1.Social support needs of medical staff based on occupational quality of life

Objective To investigate the social support needs of medical staff in Guangzhou during the post-COVID-19 period,providing a basis for relevant departments to offer targeted social support for medical staff in Guangzhou and improve their of occupational quality of life.Methods A cross-sectional survey was conducted,employing a combination of quota and conven-ience sampling to select subjects,eligible medical staff were invited to complete an online questionnaire.Results The social sup-port with a total score of 20 for medical staff in Guangzhou scored 9(7,11).Among the dimensions of social support,media sup-port scored the lowest.Social support as a whole scored with statistically significant differences across groups of different age,ed-ucational background,and job position(all P<0.05).The median score was the lowest for those under 25 years,for those with a college degree or below,and highest among administrative staff,with other positions showing equal median score.The five di-mensions of social support ranked on the top concerning the total number of the respondents included patients'understanding and trust for medical staff(93.03%),increasing in salaries and benefits(86.06%),accurate policy promotion and public opinion guidance by media(83.41%),attention to the living conditions of medical staff by media(80.29%),and enhanced regulation of false reporting and inappropriate commentary by media(76.92%).Medical staff under the age of 25 had a high need for pa-tients'following doctor's advice,and medical staff with a collegiate degree or below had a high need for family members or friends to care about their physical and mental health.The five strongest needs for social support ranked on the top concerning the total number of respondents included understanding and trust from patients(75.72%),increased salaries and benefits(53.85%),assistance with household matters by family or friends(42.55%),correct policy promotion and public opinion guidance by media(34.62%),and attention to the living conditions of medical staff from media(33.41%).Conclusion Social support for medical staff in Guangzhou are at a relatively low level,indicating a need for social support from all levels of society.
2.Secondary metabolites of mulberry leaves exert anti-lung cancer activity through regulating the PD-L1/PD-1 signaling pathway

Lung cancer ranks the top of malignancies that cause cancer-related deaths worldwide.The leaves of Morus alba L are traditional Chinese medicine widely applied in respiratory diseases.Our previous work has demonstrated the anti-lung cancer effect of secondary metabolites of mulberry leaf,but their mechanism of action has still not fully elucidated.We synthesized Moracin N(MAN)-Probe conjugated with alkyne to label lung cancer cells and identified protein targets by chemical proteomic analysis.MAN and its probe exerted similar growth-inhibitory effect on human lung cancer cells.Chemical proteomic results showed that MAN targeted the programmed death ligand 1(PD-L1)checkpoint pathway and T cell receptor(TCR)signaling pathway,indicating its immune-regulatory function.Cell-free surface plasmon resonance(SPR)results showed the direct interaction of MAN with PD-L1 protein.Molecular docking analysis demonstrated that MAN bound to E158 residue of PD-L1 protein.MAN downregulated the expression levels of PD-L1 in a time-and dose-dependent manner and disrupted the PD-L1/programmed death 1(PD-1)binding,including other secondary metabolites of mulberry leaves Guangsangon E(GSE)and Chalcomoracin(CMR).Human peripheral blood mononuclear cells(PBMCs)co-cultured with MAN-treated A549 cells,resulting in the increase of CD8+GZMB+T cells and the decrease of CD8+PD-1+T cells.It suggested that MAN exerts anti-cancer effect through blocking the PD-L1/PD-1 signaling.In vivo,MAN combined with anti-PD-1 antibody significantly inhibited lung cancer development and metastasis,indicating their synergistic effect.Taken together,secondary metabolites of mulberry leaves target the PD-L1/PD-1 signaling,enhance T cell-mediated immunity and inhibit the tumorigenesis of lung cancer.Their modulatory effect on tumor microenvironment makes them able to enhance the therapeutic efficacy of immune checkpoint inhibitors in lung cancer.
3.Value of serum exosomal circ_0023461 in diagnosis and prognosis of coronary artery disease related acute myocardial infarction

Objective To investigate the diagnostic and prognostic value of serum exosomal exocirc_0023461 in coronary artery disease(CAD)related acute myocardial infarction(AMI).Methods From December 2019 to January 2022,383 patients with CAD related AMI(AMI group),200 pa-tients with chronic stable CAD but no AMI(control group),and 200 healthy individuals identified with physical examination(healthy group)were recruited in our hospital.The serum exocirc_0023461 level was determined by real-time quantitative fluorescence polymerase chain reaction.The correlation of serum exosomal circ_0023461 level with clinicopathological features and oxida-tive stress indicators was analyzed.Results The serum level of exocirc_0023461 was significantly higher in the AMI group than the control group and healthy group[3.54(1.39,9.82)vs 0.86(0.62,1.23)and 0.65(0.41,0.79),P<0.01].ROC curve analysis showed that when the serum level of exocirc_0023461 ≥1.31,its AUC value for the diagnosis of AMI was 0.857(95%CI:0.827-0.887),with a sensitivity and specificity of 78.90%and 83.50%,respectively.Kaplan-Meier survival curve displayed that the survival time was significantly shorter in the high level AMI patients without MACE than those with low level(X2=19.390,Plog rank<0.01).Multivariate Cox regression analysis revealed that age,peripheral artery disease and serum exocirc_0023461 were independent predictors of MACE occurrence in AMI patients during follow-up(P<0.05,P<0.01).Pearson correlation analysis indicated that serum exocirc_0023461 level was negatively correlated with serum GPX and SOD levels(r=-0.395,r=-0.193,P<0.01),and positively correlated with serum MDA level(r=0.194,P<0.01).Conclusion Serum exocirc_0023461 may be a potential biomarker for the diagnosis and prognosis of CAD-related AMI,and its mechanism seems to be associated with its regulating oxidative stress and thus affecting myocardial injury.
4.Histone deacetylase inhibitors inhibit cervical cancer growth through Parkin acetylation-mediated mitophagy.

Parkin, an E3 ubiquitin ligase, plays a role in maintaining mitochondrial homeostasis through targeting damaged mitochondria for mitophagy. Accumulating evidence suggests that the acetylation modification of the key mitophagy machinery influences mitophagy level, but the underlying mechanism is poorly understood. Here, our study demonstrated that inhibition of histone deacetylase (HDAC) by treatment of HDACis activates mitophagy through mediating Parkin acetylation, leading to inhibition of cervical cancer cell proliferation. Bioinformatics analysis shows that Parkin expression is inversely correlated with HDAC2 expression in human cervical cancer, indicating the low acetylation level of Parkin. Using mass spectrometry, Parkin is identified to interact with two upstream molecules, acetylase acetyl-CoA acetyltransferase 1 (ACAT1) and deacetylase HDAC2. Under treatment of suberoylanilide hydroxamic acid (SAHA), Parkin is acetylated at lysine residues 129, 220 and 349, located in different domains of Parkin protein. In in vitro experiments, combined mutation of Parkin largely attenuate the interaction of Parkin with PTEN induced putative kinase 1 (PINK1) and the function of Parkin in mitophagy induction and tumor suppression. In tumor xenografts, the expression of mutant Parkin impairs the tumor suppressive effect of Parkin and decreases the anticancer activity of SAHA. Our results reveal an acetylation-dependent regulatory mechanism governing Parkin in mitophagy and cervical carcinogenesis, which offers a new mitophagy modulation strategy for cancer therapy.
5.The value of echocardiography in the diagnosis of persistent fifth aortic arch and its classification

Objective:To study the classification of persistent fifth aortic arch (PFAA) and the value of echocardiography in the diagnosis of PFAA.Methods:A total of 16 cases (male 6, female 10, at ages from 7 days to 4 years and 2 months old, the median age was 3 months) diagnosed with PFAA in Beijing Children′s Hospital Affiliated to Capital Medical University from January 2013 to June 2019 were studied retrospectively. The diagnosis standard, differential methods and misdiagnosed analysis of different subtypes of PFAA by echocardiography were summarized and analyzed.Results:The 16 cases included 1 case of type A1 double lumen aortic arch, 8 cases of type A2 single-lumen aortic arch, 3 cases of type B1 with pulmonary atresia and 4 cases of type B3 pulmonary artery branch arising from the distal end of ascending aorta. Only one patient of double lumen aortic arch missed diagnosis by echocardiography, and the rest were accurately diagnosed by echocardiography. CTA was performed in 13 cases, including 9 cases of type A, 1 case of type B1 and 3 cases of type B3, which confirmed the echocardiography diagnosis. Seven cases of Type A2 were operated.Conclusions:PFAA is a rare and complicated aortic arch malformation, which is divided into four major classification and multiple subtypes. Echocardiography can diagnose the PFAA and its classification, it is of great clinical significance for the early diagnosis, treatment and prognosis of children.
6.Filamentation initiated by Cas2 and its association with the acquisition process in cells.

Cas1-and-Cas2-mediated new spacer acquisition is an essential process for bacterial adaptive immunity. The process is critical for the ecology of the oral microflora and oral health. Although molecular mechanisms for spacer acquisition are known, it has never been established if this process is associated with the morphological changes of bacteria. In this study, we demonstrated a novel Cas2-induced filamentation phenotype in E. coli that was regulated by co-expression of the Cas1 protein. A 30 amino acid motif at the carboxyl terminus of Cas2 is necessary for this function. By imaging analysis, we provided evidence to argue that Cas-induced filamentation is a step coupled with new spacer acquisition during which filaments are characterised by polyploidy with asymmetric cell division. This work may open new opportunities to investigate the adaptive immune response and microbial balance for oral health.
7.Echocardiographic and clinical retrospective study of 35 patients with Kawasaki disease combined with coronary artery thrombosis

Objective To summarize the echocardiographic findings and clinical characteristics of Kawasaki disease(KD) complicated with coronary artery thrombosis (CAT).Methods Thirty-five patients with KD combined with CAT were enrolled,who were admitted to Beijing Children's Hospital,Capital Medical University between July 2005 and August 2016.The clinical characteristics and echocardiographic findings during follow-ups were retrospectively studied.According to whether the childrenhad been complicated with myocardial ischemia,the patients were divided into 2 groups:ischemic group and non-ischemic group.The duration of fever,the time when the intravenous immunoglobulin(IVIG) was first injected,the time when coronary artery aneurysms (CAA) was formed,the maximum diameter of CAA and inflammatory index inthe acute phase were compared between 2 groups.Results All of the 35 children diagnosed as KD combined with CAT suffered firom CAA,and the coronary thrombosis was detected in all the cases with aneurysms.Thirty-five patients had 99 branches of CAA,of which the maximum diameter of CAA was (9.6 ± 3.1) mm(4.0-19.0 mm).Fifty-four plots of CAT were detected in the aneurysms.The diameter of CAA that thrombosis located was larger than that of which the thrombosis was not located[(10.9 ± 2.8) mm vs.(7.9 ± 2.6) mm],and the difference was significant(P <0.01).During 4 months to 10 years and 8 months [(39.2 ±29.5) months] follow-ups,CAA regressed in 32 branches [32.3% (32/99 branches)],of which 4 branches [4.0% (4/99 branches)] completely regressed to the normal diameter.The maximum diameter of CAA regressed was smaller than the maximum diameter of CAA consistence [(7.3 ± 1.9) mm vs.(10.6 ± 3.0) mm],and the difference was significant (P < 0.01).Out of 35 patients,15 cases [42.9% (15/35 cases) had myocardial ischemia,while the other 20 cases[57.2% (20/35 cases)] didn't have.Among 15 cases with myocardial ischemia,6 cases[17.1% (6/35 cases)] had myocardial infarction,4 cases [11.4% (4/35 cases)] had heart failure,and 1 case[2.9% (1/35 cases)] died of acute heart failure complicated with severe ventricular arrhythmia.Compared with non-ischemic group,the children in the ischemic group had longer duration of fever[(19.1 ± 7.8) d vs.(12.1 ± 3.3) d],higher white blood cell account in the acute phase[(24.8 ± 13.5) × 1012/L vs.(19.7 ±4.0) × 1012/L],later treatment of IVIG [(13.9 ± 5.5) d vs.(9.8 ±3.8) d],and earlier CAA formation [(16.0 ±4.9) d vs.(20.9 ± 14.5) d],and the differences were statistically significant (all P < 0.05).Conclusions CAT of children with KD commonly originates from CAA.Patients who have more serious inflammatory reaction in the acute phase,earlier formation,heavy severity and longer consistence of CAA are prone to have myocardial ischemia.Echocardiographic study plays an important role in monitoring CAA,detecting the CAT and finding the early left ventricle dysfunction,which is of clinical significance.
8.Effects of perinatal personalized self-efficacy intervention on breastfeeding six months after delivery in primiparas

Objective To evaluate whether perinatal personalized self-efficacy can improve the rate of breastfeeding six months after delivery in primiparas by applying it in the breastfeeding by primiparas. Methods Totally 76 pregnant women who were admitted by the outpatient service of the Obstetrics Department from January to February 2016 were selected as the object of intervention by convenience sampling. Pregnant women admitted in January 2016 were divided into the control group (n=38) and received conventional nursing, while those admitted in February 2016 were divided into the observation group (n=38) and received perinatal personalized breastfeeding self-efficacy intervention. Then the breastfeeding self-efficacy one week and one month after delivery and the rate of pure breastfeeding one week, one month, three months and six months after delivery were compared between two groups.Results The pregnant women in the observation group showed higher breastfeeding self-efficacy than the pregnant women in the control group (t=25.43,22.78;P<0.05). The rates of pure breastfeeding one week, one month, three months and six months after delivery were 88.57%, 85.71%, 80.00% and 77.14%, respectively, higher than those of the pregnant in the control group, which were 66.67%, 55.56%, 47.22% and 36.11%, respectively (χ2=4.878, 7.749, 8.215, 12.146;P<0.05). Conclusions Perinatal personalized self-efficacy intervention can improve the breastfeeding self-efficacy and the rate of pure breastfeeding six months after delivery.
9.Clinical Efficacy Observation of Herpes Zoster Treated by Compound Radix Scutellariae Solution Wet Application

Objective]Observe clinical efficacy of herpes zoster treated by the compound radix scutellariae solution wet application. [Method]50 patients were randomly divided into 25 cases of the compound radix scutellariae group and 25 cases of the nitrofurazone group, respectively given wet application of the compound radix scute-llariae solution and nitrofurazone solution, at the same time given intravenousganciclovir 0.3g/time,1 time/d, oral vitamin B1 20mg/time, 3 times/d, oral cobalt amine 0.5mg/time, 3 times/d, one week as a period. Record all patients' pain, skin lesion scores before the treatment and in the course of the third, fifth, seventh day and record the time of herpes stopping hair, blister drying up, blister beginning to scab and the scabby area not less than 50%.[Result] Pain, skin lesion scores of the compound radix scutellariae group were signifycantly lower than those of nitrofurazone group, the time of herpes stopping hair, blister drying up, blister beginning to scab and the scabby area not less than 50% of the compound radix scutellariae group compared with nitrofurazone group in advance, the difference was statistically significant(P<0.05). [Conclusion]Treatment of the compound radix scutellariae solution to herpes zoster had distinct clinical efficacy, having significant advantages in terms of alleviating pain and promoting skin lesions subsided.
10.Application of quality control circle on reducing the unqualified cases of nursing documents

Objective To investigate the influence of quality control circle ( QCC ) on reducing the unqualified cases of nursing documents. Methods Through QCC, we used quality management tools to analysis the unqualified factors of nursing documents, then took corresponding improvement measures and evaluate the effect. Results Utilization of QCC, unqualified cases of nursing documents was reduced from 88 cases per month to 44 cases pre month, and achieved rate was 105%, progress rate was 50%. Unqualified cases of nursing documents before and after QCC were significantly different (P <0. 01). Conclusions Using QCC practices in the quality management of nursing documents, the members not only can enjoy greater autonomy, participation rights, and management rights, but also can play an active and creative role. In addition, the outcomes of QCC activity were standardized, so QCC activities can effectively reduce the nursing documents unqualified cases.
