1.Clinical characteristics and related factors of patients with schizophrenia in different ages of onset

Objective:To explore the clinical characteristics and related socio-demographic factors of schizo-phrenia patients with different ages of onset.Methods:Totally 2 016 patients with schizophrenia aged 15 to 70 were selected according to the diagnostic criteria for schizophrenia in the Diagnostic and Statistical Manual of Mental Disorders,Fifth Edition.All of the patients were interviewed by psychiatrists using the Mini International Neuropsy-chiatric Interview to diagnose schizophrenia,Clinical-Rated Dimensions of Psychosis Symptom Severity(CRDPSS)and the Positive and Negative Syndrome Scale(PANSS)to assess symptoms.The cut-off points were 18 and 25 years old for three age groups,i.e.early onset(EOS),youth onset(YOS)and adult onset(AOS).Statistical analy-ses were performed by analysis of variance Pearson correlation analysis,and multivariate linear regression.Results:The early-onset patients had the highest total PANSS score(73.8±28.0)and CRDPSS score(11.7±5.4).Fe-male gender,high education level,Han ethnicity,early onset age,and slower onset of illness were negatively corre-lated with the total and dimension score of PANSS scale and CRDPSS scale(standardized regression coefficient:0.04-0.47),and income level and smoking were negatively correlated with those score(standardized regression coefficient:-0.04--0.14).Conclusion:Early-onset schizophrenia patients have more severe symptoms,and fe-male,high education level,early-onset disease,and chronic onset are the risk factors of symptom severity in patients with schizophrenia.
2.Analysis of factors influencing premature birth in cases with placenta previa complicated by placenta ac-creta spectrum disorders

Objective To retrospectively analyze of factors influencing early preterm birth(EPB)and late preterm birth(LPB)in pregnancy women with placenta previa complicated by placenta accreta spectrum disorders(PAS),and assess maternal and infant outcomes.Methods We included 590 cases of pregnancy women with placenta previa complicated by PAS who underwent cesarean sections at five hospitals in Wuhan and Xianning cities between January 2018 and June 2021.These patients were divided into three groups based on delivery gesta-tional age:EPB,LPB,and term birth(TB).A multiple logistic regression model was employed to analyze the risk factors associated with EPB and LPB.Additionally,differences in early maternal and infant outcomes among these groups were examined.Results Among 590 pregnancy women with placenta previa complicated by PAS,the proportions of EPB and LPB were 9.7%and 54.4%.The use of uterine contraction inhibitors prior to cesarean section,vaginal bleeding,and previous cesarean sections history were identified as risk factors for both EPB and LPB.The proportion of severe postpartum hemorrhage was comparable between the EPB group and the LPB group;however,the incidence of neonatal asphyxia,low birth weight infants,and the rate of newborns transferred to the Neonatal Intensive Care Unit(NICU)within 24 hours after cesarean delivery were significantly higher in the EPB group compared to the LPB group.Conclusions Placenta previa complicated by PAS predominantly leads to LPB.The history of prior cesarean sections,uterine contractions,and vaginal bleeding prior to cesarean section,are sig-nificantly associated with both EPB and LPB.During the perinatal period,efforts should be made to extend gesta-tional weeks under close monitoring to minimize the incidence of premature births and thereby improve early mater-nal and infant outcomes.
3.Efficacy and safety of vancomycin in the treatment of infections caused by gram-positive coccus in children

Objective To analyze the concentration and exposure of vancomycin in children with gram-positive coccal infection,and the corresponding clinical efficacy and safety to support rational use of vancomycin in children.Methods We prospectively collected the clinical and laboratory data of 87 children with gram-positive coccal infection in the Children's Hospital of Fudan University from January 2012 to March 2021.Therapeutic drug monitoring(TDM)was conducted for vancomycin simultaneously,to acquire the data of serum through concentration(Cmin),peak concentration(Cmax),the area under the drug concentration-time curve in a 24-h interval(AUC0-24h)and the ratio ofAUC0-24h to the minimum inhibitory concentration(AUC0-24h/MIC).Results The median(P25,P75)age of the children enrolled in this study was 3.60(1.20,20.00)months.The median dose of vancomycin was 39.23(30.00,46.51)mg/kg.The median serum Cmin was 3.30(1.50,7.10)mg/L.Cmin achieved the target(5-15 mg/L)in 23 cases(26.4%).The median AUC0-24h was 213(174,293)mg·h/L and the median AUC0-24h/MIC was 221(128,349).Adaily dose of above 60 mg/kg in children could achieve the median value of AUC0-24h and AUC0-24h/MIC greater than 400,and the corresponding median age was 28.50(6.85,36.00)months.Multivariate logistic analysis showed a good correlation between Cmin and AUC0-24h(P=0.002).At the end of treatment,the clinical efficacy rate was 85.1%(74/87)and the bacterial eradication rate was 95.4%(83/87).No renal injury occurred during the treatment.Conclusions In this study,the median daily dosage,Cmin and A UC0-24h/MIC of vancomycin were below the recommended range at home and abroad.However,good clinical and microbiological efficacy were achieved in children at low vancomycin exposure.The probability of target attainment(PTA)for A UC0-24h and AUC0-24h/MIC ≥400 increased when the daily dose of vancomycin was>60 mg/kg(corresponding to the median age of 28.50 months)or Cmin ≥ 5 mg/L.
4.Effect of basic psychological needs satisfaction on phubbing behavior among high school students: the mediating effect of fear of missing out and mobile phone addiction

Objective:To investigate the mediating roles of the fear of missing out and mobile phone addiction between the basic psychological needs satisfaction and phubbing behavior among high school students.Methods:In April 2022, a cross-sectional design survey was conducted on 14 666 high school students. All participants were evaluated by the basic psychological needs scales(BPNS), generic scale of phubbing(GSP), trait-state fear of missing out scale(T-S FOMOS) and mobile phone addiction index(MPAI). The SPSS 26.0 software was used to conduct common method deviation test, descriptive statistics, and correlation analysis.PROCESS 4.1 was used to construct the model, and the Bootstrap method was used to test for mediating effects.Results:(1)Among the 14 036 high school students, there were 1 752 (12.48%) students who were addicted to mobile phones.There were significant differences in gender in the scores including BPNS(boy: 4.43±0.79, girl: 4.36±0.79), GSP(boy: 2.72±1.01, girl: 2.76±1.03) and T-S FOMOS(boy: 1.73±0.60, girl: 1.84±0.64), ( t=5.22, -10.58, -2.78, all P<0.01). Among different grades, there were significant differences in the scores of BPNS, T-S FOMOS, MPAI, and GSP( F=25.43, 39.50, 53.45, 14.59, all P<0.01). (2)Basic psychological needs score were positively correlated with fear of missing out, mobile phone addiction and phubbing( r=-0.432--0.294, all P<0.01). Phubbing were negatively correlated with fear of missing out and mobile phone addiction( r=0.744, 0.538, both P<0.01). Fear of missing out were negatively correlated with mobile phone( r=0.646, P<0.01). (3)The basic psychological needs satisfaction had a direct effect on phubbing behavior, and the effect value was -0.188 (95% CI: -0.173--0.204). The mediating effect of fear of missing out between the basic psychological needs satisfaction and phubbing behavior was -0.035(95% CI: -0.028--0.042). The mediating effect of mobile phone between the basic psychological needs satisfaction and phubbing behavior was -0.203(95% CI: -0.191--0.214). Fear of missing out and mobile phone addiction played a chain mediating role between them, and the mediating effect value was -0.134(95% CI: -0.125--0.143), which accounted for 23.93%(-0.134/-0.560) of the total effect. Conclusion:The high level basic psychological needs satisfaction can alleviate the occurrence of phubbing behavior. It may be achieved by decreasing fear of missing out and reducing mobile phone addiction.
5.Pharmacokinetics of sugammadex in reversal of rocuronium-induced muscle relaxant residual in infants and young children undergoing daytime surgery

Objective:To investigate the pharmacokinetics of sugammadex in reversal of rocuronium-induced muscle relaxant residual in infants and young children undergoing daytime surgery.Methods:One hundred and four pediatric patients of either sex, aged 3-36 months, of American Society of Anesthesiologists Physical Status classification Ⅱ, with body mass index of 18.5-28.0 kg/m 2, diagnosed with oblique inguinal hernia and/or hydrocele, scheduled for laparoscopic high ligation of hernia sac and/or high ligation of sphingoid surgery, were included in the study. Intraoperative neuromuscle relaxation was assessed by transdermal stimulation of the ulnar nerve in the wrist using a TOF Guard monitor. Rocuronium 0.9 mg/kg, propofol 3 mg/kg, and sufentanyl 0.5 μg/kg were intravenously injected for anesthesia induction, and propofol 6-8 mg·kg -1·h -1 was intravenously infused to maintain anesthesia. The pediatric patients were divided into Ⅰgroup and Ⅱ group according to the degree of postoperative neuromuscular block. In group Ⅰ, sugammadex 2 mg/kg was intravenously injected when TOF returned to T 2 recurrence. In group Ⅱ, sugammadex 4 mg/kg was intravenously injected when the single stimulation count was 1 or 2 after tetanic stimulation. At 2 and 10 min after rocuronium administration, at the end of operation, 2 and 10 min after sugammadex administration, and when the children met the standard of leaving the resuscitation room, venous blood samples were collected for determination of plasma concentrations of rocuronium and sugammadex using ultra-high performance liquid chromatography-mass spectrometry. Pharmacokinetic parameters were determined using the Pheonix WinNonlin software. The onset of rocuronium and time for recovery of TOF ratio to 90% were recorded. Results:The pharmacokinetics of sugammadex was fitted to the nonlinear mixed-effect satrioventricular model.There was no significant difference in the peak concentration, area under the drug concentration-time curve, elimination half-life, apparent clearance, apparent volume of distribution, mean retention time, and time for TOF ratio returning to 90% between the two groups ( P> 0.05). Conclusions:The pharmacokinetics of sugammadex in reversal of rocuronium-induced muscle relaxant residual is fitted to a nonlinear mixed-effect satrioventricular model, and sugammadex 2 and 4 mg/kg have similar pharmacokinetics in infants and young children undergoing daytime surgery.
6. Establishment of Geographic Information System on risk assessment regarding infectious diseases imported to China

Objectives:
To establish a geographic information application system for analyzing the spatial and temporal distribution of major infectious diseases in various regions of the world and to assess the risk of importation of those diseases, to China.
Methods:
We collected and integrated the following information on: 1) outbreaks and areas of epidemics of major infectious diseases in the world from 2000 to 2017, 2) cases of infectious diseases in arriving travelers through active surveillance at international entry-exit ports in mainland China from 2014 to 2016, 3) numbers of annual global international flights and travelers in the country. With the above information, a global space-time distribution database on major infectious diseases was then established, using the technology related to the system. Models regarding technologies on time-space analysis, probabilistic risk assessment and geographic information visualization, were applied to establish a geographic information system on risk assessment of infectious diseases that imported to China.
Results:
Through integration of information on outbreaks and epidemic areas of 60 major infectious diseases in 220 countries and regions around the world, as well as 42 kinds of infectious diseases identified among the international arrivals in mainland China, a system was then developed. Information on the distribution of major infectious diseases and their potential risks in the worldwide various regions, characteristics of spectrum and disease burden of infectious diseases imported to each province of mainland China were displayed. Thus, risks on importing infectious diseases in each province via air way were able to be evaluated and simulated by the probabilistic risk assessment model, under the information on specific kind of infectious disease, outside China.
Conclusion
Geographic Information System on Risk Assessment Regarding Infectious Diseases Imported to China provides basic data for epidemiological reconnaissance and assessment on risks of importing infectious diseases outside China, thus would be helpful for the improvement of strategies on surveillance, prevention and control regarding the importing infectious diseases, in China.
7.Analysis of prognostic factors in patients with hepatocellular carcinoma complicated by diabetes mellitus after transcatheter arterial chemoembolization

Objective To analyze the effect of diabetes mellitus (DM) on the prognosis of patients with unresectable hepatocellular carcinoma (HCC) after receiving transcatheter arterial chemoembolization (TACE).Methods A total of 858 HCC patients,who were admitted to the Affiliated Xinhua Hospital,School of Medicine,Shanghai Jiaotong University,China,during the period from January 2010 to June 2015 to receive TACE,were selected.Among the 858 HCC patients,175 patients suffered from coexisting DM (DM group) and 683 patients had no DM (non-DM group).The differences in general clinical data and overall survival between the two groups were analyzed.Kaplan-Meier method was used to calculate the survival rate,the difference between the two groups was evaluated by log-rank method,and Cox proportional hazards regression was adopted to analyze the prognostic factors of HCC patients after receiving TACE.Results There were no statistically significant differences in sex,age,serum albumin,total bilirubin,prothrombin time,alpha fetoprotein (AFP),Child-Pugh grade,tumor size and number,cirrhosis,number of TACE treatment times and BCLC staging between the two groups (P>0.05);the fasting blood glucose (FPG)level in DM group was higher than that in non-DM group (P<0.05).The 1-,3-and5-year survival rates of the DM group were 60.9%,27.5% and 10.7% respectively,which were 70.9%,36.0% and 17.6% respectively in the non-DM group;the Log-rank test showed that the difference between the two groups was statistically significant (P=0.008).Multivariate Cox proportional hazard regression analysis revealed that the largest tumor diameter >5 cm,multiple tumor lesions,AFP>20 ng/ml,BCLC stage and coexisting DM were independent risk factors that affected the prognosis of HCC patients after receiving TACE.Conclusion The coexisting DM is an independent risk factor that may affect the prognosis of patients with inoperable HCC after receiving TACE.
8.Correlations between the expressions of Smad4, estrogen receptor and the clinicopathological features of breast cancer

Objective To explore the expressions of Smad4 and estrogen receptor (ER) and their interrelation,and the relationship with the clinicopathological features of breast cancer.Methods The immunohistochemical SP method was used to detect the expressions of Smad4 and ER in 50 case of invasive cancer,12 cases of carcinoma in situ and 15 cases of normal breast tissues.The differences in different clinical stages,differentiation degrees and nodal metastases were analyzed.The correlation between Smad4 and ER was explored.Results The positive expression rate of Smad4 in invasive cancer was 52.00%,which lower than that in normal breast tissue (93.33%),with a significant difference (x2 =8.329,P =0.004),positive expression rates of ER were 60.00% and 40.00% respectively,with no significant difference (x2 =1.868,P =0.172).The positive expression rates of Smad4 in carcinoma in situ and invasive cancer were 75.00% and 52.00% respectively,with no significant difference (x2 =2.082,P =0.149).The positive expression rates of ER were 58.33% and 60.00% respectively,with no significant difference (x2 =0.011,P =0.916).The positive expression of Smad4 was related to the TNM stage (x2 =6.392,P =0.011) and the lymph node metastasis (x2 =6.738,P =0.009),but it was not associated with the histologic grade (x2 =0.542,P =0.462).The positive expression of ER was related to the lymph node metastasis (x2 =4.133,P =0.042) and histologic grade (x2 =5.357,P =0.021),but it was not associated with the TNM stage (x2 =1.159,P =0.282).There was positive correlation between Smad4 and ER in breast cancer tissue (r =0.263,P =0.032).Conclusion Smad4 is expressed at lower level in breast cancer than in normal breast tissue.The expressions of Smad4 and ER are related to the different clinicopathological features of breast cancer with positive correlation.
9.Initial study on correlation between C -reactive protein and acute cerebral infarction

Objective To find out the correlation of C -reactive protein (CRP)and acute cerebral infarc-tion,and to provide research data for acute cerebral infarction.Methods 43 cases with acute cerebral infarction were set as the observation group,and 40 healthy people were set as the control group.The CRP level of the observation group and the control group,and CRP level of different infarction volume and neurological function defect degree were compared.The influence factors of CRP level were analyzed with single factor analysis and Logistic regression.Results The CRP level of the observation group was (14.19 ±2.60)mg/L,which was significantly higher than that of the control group (t =7.453,P <0.05).With the increase of infarction volume,CRP levels increased (t =3.451,8.534, 5.930,all P <0.05).With the increase of nerve function defect degree,CRP level elevated (t =3.845,9.077, 6.730,all P <0.05).CRP level and systolic blood pressure,fasting glucose,total cholesterol,fibrinogen were positively correlated (r =0.479,0.603,0.508,0.603,all P <0.05 ).Cerebral infarction,systolic blood pressure,fasting glucose,total cholesterol and fibrinogen were the influence factors of CRP level.Conclusion There is correlation between C -reactive protein and acute cerebral infarction;detecting CRP level can assist in diagnosis of cerebral infarction.It was favorable to prevent and control the occurrence and progress of acute cerebral infarction.
10.Therapeutic Observation of Acupuncture at the Interiorly-exteriorly Related Meridians plus Rehabilitation Training for Post-stroke Shoulder-hand Syndrome

Objective To observe the therapeutic efficacy of acupuncture at the interiorly-exteriorly related meridians plus rehabilitation training in treating post-stroke shoulder-hand syndrome. Methods Sixty eligible patients with post-stroke shoulder-hand syndrome were randomized into a treatment group and a control group, 30 in each group. The treatment group was intervened by acupuncture at the interiorly-exteriorly related meridians plus rehabilitation training, while the control group was by rehabilitation training alone. After 2 treatment courses, the Fugl-Meyer Assessment (FMA), visual analogue scale (VAS) and edema score were observed and compared. Results The FMA, VAS, and edema scores were significantly changed in both groups after intervention (P<0.01). There were significant differences in comparing the FMA, VAS, and edema scores between the two groups after intervention (P<0.05). The total effective rate was 83.3%in the treatment group versus 76.7%in the control group, and the difference was statistically significant (P<0.05). Conclusion Acupuncture at the interiorly-exteriorly related meridians plus rehabilitation is an effective method in treating post-stroke shoulder-hand syndrome.
