1.Experimental study of en-bloc resection of bladder tumors by transurethral single-port laparoscopy in vivo animal models

【Objective】 To explore the feasibility of en-bloc resection of bladder tumors by flexible cystoscope combined with laparoscopic instruments through urethra and to provide reference for the clinical application of this technique. 【Methods】 Self-designed and processed transurethral single-hole PORT and Olympus electronic cystoscope were used as observation mirror; Φ1.8 mm soft grasper, tissue scissors, electric hook, and ultrasonic scalpel were used as instruments; the porcine bladder was used as a model.The PORT was placed through the urethra, and the cystoscope was inserted to observe the inner wall of the bladder and the condition of the mucosa.After the lesion site was identified in the bladder cavity, the soft grasper was inserted to pull the mucosa to be removed, which was then fixed with tension at the target position to maintain a satisfactory feild of view.The surgeon held the cystoscope in the left hand, and operated the laparoscopic instruments into the bladder cavity through the PORT with the right hand.Observing with the cystoscope and lifting and pulling the mucosa with the grasper, the surgeon simulated the cutting and pushing actions to realize the en-bloc resection of the lesioned mucosa. 【Results】 The mucosa at 4 different locations were successfully resected on 2 in vitro porcine bladder models. 【Conclusion】 The in vitro experiments show that the combination of flexible electronic cystoscope and laparoscopic instruments achieves synergistic effects in en-bloc resection of bladder tumor by transurethral single-hole laparoscope without additional iatrogenic bladder injury caused by percutaneous bladder incision.This method is feasible in the treatment of bladder tumors, and has the potential of clinical application after further optimization.
2.Mechanism of Biochanin A in treating gliomas based on network pharmacology and molecular docking

Objective To analyze the mechanism of Biochanin A in the treatment of Gliomas based on network pharmacology and molecular docking.Methods Traditional Chinese medicine systems pharmacology database and analysis platform(TCMSP),TargetNet,Swiss Target Prediction were used to search the active components and targets of Biochanin A.DisGeNET,GeneCards databases were used to search the corresponding targets of Gliomas.The intersection of active components of Biochanin A and gliomas target were selected to obtain the potential target of Biochanin A in treating Gliomas.Protein gene interaction data were obtained by STRING database,and protein-protein interaction network was constructed by importing into Cytoscape software.Gene ontology(GO)function and Kyoto encyclopedia of genes and genomes(KEGG)pathway enrichment analysis of the same target proteins of drug and disease were carried out by DAVID database.Molecular docking was performed by using DockThor and Pymol software.Results A total of 149 targets of Biochanin A,5654 gliomas relate-genes,97 common targets of Biochanin A and gliomas are collected.The key targets were epidermal growth factor receptor(EGFR),estrogen receptor(ESR1),heat shock protein(HSP)90AA1,matrix metalloproteinase(MMP)9,PPARG and PTGS2.The targets were mainly play an essential role in cell proliferation,invasion,cell apoptosis,and other biological pathways.GO enrichment analysis demonstrated that Biochanin A could involve the treatment of Gliomas in biological process,cell composition and molecular function.KEGG 108 signaling pathways mainly related to pathways in cancer,chemical carcinogenesis-receptor activation,Lipid and atherosclerosis,PI3K/Akt pathway.Molecular docking indicated that Biochanin A had a good bonding activity with the key targets.Conclusion Biochanin A may play a role in the treatment of glioma by inhibiting tumor cell proliferation,inducing apoptosis and enhancing chemotherapy sensitivity.The study built a foundation for drug development and innovative research.
3.Development of large-scale amplification process of rabies virus in 150 L bioreactor

Objective To develop a large-scale culture process for rabies virus(RABV)in 150 L bioreactor,and lay a foundation for the further development of a larger-scale and high-density microcarrier reactor process.Methods Vero cells and RABV strain CTN-1V were cultured in 30 L(model:C30-2)and 150 L(model:VESSEL FERMENTER 300L)bioreactors by perfused culture with 20 g/L Cytodex-1 microcarrier and DO 20%-60%,at culture temperature 36-38 ℃ and pH 7.0-7.4.During the culture process,the cell density and virus titer were measured.The virus culture media was harvested for consecutive 13 d and detected for the sterility,mycoplasma,and the residues of antigen,host cell protein(HCP),bovine serum albumin(BSA)and DNA.Results The density of cultured cells in 30 L and 150 L bioreactors all reached above 1.2 ×10~7cell/mL.There was no significant difference in cell density at different time points during the culture(t = 0.225-2.173,P = 0.096-0.833).The highest virus titer(8.5 lgLD_(50)/mL)was found in the both bioreactors 6 d after infection with no significant difference(t = 1.000,P = 0.374).The residues of antigen,HCP,BSA and DNA in the virus suspension from the two bioreactors were basically the same.Conclusion 150 L bioreactor can be used for the large-scale culture of RABV,and the harvested virus conformed to the relevant standards in Chinese Pharmacopoeia(Volume Ⅲ,2020 edition).
4.Non-target metabolomic analysis of buccal pouch squamous cell carcinoma in Chinese hamsters

Objective:To explore the metabolic basis and related molecular mechanism of oral squamous cell carcinoma(OSCC)path-ogenesis.Methods:20 Chinese hamsters were divided into 2 groups(n=10).OSCC models were induced by dimethylbenzanthracene(DMBA)in 10 of the animals and the other 10 were used as the controls.LC-MS chromatography-mass spectrometry was used to iden-tify the metabolites in the buccal pouch,and multidimensional statistical analysis of the metabolites was performed with the orthogonal partial least squares discriminant analysis model.Variable Importance for the Projection(VIP)>1 and P<0.05 were used as the criteria to screen the differential metabolites between the 2 groups.KEGG pathway annotation and enrichment analysis for the metabolites were performed to screen the significantly differential pathways.Results:The hamster cheek pouches painted with 0.5%DMBA for 18 weeks were diffused with leukoplakia and loaded obvious papillary protrusions,which were diagnosed as OSCC by pathological examination.Lipids and lipid-like molecules were the main differential metabolites.Reprogramming of unsaturated fatty acid biosynthesis,cholesterol accumulation,enhanced catabolism of tryptophan,up-regulation of aspartate,increased synthesis of pyrimidine and purine,etc.were important metabolic features in the occurrence and development of OSCC.Conclusion:Molecular intervention targeting the related met-abolic pathways is expected to inhibit OSCC pathogenesis and progression.
5.Risk factors for ultrasound-guided closed reduction failure of unstable lateral humeral condylar fractures in children

Objective:To explore the risk factors of ultrasound-guided closed reduction failure of unstable lateral humeral condylar fractures in children.Methods:A retrospective case-control study was conducted to analyze the clinical data of 158 children with unstable lateral humeral condyle fractures admitted to Anhui Provincial Children′s Hospital from January 2022 to August 2023, including 102 males and 56 females, aged 1-14 years [5(4, 8)years]. The patients were divided into reduction success group ( n=136) and reduction failure group ( n=22) according to the results of ultrasound-guided closed reduction. The gender, age, body mass index, injury side, time from injury to surgery, direction of sagittal plane displacement of the fracture fragment, Milch classification, Song classification, concomitant subcutaneous bruising or not, nighttime surgery or not, surgeons′ lack of experience or not, and concomitant elbow dislocation or not were recorded in both groups. Univariate analysis and binary Logistic regression analysis were used to evaluate and identify the independent risk factors for ultrasound-guided closed reduction failure of unstable lateral humeral condylar fractures in children. Results:Univariate analysis showed that there were significant differences in the time from injury to surgery, direction of sagittal plane displacement of the fracture fragment, and surgeons′ lack of experience or not between the reduction success group and reduction failure group ( P<0.01), while there were no significant differences in gender, age, body mass index, injury side, Milch classification, Song classification, concomitant subcutaneous bruising or not, nighttime surgery or not, or concomitant elbow dislocation or not between the two groups ( P>0.05). Binary Logistic regression analysis showed that the time from injury to surgery≥5 days ( OR=1.47, 95% CI 1.17, 1.86, P<0.01), sagittal anterior displacement of the fracture fragment ( OR=7.07, 95% CI, 1.79, 27.98, P<0.01) and surgeons′ lack of experience ( OR=4.67, 95% CI, 1.21, 18.05, P<0.05) were significantly correlated with ultrasound-guided closed reduction failure of unstable lateral humeral condylar fractures in children. Conclusion:The time from injury to surgery ≥5 days, sagittal anterior displacement of the fracture fragment and surgeons′ lack of experience are the independent risk factors for ultrasound-guided closed reduction failure of unstable lateral humeral condylar fractures in children.
6.Investigation results of drinking-water-borne endemic arsenic poisoning in Henan Province in 2019

Objective:To learn about the progress of prevention and control of drinking-water-borne endemic arsenic poisoning in Henan Province, and provide scientific basis for achieving the goal of eliminating high arsenic hazards as scheduled.Methods:From July to August 2019, in accordance with the requirements of the National Monitoring Program for Drinking-water-borne Endemic Arsenic Poisoning, a general survey was carried out in 26 high-arsenic villages in 6 counties of Henan Province to investigate the water improvement situation and the operation of water improvement projects, and the arsenic content in drinking water of households was measured, meanwhile, the arsenic poisoning status of permanent residents in high-arsenic villages was investigated.Results:All 26 villages with high arsenic content in the province had undergone water improvement, with a water improvement rate of 100.00%. A total of 18 water improvement projects were investigated in 26 high-arsenic villages, all of which were operating normally. Twenty-six water samples were collected, and the arsenic content in the household water was < 0.01 mg/L, which met the sanitary standards for drinking water. No patient of endemic arsenic poisoning was found during the survey.Conclusion:In Henan Province, the drinking-water-borne endemic arsenic poisoning has been effectively controlled, and the prevention and control achievements should continue to be consolidated in the future.
7.Analysis of monitoring results of drinking water-borne endemic fluorosis in Henan Province in 2021

Objective:To investigate the epidemic situation of drinking water-borne endemic fluorosis in Henan Province, evaluate the effectiveness of prevention and control measures, and provide a basis for formulating further prevention and control policies.Methods:From April to November 2021, a census of all diseased villages registered in 112 drinking water-borne endemic fluorosis counties (cities, districts, hereinafter referred to as counties) of 17 provincial-level cities and 9 provincial-level counties (cities) in Henan Province was conducted to collect the basic information of diseased counties and villages. The content of fluoride in drinking water and the prevalence of dental fluorosis in children aged 8 - 12 years old were measured on a village by village basis.Results:By the end of 2021, drinking water-borne endemic fluorosis was prevalent in 112 diseased counties in Henan Province, involving 17 450 diseased villages. Among them, 17 433 villages had implemented measures to reduce fluoride and improve water quality, with a water improvement rate of 99.90% (17 433/17 450). There were 14 920 villages with qualified water, and the qualified rate of water fluoride was 85.50% (14 920/17 450). There were a total of 4 232 water improvement projects in all diseased villages, with a normal operation rate of 97.09% (4 109/4 232) and a qualified rate of 93.03% (3 937/4 232) for water fluoride in the projects. The total detection rate of dental fluorosis in children in the province was 9.64% (39 391/408 584), with moderate to severe dental fluorosis accounted for 8.08% (3 182/39 391), the dental fluorosis index was 0.18. The prevalence of fluorosis was negative.Conclusions:In 2021, the prevalence of drinking water-borne endemic fluorosis is widespread in Henan Province, and measures to reduce fluoride and improve water quality in diseased villages have been basically implemented. However, the fluoride content of water in some villages still does not meet the standard, and some water improvement projects cannot operate normally throughout the year. In the future, it is necessary to address the above issues one by one and consolidate the achievements in prevention and control of drinking water-borne endemic fluorosis.

The authors summarized its main practices in promoting the integration of the management system and optimizing operating mechanism of countywide medical and health institutions, promoting the reform of the county medical and health governance system, as well as the achievements and shortcomings, with a view to providing reference for promoting the high-quality development of the countywide medical and health service system.

Femoral neck fracture (FNF) in the elderly patients is currently a major health challenge worldwide, with excessive consumption of medical resources, high incidence of complications as well as suboptimal outcome and prognosis. Hip joint arthroplasty (HJA) has been the mainstream treatment for FNF in the elderly, but the conventional surgical approaches and techniques are still confronted with a series of bottlenecks such as dislocation, limp and limb length discrepancy. In recent years, direct anterior approach (DAA) for HJA (DAA-HJA) has been a major new choice in the field of joint replacement, which achieves improved clinical effectiveness of HJA in the treatment of elderly FNF, due to the fact that DAA approach involves the neuromuscular interface and accords with the idea of soft tissue retention and enhanced recovery after surgery. However, there is still a lack of unified understanding of standard technique and procedure of DAA-HJA in the treatment of elderly FNF. Therefore, relevant experts from the Hip Joint Group of Chinese Orthopedics Association of Chinese Medical Association, Youth Arthrology Group of Orthopedic Committee of PLA, Orthopedic Committee of Chongqing Medical Association, Branch of Orthopedic Surgeons of Chongqing Medical Doctor Association and Sport Medicine Committee of Chongqing Medical Association were organized to formulate the " Chinese expert consensus on the technical standard of direct anterior hip arthroplasty for elderly femoral neck fracture ( version 2023)" based on evidence-based medicine. This consensus mainly proposed 13 recommendations covering indications, surgical plans, prosthesis selections, surgical techniques and processes, and postoperative management of DAA-HJA in elderly patients with FNF, aiming to promote standardized, systematic and patient-specific diagnosis and treatment to improve the functional prognosis of the patients.
10.Evolution of ischial thickness in unilateral developmental dysplasia of the hip in 1 - 15 years old children

Objective :
To investigate the natural evolution of the sciatic bone thickness in pediatric untreated unilateml developmental dysplasia of the hip ( DDH)aged 1-15 years.
Methods:
329 cases of DDH childrn aged 1-15 years with unilateral disloeation were retrospectively reconled. The connection lines were defined on the coronal plane or axial plane of CT. The connection lines of the Y ⁃shaped cartilage center on both sides were line H , the connection lines of the lowest edge of the ischia on both sides were line b , and the middle part of the two lines were divided into Zone 1 and Zone 2. Zone 1 represented the marginal area , and Zone 2 represented the central area. The thickness of ischium , epiphyseal plate , iliac bone thickness and epiphyseal quotient of femoral head on both sides were measured and compared.
Results :
In coronal CL1 ⁃CL4 , the ischial thickness gradually increased with age from 1 to 10 years old , and decreased from 11 to 15 years old. The range of ischial thickness of CL1 ⁃CL4 was 2. 1 - 16. 7 mm , 3. 3 - 18. 9 mm , 2. 4 - 13. 6 mm and 3. 0 - 14. 9 mm , respectively. The width of the epiphyseal plate in coronal position , the width of the epiphyseal plate in axial position , and the thickness of the iliac bone in the affected side were greater than those in the opposite side and had statistical differences. In the correlation test of ischial thickness with age and degree of dislocation , the thickness of ischial bone in coronal and axial positions was moderately correlated with age ( r = 0. 413 - 0. 570 , P < 0. 05) , and had no correlation with the degree of dislocation ( r = 0. 024 - 0. 073 , P > 0. 05) . In the correlation tests of ischial thickness and epiphyseal thickness CD , epiphyseal quotient , coronal iliac thickness IL on the affected side , the thickness of ischial bone in different parts and sections were positive ( r = 0. 427 - 0. 681 , P < 0. 05) , and the thickness of ischial bone was negatively correlated with the epiphyseal quotient ( r = 0. 130 - 0. 241 , P < 0. 05) .
Conclusion
The ischial thickness in coronal zone 1 and zone 2 of 1 - 10 years old children with unilateral DDH increased at a stable rate with age , and the growth rate decreased gradually in 11 - 15 years old. The thickness of ischia on the affected side in different sections and areas were greater than that on the opposite side. The difference in the central area of the hip joint was greater than that in the marginal area. The thickness of ischia was positively correlated with acetabular cartilage index , epiphyseal plate thickness , and coronal iliac bone thickness.
