Public hospitals serve as important windows for the Party to lead the people and serve the people,making the exploration of Party-building-led high-quality development in public hospitals highly significant in practical terms.This study delves into the current challenges faced in guiding the high-quality development of public hospitals through party building.It presents the practical exploration and outcomes of Dongguan People's Hospital,which has taken the initiative to establish a provincial demon-stration site for the"Four Orientations"project(Leading groups work effectively,Party branches work with promising methods,Party building achieves a fame with brand,and each unit has its own model)within the public hospital party-building framework.The hospital has deepened the practical application of the"Four Endeavors"project by practicing the project.These practices offer valuable insights into and references for high-quality development in public hospitals under the guidance of Party building.
2.Status quo of pain catastrophizing in patients with diabetic peripheral neuropathic pain and influencing factors analysis

Objective To investigate the status quo of pain catastrophizing(PC)in the patients with di-abetic peripheral neuropathic pain(DPNP),and to analyze the influencing factors to provide reference for for-mulating clinical preventive intervention strategies.Methods A total of 206 patients with DPNP admitted and treated in the People's Hospital of Guangxi Zhuang Autonomous Region were selected as the research sub-jects by convenience sampling method.The general data questionnaire,Numerical Rating Scale(NRS),Pain Catastrophizing scale(PCS),Perceived Social Support Scale(PSSS)and diabetes distress scale(DDS)were used to conduct the investigation.Results The incidence rate of PC in 206 cases of DPNP patients was 44.66%(92/206),and the total score of PCS was(30.10±5.16)points.The results of multiple linear regres-sion analysis showed that the gender,duration of diabetes(≥10 years),multiple drug use,number of compli-cations(>5),NRS score,PSSS score and scores of DDS dimensions were the main influencing factors of PC(all P<0.05),which could explain 92.3%of the total variation of PC.Conclusion The PC incidence rate in the patients with DPNP is high.Clinical healthcare workers should pay attention to the evaluation of PC in these patients,and formulate the scientific and effective targeted intervention measures according to the main influen-cing factors to help the patients to reduce the pain burden in order to reduce the level of PC.
3.Influencing factors of maintenance and promotion of peripherally inserted central catheter in community under the medical alliance model

Objective:To understand the influencing factors of the maintenance and promotion of peripherally inserted central catheter (PICC) in the community under the medical alliance model, and provide a basis for realizing the full coverage of PICC maintenance clinics at each site in the medical community.Methods:Using the cluster sampling method, a total of 79 patients with PICC in 10 communities participating in the medical alliance model in Hangzhou and 67 community nurses participating in PICC maintenance were selected as the research objects from January to February 2021. The patients and nurses were surveyed by using the patient questionnaire and the nurse questionnaire, respectively. Univariate analysis and Logistic regression were used to analyze the influencing factors of community PICC maintenance and promotion under the medical alliance model, and the theoretical knowledge and operation of PICC maintenance for community nurses were assessed. According to the 8-month promotion of communities, 3 communities with more than or equal to 40 PICC maintenance patients were selected as the promotion advantage communities, and the remaining 7 communities were selected as the promotion disadvantage communities. And the influencing factors of promotion results among the superior and inferior communities were compared and analyzed.Results:In the univariate analysis of the patient survey, payment method, mobility, transportation mode, transportation cost, receiving follow-up, satisfaction with community consultation process and maintenance technology were the influencing factors of community PICC maintenance and promotion under the medical alliance model ( P<0.05) . In the univariate analysis of the nurse survey, publicity channel, performance distribution, shift arrangement, family doctor team and door-to-door maintenance were the influencing factors of community PICC maintenance and promotion under the medical alliance model ( P<0.05) . There were statistically significant differences in knowledge and operation skills of PICC complications among superior and inferior community nurses ( P<0.05) . Logistic regression analysis showed that patients' behavior ability and whether they were followed up were the influencing factors of community PICC maintenance and promotion ( P<0.01) . Conclusions:Based on the medical community model, the homogenization training and management of PICC maintenance should be carried out in the community, the family doctor team should be used to carry out regular follow-up for patients with catheterization, the maintenance process of PICC in the community should be simplified, the publicity of various ways should be strengthened, the ability of community nurses to deal with PICC complications should be continuously improved and the performance allocation scheme should be adjusted to improve the initiative of nurses. And strengthening the two-way referral linkage mechanism between the general hospital and the community, providing consultation support to the community and solving the reimbursement channels for diseases specified in the maintenance of community PICC can promote the promotion and maintenance of PICC in community.

5.Construction of nursing evaluation system for patients with enterostomy

Objective:To construct a nursing evaluation system for patients with enterostomy to provide a basis for formulating individualized nursing programs for patients with enterostomy.Methods:From November 2020 to June 2021, based on the conceptual framework of health-related quality of life proposed by the World Health Organization and the cross-cultural nursing theory as theoretical guidance, the first draft plan was formed through the extraction of evidence-based evidence and the results of qualitative interviews. Using the purposeful sampling method, 20 medical and nursing experts in enterostomy-related fields were selected to carry out Delphi expert letter inquiries, the first draft plan was revised, and a nursing evaluation system for colostomy patients was established.Results:The effective recovery rate of the two rounds of expert letter questionnaires was 100.00%. Expert authority coefficients were both 0.89. Kendall's coordination coefficients were 0.23 and 0.32, respectively ( P<0.01) . Finally, a nursing evaluation system for enterostomy patients was formed, including 4 first-level indexes, 22 second-level indexes and 102 third-level indexes. Conclusions:The framework of the nursing evaluation system for patients with enterostomy is complete, the construction process is scientific and reliable, and it has guiding value for the overall nursing evaluation of patients with enterostomy.
6.Study on the effects of periodontal subgingival scaling under the mode of standardized residency training of stomatology general medicine

Objective:To observe the effects of the periodontal subgingival scaling in different stages of standardized residency training of stomatology general medicine.Methods:Thirty residents in Affiliated Stomatology Hospital of Zhejiang University School of Medicine were recruited by stratified randomization and divided into three groups, including 1-month group, 3-month group and 6-month group. The residual ratio of clinical calculus was compared after periodontal subgingival scaling among groups. SPSS 23.0 was conducted for Kruskal-Wallis H test. Results:The mean residual ratio of clinical calculus in 3-month group and 6-month group were significantly lower than that in the 1-month group. In the 3-month group, the scaling could only achieve the effective results at the shallow pockets and anterior tooth area. Compared with the 1-month group, the 6-month group significantly decreased the residual ratio of clinical calculus for the sites with pocket depth of 4-6 mm, but there were still insufficiency to deal with the posterior tooth area.Conclusion:The 3-6 months residency training of periodontology could improve the effectiveness of the periodontal subgingival scaling. We should establish reasonable assessment and evaluation system according to different stages of residents, and the subgingival scaling training in the posterior areas and the distal interproximal sites should be reinforced in order to more effectively improve the quality of the standardized residency training.
7.Effects of perioperative emergency scene two-step simulation drill for cancer patients in medical and nursing staff training

Objective:To explore the effects of perioperative emergency scene two-step simulation drill for cancer patients in medical and nursing staff training.Methods:From August 2018, this study selected 19 nurses and 10 doctors in Oncology Surgery of the First People's Hospital of Fuyang Hangzhou as subjects by convenience sampling. Subjects all received the emergency scene two-step simulation drill. We compared the theoretical knowledge and operational skills before and after training for medical staff.Results:After training, the theoretical knowledge and operational skills of medical and nursing staff were higher than those before training with statistical differences ( P<0.05) . Conclusions:Perioperative emergency scene two-step simulation drill for medical staff of Oncology Surgery can creative a good environment for medical staff and improve the theoretical knowledge and operational skills of medical staff so as to ensure the security of perioperative cancer patients.
8.Associations of cognitive function with APOE gene polymorphism and chronic diseases among long-lived people in Zhongxiang City of Hubei Province

Objective To investigate the associations of cognitive function with apolipoprotein E (APOE) gene polymorphism and chronic diseases among long-lived people in Zhongxiang City of Hubei Province.Methods A total of 110 long-lived elderly residents aged 90 years and over were collected.Their cognitive function was determined face-to-face using questionnaires by trained interviewers.According to mini-mental state examination(MMSE) scores,subjects were divided into a no dementia risk group and a high dementia risk group.General demographic characteristics,activities of daily living,depression state and nutrition status were compared between the two groups.Correlations of dementia with APOE gene polymorphism and chronic diseases were analyzed.Results The average MMSE score was 22.3±4.8.Among the 110 long-lived people,18 cases had a high risk for dementia,accounting for 16.4%,and 92 cases had no risk of dementia,accounting for 83.6%.The risk of dementia in long-lived elderly people was correlated with activities of daily living,mental state,nutritional status and falls(all P<0.05).There were 8 cases with the APOE gene ε4/ε3 genotype in the high dementia risk group and 16 cases with the APOE gene ε4/e3 genotype in the no dementia risk group,with the former group showing a higher frequency of the APOE ε4/ε3 genotype (44.4% vs.17.4%,x2 =6.46,P<0.05).The former group also seemed to have a higher APOE ε4 frequency,but the difference was not statistically significant(22.2 % vs.10.3 %,x2 =3.96,P =0.055)Chronic diseases prevalent in the long-lived people were hypertension(86 cases,78.2 %),hearing loss (72 cases,65.5%),hyperlipidemia(56 cases,50.9%),anemia(43 cases,39.1%),impaired vision(39 cases,35.5 %),chronic kidney diseases(25 cases,22.7 %),chronic heart diseases (18 cases,16.4 %) and osteoarthritis(18 cases,16.4%).No correlation was found between the risk of dementia and chronic diseases(P>0.05).Conclusions Cognitive function is highly correlated with activities of daily living,mental state and nutritional status among long-lived elderly people in Zhongxiang City.The risk of dementia has a correlation with the APOE gene e4/e3 genotype but not with chronic diseases in long-lived people.
9.Effect of calcium dobesilate tablets combined with mecobalamin injection on diabetic peripheral neuropathy and its effects on oxidative stress and neurotrophic factor

Objective To investigate the calcium dobesilate tablets combined with mecobalamin injection in treatment of diabetic peripheral neuropathy and effects on oxidative stress and neurotrophic factor.Methods 82 cases of diabetic peripheral neuropathy(DPN) patients from February 2015 to October 2016 in our hospital were selected and randomly divided into observation group and control group with 41 cases in each group.The control group was treated with mecobalamin injection, the observation group were treated with calcium dobesilate tablets.The course of treatment was two weeks in two groups.Results The total efficiency of the observation group (90.24%) was higher than the control group (68.29%) , the difference was statistically significant ( P<0.05 ) , patients in the observation group after the treatment of common peroneal nerve and median nerve SCV and MCV higher than the control group, the difference was statistically significant (P <0.05), the level of serum malondialdehyde (MDA) in the observation group was lower than that in the control group and the level of superoxide dismutase ( SOD) in the observation group was higher than that in the control group, the difference was statistically significant (P<0.05), the levels of serum NGF and BDNF in the observation group were higher than those in the control group, the difference was statistically significant (P<0.05), the two groups were no obvious adverse reactions.Conclusion Calcium dobesilate tablets and mecobalamin injection curative effect in the treatment of diabetic peripheral neuropathy , and patients could improve oxidative stress and neurotrophic factors, and the safety is good, has the important research significance.
10.Nursing care in reservation of the catheters in eleven nasopharyngeal carcinoma patients with symptomatic thrombosis after implantation of double-lumen PICCs

Objective To summarize the key points of nursing care in reservation of catheters in 11 cases of nasopharyngeal carcinoma patients who received concurrent chemo-radiotherapy with symptomatic thrombosis after ultrasound-guided implantation of double-lumen PICCs(PowerPICC).Methods From January,2014 to December,2015,totally 11 cases with symptomatic venous thromboembolism were identified among 109 cases of nasopharyngeal carcinoma patients receiving concurrent radiotherapy and chemotherapy,and observation and nursing care were provided at the early stage of thrombosis and during thrombosis.Results All double-lumen PICCs were reserved,and no recurrence or aggravation of thrombosis was recorded to the end of the treatment.The duration of carrying PICCs was 67~89(77.45±6.65) days.Conclusion With careful treatment and nursing,PICC catheter-related complications can be reduced and the duration of carrying catheters can be prolonged,which leads to accomplishment of the treatment plan for patients.
