In order to provide basic information for the utilization and development of famous classical formulas containing Bletillae Rhizoma, this article systematically analyzes the historical evolution of the name, origin, harvesting and processing of Bletillae Rhizoma by reviewing the ancient materia medica, prescription books, medical books and modern literature. The research results showed that Baiji(白及) was the main name, some scholars took Baiji(白芨) as its main name, and there were many other names such as Baiji(白给), Baigen(白根), Baiji(白苙). The mainstream source of Bletillae Rhizoma was the tubers of Bletilla striata, and drying, large, white, solid, root-free and skin removed completely were the good quality standards. With the promotion of wild to cultivated medicinal materials, there were certain differences between their traits, and the quality evaluation indexes should be adjusted accordingly. The origin of records in the past dynasties was widely distributed, with Guizhou and Sichuan having high production and good quality in modern times. The harvesting period is mostly in spring and autumn, and harvested in autumn was better. The processing and processing technology is relatively simple, and it was used fresh or powdered in past dynasties, while it is mainly sliced for raw use in modern times. Based on the results, it is suggested that the tubers of Bletilla striata of Orchidaceae should be used in the famous classical formulas, and it should be uniformly written as Baiji(白及). And if the original formula indicates the requirement of processing, it should be operated according to the requirement, if the requirement of processing is not indicated, it can be used in raw form as medicine.

By consulting the ancient and modern literature, the name, origin, quality evaluation, harvesting and processing methods of Curculiginis Rhizoma(CR) were systematically combed and verified, in order to provide a basis for the development and utilization of famous classical formulas containing CR. The results of herbal textual research showed that the name Xianmao was first recorded in Leigong Paozhilun, the name of CR was used in all dynasties and this name came from its efficacy and morphological characteristics, the mainstream source for CR of the past dynasties was the rhizome of Curculigo orchioides or C. capitulata, since modern times, C. orchioides has been the main source of commodities. In ancient times, most of the places of origin of the description were the western regions and southwest China, while in modern times, Sichuan and Guizhou were regarded as genuine places. Since modern times, its quality has been summarized as the best with thick roots, firm texture and black-brown surface, the harvesting and processing methods recorded in the past dynasties are mainly sun drying after harvest in the second, eighth and ninth months of the lunar calendar, and most of them are harvested in autumn and winter in modern times. In ancient times, there were many processing methods of CR, mainly in processing with rice swill, while in modern times, stir-frying with wine was the main processing method. The nature, taste, meridian tropism, functions and indications of CR are basically consistent from ancient to modern times, the taboos for taking are to avoid iron, cow's milk, and beef. Although there are some differences in the understanding of the toxicity of CR in the past dynasties, most of the materia medica are clear that it has a certain toxicity. Based on the research conclusion, it is suggested that the rhizome of C. orchioides of Lycoris family should be used as its source in the famous classical formulas, and the corresponding processing method should be selected according to the processing requirements in the formulas, while the raw products is recommended to be selected as medicine if the processing requirement is not specified.

OB JECTIVE To study the protective effects of saponins from Gleditsia sinensis on ischemic stroke with phlegm and blood stasis (ISPBS)model rats ,and to explore its mechanism. METHODS Totally 119 rats were randomly divided into normal group (normal saline ),sham operation group (normal saline ),model group (normal saline ),nimodipine group (positive control group ,5 mg/kg),G. sinensis saponin low-dose ,medium-dose and high-dose groups (3.21,6.42 and 12.84 mg/kg),with 17 rats in each group. Except for normal group ,other groups were all given high-fat diet+suture-occluded method to induce ISPBS model. The neurological function score ,water content of brain tissue ,pathological morphology of brain tissue ,the changes of hemorheology indexes (whole blood viscosity , erythrocyte aggregation index , Casson-viscosity), four items of blood lipid [triacylglycerol (TG),total cholesterol (TC),low-density lipoprotein cholesterol (LDL-C),high-density lipoprotein cholesterol(HDL-C)] and inflammatory factors in serum and oxidative stress indexes [malondialdehyde (MDA),nitric oxide (NO),superoxide dismutase (SOD)] in brain tissue were determined or observed in rats. The protein expressions of B lymphocytoma 2(Bcl-2),Bcl-2 associated X protein (Bax)and caspase- 3 in cerebral tissue were also detected. RESULTS Compared with normal group ,the score of nerve function ,5 kinds of serum indexes (TC,TG,LDL-C,TNF-α,IL-1β), hemorheology indexes ,the contents of MDA and NO and protein expressions of Bax and caspase- 3 in cerebral tissue were all increased significantly in model group (P<0.01). The levels of HDL-C and IL- 10 in serum ,SOD activity and protein expression of Bcl- 2 in cerebral tissue were decreased significantly (P<0.01),and obvious lesions such as nuclear pyknosis and cell membrane fragmentation occurred in brain tissue. Compared with model group ,above indexes of administration groups were improved to different extents ,among which there was statistical significance in above indexes of G. sinensis saponin high-dose group (P< 0.01). CONCLUSIONS Saponin from G. sinensis has a good protective effect on ISPBS model rats. Its mechanism may beassociated with reducing oxidative damage , reducing the production of pro-inflammatory mediators and resisting neuronal apoptosis.

Objective:To investigate the correlations between subacromial impingement syndrome (SIS) and acromial morphology and subacromial intervals after surgery of proximal humerus fracture. Methods:A retrospective study was conducted of the 62 patients with proximal humerus fracture who had been treated by internal fixation with a locking titanium plate from December 2014 to December 2019 at Department of Orthopedics, People's Hospital of Gaoming District. They were 40 men and 22 women, with an average age of 53.5 years (from 35 to 71 years). By the Neer classification, there were 38 three-part and 24 four-part fractures. The incidence of SIS was determined by Nikolaus's diagnostic criteria. The patients were divided into a SIS group and a non-SIS group. The acromial morphology was observed and the subacromial intervals [acromio-greater tuberosity of humerus interval (AGI) and acromio-plate interval (API)] were measured on postoperative X-ray films. The correlations were analyzed between them and SIS.Results:All patients completed treatments and follow-ups from 3 to 15 months (average, 10 months). There was no incision infection or internal fixation failure. There were 18 cases in the SIS group and 44 ones in the non-SIS group. In the SIS group, 3 cases had type Ⅰ acromion while 15 ones type Ⅱ or type Ⅲ acromion; in the non-SIS group, 20 cases had type Ⅰ acromion while 24 ones type Ⅱ or type Ⅲ acromion. The proportion of patients with type Ⅱ or type Ⅲ acromion in the SIS group was significantly higher than that in the non-SIS group ( P<0.05). AGI and API in the SIS group were (11.6±2.1) mm and (14.2±2.4) mm, significantly shorter than those in the non-SIS group [(15.7±2.8) mm and (18.5±3.2) mm] ( P<0.05). Conclusions:SIS is a common complication after surgery of proximal humerus fracture. Abnormal acromial morphology (type Ⅱ or type Ⅲ acromion), poor fracture reduction (reduced AGI), and high placement of a titanium plate (reduced API) may be all important factors leading to SIS.

Integrated medical education, namely medical education under the concept of integration, is an important trend in the development of global medical education, and also an important task in the re-form of medical universities and colleges in China. The integrated medical education abroad originated from Europe and America, and developed relatively well. It started late in China and was limited to integration of some subjects. At present, the reform of integrated medical education in domestic colleges and universities is still facing the problems and challenges such as lag of teaching ideas, limited content of reform, insufficient teachers' ability, difficulty to change learning habits, deviations of learning purposes, lack of integrated learning, lack of system, poor mechanism, weak guarantee and so on. In the future, we should make efforts to teach, learn and manage, give full play to the leading role of "teaching", improve the main position of"learning", and constantly improve the guarantee function of "management".
