1.Evolution of Medication Patterns in Traditional Chinese Medicine for Treating Inflammatory Bowel Disease

		                        			ObjectiveTo explore the evolution of medication patterns and syndrome-herb associations of traditional Chinese medicine (TCM) in treating inflammatory bowel disease (IBD), providing a theoretical foundation for precise syndrome differentiation and treatment in clinical practice. MethodsMedical case literature on TCM treatment of IBD from 1960 to 2024 was retrieved to establish a database. Frequency statistics, cluster analysis, change point detection, and association rule mining were employed to comprehensively analyze the syndrome distribution, therapeutic methods, medication patterns, and their temporal variations. ResultsA total of 685 medical cases were included. Common syndromes were dampness-heat (66.42%) and spleen deficiency (56.20%). Primary therapeutic methods included heat clearing (63.65%), spleen invigorating (47.45%), and dampness draining (36.79%). High-frequency herbs included Coptidis Rhizoma (354), Paeoniae Radix Alba (303), Aucklandiae Radix (292), Codonopsis Radix (253), and Glycyrrhizae Radix et Rhizoma (244). Initial prescription clustering revealed three core therapeutic method combinations: heat clearing and detoxifying (represented by Baitouweng Tang), spleen invigorating and Qi reinforcing (represented by Shenling Baizhusan), and cold-heat regulation (represented by Wumeiwan combined with Shaoyao tang). Temporal analysis identified 2008 as a key transition point in TCM treatment of IBD, with significantly increased usage frequency of heat-clearing and dampness-drying herbs such as Fraxini Cortex, Phellodendri Chinensis Cortex, Sophorae Flavescentis Radix, and Scutellariae Radix as well as hemostatic herbs such as carbonized Sanguisorbae Radix, Bletillae Rhizoma, Agrimoniae Herba, and Notoginseng Radix et Rhizoma. Follow-up efficacy analysis showed median improvement rates of 64.0% at the first follow-up, 76.0% at the second follow-up, and 78.7% at the third follow-up. Syndrome-drug association analysis revealed specific herb pairs with significant therapeutic advantages, including Notoginseng Radix et Rhizoma + Coicis Semen, Sanguisorbae Radix + Coptidis Rhizoma, and Codonopsis Radix + Aconii Lateralis Radix Praeparaia. ConclusionTCM medication patterns for treating IBD demonstrate distinct temporal evolution characteristics, with significantly increased usage frequency of herbs such as Fraxini Cortex, Notoginseng Radix et Rhizoma, and Agrimoniae Herba. Significant therapeutic method-herb associations and syndrome-herb association patterns exist, with the formation of specific herb pairs, providing evidence-based support for precise syndrome differentiation and treatment of IBD. 
		                        		
		                        		
		                        		
		                        	
2.Study of Lkb1 regulates epithelial regeneration in asthma using airway organoid

		                        			Objective To explore the mechanism of Lkb1 regulated epithelial regeneration in asthma by airway organoid culture.Methods Lkb1f/f(the control group,n=10)and Scgb1a1CreER;Lkb1f/fmice(the Lkb1 knockout group,n=9)were taken to establish allergic asthma models by aerosol inhalation of ovalbumin(OVA).Bronchial lavage fluid(BALF)and lung tissue were collected.The number of inflammatory cells in BALF were counted.The amount of CLCA3 positive cells was compared by immunofluorescence staining of lung tissue sections.Club cells were selected by flow cytometry for organoid culture.The average diameter of organoids and organoid formation rate were calculated.Expression levels of goblet cell marker CLCA3,cilia cell markers FOXJ1 and AMPK in Club cells were detected by RT-PCR.Results There were no significant differences in the number of macrophages,eosinophils,neutrophils and lymphocytes in BALF between the control group and the Lkb1 knockout group.The number of CLCA3 positive cells were decreased after Lkb1 knockout.Results of organoid culture showed that the average diameter of organoids derived from Club cells and organoid formation rate were decreased after the absence of Lkb1.The expression of FOXJ1 was reduced.After Lkb1 deletion,the expression of AMPKα in Club cells were decreased and the proliferation of Club cells was inhibited.Activation of AMPK,the downstream signaling pathway of Lkb1,could attenuate the effect of Lkb1 deficiency on the regeneration of Club cells.Conclusion Lkb1 promotes the proliferation of airway progenitor cells by AMPK pathway.
		                        		
		                        		
		                        		
		                        	
3.Discussion of Hp(3) calibration with two thermoluminescent dosimeters in the same standard X-ray RQR radiation field

		                        			Objective To compare Hp(3) calibration with a homemade (A) thermoluminescent dosimeter (TLD) and an imported (B) TLD in a standard X-ray RQR radiation field, to explore the different responses of A and B, and to provide foundation for the calibration of Hp(3). Methods A column mode was selected. Hp(3) calibration was performed using A and B in a standard X-ray RQR radiation field in the Secondary Standard Dosimetry Laboratory, National Institute for Radiological Protection, China Center for Disease Control and Prevention. Angle response, energy response, and linear response were calibrated with RQR4 (60 kV), RQR7 (90 kV), and RQR9 (120 kV), respectively. Results In terms of angle response, the calibration results of A were relatively high, while the calibration results of B were relatively low. In terms of energy response, the calibration results showed a similar pattern to angle response. In terms of linear response, the calibration results of both A and B were satisfactory. Conclusion Both A and B can be used for normal calibration of Hp(3) in a standard X-ray RQR radiation field. 4.Establishment of a method for rapid detection of the minimum inhibitory concentration of imipenem in KPC-producing Klebsiella pneumoniae based on ompK36 mutation

		                        			Objective:To establish a rapid method to detect the minimum inhibitory concentration (MIC) of imipenem in Klebsiella pneumoniae carbapenemase-producing Klebsiella pneumoniae (KPC-Kp) based on ompK36 gene′s GD mutation. Methods:This was a methodological evaluation study. A total of 258 isolates of Klebsiella pneumoniae were collected from Lishui Municipal Central Hospital from March 2011 to December 2019. Porin gene ompK36 and carbapenemase genes blaKPC, blaNDM, blaIMP and blaOXA-48 were amplified by PCR and confirmed by sequencing. The MIC was detected and confirmed by microbroth dilution susceptibility test, and the corresponding patterns of genotype and MIC were constructed. Based on the patterns, a method for rapid detection of imipenem MIC by real-time fluorescence PCR (RT-PCR) was designed and established. The 159 isolates of non-repetitive Klebsiella pneumoniae collected by Lishui Disease Prevention and Control Center (CDC) from 2017 to 2019 were used for further verification. The sensitivity and specificity were calculated by fourfold table. Kappa test was used to compare the consistency between RT-PCR and microbroth dilution susceptibility test. Results:Among 258 isolates, 109 isolates did not carry carbapenemase gene, 65 isolates carried ompK36 gene GD mutation, 127 isolates carried blaKPC, 15 isolates carried blaNDM, 9 isolates carried blaIMP, and blaOXA-48 was not detected. With mircobroth dilution susceptibility test as the standard, there were 3 corresponding patterns between the drug resistance gene and the imipenem MIC of Kp: when all the 4 carbapenemase genes were negative, MIC≤1 mg/L, the sensitivity was 100% (107/107) and the specificity was 98.4% (125/127); when blaKPC was positive and ompK36 gene GD mutation was negative, 4 mg/L≤MIC≤16 mg/L, the sensitivity was 88.2% (60/68) and the specificity was 98.8% (164/166); when blaKPC and ompK36 gene GD mutation were both positive, MIC≥32 mg/L, the sensitivity was 96.6% (57/59) and the specificity was 96.6% (169/175). RT-PCR detected blaKPC, blaNDM, blaIMP, blaOXA-48 genes accurately.The RT-PCR results of ompK36 gene GD mutation in the KPC-producing isolates were 100% consistent with the sequencing results. In the 159 isolates from Lishui CDC, the sensitivity and specificity of imipenem MIC detected by RT-PCR were higher than 95% in all 3 patterns with mircobroth dilution susceptibility test as the standard, and Kappa value was 0.971. Conclusion:The RT-PCR based on ompK36 gene GD mutation was helpful to quickly determine the MIC range of imipenem in KPC-Kp.
		                        		
		                        		
		                        		
		                        	
5.Development and reliability and and validity testing of a nutritional literacy scale for patients with inflammatory bowel disease

		                        			Objective:To develop a nutritional assessment scale for patients with inflammatory bowel disease and examine its reliability and validity for assessing nutritional literacy in patients with inflammatory bowel disease.Methods:Based on the Nutbeam health literacy stratification model and knowledge-attitude-practice model, a preliminary scale was developed through literature review, semi-structured interviews, expert consultation and pre-surveys. A convenient sampling method was used to select 376 inflammatory bowel disease patients admitted to Tenth People′s Hospital Affiliated to Tongji University from September, 2022 to April, 2023 for questionnaire surveys, and reliability and validity tests were conducted to form the final scale.Results:The nutritional assessment scale for inflammatory bowel disease included 39 items. Exploratory factor analysis identified five common factors: nutritional knowledge, nutritional attitudes, nutritional practices, information interaction ability, and information evaluation ability. These factors explained 65.431% of the total variance. The content validity index of the scale was 0.857, and the item-level content validity index ranged from 0.800 to 1.000. The Cronbach α coefficient of the scale was 0.869, and the Cronbach α coefficients of each dimension ranged from 0.847 to 0.922. Conclusions:The developed nutritional assessment scale for inflammatory bowel disease demonstrates good reliability and validity, allowing for effective evaluation of patients′nutritional status.
		                        		
		                        		
		                        		
		                        	
6.Diagnostic value of pepsinogen determination in children peptic ulcer

		                        			Objective To investigate the value of pepsinogen(PG)in diagnosing children peptic ulcer.Methods A total of 855 children patients completing gastroscopic examination in this hospital due to nausea,vomiting,abdominal pain,gastrointestinal bleeding and other gastrointestinal symptoms from July 2019 to January 2024 were selected as the study subjects.According to the gastroscopic examination results,they were divided into the healthy physical examination group(n=223),chronic gastritis group(n=418),chronic gas-tritis combined with duodenal bulbar inflammation(n=168)and gastric ulcer and duodenal bulbous ulcer group(peptic ulcer group,n=46).A total of 503 study subjects perfected the 13C breath test and divided into Helicobacter pylori(Hp)positive and Hp negative according to the test results.Serum PGⅠ,PGⅡ and PGⅠ/PGⅡ levels in all study subjects were measured by latex immunoturbidimetry.The differences of this indi-cator among different ages and different gastric diseases and its diagnostic value in peptic ulcer were analyzed.Results The proportion of female children patients in the peptic ulcer group was significantly lower than that in the other 3 group,and the difference was statistically significant(P<0.05).The PGⅠ level of children pa-tients<7 years old in the chronic gastritis group was lower than that of the children patients>12 years old group,and the difference was statistically significant(P<0.05).The PG level of the children patients<7 years old in chronic gastritis combined with duodenal bulbar inflammation group was lower than that in the children patients>12 years old group,the PGⅡ level in the children patients<7 years old,7-12 years was lower than that in the children patients>12 year old,and the difference was statistically significant(P<0.05).Compared with the healthy physical examination group,the PGⅠ level in the chronic gastritis group was increased,the PGⅠ and PGⅡ levels in the chronic gastritis combined with duodenal bulbar inflammation group were increased,the PGⅠ/PGⅡ ratio was decreased,the PGⅠ and PGⅡ levels in the peptic ulcer group were increased and the PGⅠ/PGⅡ ratio was decreased,and the differences were statistically significant(P<0.05).Compared with the chronic gastritis group,the PGⅠ/PGⅡ ratio in the chronic gastritis combined with duodenal bulbar inflammation group was decreased,the PGⅠ and PGⅡ levels in the peptic ulcer group were increased,the PGⅠ/PGⅡ ratio was decreased,and the differences were statistically significant(P<0.05).Compared with chronic gastritis combined with duodenal bulbar inflammation group,the PGⅠ and PGⅡ lev-els in the peptic ulcer group were increased,the PGⅠ/PGⅡ ratio was decreased,and the differences were sta-tistically significant(P<0.05).The area under the curve(AUC)of PGⅠ and PGⅡ for predicting peptic ul-cer was 0.689 and 0.785 respectively,the maximal Youden index served as the optimal diagnostic cutoff val-ue,when PGⅠ was 69.25 μg/L,its Youden index reached the maximal value,the sensitivity for diagnosing peptic ulcer was 67.4%,the specificity was 72.3%;when PGⅡ was 12.25 μg/L,its Youden index reached the maximal value,the sensitivity for diagnosing peptic ulcer was 71.7%,the specificity was 76.1%.Compared with the Hp negative,Hp positive PGⅠ and PGⅡ in the chronic gastritis combined with duodenal bulbar in-flammation group were increased,the PGⅠ/PGⅡ ratio was decreased,the Hp positive PGⅠ/PGⅡ ratio in the peptic ulcer group was decreased,and the differences were statistically significant(P<0.05).Conclusion PG could be used to diagnose children peptic ulcer.
		                        		
		                        		
		                        		
		                        	
7.Status and influencing factors of participation in cardiac rehabilitation of cardiovascular nurses

		                        			Objective:To explore the status and influencing factors of participation in cardiac rehabilitation of cardiovascular nurses.Methods:This is a cross-sectional study. From January to February 2022, a total of 3 047 cardiovascular nurses in members of China Cardiovascular Health Alliance were selected as research objects by convenience sampling method. General data questionnaire and Cardiovascular Nurses Participated in the Cardiac Rehabilitation Questionnaire (CNPCRQ) were used to investigate cardiovascular nurses. Multiple linear regression analysis was used to investigate the influencing factors of participation in cardiac rehabilitation of cardiovascular nurses. A total of 3 047 questionnaires were distributed in this study, and 2 704 valid questionnaires were effectively received, with an effective recovery rate of 88.74%.Results:The total score of CNPCRQ of cardiovascular nurses was (93.23±31.58), which was at a moderate level. Multiple linear regression analysis results showed that age, education, professional title, hospital location, hospital level, position type and hospital type were the influencing factors of participation in cardiac rehabilitation of cardiovascular nurses.Conclusions:The status of participation in cardiac rehabilitation of cardiovascular nurses in China needs to be improved. Role and job content of cardiovascular nurses should be further clarified, and cardiac rehabilitation knowledge and skills training should be strengthened in the future, so as to promote the profession development of cardiac rehabilitation and improve patients' health outcomes.
		                        		
		                        		
		                        		
		                        	
8.HIV infection rate, high-risk behavior and pre-exposure prophylaxis/post-exposure prophylaxis in men who have sex with men in Beijing

		                        			Objective:To understand the incidence of HIV infection, high risk behaviors and pre-exposure prophylaxis/post-exposure prophylaxis (PrEP/PEP) utilization in men who have sex with men (MSM) in Beijing.Methods:Sample size was estimated to be 1 227 persons and 207 person year respectively in the survey and the cohort by using Epi Info 7.0 software. Using convenient sampling method, MSM were recruited by using Wechat app. Questionnaire was completed online to collect the information about demographic characteristics, high risk behavior, and utilization of PrEP/PEP of the MSM. MSM collected dry blood spot (DBS) samples by themselves, and mailed the DBS samples to laboratory for HIV nucleic acid testing. Open cohort was established and those with HIV negative nucleic acid testing results were followed up. Non-conditional binary logistic regression method was used to identify the associated factors for high risk anal sex in the last month and having multiple homosexual partners in the last month.Results:A total of 1 147 MSM were recruited, and follow up for 236 person years was conducted in 956 MSM with negative HIV nucleic acid testing results. The detection rate of new HIV infection was 1.3 per 100 person-years (3/236). During the last month, the proportions of consistent condom use in anal sex and oral sex were 50.7% (238/469) and 4.9% (23/469). In the MSM, 5.9% (43/723) had sex with HIV positive partners in the last month. 9.8% (103/1 049) used PrEP, and 8.7% (91/1 049) used PEP. The proportion of consistent condom use in PrEP and PEP were 34.3% (24/70) and 72.2% (39/54) respectively. Logistic regression analysis revealed that compared with those who used no PrEP/PEP, those who used PrEP/PEP were more likely to have unprotected anal sex in the last month (a OR=3.16, 95% CI:1.45-7.18), and more likely to have multiple homosexual partners in the last month (a OR=2.64, 95% CI:1.19-6.30), and compared with those who used no Rush Popper or drugs in the last month, those who used Rush Popper or drugs in the last month were more likely to have unprotected anal sex in the last month (a OR=2.34, 95% CI:1.67-3.30), and more likely to have multiple homosexual partners (a OR=2.42,95% CI:1.76-3.33). Conclusions:It is necessary to strengthen the health education to promote condom use and introduce the harm of drug use in MSM. 9.Efficacy and safety of dupilumab in the treatment of 123 cases of atopic dermatitis

		                        			Objective:To investigate clinical efficacy and safety of dupilumab in the treatment of atopic dermatitis (AD) .Methods:An ambispective study was conducted on 123 AD patients treated with dupilumab in Department of Dermatology, the Second Xiangya Hospital of Central South University from July 2020 to March 2022, clinical data were collected, and efficacy and safety of dupilumab were evaluated. Primary outcomes included scores of eczema area and severity index (EASI) , patient-oriented eczema measure (POEM) , peak pruritus numerical rating scale (NRS) and dermatology life quality index (DLQI) before and after 4-, 8-, 12- and 16-week treatment, and adverse reactions and events were recorded. Comparison of scores before and after treatment was performed using paired t test or repeated measures analysis of variance, Mann-Whitney U test was used for the comparison of efficacy among patients with different types of skin lesions or different IgE levels, and multiple regression model based on robust standard errors was used to analyze factors influencing the efficacy. Results:Among the 123 AD patients, 107 were enolled into the efficacy analysis, and 85 (79.44%) completed at least 4 weeks of treatment, including 6 (7.06%) achieving EASI75 and 23 (27.06%) achieving EASI50, and the EASI, NRS, POEM, DLQI scores (10.41 ± 6.72, 4.12 ± 1.74, 8.60 ± 4.29, 7.81 ± 4.38, respectively) significantly decreased compared with those before treatment (18.08 ± 10.69, 7.21 ± 2.01, 16.88 ± 5.74, 12.95 ± 5.95, respectively; all P < 0.001) in the 85 patients. Among the 107 patients, 47 (43.93%) completed at least 16 weeks of treatment. Among the 47 patients, 23 (82.14%) of 28 adults and 17 (89.47%) of 19 adolescents and children achieved 75% or greater improvement in EASI score; the EASI, NRS, POEM and DLQI scores before the treatment all significantly differred from those 4, 8, 12, 16 weeks after the treatment (all P < 0.001) , and all the scores were significantly lower at weeks 4, 8, 12 and 16 than at the previous adjacent time points (all P < 0.05) . At week 4 during the treatment, the EASI improvement rate was significantly lower in the AD patients with prurigo nodularis than in those without ( U = 151.00, P = 0.006) , while there was no significant difference in the EASI improvement rate between the AD patients with xeroderma and those without ( P > 0.05) ; at week 16 during the treatment, there was no significant difference in the EASI improvement rate between patients with prurigo nodularis or xeroderma and those without (both P > 0.05) . Multiple regression analysis based on robust standard errors at week 16 showed that the improvement degree in the EASI score was not correlated with the type of skin lesions ( β = 3.20, P = 0.075) , but correlated with age ( β = -0.22, P = 0.030) , whether patients were in adulthood ( β = 9.54, P = 0.049) , immediate family history ( β = 7.46, P = 0.017) ; the improvement degree in the NRS score was correlated with the type of skin lesions ( β = 0.55, P = 0.032) , age ( β = -0.04, P = 0.033) , weight ( β = -0.05, P = 0.020) , whether patients were in adulthood ( β = 2.06, P = 0.003) and whether patients received combined treatment with antihistamines ( β = -1.91, P = 0.001) . Adverse reactions: among the 123 patients, 6 (4.88%) developed conjunctivitis, and 2 (1.63%) developed facial erythema. Adverse events: vitiligo-like changes occurred on the right forehead of 1 patient, and 3 patients discontinued the treatment with dupilumab due to Henoch-Sch?nlein purpura, distal axonal damage in peripheral nerves in both upper limbs, and epilepsy, respectively. The causal relationship between these adverse events and dupilumab was unclear. Conclusion:Dupilumab is effective in the treatment of AD with high overall safety, and can serve as a new treatment option for AD patients with an unsatisfactory response to traditional treatment.
		                        		
		                        		
		                        		
		                        	
10.Etiological diagnostic value of cerebrospinal fluid metagenomic next-generation sequencing in central nervous system infection

		                        			Objective:To investigate the etiological diagnostic value of metagenomic sequencing in central nervous system (CNS) infectious diseases.Methods:A total of 170 patients with central nervous system infection admitted to the First Affiliated Hospital of Zhengzhou University from January 2018 to June 2020 were selected as the study subjects according to inclusion and exclusion criteria. General clinical data and pathogen test results were collected. All included patients underwent routine examination and mNGS test, and were divided into the conventional method test group and mNGS test group according to the test results. The measurement data conforming to normal distribution were represented by ± s; The measurement data that did not conform to normal distribution were represented by median and interquartile range. The classification data were expressed by the number of cases and percentage( n,%), and were compared by χ2 test or Fisher's exact test. Consistency test was represented by Kappa value. The detection of pathogenic microorganisms by the two methods and the rule of pathogen spectrum were compared and analyzed. Results:The overall positive rate of mNGS in CNS infectious diseases was higher than that of conventional methods (58.23% vs. 18.82%), and the difference was statistically significant ( P<0.01). Among the 20 samples which were both positive by the two methods, 10 cases were completely pathogenic, 5 cases were partially consistent and 5 cases were completely inconsistent. In the detection of tuberculous nervous system infection, the positive rates were 66.7%, 53.8%, 44.0%, 40.0%, 4.0% in blood T-SPOT, cerebrospinal fluid mNGS, ADA, Mycobacterium tuberculosis DNA and tuberculous specific antibody, respectively. The positive rate of acid-fast staining was 0. The positive rate of mNGS combined with conventional method was 80.8%. Conclusions:The detection rate of mNGS in CNS infection is better than that of conventional methods. However, it does not show obvious superiority in the detection rate of Mycobacterium tuberculosis associated nervous system infection. In general, mNGS detection of pathogenic bacteria is more extensive, which is conducive to a thorough and comprehensive understanding of the bacterial characteristics of central nervous system infection.
