1.Predictive value of early thyroid function changes for the curative effect of 131I therapy in patients with Graves′ disease

Objective:To investigate the predictive value of early thyroid function changes on the efficacy of patients with Graves′ disease (GD) after 131I therapy. Methods:Data of patients with GD (59 males, 214 females; age (37.4±11.4) years) who underwent single therapy of 131I in Tianjin Medical University General Hospital from November 2017 to January 2019 were retrospectively analyzed. Symptoms, signs and laboratory tests (serum free triiodothyronine (FT 3) and serum free thyroxine (FT 4)) of patients were observed to assess the efficacy of 131I treatment. Efficacy was divided into complete remission (CR), partial remission (PR), non-remission (NR) or relapse. The changes of thyroid function (ΔFT 3=FT 3 before treatment-FT 3 after treatment)/FT 3 before treatment×100%; ΔFT 4=FT 4 before treatment-FT 4 after treatment)/FT 4 before treatment×100%) 1 month after 131I therapy in each efficacy group and differences among them were compared by using independent-sample t test, χ2 test, one-way analysis of variance and the least significant difference t test. ROC curves were drawn to analyze the predictive values of early thyroid function changes on the efficacy of 131I treatment for GD. Logistic regression analyses were performed to identify the influencing factors for the efficacy of 131I therapy. Results:CR rate and total effective rate of 273 GD patients after single therapy of 131I were 67.03%(183/273) and 92.67%(253/273), respectively. After 1 month, CR rate of euthyroidism group ( n=95) was significantly higher than that of hyperthyroidism group ( n=178; 81.05%(77/95) vs 59.55%(106/178); χ2=4.60, P=0.032). ΔFT 3 and ΔFT 4 at the first month were statistically significant and decreased sequentially in the CR group ( n=183), PR group ( n=70), NR or relapse groups ( n=20; F values: 15.40, 12.54, both P<0.001). ROC curve analysis showed that patients with ΔFT 3≥73.64% and (or) ΔFT 4≥59.03% had a higher probability of achieving CR, with sensitivities of 84.3% and 86.7%, and specificities of 62.6% and 62.6%, respectively. Logistic regression analysis showed that 24 h radioactive iodine uptake (odds ratio ( OR)=1.095, 95% CI: 1.031-1.139), dose of 131I given per gram of thyroid tissue ( OR=1.562, 95% CI: 1.321-1.694), ΔFT 3 ( OR=1.354, 95% CI: 1.295-1.482), ΔFT 4 ( OR=1.498, 95% CI: 1.384-1.608) were factors affecting the outcome of patients with GD treated with 131I treatment (all P<0.05). Conclusion:Effects of 131I treatment can be predicted based on the change of the thyroid function at the first month after 131I treatment in patients with GD.
2.Genetic analysis of a child with Complex cortical dysplasia with other brain malformations type 6 due to a p.M73V variant of TUBB gene.

OBJECTIVE:
To explore the genetic basis for a child with multiple malformations.
METHODS:
A child who had presented at Shanxi Provincial Children's Hospital in February 2021 was selected as the study subject. Clinical data of the patient was collected, and whole exome sequencing (WES) was carried out to screen pathogenic variants associated with the phenotype. Candidate variant was validated by Sanger sequencing of her family members.
RESULTS:
The child had normal skin, but right ear defect, hemivertebral deformity, ventricular septal defect, arterial duct and patent foramen ovale, and separation of collecting system of the left kidney. Cranial MRI showed irregular enlargement of bilateral ventricles and widening of the distance between the cerebral cortex and temporal meninges. Genetic testing revealed that she has harbored a heterozygous variant of NM_178014.4: c.217A>G (p.Met73Val) in the TUBB gene, which was unreported previously and predicted to be likely pathogenic based on the guidelines from the American College of Medical Genetics and Genomics (ACMG). The child was diagnosed with Complex cortical dysplasia with other brain malformations 6 (CDCBM6).
CONCLUSION
CDCBM is a rare and serious disease with great genetic heterogeneity, and CDCBM6 caused by mutations of the TUBB gene is even rarer. Above finding has enriched the variant and phenotypic spectrum of the TUBB gene, and provided important reference for summarizing the genotype-phenotype correlation of the CDCBM6.

3.Correlation between depression, anxiety and social support among grade three senior high school students under in-school quarantine during the COVID-19 pandemic

ObjectiveTo assess the level of depression, anxiety and social support among grade three senior high school students under in-school quarantine during the COVID-19 pandemic, so as to provide references for targeted psychological intervention of them. MethodsA total of 724 grade three senior high school students from three key schools in Binzhou City, Shandong Province were selected using random sampling method. Generalized Anxiety Disorder (GAD-7), Patients' Health Questionnaire Depression Scale-9 item (PHQ-9) and Perceived Social Support Scale (PSSS) were used to survey the psychological status including anxiety, depression and the level of social support. Partial correlation analysis was adopted to discuss the correlation of PSSS score with GAD-7 and PHQ-9 scores, meantime, the multiple linear regression analysis was conducted to explore the relationship between PSSS and PHQ-9 scores. ResultsAmong grade three senior high school students, 341 (48.64%) and 383 (54.64%) were found to have symptoms of anxiety and depression, respectively. PSSS total score as well as the dimension score of family support and other support of female students were higher than those of male students, with statistical significance (t=-3.431, -3.048, -4.482, P<0.01). GAD-7, PSSS total scores and the dimension scores of family support and other support demonstrated statistical differences among grade three senior high school students with different selections of academic subjects for college entrance examination (F=1.992, 2.660, 2.074, 3.138, P<0.05 or 0.01). PHQ-9 score was negatively correlated with the total score and each dimension score of PSSS (r=-0.329, -0.283, -0.284, -0.284, P<0.01). The dimension score of family support and friend support in PSSS could negatively predict the PHQ-9 score (B=-0.213, -0.308, P<0.01). ConclusionGrade three senior high school students under in-school quarantine during the COVID-19 pandemic suffer a high prevalence rate of anxiety and depression, while students with higher perceived social support scores report fewer symptoms of depression. Hence, improving the level of social support may alleviate depression in students.
4.Re-evaluation of systematic reviews of application effect of virtual reality technology in pain management of children

Objective:To re-evaluate the systematic reviews of the effectiveness of virtual reality technology in pain management in children.Methods:Using computers to search for systematic reviews of the effectiveness of virtual reality technology in pain management for pediatric patients in China National Knowledge Infrastructure (CNKI), Wanfang Database, VIP, China Biology Medicine disc, Cochrane Library, PubMed, Embase, CINAHL and Web of Science. The retrieval period was from the establishment of the databases to November 4, 2022. Literature screening and data extraction were conducted independently by two researchers, methodological quality evaluation was performed for the included studies using the A Measurement Tool to Assess Systematic Reviews 2 (AMSTAR 2) checklist, and the evidence quality rating was evaluated using Grades of Recommendations Assessment, Development and Evaluation (GRADE) system.Results:A total of 14 systematic reviews were included. AMSTAR 2 evaluation results showed that the methodological quality of 2 systematic reviews was low, and 12 were extremely low. GRADE was used to evaluate the quality of evidence of the outcome indicators, and the results showed that among the 26 outcome indicators included, 23.08% (6/26) were medium evidence, 76.92% (20/26) were low evidence, and the quality of no evidence was high.Conclusions:There is a lack of high-quality evidence to support the conclusion that virtual reality technology can relieve pain in children. The methodological quality and evidence quality of systematic reviews are low, and more standardized and rigorous studies need to be carried out in the future to further verify the application effect of virtual reality technology in pain management of children.
5.Establishment of the fingerprint of Temurin- 5 powder and content determination of 4 components

OBJECTIVE To establish the fing erprint of Temurin- 5 powder,conduct chemical pattern recognition analysis ,and determine the contents of 4 components simultaneously. METHODS The fingerprints of 10 batches of Temurin- 5 powder were established and similarity evaluation was performed by using high performance liquid chromatography (HPLC)combined with the Similarity Evaluation System of Chromatographic Fingerprints of Traditional Chinese Medicine (2012 edition);common peaks were identified by comparing with mixed substance control. The common peaks were analyzed by systematic cluster analysis and principal component analysis with SPSS 26.0 software. The HPLC method was used to determine the contents of gallic acid , geniposide,chlorogenic acid and ellagic acid in 10 batches of samples. RESULTS A total of 15 common peaks were identified from the fingerprints of 10 batches of Temurin-5 powder,and the similarity was 0.997-0.999. It was identified that peak 1 was gallic acid ,peak 3 was geniposide ,peak 5 was chlorogenic acid and peak 12 was ellagic acid. Among the 10 batches of samples , S4 and S 9 were grouped into one category ,S6-S8 were grouped into one category ,and the other batches of samples were grouped into one category. The accumulative variance contribution rate of first three principal components was 89.245%. The linear ranges of gallic acid ,geniposide,chlorogenic acid and ellagic acid were 5.55-177.5,15.98-511.5,2.56-82.0 and 13.48-431.5 μg/mL, respectively. RSDs of precision ,stability(24 h)and repeatability tests were all less than 2%(n=6 or n=7). The average recoveries were 101.56%,102.21%,98.60% and 96.62%,respectively,RSDs were 1.90%,1.61%,1.58% and 1.73%(n=6). Average contents of above components were 5.03-5.64,10.38-12.16,1.40-1.69,6.47-7.11 mg/g,respectively. CONCLUSIONS The established fingerprint is stable and feasible ,and the content determination method meets the relevant regulations. Combined with chemical pattern recognition analysis ,it can be used for the quality control of Temurin- 5 powder.
6.Adult linear scleroderma with ptosis: a case report and literature review

Objective:To discuss the clinical features, diagnosis and treatment of linear scleroderma (LS).Methods:A case of LS diagnosed in the Second Hospital of Shandong University in October 20, 2020, was reported and the clinical features and pathological documentation of the disease reported in the literature were reviewed.Results:A 24-year-old woman presented cicatricial alopecia on the left frontoparietal area and facial atrophy for about 10 years. Two years before, she began to suffer ptosis and neurological complaints. Clinical features of different stages of the disease are presented. All 15 patients reported in the literature were analyzed, with a median of 22 years and a male to female ratio of 9∶6. There were 4 cases of linear scleroderma with ipsilateral drooping eyelids and lateral contraction, 3 cases of linear scleroderma with demyelinating lesions, combined with lateral contraction, 3 cases of linear scleroderma combined with lateral atrophy, and 1 case of linear scleroderma with ipsilateral facial spasm. Two cases were with the chest sclerosing spot. Two cases of linear scleroderma were with epileptic seizure and white matter demyelination lesion. Six cases were treated with hormone, 2 cases were treated with methotrexate. One case was treated with both hormone and methotrexate. One case was treated with botulinum toxin. Three cases were treated with surgical correction of eyelid ptosis. One case was treated with ultraviolet A1 radiation phototherapy and 1 case was treated with vitamin therapy.Conclusions:Patients with scleroderma may have ipsilateral facial atrophy, blepharoptosis and facial spasm. Some patients involving the nervous system may have epilepsy and myelitis. And demyelinating lesions can be seen in magnetic resonance imaging. Localized scleroderma may develop into systemic scleroderma. Therefore, it is recommended to combine immunosuppressants as soon as possible to control the development of the disease if necessary.
7.Research on electroencephalogram power spectral density of stroke patients under transcranial direct current stimulation.

Transcranial direct current stimulation (tDCS) has become a new method of post-stroke rehabilitation treatment and is gradually accepted by people. However, the neurophysiological mechanism of tDCS in the treatment of stroke still needs further study. In this study, we recruited 30 stroke patients with damage to the left side of the brain and randomly divided them into a real tDCS group (15 cases) and a sham tDCS group (15 cases). The resting EEG signals of the two groups of subjects before and after stimulation were collected, then the difference of power spectral density was analyzed and compared in the band of delta, theta, alpha and beta, and the delta/alpha power ratio (DAR) was calculated. The results showed that after real tDCS, delta band energy decreased significantly in the left temporal lobes, and the difference was statistically significant ( P < 0.05); alpha band energy enhanced significantly in the occipital lobes, and the difference was statistically significant ( P < 0.05); the difference of theta and beta band energy was not statistically significant in the whole brain region ( P > 0.05). Furthermore, the difference of delta, theta, alpha and beta band energy was not statistically significant after sham tDCS ( P > 0.05). On the other hand, the DAR value of stroke patients decreased significantly after real tDCS, and the difference was statistically significant ( P < 0.05), and there was no significant difference in sham tDCS ( P > 0.05). This study reveals to a certain extent the neurophysiological mechanism of tDCS in the treatment of stroke.

8.Research on characteristics of brain functional network in stroke patients during convalescent period under transcranial direct current stimulation.

Transcranial direct current stimulation (tDCS) is an emerging non-invasive brain stimulation technique. However, the rehabilitation effect of tDCS on stroke disease is unclear. In this paper, based on electroencephalogram (EEG) and complex network analysis methods, the effect of tDCS on brain function network of stroke patients during rehabilitation was investigated. The resting state EEG signals of 31 stroke rehabilitation patients were collected and divided into stimulation group (16 cases) and control group (15 cases). The Pearson correlation coefficients were calculated between the channels, brain functional network of two groups were constructed before and after stimulation, and five characteristic parameters were analyzed and compared such as node degree, clustering coefficient, characteristic path length, global efficiency, and small world attribute. The results showed that node degree, clustering coefficient, global efficiency, and small world attributes of brain functional network in the tDCS group were significantly increased, characteristic path length was significantly reduced, and the difference was statistically significant (

9.Construction and implementation of training base for anesthesia specialized nurses based on IFNA evaluation criteria

Based on the International Federation of Nursing Anesthetists (IFNA) education and training base, our hospital has built a training base for anesthesia specialized nurses in Nanjing from the following aspects: the application for training base of anesthesia specialized nurses, the qualification and examination of students' and teachers' qualification, the settings of training curriculum, the examination contents and methods, and the evaluation of post-training effect. This article summarizes the construction experience of this base, therefore, providing support and standard for the training of anesthesia specialized nurses.
10.Effects of suspended leukocyte depleted red blood cell infusion on HO-1 expression and T lymphocyte subsets in peripheral blood of patients with MDS

Objective:To investigate the effect of suspended leukocyte poor red blood cell transfusion on hemeoxygenase-1 (HO-1) expression and T lymphocyte subsets in peripheral blood of patients with myelodysplastic syndrome.Methods:50 cases of MDS patients treated in Shandong Blood Center from January 2016 to December 2019 were selected as the research object. According to the different infusion of blood components, they were randomly divided into the observation group and the control group, with 25 cases in each group.The patients in the observation group were treated with s suspended leukocyte poor red blood cell transfusion, and the patients in the control group were treated with washing red blood cell infusion.The level of HO-1, interleukin-6 (IL-6) and TNF-α and the T cell subset such as CD 3+, CD 4+, CD 8+ and CD 4+/CD 8+ in the serum before and after transfusion were tested by enzyme linked immunosorbent assay and flow cytometry, respectively. Results:There was no statistically significantdifference in the level of HO-1, IL-6 and TNF-α in the serum of the two groups before transfusion ( P>0.05); After transfusion, TNF-α in the serum of the observation group significantly decreased, compared with that in control group: (152.10 ± 21.89) ng/L vs. (201.14 ± 28.90) ng/L, (1.34 ± 0.45) ng/L vs. (2.89 ± 1.01) ng/L. However, the HO-1 significantly increased: (78.91 ± 15.74) μg/L vs. (58.99 ± 13.33) μg/L, andthe difference was statistically significant ( P<0.05). There was nostatistically significantdifference in the immunologic functions of the two groups before transfusion ( P>0.05); After transfusion, CD 4+/CD 8+of the observation group both significantly increased, compared with that inthe control group: (49.11 ± 19.13)% vs. (47.13 ± 12.84)%, (25.23 ± 10.80)% vs. (21.09 ± 12.28)%, (24.74 ± 10.84)% vs. (21.88 ± 11.18)%, 1.02 ± 0.25 vs. 0.96 ± 0.20, and the difference was statistically significant ( P<0.05). Conclusions:The expression level of HO-1 in peripheral blood and immune function of patients with myelodysplastic syndrome can be improved by suspended leukocyte poor red blood cell transfusion.
