1.Evaluation of the efficacy and safety of Xiao′er Huangjin Zhike Granules in the treatment of acute bronchitis-caused cough (syndrome of phlegm-heat obstructing the lung) in children

		                        			Objective:To evaluate the efficacy and safety of Xiao′er Huangjin Zhike Granules in the treatment of cough caused by acute bronchitis in children, which is defined in TCM terms as a syndrome of phlegm-heat obstructing the lung.Methods:This was a block-randomized, double-blind, placebo-controlled, multicenter clinical trial.From January 2022 to September 2023, 359 children aged 3 to 7 years old diagnosed as acute bronchitis (lung-obstructing phlegm-heat syndrome) were enrolled from 21 participating hospitals and randomly assigned to the experimental group and placebo group in a 3︰1 ratio, and respectively treated with Xiao′er Huangjin Zhike Granules and its matching placebo.Cough resolution/general resolution rate after 7 days of treatment was used as the primary efficacy outcome for both groups.Results:(1)On the seventh day of treatment, the rate of cough disappearance/basically disappearance in the experimental group and placebo group were 73.95% and 57.61% retrospectively, which had statistically significance ( P=0.001).(2)After 7 days of treatment, the median duration of cough disappearance/basic disappearance were 5 days and 6 days in the two groups , with a statistically significant difference ( P=0.006).The area under the curve of cough symptom severity time was 7.20 ± 3.79 in the experimental group and 8.20±4.42 in the placebo group.The difference between the two groups was statistically significant ( P=0.039).(3) After 7 days of treatment, the difference between TCM syndrome score and baseline was -16.0 (-20.0, -15.0) points in the experimental group and -15.0 (-18.0, -12.0) points in the placebo group, with significant difference between the two groups ( P=0.004).In the experimental group, the clinical control rate, the markedly effective rate, the effective rate and the ineffective rate were 49.04%, 28.35%, 16.48% and 6.13% severally; and in the placebo group, the clinical control rate, the markedly effective rate, the effective rate and the ineffective rate were 38.04%, 26.09%, 29.35%, and 6.52% separately, which had statistically significant ( P=0.014).(4) There was no significant difference in the incidence of adverse events or adverse reactions during the trial between both groups.Moreover, while adverse reactions in the form of vomiting and diarrhea were occasionally reported, no serious drug-related adverse event or adverse reaction was reported.(5)The tested drug provided good treatment compliance, showing no statistically significant difference from the placebo in terms of compliance rate. Conclusions:Based on the above findings, it can be concluded that Xiao′er Huangjin Zhike Granules provides good safety, efficacy, and treatment compliance in the treatment of cough caused by acute bronchitis, and lung-obstructing phlegm-heat syndrome, in children.
		                        		
		                        		
		                        		
		                        	
2.Analysis and Recommendations on the Current Status of Pharmaceutical Management in County Medical Communities in Hubei Province

		                        			Objective This study aims to assess the current status of pharmaceutical management in county medical communities in Hubei province,and provide recommendations for the homogenization,standardization,and regulation of pharmaceutical management in these communities.It also intends to offer decision-making support for health administrative departments,and provide reference experiences for management in other regions.Methods The current status of pharmaceutical management in county medical communities in Hubei province was conducted through a questionnaire survey and field research.Existing problems were analyzed,key management areas were identified,and reasonable recommendations were proposed.Results Pharmaceutical management in county medical communities has significant shortcomings in organizational structure,system construction,personnel allocation,key link control,and the leading unit's outreach capabilities.These deficiencies are not aligned with the high-quality development of pharmacy in the new era.Conclusions It is recommended that county medical communities should establish a comprehensive pharmaceutical management quality control system.This can be achieved by improving organizational management,strengthening talent development,enhancing core systems,setting monitoring indicators,and increasing outreach capabilities.Additionally,evaluation standards for the quality control system of pharmaceutical management should be established to enhance management capabilities through scientific assessment and positive feedback.
		                        		
		                        		
		                        		
		                        	
3.The mediating effect of fear-avoidance beliefs in neck pain and quality of life among nursing staff in a hospital in Shanghai

		                        			Objective:To test whether the fear-avoidance beliefs (FAB) of nursing staff in a hospital in Shanghai have a mediating effect between neck pain and quality of life.Methods:The cross-sectional survey method was used to obtain the general information, neck pain, FAB and quality of life scores of the subjects. The mediating effect analysis was used to verify potential mediating effect of FAB on neck pain and quality of life.Results:A total of 103 valid questionnaires were collected, 60 (58.25%,60/103) in the neck pain group and 43 (41.75%, 43/103) in the pain-free group. The independent sample T test found that the FABQ score was higher in the pain group than in the pain-free group ( P<0.05). The quality of life score was lower in the pain-free group, and the between-group difference of physical component summary (PCS) was statistically significant ( P < 0.05). The results of pearson correlation analysis showed a negative correlation between FABQ and quality of life scores ( rFABQ1-PCS=-0.32, rFABQ2-PCS=-0.34, rFABQ1-MCS=-0.20, rFABQ2-MCS=-0.32, P<0.05). Both Physical Activity Fear Avoidance Belief (FABQ-1) and Work Fear Avoidance Belief (FABQ-2) have a complete mediating effect between neck pain and PCS. The mediating effect magnitude of FABQ-1 was 0.575, and the mediating effect of FABQ-2 was 0.552. The mediating effect magnitude of FABQ-1 was enlarged and FABQ-2 was reduced after adding age as a covariate in the mediating effect analysis. The mediating effect of FABQ-1 was 0.601, and the mediating effect magnitude of FABQ-2 was 0.501. Conclusion:The fear avoidance belief of nursing staff in a hospital in Shanghai presents a complete mediating effect between neck pain and quality of life, and the mediating effect of Physical Activity Fear Avoidance Belief is stronger with age but the mediating effect of Work Fear Avoidance Belief is weaker.
		                        		
		                        		
		                        		
		                        	
4.Effects of WeChat-based health education on management of hypertensive outpatients under medical consortium

		                        			Objective:To explore the effects of WeChat-based health education on management of hypertensive outpatients under medical consortium.Methods:Totally 180 hypertensive patients who attended the Multidisciplinary Clinics of Beijing Anzhen Hospital, Capital Medical University, and Datun Community Health Service Center from communities under our medical consortium from November 2018 to April 2019 were selected and divided into the observation group ( n=91) and the control group ( n=89) according to the visit time. Patients in the control group received routine health education, while patients in the observation group received WeChat-based health education under medical consortium for 6 months. The self-management ability (using Hypertension Patients Behavior Rating Scale, HPSMBRS) , treatment compliance (using Therapeutic Adherence Scale for Hypertensive Patients, TASHP) , the 24 h average systolic and diastolic blood pressure, the average systolic and diastolic blood pressure at daytime, and the average systolic blood pressure and diastolic blood pressure at night were compared between the two groups. Results:After the intervention, the diet management, medication management, disease monitoring, exercise management, work and rest and emotional management scores of the observation group were higher than those of the control group, and the differences were statistically significant ( P<0.05) . After the intervention, the medication-taking behavior, bad medication-taking behavior, daily activity management behavior and tobacco and alcohol addiction scores were higher than those of the control group, and the differences were statistically significant ( P<0.05) . After the intervention, the 24 h average systolic and diastolic blood pressure, average systolic and diastolic blood pressure at daytime and average systolic and diastolic blood pressure at night decreased in the observation group, and the differences were statistically significant compared with the control group ( P<0.05) . Conclusions:The WeChat platform under medical consortium provides standardized, consistent, continuous and effective health education, which can improve the self-management ability and treatment compliance of hypertensive outpatients, and can effectively control blood pressure levels. 5.Effectiveness and Safety of Yinzhihuang Oral Liquid in the Adjuvant Treatment of Neonatal Jaundice:A Sys-tematic Review

		                        			OBJECTIVE:To systematically review the effectiveness and safety of Yinzhihuang oral liquid in the adjuvant treat-ment of neonatal jaundice,and provide evidence-based reference for clinical treatment. METHODS:Retrieved from CJFD,VIP Da-tabase,Wanfang Database,Medline and EMBase,randomized controlled trials(RCT)about the effectiveness of Yinzhihuang oral liquid(test group)based on the canventional treatment(control group)in the adjuvant treatment of neonatal jaundice were collect-ed,and Meta-analysis was performed by using Rev Man 5.2 software after data extracting and quality evaluating by modified Jadad. RESULTS:Totally 12 RCTs were enrolled,involving 1585 patients. Results of Meta-analysis showed total effective rate [OR=0.19,95%CI(0.12,0.30),P<0.001],serum total bilirubin levels [MD=-40.78,95%CI(-42.68,-38.89),P<0.001] and time of bilirubin decreased to normal [MD=-2.56,95%CI(-2.72,-2.40),P<0.001] in test group were significantly better than control group,the differences were statistically significant between 2 groups;3 reports of adverse reactions showed scme children had vomiting and mild diarrhea,they were improved after symptomatic treatment,and it did not affect the treatment. CONCLU-SIONS:The effectiveness of Yinzhihuang oral liquid is good in the adjuvant treatment of neonatal jaundice,with mild adverse reac-tions.
		                        		
		                        		
		                        		
		                        	
6.Efficacy of Yixinshu Capsule in the Adjuvant Treatment of Angina Pectoris:A Meta-analysis

Meta-analysis was performed bu using Rev Man 5.2 software after data extraction and quality evaluation by modified Jadad. RESULTS:Totally 11 RCTs were enrolled,involving 1 827 pa-tients. Results of Meta-analysis showed the total effective rate of angina pectoris improvement [OR=0.30,95%CI(0.23,0.40),P<0.001] and electrocardiogram improvement [OR=0.52,95%CI(0.41,0.66),P<0.001] in test group were significantly higher than control group,there was significant difference between 2 groups. 7.Study on Formulation Optimization of Nicorandil Sustained-release Matrix Tablet and Drug Release in vitro

		                        			OBJECTIVE:To optimize the formulation optimization of Nicorandil sustained-release matrix tablet,and evaluate its drug release properties in vitro. METHODS:Based on single factor test,powder direct compression method was used,using nicorandil cumulative release rate (Q) in 1,4,8,12 h as evaluation indexes,central composite design-response surface method was adopted to optimize the amount of hydroxypropyl methylcellulose(HPMC)and ethyl cellulose(EC);Q values within 12 h in different pH (1.0,5.0,6.8,7.4) media were compared. RESULTS:The optimized formulation (every tablet) was nicorandil 10 mg,HPMC 150 mg,EC 90 mg,microcrystalline cellulose 80 mg,lactose 60 mg,magnesium stearate 2%. Q1 h,Q4 h,Q8 h and Q12 h of the obtained formulation were 23.6%,51.3%,83.7% and 96.9%,respectively;deviation from the predicted values were 2.1%,1.6%,1.0%,0.2%. Q values were similar in pH 1.0-7.4 at different time points. CONCLUSIONS:The obtained Nicor-andil sustained-release matrix tablet by optimal formulation shows sustained-release effect,and the change of pH 1.0-7.4 has no in-terference in the release characteristics of main drug.
		                        		
		                        		
		                        		
		                        	
8.Efficacy of Qijiao Shengbai Capsule in the Adjuvant Treatment of Leucopenia:A Meta-analysis

		                        			OBJECTIVE:To systematically review the efficacy of Qijiao shengbai capsule in the adjuvant treatment of leukope-nia,and provide evidence-based reference for the clinical treatment. METHODS:Retrieved from CJFD,VIP,Wanfang Database and Medline and EMBase,randomized controlled trials(RCT) about the efficacy of Qijiao shengbai capsule (test group) versus blank control(control group) in the adjuvant treatment of leucopenia were collected. Meta-analysis was performed by using Rev Man 5.2 software after the quality assessment and data extraction. RESULTS:A total of 15 RCTs were included,involving 1 383 patients. Results of Meta-analysis showed the total effective rate [OR=0.31,95%CI(0.23,0.43),P<0.001],obvious effective rate [OR=0.31,95%CI(0.24,0.41),P<0.001],leukocyte count [MD=-1.04,95%CI(-1.10,-0.98),P<0.001] and myelosuppres-sion reduction [OR=0.27,95%CI(0.14,0.52),P<0.001] in test group were significantly higher than control group,there were sig-nificant differences. Only 9 patients had adverse reactions of mild diarrhea,nausea and other symptoms,and it relieved after symp-tomatic treatment. CONCLUSIONS:Qijiao shengbai capsule has achieve good efficacy in the adjuvant treatment of leukopenia. 9.Relationship between Homocysteine,Glycosylated Hemoglobin, Lipids and Posterior Circulation Ischemia

		                        			Objective To explore the relationship between homocysteine(HCY),glycosylated hemoglobin(HbA1c),lipids and posterior circulation ischemia and the clinical significance of their levels in the posterior circulation ischemic (PCI)diseases. Methods Difference between PCI were diagnosed in 140 examinees and a healthy control group,with fasting serum,HCY, HbA1c,total cholesterol (TC),triglyceride (TG),high density lipoprotein cholesterol (HDL-C)and low-density lipoprotein cholesterol (LDL-C)were detected.Results HCY and HbA1c were significantly higher,HDL-C was significantly lower in the patients with PCI,two independent sample t-test showed a significant difference between the test group and control group (P <0.001 and P =0.001),the remaining lipid indicators was not statistically significant.Spearman rank correlation analysis showed that HCY levels had significant positive correlation (P <0.001)and TG levels had significant negative cor-relation (P =0.013)with the subjects age,HCY levels had significant negative correlation with TG levels(P = 0.028), HbA1c levels had significant positive correlation with TG levels and LDL-C levels(P =0.001 and P =0.027).Conclusion High levels of HCY and HbA1c were closely associated with PCI,which HCY and HbA1c should be attached great impor-tance to the effective prevention and treatment and improve of PCI.
		                        		
		                        		
		                        		
		                        	
10.Practice of Clinical Pharmacists Participating in the Treatment of One COPD Patient with Pulmonary In-fection

		                        			Objective:To discuss the choice of anti-infective drugs and the significance of pharmaceutical care for COPD patients with pulmonary infection. Methods:Clinical pharmacists performed pharmaceutical care for a COPD patient with pulmonary infection including such aspects as drug selection, treatment course, drug interaction, adverse reactions and so on. Results:By providing phar-maceutical care for the patient, the problems in the treatment were identified and solved in time, and the rationality of drug treatment was improved.
