1.Real-life experience of implantable venous access ports for cancer patients: a Meta-synthesis of qualitative research

		                        			Objective To analyze and summarize the real-life experiences of cancer patients using infusion ports both domestically and internationally. Methods Literatures were searched in the Cochrane Library, Embase, CINAHL, PubMed, Web of Science, ProQuest Dissertations & Theses Global (PQDT), CNKI, Wanfang Database, VIP, and China Biology Medicine up to May 1, 2023. After rigorous evaluation of the retrieved literature, the themes related to the real-life experiences of cancer patients with infusion ports were analyzed and synthesized. Results A total of 13 articles were included, with 6 in Chinese and 7 in English. From these, 64 findings were extracted, comprehensively categorized into 7 groups, and further consolidated into 3 main results: diverse needs across different stages of cancer patients with infusion ports, dynamically intertwined positive and negative psychological states in response to encountered events, and the establishment of a new normal in life experiences for cancer patients with infusion ports. Conclusion Clinical nurses should recognize and address the varying needs of cancer patients with implantable venous access ports at different stages, strengthen their positive experiences, alleviate their negative experiences, assist in adapting to life with implantable venous access ports, improve the quality of nursing services, and enhance patients' sense of happiness.
		                        		
		                        		
		                        		
		                        	
2.Relationship between polymorphic interaction of glutamate pathway genes and anhedonia

		                        			Objective·To explore the association between gene-gene interaction of glutamate pathway and anhedonia.Methods·A total of 279 patients with schizophrenia(SZ)and 236 patients with major depression disorder(MDD)recruited in the outpatient department and ward of Shanghai Mental Health Center,Shanghai Jiao Tong University School of Medicine,and 236 healthy controls(HC)recruited in the community from January 2017 to August 2020 were included in the study.General demographic data and clinical characteristics of the three groups were collected and compared.The Chinese version of Temporal Experience of Pleasure Scale(TEPS)was used to evaluate the pleasure experience ability of the three groups.Generalized multifactor dimensionality reduction(GMDR)method was used to establish the interaction model of the single nucleotide polymorphism(SNP)in glutamate pathway genes(NOS1AP,GSK3β,DAOA,DISC1 and GRIN2A).According to the interaction model,SZ and MDD patients were divided into high-risk group and low-risk group,and the differences in pleasure experience ability were compared between the two groups,so as to analyze the effect of gene-gene interaction on anhedonia.Results·There were significant differences in age and years of education among the three groups,and in age of onset and duration of illness between SZ and MDD groups(all P=0.000).There were significant differences among the three groups of participants in terms of overall pleasure experience,anticipatory pleasure experience and consummatory pleasure experience(all P=0.000);the overall pleasure experience,anticipatory pleasure experience and consummatory pleasure experience in the SZ and MDD group were lower than those in the HC group(all Pcorr=0.000),and there was marginal statistical difference in anticipatory pleasure experience between the SZ and MDD groups(Pcorr=0.051).Through GMDR modeling,it was found that the 2-loci interaction model composed of DAOA-rs3916965 and DISC1-rs821577 had a predictive effect on the overall pleasure experience ability of SZ patients(P=0.003),and the 2-loci interaction model composed of NOS1AP-rs1858232 and GRIN2A-rs1014531 had a predictive effect on the anticipatory pleasure experience ability of MDD patients(P=0.037);moreover,the overall pleasure experience ability of patients in the SZ high-risk group and anticipatory pleasure experience ability of patients in MDD high-risk groups were lower than those in their low-risk groups(t=3.443,P=0.000;t=3.471,P=0.001).Conclusion·The interaction of glutamate pathway gene polymorphisms may be involved in the occurrence of anhedonia.
		                        		
		                        		
		                        		
		                        	
4.Clinical prediction model for complicated appendicitis in children under five years old

		                        			Objective:To retrospectively analyze the independent risk factors of complicated appendicitis(CA)in children under five years old and establish a clinical prediction model, and to evaluate the clinical application of this model.Methods:A retrospective analysis was performed on children under five years old who underwent appendectomy at Children′s Hospital of Shanghai Jiao Tong University School of Medicine from January 2018 to December 2021.The children were divided into CA group and uncomplicated appendicitis group according to whether there was sign of perforation or gangrene in appendiceal tissue after operation.The differences in clinical features and preoperative laboratory test results between two groups were compared.The independent risk factors of CA were identified and a clinical prediction model was established.The clinical prediction model was verified by receiver operating characteristic curve.Results:A total of 140 children were enrolled in this study, including 84 cases in the CA group and 56 cases in uncomplicated appendicitis group.Univariate and binary Logistic regression analysis showed that the duration of symptoms>23.5 h( OR=6.650, 95% CI 2.469-17.912, P<0.05), abdominal muscle tension( OR=3.082, 95% CI 1.190-7.979, P<0.05) and C-reactive protein>41 mg/L ( OR=3.287, 95% CI 1.274-8.480, P<0.05) were independent risk factors for CA( P<0.05). The clinical prediction model of CA was constructed by the above mentioned three independent risk factors.The area under the receiver operating characteristic curve of the clinical prediction model was 0.881(95% CI 0.825-0.936), the sensitivity was 77.4%, the specificity was 87.5%, the positive predictive value was 91.3% and the negative predictive value was 70.0%. Conclusion:Acute appendicitis in children under five years old is more likely to progress to CA if the duration of symptoms>23.5 h, the level of C-reactive protein is increased, and the abdominal muscle tension is accompanied.The clinical prediction model of CA constructed by common clinical information in pediatric clinics has good prediction efficiency, which provides a simple and feasible reference method for clinicians to distinguish CA from uncomplicated appendicitis.
		                        		
		                        		
		                        		
		                        	
5.Construction and application of a decision tree model for children with complicated appendicitis

		                        			Objective:To establish a decision tree model of pediatric complicated appendicitis (CA) based on Pediatric Appendicitis Score (PAS) combined with inflammatory indicators, and to evaluate its clinical application efficacy in pediatrics.Methods:The clinical data of 544 children diagnosed with appendicitis in Children′s Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from January 2018 to December 2021 was retrospectively analyzed. According to postoperative pathology, the children were divided into uncomplicated appendicitis group and CA group. The independent risk factors of CA were screened by univariate and multivariate logistic regression analysis, and these parameters were included to establish the decision tree model. The accuracy of the decision tree model was verified by receiver operating characteristic (ROC) curve.Results:Binary logistic regression analysis indicated that the PAS, C-reactive protein (CRP) and neutrophil to lymphocyte ratio (NLR) were identified as independent risk factors for complicated appendicitis in children (all P<0.05). PAS, CRP and NLR were included as covariables to construct the decision tree model and binary logistic regression model for predicting CA. The decision tree demonstrated an overall accuracy of 79.2% with a sensitivity of 86.7% and specificity of 71.9%, and achieved an area under curve (AUC) of 0.821(95% CI: 0.786-0.857). The binary logistic regression model had a sensitivity of 79.6% and specificity of 69.1%, with an overall accuracy of 75.1% and achieved an AUC of 0.808(95% CI: 0.770-0.845). Conclusions:The decision tree model based on PAS score combined with CRP, NLR is a simple, intuitive and effective tool , which can provide pediatric emergency physicians a reliable basis for diagnosis of pediatric CA.
		                        		
		                        		
		                        		
		                        	
6.Monitoring and analysis of food microbial contamination in rural compulsory education schools in Baiyin City from 2013 to 2021

		                        			To analyze the level and trend of microbial contamination in food in rural compulsory education schools in Baiyin City, so as to provide a reference for playing an early warning role in eliminating potential food safety hazards as soon as possible.
		                        		
		                        			Methods:
		                        			According to the "National Food Contaminants and Harmful Factors Risk Monitoring Workbook", etc., 8 pathogenic bacteria such as Salmonella, Listeria monocytogenes, Staphylococcus aureus, total bacterial count, and 2 hygienic indicator bacteria of coliform were tested.
		                        		
		                        			Results:
		                        			A total of 908 food samples were collected from 2013 to 2021, 47 were positive and the overall positive rate was 5.18%. The positive rate of samples collected in different years showed a downward trend year by year. The detected pathogenic bacteria ranked from high to low as follows: Escherichia coli (1.87%), Bacillus cereus (0.99%), Staphylococcus aureus (0.44%), mold (0.33%), Listeria monocytogenes (0.22%). Hygiene indicator bacteria total number of bacteria exceeded the standard rate was higher (2.09%). Among all kinds of food, milk and dairy products, frozen food, cold food, barbecue, rice and flour products had higher positive rates. According to quarterly statistics, the positive rate in the first and second quarters was relatively high.
		                        		
		                        			Conclusion
		                        			There are different degrees of microbial contamination risks in the food of rural compulsory education schools. All relevant departments should pay attention to the current situation of school food hygiene and safety in poverty stricken areas, and strengthen rural compulsory education school food, especially for milk and dairy products, frozen food, cold food, Supervision of key foods such as barbecue, rice and flour products, and efforts to improve their safety standards, do a good job in school food hygiene and safety, and ensure the healthy growth of young people.
		                        		
		                        		
		                        		
		                        	
7.Spinal Angiolipomas : Clinical Characteristics, Surgical Strategies and Prognosis

		                        			 Objective:
		                        			: Angiolipomas are usually found in the subcutaneous tissue of the trunk and limbs. Spinal angiolipomas (SALs) are uncommon and have rarely been reported. We report a series of nine SALs patients who received surgical treatment in our department. To summarize the clinical characteristics of SALs, propose our surgical strategies, and evaluate the effects of the operation. 
		                        		
		                        			Methods:
		                        			: This was a retrospective review of nine SALs patients who received surgical treatment from August 2015 to March 2020. Total or subtotal resection was determined by the axial localization (dorsal or ventral) and the extent of intervertebral foramen involvement. The outcomes were assessed based on the modified Japanese Orthopaedic Association (mJOA) scoring system utilized before surgery and at various follow-up points. 
		                        		
		                        			Results:
		                        			: Among the nine patients, the mean mJOA score before surgery was 6.6±2.3, compared with 10.1±1.1 at the last follow-up time point (33.4±11.8 months). All patients achieved good outcomes, even in cases of subtotal resection. 
		                        		
		                        			Conclusion
		                        			: Early surgical resection of SALs is recommended, and the specific procedures should be determined by the axial localization (dorsal or ventral) and the extent of intervertebral foramen involvement. 8. High Expression of GADD45B Induced by Acid and Bile Acid is A Potential Prognostic Marker for Barrett's Esophagus-associated Adenocarcinoma

		                        			 Background: Esophageal mucosal injury induced by gastroesophageal reflux is a key link to the development of Barrett's esophagus-associated adenocarcinoma. However, the molecular mechanism is still not elucidated. Aims: To investigate the role of differentially expressed genes (DEGs) after stimulating esophageal cells with acid and bile acid in the development of esophageal adenocarcinoma (EAC). Methods: The DEGs were obtained through bioinformatics methods after stimulating esophageal cells with low pH and deoxycholic acid, and GO, KEGG enrichment analysis were performed. Protein-protein interaction (PPI) network was performed to screen the hub genes, and their relationships with prognosis and tumor stage of EAC patients were analyzed. The role of co-expressed genes of GADD45B in EAC was also analyzed. Results: Thirty-one overlapping DEGs were obtained after stimulating esophageal cells with low pH and deoxycholic acid, which mainly enriched in the cytokine-cytokine receptor interaction, transcription factors activity, and regulation of cell proliferation and apoptotic process. High expression of GADD45B was correlated with the survival prognosis and tumor stage of EAC patients. GADD45B and its co-expressed genes were involved in the production of tumor necrosis factor. Conclusions: The high expression of GADD45B induced by acid and bile acid is correlated with the prognosis and tumor stage of EAC patients, and is a potential diagnosis and treatment target for Barrett's esophagus-associated adenocarcinoma. 
		                        		
		                        		
		                        		
		                        	
9.Study on Pharmaceutical Care Competency of Retail Pharmacy Licensed Pharmacists in Guizhou Province

		                        			OBJECTIVE: To provide reference for strengthening the construction of retail pharmacy pharmacists and ensure safe and rational use of drugs.  METHODS: A simple random sampling method was used to determine 18 cities (counties) in Guizhou province as survey sites to visits all retail drugstores one by one. According to the Pharmacist Pharmaceutical Care Competency Evaluation Standard(Trail) issued by the Chinese Pharmacists Association, questionnaire survey and face-to-face interviews were conducted with licensed pharmacists in Guizhou province’s retail pharmacies, and multiple linear regressions were conducted on the factors affecting the pharmacy’s pharmaceutical care competency.  RESULTS: A total of 943 retail pharmacies in Guizhou province were visited, and 270 licensed pharmacists were investigated. The total self-assessment score of pharmaceutical care competence was (79.62±11.23), with 11.5% of undergraduate education and above, and 53.0% of pharmacy related majors. Among self-assessment score of pharmaceutical care competency, score of basic knowledge item such as foreign language knowledge, statistical knowledge and computer knowledge was the lowest, followed by clinical thinking ability and drug treatment evaluation ability. The results of multiple linear regression showed that age had significant influence on the scores of basic skills and professional knowledge. The influence of academic qualifications on personal accomplishment, basic knowledge, basic skills, professional knowledge and professional skills score was statistically significant; the influence of professional and technical positions on total score, personal accomplishment, basic skills and professional skills score was statistically significant (P<0.05). CONCLUSIONS: The overall educational level of licensed pharmacists in Guizhou retail pharmacies is low and their professional level is limited. It is suggested that the supervision of licensed pharmacists should be strengthened and the access conditions of licensed pharmacists should be improved; the education and training of professional competency of licensed pharmacists in pharmaceutical care should be strengthened, the functional orientation of licensed pharmacists in retail pharmacies should be clarified, and public safety and rational drug use should be effectively guaranteed.
