT, were retrospectively analyzed by two experienced radiologists. BF,BV and PI according to DC and MS algorithms were compared with t paired test, Pearson correlation and Bland-Altman agreement analysis. Interobserver agreement for all perfusion parameters was calculated using intraclass correlation coefficients (ICC). Results Interobserver agreement measured with ICC was very good for all perfusion parameters (≥0.95). BFdc and BVdc exceeded the BFms and BVms in normal hepatic segments and neoplasms (P<0.05); PIdc significantly exceeded the PIms in normal hepatic segments(P<0.05) ,while no difference were found in hepatic neoplasms(P>0.05). Both pairs of perfusion measurements significantly correlated with each other(r>0.9, P<0.01),but the agreement of BF, BV and PI according to DC and MS algorithms was not good. Conclusions CT perfusion values such as BF,BV and PI obtained by DC and MS algorithms correlated significantly with each other, but with poor agreement.
7.Constituent analysis,analgesic and anti-inflammatory activities of leaves essential oil of Schef f lera octophylla

Objective To investigate the constituents and analgesic and anti‐inflammatory effects of leaves essential oil in Schef fleraoctophylla .Methods SOLEO was dilated by water vapor .The mouse acetic acid‐induced twisting test and mouse auricle swelling test induced by xylene were employed to evaluate the anti‐ inflammation and analgesia effects of SOLEO . Results 55 constituents of SOLEO were identified by GC‐MS .The major constituents ,4‐terpenol ,(‐)‐spthulenol ,caryo‐phllene oxide andβ‐linalool with total percent of 51 .86% were found .The twisting number and relieve swollen auricle of mice induced by xylene were significantly reduced by SOLEO .Conclusion SOLEO was the major active components of anti‐inflam‐mation and analgesia .
8.Application effects of the home health care platform and mobile care clinic on the continuing nursing in patients with colostomy

Objective To practice the distance nursing and healthcare through mobile care clinic, and to provide a higher quality of specialist care to colostomy patients. Methods 76 colostomy patients who were operated during August 2015 to January 2016 were investigated and randomly divided into the intervention group ( n=40) and the control group ( n=36 ) . Based on the conventional nursing care, the intervention group received additional continuing care including the subsequent visit and telephone follow-up with the help of mobile care clinic. After one month of intervention, patients′satisfaction level and their awareness of health knowledge were surveyed using an autonomous questionnaire. Results The intervention group had much higher level of satisfaction compared to the control group [ ( 98. 4 ± 1. 6 ) vs ( 91. 3 ± 1. 7 ); t=18. 75, P<0. 05 ] . They also performed better wound care and knowledge awareness compared to the control group ( 92. 5% vs 69. 4%;82. 50% vs 58.33%; χ2=6.70, 5.38; P<0.05). Furthermore, the intervention group had significantly fewer cases of fecal dermatitis than the control group (2 vs 11; χ2=8.73,P<0.05).Conclusions With the help of home health care platform and mobile care clinic, more professional nursing guidelines can be provided in time. It can also improve the nursing satisfaction and reduce related compliances.
9.Application of continuous nursing on the home care of colostomy patients

Objective To explore the effects of continuous nursing in the home care of colostomy patients. Methods A total of 96 patients who received colostomy between January and December 2014 were randomly divided into observation group (48 cases) and control group (48 cases). Patients in control group were received routine care, while patients in observation group were provided continuous nursing. The complications, patients′ satisfactions, correct nursing of colostomy and self care ability were evaluated after intervention. Results The incidence rate of complications including fecal dermatitis and parastomal hernia in observation group was significantly lower than that in control group (P <0. 05). The score of patients′ satisfaction in observation group was higher than that in control group [(98. 4 ± 1. 6) vs. (91. 3 ± 1. 7)]. Furthermore, the percentage of correct nursing of colostomy in observation group and control group were 89. 6% and 64. 6%. The difference between two groups was statistically significant (t/χ2 =21. 07,16. 34;P<0. 05). The scores of self care ability in self care skills, self-responsibility, self-concept and health knowledges in observation group were higher than those in control group. The differences between two groups were statistically significant ( t=9. 42, 4. 31,5. 86,6. 05 and 5. 41;P<0. 05). Conclusions Continuous nursing can significantly increase patients′satisfaction and self care ability, reduce complications, and improve home care quality of colostomy patients.
10.Clinical application of 320-detector CT in interventional treatment of bronchial artery hemoptysis

Objective To evaluate the clinical value of 320-detector CT in interventional treatment of bronchial artery hemoptysis. Methods CTA and DSA images of 30 patients with bronchial artery hemoptysis were retrospectively analyzed.Spatial anatomical characters of the bronchial arteries,such as the type of branches,origin and opening positions of the bronchial arteries were observed and recorded.Results In 30 patients,6 bronchial arteries distribution patterns were found,and the most common type was R1 L1 (43.3%).83 bronchial arteries were identified using CTA,including 38 on the right and 45 on the left.The right bronchial arteries mainly originated from the intercostal artery (52.6%),while the left bronchial arteries mainly from the descending aorta and aortic arch (82.2%).The opening positions of right and left bronchial arteries were mainly located at the right wall of the descending aorta (78.9%),and anterior wall of the descending aorta (62.2%),respectively.When the cacarina of trachea was used as the reference position,the left and right bronchial arteries were mainly located in the range of above 2 cm to below 1 cm from tracheal bifurcation, accounting for 80% and 89.5%,respectively.Compared with DSA,the sensitivity and specificity of CTA were 97.5% and 100%, respectively.Conclusion 320-detector CT can be used to clearly display the distribution patterns,origin and opening positions of bronchial arteries,and especially to find bronchial arteries with ectopic origin.It is possible to apply 320-detector CT in preoperative routine examination and postoperative evaluation of massive hemoptysis.
