1.Health state utility value and influencing factors of Tibetan elderly in Tibet

Objective To understand the current situation and influencing factors of health state utility value of the Tibetan elderly in Tibet Autonomous Region.Methods The multi-stage stratified cluster sampling method was used to recruit 1 690 elderly aged 60 and above from 7 cities of Tibet Autonomous Region as the research subjects.The general status of the elderly was investigated by questionnaire survey,and health utility value of the population was measured and computed by EuroQol Five Dimensions Questionnaire(EQ-5D-3L).The influencing factors were analyzed using Tobit regression model.Results The mean visual analogue scale(VAS)score of EQ-5D-3L and the mean health state utility value were 56.36±21.19 and 0.901±0.145,respectively.Those who were male,aged between 60 to 69 years,received middle school education and above,got married,were employed,had medical examination in the past year,and had no hypertension,rheumatoid arthritis or other chronic diseases had higher health state utility value and VAS score.Conclusion The mean health state utility value of Tibetan elderly aged 60 and above in Tibet Autonomous Region was higher than that of other regions in China,while the average score of self-assessment EQ-VAS was lower than that of other regions in China.The health state utility value was affected by gender,age,employment status,education level,living area,different medical examination status in the past year,and chronic disease status.In the future,the health care of the elderly who are aged 70 years and above,are female,have never been to school,live in rural areas,are unemployed,and have chronic diseases should be strengthened.Regular medical examination should be performed to identify health problems timely so as to improve the quality of life of the elderly.
2.COVID-19 surveillance via wastewater:Introduction of wastewater-based epidemiology

Since the outbreak of the COVID-19 global pandemic in 2019, monitoring COVID-19 infection status and trend through wastewater, known as wastewater-based epidemiology (WBE), has been widely used in many countries and regions. WBE consists of five steps:wastewater sample collection, viral concentration, viral nucleic acid extraction, quantification of virus using quantitative RT-PCR, and dissemination of the wastewater surveillance results. This method could be used for early warning of COVID-19 outbreak in a population, monitoring COVID-19 distributions and epidemic trend, prediction of COVID-19 prevalence rate, understanding of temporal trend of SARS-COV-2 variants, and simultaneous surveillance of multiple pathogens. WBE and clinical surveillance can be used concurrently and the former is a good complement to the latter.
3.Efficacy and safety of pomalidomide combined with cyclophosphamide and dexamethasone in treatment of relapsed/refractory multiple myeloma

Objective:To investigate the therapeutic effect and safety of pomadomide combined with cyclophosphamide and dexamethasone (PCD) in the treatment of relapsed/refractory multiple myeloma (MM).Methods:The clinical data of 20 relapsed/refractory MM patients receiving PCD regimen in the Second People's Hospital of Lianyungang Affiliated to Bengbu Medical College from March 2021 to June 2022 were retrospectively analyzed; and 29 relapsed/refractory MM patients receiving other regimens including DECP (dexamethasone+etoposide+cyclophosphamide+cisplatin, 13 cases) and VCD (bortezomib+ cyclophosphamide+ dexamethasone, 16 cases) during the same period were treated as the control group. The efficacy and adverse effects of both groups were compared after 4 cycles of treatment.Results:After 4 cycles of treatment, the overall response rate (ORR) and the clinical benefit rate (CBR) of 20 cases in PCD group was 70.0% (14/20) and 85.0% (17/20), respectively; among 20 cases, there were 5 cases of complete response (CR), 4 cases of very good partial remission (VGPR), 5 cases of partial remission (PR), 3 cases of minimal remission (MR), 2 cases of stable disease (SD), 1 case of the progression of the disease (PD). ORR and CBR of 29 cases in the control group was 41.4% (12/29) and 65.5% (19/29), respectively; among 29 cases, there were 2 cases of CR, 3 cases of VGPR, 7 cases of PR, 7 cases of MR, 5 cases of SD, 5 cases of PD. There was a statistically significant difference in ORR of both group ( χ2 = 3.89, P = 0.048), while the difference in CBR of both group was not statistically significant ( χ2 = 2.30, P = 0.129). There were 2 patients with renal impairment achieving CR in PCD group and 1 patient with renal impairment achieving CR in the control group ( P = 0.152); 1 genetically high-risk patient achieved CR in PCD group and none of patients in the control group achieved CR, and the difference was statistically significant ( P>0.05). The common hematological adverse effects of two groups were anemia, neutropenia, thrombocytopenia; the common non-hematological adverse effects were malaise, infection and fatigue, and the differences were statistically significant (all P>0.05). The incidence of grade 3-4 infection was 25.0% (5/20) in PCD group and the disease was under the control after anti-infective therapy, and the incidence of grade 3-4 infection was 24.1% (7/29) in the control group; and the difference was not statistically significant ( P > 0.05). Conclusions:PCD regimen has good clinical efficacy and safety in treatment of relapsed/refractory MM.
4.Associations between cancer family history and esophageal cancer and precancerous lesions in high-risk areas of China.

BACKGROUND:
Family clustering of esophageal cancer (EC) has been found in high-risk areas of China. However, the relationships between cancer family history and esophageal cancer and precancerous lesions (ECPL) have not been comprehensively reported in recent years. This study aimed to provide evidence for identification of high-risk populations.
METHODS:
This study was conducted in five high-risk areas in China from 2017 to 2019, based on the National Cohort of Esophageal Cancer. The permanent residents aged 40 to 69 years were examined by endoscopy, and pathological examination was performed for suspicious lesions. Information on demographic characteristics, environmental factors, and cancer family history was collected. Unconditional logistic regression was applied to evaluate odds ratios between family history related factors and ECPL.
RESULTS:
Among 33,008 participants, 6143 (18.61%) reported positive family history of EC. The proportion of positive family history varied significantly among high-risk areas. After adjusting for risk factors, participants with a family history of positive cancer, gastric and esophageal cancer or EC had 1.49-fold (95% confidence interval [CI]: 1.36-1.62), 1.52-fold (95% CI: 1.38-1.67), or 1.66-fold (95% CI: 1.50-1.84) higher risks of ECPL, respectively. Participants with single or multiple first-degree relatives (FDR) of positive EC history had 1.65-fold (95% CI: 1.47-1.84) or 1.93-fold (95% CI: 1.46-2.54) higher risks of ECPL. Participants with FDRs who developed EC before 35, 45, and 50 years of age had 4.05-fold (95% CI: 1.30-12.65), 2.11-fold (95% CI: 1.37-3.25), and 1.91-fold (95% CI: 1.44-2.54) higher risks of ECPL, respectively.
CONCLUSIONS:
Participants with positive family history of EC had significantly higher risk of ECPL. This risk increased with the number of EC positive FDRs and EC family history of early onset. Distinctive genetic risk factors of the population in high-risk areas of China require further investigation.

5.Relationship of health literacy with COVID-19 prevention and control knowledge, attitude and practice in general population of Baoji city

【Objective】 To explore the relationship of health literacy with COVID-19 prevention and control knowledge, attitude and practice (KAP) in general population so as to contribute scientific evidence for strengthening health education and promoting health literacy to resist the threat of major infectious disease outbreaks. 【Methods】 In September 2020, a questionnaire survey was conducted in residents selected by a multi-stage random sampling across all the twelve counties/districts of Baoji city. The questionnaire, which was issued by the Chinese Health Education Center, consisted of a health literacy questionnaire and a COVID-19 prevention and control KAP questionnaire. According to the national unified scoring method, the participants were divided into two groups: those who met and those who failed to meet the overall standard of health literacy. The results of the answer to each KAP question were compared between the two groups by Chi-square test or rank sum test. Multivariate binary Logistic regression was used to control confounding effects of socio-demographic characteristics to draw relatively reliable conclusions. 【Results】 A total of 4 544 valid questionnaires were collected, in which 664 (14.60%) met the overall standard of health literacy, but 3 880 (85.40%) failed to do so. Compared with the unmet group, the met group had a higher correct answer rate in 10 of the 11 knowledge-related questions (all P<0.001); showed more positive answer to each attitude-related question in the three aspects, namely, responsibility for the prevention and control of infectious disease transmission, evaluation for COVID-19-related information release and reporting, and evaluation for the government’s COVID-19 prevention and control results (all P<0.001); and acted more actively in 6 of the 7 practice concerning appropriate self-prevention and control behaviors during the COVID-19 outbreak (all P<0.001). Logistic regression analyses confirmed that achieving the overall standard of health literacy played a positive role in each of the contents of COVID-19 prevention and control KAP in study (ORs were between 1.44 and 4.09, all P<0.001). Moreover, the absolute value of regression coefficient of the overall standard of health literacy was the largest compared with all the socio-demographic factors. Logistic regression was used to further analyze relationships between each of the six health dimensions of health literacy and COVID-19 prevention and control KAP, which revealed that the association with safety and first aid, infectious diseases prevention, and health information was the closest. 【Conclusion】 Health literacy is closely related to COVID-19 prevention and control KAP in the general population of Baoji city. Promoting residents' health literacy by targeted health education can play an important and positive role in dealing with the threat of major infectious diseases outbreaks.

Following the World Health Organization Handbook for Guideline Development, this guideline integrated the most up-to-date evidence of liver cancer screening, China's national conditions, and existing practical experience in liver cancer screening. Evidence-based recommendations on the target population, screening technologies, surveillance strategies, and other key points across the process of liver cancer screening and surveillance management were provided. This guideline would help standardize the practice of liver cancer screening in China.

8.Progress of bendamustine in treatment of multiple myeloma

Bendamustine, a bifunctional derivate of nitrogen mustard, is an attractive treatment option for multiple myeloma (MM) due to its specific mode of activity, favorable toxicity profile, and clinical activity in patients resistant to alkylating agent. Bendamustine single agent and its combination with immunomodulators or proteasome inhibitors have been widely used in the relapsed/refractory MM patients. Bendamustine has brought the longer progression-free survival, overall survival time and deeper remission in the autologous stem cell transplantation pre-conditioning for MM patients. This paper reviews the treatment progress of bendamustine for MM.
